Comparative effectiveness of hybrid and laparoscopic techniques for repairing complex incisional ventral hernias: a systematic review and meta-analysis.

BACKGROUND: The recently developed Hybrid Hernia Repair technique (HHR), an adaptation of the laparoscopic method, has been proposed as a potential alternative for the treatment of complex Incisional Ventral Hernias (IVH). While single-arm studies have reported promising outcomes, a comprehensive meta-analysis affirming these benefits is lacking. This meta-analysis aims to compare the clinical outcomes of HHR and Laparoscopic Hernia Repair (LHR) in the management of IVH. METHODS: An exhaustive search of the literature was conducted, targeting publications in both English and Chinese that comp

Diaphragm pacing in spinal cord injury can significantly decrease mechanical ventilation in multicenter prospective evaluation.

BACKGROUND: Cervical spinal cord injury (SCI) can lead to dependence on mechanical ventilation (MV) with significant morbidity and mortality. The diaphragm pacing system (DPS) was developed as an alternative to MV. METHODS: We conducted a prospective single-arm study of DPS in MV-dependent patients with high SCI and intact phrenic nerves. Following device acclimation, pacing effectiveness to provide ventilation was evaluated. The primary endpoint was the number who could use DPS to breathe for 4 continuous hours without MV. Secondary endpoints included the number of patients that could use DP

A Podcast Discussing Aldosterone and Mineralocorticoid Receptor Antagonists in 2021: A Paradigm Shift.

The classic focus on the mechanisms of action of aldosterone was directed primarily on its role in modulating renal excretory function and maintaining volume homeostasis. In contrast, many recent studies have demonstrated a much wider and expanded role for aldosterone and for the mineralocorticoid receptor (MR). Activation of the MR promotes inflammation, collagen formation, fibrosis, and necrosis with consequent renal injury. Increasing evidence has accrued that implicates the pathophysiological overactivation of the MR as a major determinant of progression of both diabetic and nondiabetic ch

Validity and Reliability of Prognosis Brachial Plexus Injury Score for Traumatic Brachial Plexus Injury.

BACKGROUND: Brachial plexus injury (BPI) is a peripheral nerve injury that results in severe functional impairment and disability. Even after prompt treatment, predicting the prognosis of BPI is not easy as it involves various factors. An objective and valid scoring system would aid clinicians in informing families and anticipating problems related to the recovery of BPI. Prognosis BPI (PRO-BPI) score, a new prognostic score to predict the outcome of traumatic BPI (TBPI), was developed in 2019 by Suroto and Rahman. This study aimed to evaluate its validity and reliability. METHODS: Retrospect

An unusual etiology of a scrotal dog bite injury and review of the literature.

Soft tissue injuries from animal bites are encountered occasionally in rural areas, resulting from attacks by, for example, dogs, wolves, horses, donkeys, and cats. The commonly affected body parts include the face, head and neck, nose, ears, hands, arms, and legs. The traumatic exposure of the external genital organs following an animal bite is a highly rare condition. Dog bite injuries in this area are a clinical condition that requires careful management due to the bacterial density of the oral flora of dogs, and also the potential bac-terial flora in the genital area, resulting in a high r

Transdeltoid Approach to Axillary Nerve Repair: Anatomical Study and Case Series.

PURPOSE: In cases of isolated paralysis of the axillary nerve, dissection of the distal stump at the posterior deltoid border can be difficult because of scarring from an injury or previous surgery. To overcome this, we propose dissecting the anterior division of the axillary nerve (ADAN) using a deltoid-splitting approach. We investigated the anatomy of the ADAN as it pertains to the transdeltoid approach and report the clinical application of this approach in 9 patients with isolated axillary nerve injury. METHODS: The axillary nerve and its branches were dissected in 9 fresh cadaver specim

Administration of diphenyl diselenide (PhSe)2 following repetitive mild traumatic brain injury exacerbates anxiety-like symptomology in a rat model.

Approximately 10-15 % of people that sustain mild traumatic brain injury (mTBI) develop post-concussive syndrome (PCS). PCS is a complex array of symptoms that can result in physical, cognitive and emotional impairments. Following mTBI, there are also complex changes in the oxidative stress system and engagement of the inflammatory system, within the brain. Diphenyl diselenide (PhSe)2 is an organoselenium compound which can play a role in anti-oxidant and anti-inflammatory activities. (PhSe)2 also has many interesting properties including anti-anxiety, anti-depressant and anti-nociception effe

Ketamine infusion for pain control in elderly patients with multiple rib fractures: Results of a randomized controlled trial.

BACKGROUND: Rib fractures are associated with increased mortality, particularly in the elderly. While opiate-based pain regimens remain the cornerstone of rib fracture management, issues related to opioids have driven research into alternative analgesics. Adjunctive ketamine use in lieu of opioids continues to increase but little evidence exists to support its efficacy or safety within the elderly trauma population. METHODS: A prospective, randomized, double-blind placebo-controlled trial of elderly patients (age, ≥65 years) with three or more rib fractures admitted to a Level I trauma center

Changes in propulsion technique and shoulder complex loading following low-intensity wheelchair practice in novices.

BACKGROUND: Up to 80% of wheelchair users are affected by shoulder pain. The Clinical Practice Guidelines for preservation of upper limb function following spinal cord injury suggest that using a proper wheelchair propulsion technique could minimize the shoulder injury risk. Yet, the exact relationship between the wheelchair propulsion technique and shoulder load is not well understood. OBJECTIVE: This study aimed to examine the changes in shoulder loading accompanying the typical changes in propulsion technique following 80 min of low-intensity wheelchair practice distributed over 3 weeks.

Trunk Stability Enabled by Noninvasive Spinal Electrical Stimulation after Spinal Cord Injury.

Electrical neuromodulation of spinal networks improves the control of movement of the paralyzed limbs after spinal cord injury (SCI). However, the potential of noninvasive spinal stimulation to facilitate postural trunk control during sitting in humans with SCI has not been investigated. We hypothesized that transcutaneous electrical stimulation of the lumbosacral enlargement can improve trunk posture. Eight participants with non-progressive SCI at C3-T9, American Spinal Injury Association Impairment Scale (AIS) A or C, performed different motor tasks during sitting. Electromyography of the tr
