Left External Carotid Artery to the Middle Cerebral Artery Bypass with Radial Artery Graft and Clip Reconstruction of a Large Middle Cerebral Artery Fusiform Aneurysm: 3-Dimensional Operative Video.

This 16-yr-old boy presented with episodes of severe headaches, blurred vision, dizziness, and muffled hearing and was discovered to have a large fusiform aneurysm of the left middle cerebral artery (MCA), M1 segment, 20 × 12 mm in dimension. The lenticulostriate arteries were arising proximal and distal to the aneurysm, but the anterior temporal artery was arising from the aneurysm. The aneurysm culminated in the distal M1 segment, and M1 immediately branched into 3 M2 vessels, the lower one being the larger. Due to origin of the lenticulostriate arteries and the anterior temporal artery and

Blood supply of the main bile duct from the caudate artery and medial subsegmental artery of the hepatic artery: Evaluation using images obtained during transcatheter arterial chemoembolization for hepatocellular carcinoma.

AIM: Main bile duct necrosis develops after transcatheter arterial chemoembolization (TACE) through the caudate artery (A1) and medial subsegmental artery (A4) of the hepatic artery in the treatment of hepatocellular carcinoma. The aim of this study was to evaluate the bile duct branch (BD branch) from A1 and A4. METHODS: We evaluated the origin and vascular territory of the BD branch in 11 patients who underwent selective A1 and/or A4 arteriography using arteriograms, cone-beam computed tomography (CBCT) and CT obtained 1 week after TACE. Follow-up CT and/or magnetic resonance imaging were a

Universal external carotid artery to proximal middle cerebral artery bypass with interposed radial artery graft prior to approaching ruptured blood blister-like aneurysm of the internal carotid artery.

Blood blister-like aneurysms are dangerous aneurysms with fragile walls arising from the supraclinoid internal carotid artery (ICA). Primary treatment of these aneurysms in the acute stage is challenging, due to the substantial risk of periprocedural bleeding. We describe a series of 4 patients who presented with ruptured blister-like aneurysm of the ICA and were treated with completion of extracranial-intracranial high-flow bypass followed by inspection and trapping of the aneurysm. All patients were treated in the acute stage, within 48 hours of bleeding. External carotid artery to proximal

Sudden Cardiac Arrest in a Previously Healthy Young Male: Perimyocarditis Presenting as Myocardial Infarction With Non-obstructive Coronary Arteries.

Myocardial infarction with non-obstructive coronary arteries (MINOCA) is a heterogeneous clinical entity that may mimic acute coronary syndromes despite the absence of significant epicardial coronary artery disease. Inflammatory myocardial conditions, such as myocarditis and perimyocarditis, represent important non-ischemic causes of MINOCA and may present with life-threatening complications, including malignant ventricular arrhythmias and sudden cardiac arrest. We report a case of a 37-year-old previously healthy male who presented with chest discomfort, syncope, and sudden cardiac arrest due

High-Origin Visceral and Renal Arteries with a Symptomatic Right Renal Artery Aneurysm: A Case Report and Literature Review.

Background: Renal artery aneurysms (RAAs) are uncommon visceral arterial lesions whose management depends on symptoms, patient-specific rupture risk, and arterial anatomy. Markedly high origin renovisceral arteries may further complicate endovascular access and reconstruction. This report describes the management of a symptomatic RAA in a young woman with multifocal aneurysmal disease and unusual renovisceral arterial origins at the 11th and 12th thoracic vertebral levels (T11-T12). Case presentation: A 35-year-old woman presented with intermittent right loin pain. Computed tomography angiogra

Impact of virtual monoenergetic images on the assessability of lower extremity arteries in (Poly-) trauma photon-counting detector CT.

BACKGROUND: Computed tomography (CT) is state of the art imaging modality in patients after severe trauma. A double-bolus contrast media protocol can be used to combine both arterial and parenchymal contrast in one scan. In case of lower extremity trauma, the CT scans can be extended to include the legs in order to rule out vascular pathologies. Virtual monoenergetic (VME) images derived from a Photon-Counting detector CT (PCDCT) proved to be helpful in vascular diagnostics and could therefore also add information in these extended trauma scans. Aim of this study was to determine the VME recon

Bayesian network meta-analysis and systematic review of endovascular revascularization strategies for infrapopliteal arteries in chronic limb-threatening ischemia.

BACKGROUND: In 2024, several researchers introduced the concept of "woundosome" in an editorial, highlighting the need to prioritize perfusion and microcirculation function within patient wound areas. Building upon this foundation, we systematically compared this concept with the previously established angiosome theory. METHODS: A comprehensive search was performed across the Cochrane Central Register of Controlled Trials, Embase, and PubMed. Twenty-five relevant studies met the inclusion criteria, including four investigations using wound blush (WB). We implemented a Bayesian network meta-an

Evaluation of the systemic right ventricle in transposition of the great arteries using fast strain encoded cardiovascular magnetic resonance.

BACKGROUND AND AIMS: Patients with a systemic right ventricle (SRV) experience increased morbidity and mortality, highlighting the need for precise assessment of systolic function. Fast Strain-ENCoded imaging (fSENC) is a novel strain measurement method that allows early detection of cardiac impairment and has demonstrated significant diagnostic and prognostic value. This study aimed to evaluate global and regional cardiac function using fSENC in patients with SRV due to transposition of the great arteries following Senning/Mustard repair (TGA) or congenitally corrected TGA (ccTGA). METHODS:

Preoperative Heart Rate Variability and Parasympathetic Modulation in Newborns with Transposition of the Great Arteries Undergoing Arterial Switch Operation.

INTRODUCTION: We retrospectively assessed preoperative cardiac autonomic regulation in neonates with TGA who subsequently underwent arterial switch operation. MATERIALS AND METHODS: Neonates treated between 1 January and 31 December 2024 were studied. Heart rate variability was derived from continuous ECG recordings, with low-frequency and high-frequency spectral components calculated from RR intervals. Longitudinal associations between HRV indices and the interval to surgery were evaluated using linear mixed-effects models with adjustment for relevant clinical covariates. RESULTS: Twenty ne

Outcomes for congenitally corrected transposition of great arteries using various surgical techniques: a systematic review and network meta-analysis.

INTRODUCTION: Congenitally corrected transposition of the great arteries (ccTGA) is a rare disorder. Here, we evaluated the effectiveness of physiological, anatomical and Fontan repair techniques for ccTGA. METHODS: A systematic database search in PubMed, Embase and Cochrane Central Register of Controlled Trials was conducted from inception to 5 June 2023. Random-effects network meta-analysis was performed, comparing mortality, postoperative arrhythmias, reintervention after definitive surgery and transplant-free survival in patients undergoing physiological, anatomical and Fontan repair. Surgical Outcomes of anomalous aortic origin of coronary arteries: early and mid-term results from a single-center experience.

BACKGROUND: Anomalous aortic origin of a coronary artery (AAOCA) is the second leading cause of sudden cardiac death in young athletes. While anomalous aortic origin of the right coronary artery (AAORCA) predominates, anomalous aortic origin of the left coronary artery (AAOLCA) carries higher mortality. Surgical correction requires anatomical precision due to heterogeneous anatomy. METHODS: A retrospective analysis (June 2016–June 2023) included 17 AAOCA patients (mean age: 11.9 ± 3.8 years). Surgical techniques included unroofing (94.1%), coronary reimplantation (11.8%), pulmonary translocati

Cardiovascular safety profile of prostaglandin E and prostacyclin analogues: Clinical reports and ex vivo studies on human coronary arteries.

Prostaglandin E1 (PGE1), prostaglandin E2 (PGE2), prostacyclin and their analogues are widely used in clinical practice for their potent biological effects. PGE1/PGE2 analogues, including misoprostol and sulprostone, are primarily used in obstetrics for labour induction, abortion, and postpartum haemorrhage management, whereas PGI2 analogues such as iloprost and treprostinil are key drugs in pulmonary arterial hypertension treatment. Despite structural similarities, these compounds produce contrasting vascular effects due to differences in receptor selectivity, signalling pathways, and tissue

A physician-modified endograft for intercostal arteries preservation and its role as an endovascular strategy in chronic aortic dissection.

Treatment of thoracoabdominal aortic aneurysm post aortic dissection is challenging and carries a significant risk of spinal cord ischemia. Open surgery allows reimplantation of intercostal arteries (ICAs), whereas conventional endovascular treatment cannot. We report a 46-year-old patient treated with a physician-modified endograft with posterior fenestrations to preserve perfusion of ICAs. The device was deployed in the true lumen to maintain sealing, with a large fenestration aligned to the target ICAs. Two custom-made false lumen endoprostheses were also placed in the false lumen. The post

Prognostic incremental value of perivascular adipose tissue in myocardial infarction with non-obstructive coronary arteries: a multicenter cohort study.

BACKGROUND: Perivascular adipose tissue (PVAT) is recognized as a key contributor to atherosclerosis and adverse clinical outcomes. However, its relationship with metabolic status and prognosis in patients with myocardial infarction with non-obstructive coronary arteries (MINOCA) remains unclear. The aim of this study was to investigate the relationship between PVAT and clinical prognosis in patients with MINOCA, and to assess its incremental predictive value beyond the traditional metrics. METHODS: A simple scoring tool, the PVAT score, was constructed based on cardiac magnetic resonance (CM
