Neonatal liver (storage) but not heart (nonstorage) tissue iron concentrations were reduced by 60% at autopsy in 15 newborn infants who had gestations complicated by uteroplacental insufficiency because of maternal hypertension or Potter syndrome. The hepatic iron reductions in term and preterm infants, and with either antecedent condition, were similar.

A 2-year-old patient with diffuse supravalvular aortic stenosis and the Williams-Beuren syndrome at autopsy was found to have dissection of the ascending aorta in addition to severe coronary artery disease and microfocal myocardial fibrosis. These findings emphasize the need for early surgical intervention in supravalvular aortic stenosis.

A 66-year-old male with chronic pericarditis accompanied by idiopathic retroperitoneal fibrosis was originally admitted for cardiac tamponade. A firm diagnosis was obtained only after the patient's sudden death when autopsy was performed. Chest X-rays showed cardiac enlargement and pleural effusion. Echocardiogram revealed bright echo density on the myocardium.

A diabetic man developed a severe hyperosmolar state resulting in coma. CT of the head showed bilateral cerebellar hemorrhages. Despite medical treatment, he deteriorated and died. At autopsy, the straight sinus was thrombosed. There were bilateral, hemorrhagic, cerebellar venous infarctions. This condition is rare because of abundant collateral venous drainage.

A fatal case of malakoplakia of the prostate is described. The malakoplakia developed after 20 years of chronic prostatitis caused by E. coli and was not prevented by careful antibiotic treatment. Three years after the histological verification malakoplakia invasion caused fistula to rectum. Autopsy revealed no signs of any debilitating diseases.

We report an autopsy case of an 8-month-old male infant with bilateral cystic nephroblastomas, a botryoid sarcoma involving the urinary bladder, microencephaly, the Dandy-Walker syndrome, bilateral cataracts, hypospadias, and male pseudohermaphroditism. An unusual combination of congenital malformation and tumors may reflect the presence of a mutant gene.

Sudden unexpected death caused by the failure of a cardiac valve prosthesis is a rare occurrence. A 45-year-old female suddenly died three years following mitral valve replacement for rheumatic mitral stenosis. At autopsy, death was determined to be due to a metallic fatigue of the mitral valve prosthesis.

A known hypertensive presented to the hospital with sudden onset of chest pain and shock. ECG, cardiac enzymes, aortogram and echocardiogram were normal. Hemodynamic monitoring and radionucleotide angiogram demonstrated a severely dyskinetic right ventricle. Autopsy revealed proximal aortic dissection with extramural compression of right coronary artery, resulting in right ventricular dysfunction.

An evaluation of 1500 perinatal autopsies is presented and the findings of umbilical cord anomalies are related to fetal malformations. The most frequent pathologies of the umbilical cord were one umbilical artery and stenosis of the cord. The author highlights the importance of a thorough examination of the umbilical cord and of the placenta.

A case of an infant born in association with a large chorioangioma of the placenta is reported. The mother was complicated with polyhydramnios and premature labor. The fetal complications were congestive heart failure and hydrops fetalis. This case serves to emphasize that it is necessary to examine the placenta in every perinatal autopsy case.

Cardiac rhabdomyomas of the fetus and neonate are benign cardiac muscle hamartomas with an incidence of 0.027% in pediatric autopsies. This is the third reported case of prenatally diagnosed cardiac rhabdomyoma, the second with severe hydrops and the only one associated with aortic valve hypoplasia and hypoplastic lungs.

Disseminated aspergillosis is a major complication in immunosuprressed patients. Infiltration of the thyroid with Aspergillus organisms occurs in approximately 20% of autopsies in patients dying with disseminated disease. The present report describes Aspergillus thyroiditis, Hashimoto's thyroiditis, and possible hypothyroidism in a patient with disseminated aspergillosis.

BACKGROUND: Aortic dissection (AD) is still a challenging diagnosis with poor prognosis, and its misdiagnosis remains an unresolved problem. Postmortem examination has long been deemed important for understanding the diseases and determining the cause of death. OBJECTIVES: Adrenalectomy is a part of radical nephrectomy because of the surgical oncology principle of a 'wide margin beyond the malignancy' and due to concern over possible metastases to the ipsilateral adrenal gland, especially in upper pole tumors.
