Alpha-adrenoreceptor subtypes in blood vessels: physiology and pharmacology.

A significant advance in the field of neurotransmission was made with the discovery of presynaptic release-modulating alpha-adrenoreceptors on noradrenergic nerve terminals. The concept of presynaptic modulation of noradrenaline release developed in parallel with the pharmacological evidence for two subtypes of alpha-adrenoreceptors as defined by a different profile of affinity and relative order of potencies for agonists and for antagonists. The alpha 1-adrenoreceptor is stimulated preferentially by methoxamine and cirazoline and blocked selectively by prazosin or corynanthine. The alpha 2-ad

Responses of the stenosed and contralateral kidneys to [Sar1, Thr8] AII in human renovascular hypertension.

To better define the intrarenal hemodynamic effects of angiotensin in human renovascular hypertension, 10 patients underwent renal hemodynamic and functional measurements before and during infusion of a competitive angiotensin analog, [Sar1, Thr8] AII. Eight had technically satisfactory split function studies. Despite a fall in mean arterial pressure (132 +/- 6 to 121 +/- 6 mm Hg, p less than 0.05) and humoral changes consistent with angiotensin-mediated hypertension, the intrarenal effects of this analog were commonly those of an angiotensin agonist, producing vasoconstriction and sodium rete

[Surgical treatment of total anomalous pulmonary venous connection].

Surgical experience of 70 patients with TAPVC between 1966 and 1982 were reviewed. Their age ranged from 2 days to 13 years. Fifty-two patients (74%) were below 6 months of age, 36 (51) below 3 months and 17 (24%) were neonates, respectively. Thirty-seven patients (56%) were with supracardiac type, 14 (20%) were with cardiac type and 12 (17%) were with intracardiac type. Results of operation below 6 months of age were unsatisfactory. Only 10 patients including 3 neonates out of 52 survived. However, 16 of 18 patients over 6 months of age survived. Early referral and emergency intracardiac repa

Rhythmic activity of cat pial vessels in vivo.

Rhythmic activity was observed in over 100 pial vessels in 58 cats with a photometric technique through a cranial window. Diameter oscillations with a frequency of mostly 5/min were present at normal mean arterial pressure (MAP) in about half of the animals. When spontaneous activity was absent the same type of oscillations could be provoked by varying MAP, mostly by lowering it to a level of around 60 mm Hg. If a rhythmic activity was present, it could be abolished by increasing and decreasing MAP. Likewise, rhythmic activity ceased during strong vasodilatation caused by hypercapnia, hypoxia,

Cardiovascular and behavioral effects of intracisternal 6-hydroxydopamine in the rabbit.

Acute effects of 6-hydroxydopamine (6-OHDA, 400--900 microgram kg-1 intracisternally, i.c.i.) consisted of bradycardia and hypertension, maximal 2--3 h after injection and preceded after some doses by a phase of hypotension. This pattern was obtained in completely conscious rabbits and after propanidid and sodium pentobarbitone anesthesia. After 600 microgram kg-1 i.c.i. 6-OHDA the peak rise in blood pressure (25 +/- 3.8 mm Hg) was due to a rise in peripheral resistance involving particularly renal and intestinal beds. Suprapontine mechanisms contributed to both hypertension and bradycardia. G

Effects of acetylsalicylic acid on the ductus arteriosus and circulation in fetal lambs in utero.

Intra-arterial and intravenous catheters were inserted in six fetal lambs at 125-130 days of gestation. On the following day, fetal arterial pressures and blood gases were monitored and fetal cardiac output and its distribution were measured by injection of radionuclide-labeled microspheres 15 mum in diameter. Acetylsalicylic acid, 55-90 mg/kg of estimated fetal weight, then was administered into the fetal stomach. Fetal pulmonary arterial pressure rose significantly after an average of 58 minutes, increasing the pressure difference between the pulmonary artery and the aorta from 2 +/- 0.3 (SE

The graft-reposition-on-flap technique for fingertip amputations: a patient-reported outcomes study of 111 patients.

PURPOSE: Graft-reposition-on-flap (GRF) is a reconstruction method described for non-replantable fingertip amputations. We hypothesised that this technique provides acceptable outcomes for such an injury, as measured by a patient-reported survey. METHODS: This retrospective study evaluated 111 patients [118 digits], operated between January 2019 and March 2023 for Allen 3 or 4 fingertip amputations. All patients had crush avulsion amputations not fit for replantation and had undergone flap cover with grafting of the amputated nail bed and bone. 80 digits had undergone a flap with bone and nai

Analysis of Failure to Achieve the Critical View of Safety (CVS) in Laparoscopic Cholecystectomy: A Prospective Observational Study.

Background The critical view of safety (CVS) is the principal intraoperative strategy to prevent bile duct injury during laparoscopic cholecystectomy (LC). However, CVS may not be achieved in all cases. Factors that predict such failure remain understudied. The aim of this study was to determine the rate of failure to achieve CVS during LC and to identify various preoperative and intraoperative predictors of failure to achieve CVS. Methods In this prospective observational study, 41 adult patients undergoing LC for gallstone disease at a single center were included. LC was performed using the

Clinical and polysomnographic changes in idiopathic hypersomnia ten years after the diagnosis.

PURPOSE: Idiopathic hypersomnia (IH) is characterized by excessive daytime sleepiness and an excessive need for sleep. It is unknown whether the disorder is stable or improves with time. Our goal was to characterize the evolution of clinical and sleep patterns (including measures of excessive sleep need) in patients with IH over a ten-year period. METHODS: A standardized questionnaire focusing on the progression of clinical symptoms was sent to all patients with IH. At the time of diagnosis, all patients underwent overnight polysomnography and a multiple sleep latency test (MSLT), followed by

Single- versus multi-fraction spine stereotactic radiosurgery (ALL-STAR) for patients with spinal metastases: a randomized phase III trial protocol.

BACKGROUND: For patients with spine metastases, stereotactic radiosurgery (SRS) provides excellent local control and pain response. Despite increasing use of this treatment modality, there is no consensus on the optimal dose and fractionation of spine SRS for efficacy and toxicity. We have initiated a single-center phase III randomized trial that compares two dose regimens with similar biological equivalent dose (BED) to determine the isolated effect of SRS fractionation on local control. METHODS: Patients with one to three cervical, thoracic, or lumbar spine metastases spanning no more than

Understanding the impact of radiotherapy fractionation on overall survival in a large head and neck squamous cell carcinoma dataset: a comprehensive approach combining mechanistic and machine learning models.

INTRODUCTION: Treating head and neck squamous cell carcinomas (HNSCC), especially human papillomavirus negative (HPV-) and locally advanced cases, remains difficult. Our previous analyses of radiotherapy-only HNSCC clinical trials data using mechanistically-motivated models of tumor repopulation and killing by radiotherapy predicted that hyperfractionation with twice-daily fractions, or hypofractionation involving increased doses/fraction and reduced treatment durations, both improve tumor control and reduce late normal tissue toxicity, compared with standard protocols using 35×2 Gy. Here we f

Effect of Nightly Lower Body Negative Pressure on Choroid Engorgement in a Model of Spaceflight-Associated Neuro-ocular Syndrome: A Randomized Crossover Trial.

IMPORTANCE: Astronauts returning from long-duration spaceflight experience ocular remodeling related to cephalad fluid shifts induced by microgravity. It is hypothesized that the absence of diurnal reductions in intracranial pressure in microgravity creates a low but persistent pressure gradient at the posterior aspect of the eye, which results in ocular remodeling and space-associated neuro-ocular syndrome (SANS) over many months. OBJECTIVE: To determine whether partial reintroduction of footward fluid shifts during simulated microgravity via lower body negative pressure (LBNP) during sleep

Time-Series Analysis of Health Care-Associated Infections in a New Hospital With All Private Rooms.

IMPORTANCE: Health care-associated infections are often caused by multidrug-resistant organisms and substantially factor into hospital costs and avoidable iatrogenic harm. Although it is recommended that new facilities be built with single-room, low-acuity beds, this process is costly and evidence of reductions in health care-associated infections is weak. OBJECTIVE: To examine whether single-patient rooms are associated with decreased rates of common multidrug-resistant organism transmissions and health care-associated infections.
