BACKGROUND: Chronic total occlusion (CTO) that cannot be crossed with balloons or microcatheters despite successful guidewire passage represent one of the most challenging scenarios in percutaneous coronary intervention (PCI). Excimer laser coronary angioplasty (ELCA) is an established adjunct for device-uncrossable lesions; however, the safety of its application within the subintimal space remains limited.
CASE SUMMARY: A 67-year-old male with a complex cardiac history, including prior recurrent interventions and coronary artery bypass grafting, was referred for staged PCI of a right coronar

Notably, most ventricular tachycardia (VT) episodes in patients with VT attributable to structural heart disease are not hemodynamically tolerated. Therefore, techniques for substrate mapping during stable intrinsic or paced rhythm have been developed that eliminate the need to induce VT. Moreover, advances in catheter technology, enabling high-density multi-electrode mapping of abnormal electrograms, have improved the ability of electrophysiologists to identify the substrate responsible for scar-related VT. In addition to the conventional identification of late potentials and local abnormal v

Iatrogenic femoral pseudoaneurysms and arteriovenous fistulas are uncommon but clinically significant complications of femoral access procedures. Simultaneous occurrence of both lesions is rare and poses diagnostic and therapeutic challenges. Multimodal imaging plays a critical role in defining lesion anatomy, hemodynamics, and selecting appropriate management. A 63-year-old male with a complex cardiac history, including mechanical aortic valve replacement, underwent catheter ablation for persistent atrial flutter via bilateral femoral venous access. Seven days postprocedure, he presented with

BACKGROUND: Achieving adequate lesion depth during ventricular tachycardia (VT) ablation within scar is challenging with conventional radiofrequency energy (RF). The lattice-tip catheter enables dual-energy ablation (RF & pulsed field [PF]) with enhanced tissue penetration.
OBJECTIVE: We report on 3 cardiac implantable electronic device (CIED) patients with transmural apical left ventricular (LV) scars and drug-refractory VT. Each underwent endocardial LV ablation using RF & stacked PF. Ablation included delivery near the opposing surface of CIED leads, at sites critical for the VT circuit. C

BACKGROUND: Arrhythmogenic cardiomyopathy (ACM) is an inherited disease characterized by fibrofatty replacement, most often involving the right ventricle but potentially affecting the left or both ventricles. It predisposes to severe ventricular arrhythmia (VA). Differentiating ACM from athlete's heart remains difficult, particularly in gene-elusive patients. Implantable cardioverter-defibrillators (ICDs) and catheter ablation are key therapeutic options. Epicardial ablation may be required for VA circuits inaccessible in an endocardial approach, but data on its feasibility in patients with ex

INTRODUCTION: Temporary epicardial pacing wires (TEPWs) are utilized for arrhythmia management in postoperative cardiac surgery. TEPWs that cannot be removed are sometimes left in situ, which can lead to rare complications such as vessel stenosis and thrombosis. This case report describes the use of small biopsy forceps advanced to the superior mesenteric artery (SMA) through a steerable sheath to remove a partially endothelialized TEPW.

BACKGROUND: Rotational atherectomy (RA) burr entrapment is an uncommon but potentially life-threatening complication. Additional percutaneous bailout is necessary when conventional percutaneous bailout strategies are unsuccessful in patients with prohibitive surgical risk.
CASE SUMMARY: A 91-year-old man with a history of coronary artery bypass graft surgery presented with unstable angina caused by severely calcified diffuse stenosis of the right coronary artery. During lesion preparation, an RA burr became entrapped after crossing a heavily calcified segment. Multiple conventional percutaneo

BACKGROUND: Neonatal trauma resuscitation is particularly challenging in cases of profound hemorrhagic shock. Low-titer group O+ whole blood (LTOWB+) has emerged as a potentially effective option in pediatric trauma, but its use in neonates is debated due to risks such as D-alloimmunization. In life-threatening emergencies, decisions must carefully balance immediate survival benefits against long-term risks.
STUDY DESIGN AND METHODS: We present a case report of a 4-day-old neonate transported as a Level 1 trauma following a dog attack to the head, resulting in hemorrhagic shock and cardiac ar

Atrial fibrillation (AF) is a clinically heterogeneous syndrome where traditional 'one-size-fits-all' management strategies are often suboptimal. This review synthesizes the contemporary application of machine learning (ML) and deep learning (DL) in identifying distinct clinical AF phenotypes to advance personalized treatment. We provide a comprehensive overview of over a dozen key phenotyping studies, highlighting the consistent identification of core patient subgroups across diverse international cohorts, including low-risk/younger, atherosclerotic/high-cardiovascular-risk, and elderly/multi

BACKGROUND: Incessant atrial tachycardia (AT) in patients with heart failure and congenitally corrected transposition of the great arteries (ccTGA) is rare and challenging for ablation.
CASE SUMMARY: A 49-year-old man with ccTGA and membranous ventricular septal aneurysm presented with incessant AT leading to heart failure, refractory to antiarrhythmic drugs and electrical cardioversion. Initial extensive atrial mapping suggested the left atrial anterior wall as the site of earliest activation, but ablation at this site was ineffective. BACKGROUND: Situs viscerum inversus with dextrocardia and interrupted inferior vena cava (I-IVC) are rare congenital diseases that might challenge interventional procedures, such as transcatheter ablation for atrial fibrillation (AF), when transseptal puncture (TSP) is required for left atrial access. Recently, one-shot pulsed field ablation (PFA) has emerged as a safe and effective energy modality for AF ablation. We present a case report on jugular TSP guiding one-shot PFA in a patient presenting dextrocardia and an I-IVC, addressing the unique anatomical challenges.

Venovenous extracorporeal membrane oxygenation (VV-ECMO) is a lifesaving procedure for patients with acute respiratory distress syndrome (ARDS) due to severe coronavirus disease (COVID-19) pneumonia. COVID-19 infection is a risk factor for the development of coagulopathy and anticoagulant treatment is recommended. Moreover, anticoagulant therapy is often used to prevent circulatory coagulopathy during ECMO therapy. A man on dual antiplatelet therapy was hospitalized with severe COVID-19 pneumonia and treated with standard therapy. Apixaban was administered for a suspected coagulopathy. His med

Structural valve deterioration (SVD) refers to intrinsic and irreversible pathological changes in the components of prosthetic heart valves, manifesting as fibrosis, calcification, wear and tear, loosening, as well as strut fracture or deformation of the valve framework. These changes ultimately lead to valve stenosis and/or regurgitation. The mechanisms may be related to mechanical stress, immune response and abnormal calcium-phosphorus metabolism. Studies have shown that risk factors for SVD include patient factors (such as age, underlying cardiovascular disease and comorbidities), valve fact

BACKGROUND: Right atrial (RA) diverticulum is a rare malformation that can, in some cases, harbour an accessory pathway (AP). This report describes the case of a patient with RA diverticulum associated with a high-risk right posteroseptal AP. Successful radiofrequency catheter ablation was performed, eliminating the AP and resolving the patient's pre-excitation.
CASE SUMMARY: A 33-year-old man with a 15-years history of an AP presented with two isolated, self-limited episodes of palpitations. A 12-lead ECG during normal sinus rhythm showed pre-excitation consistent with a right posteroseptal

BACKGROUND: Multiple studies have demonstrated the safety of pulsed-field ablation (PFA) systems; however, patients with cardiac implantable electronic devices (CIEDs) have been excluded because of concerns about the impact of strong electrical fields on device function and integrity.
OBJECTIVE: This study assesses the effect of PFA on the function and integrity of CIEDs.
METHODS: Patients with atrial fibrillation undergoing ablation were studied at 2 sites. PFA was performed to achieve pulmonary vein and posterior wall isolation. CIEDs were interrogated before and after PFA to assess functi

Blood pressure gradient ( Δ P ) across an aortic coarctation (CoA) is an important measurement to diagnose CoA severity and gauge treatment efficacy. Invasive cardiac catheterization is currently the gold-standard method for measuring blood pressure. The objective of this study was to evaluate the accuracy of Δ P estimates derived non-invasively using patient-specific 0D and 3D deformable wall simulations. Medical imaging and routine clinical measurements were used to create patient-specific models of patients with CoA (N = 17). 0D simulations were performed first and used to tune boundary c

BACKGROUND: Left ventricular (LV) diastolic function is a key determinant of cardiac output and impairments of diastolic function can lead to heart failure. Assessment of diastolic function is challenging due to several factors, including the load dependence of ventricular filling. We developed a method using cardiovascular magnetic resonance (CMR) imaging to model the untwisting motion of the LV as a viscoelastic damped oscillator to derive myocardial torsional modulus (µ) and frictional damping characteristics, and hypothesized that the torsional modulus would correlate with invasive measure

BACKGROUND: Pulse field ablation (PFA) is a novel catheter ablation technology with potential safety benefits due to its tissue selectivity. It has the potential to directly damage or interact with the functionality of cardiac implantable electronic devices (CIEDs) in the form of electromagnetic interference (EMI). The aim of our study was to assess the impact of PFA on CIEDs.
METHODS: PFA lesions (45 per CIED) were applied from the Farapulse system to CIEDs (< 5 cm from the lead tip and < 15 cm from the generator). All devices were checked before and after PFA application for proper sensing

The purpose of this study is to evaluate whether early initiation of catheter-directed thrombolysis (CDT) in patients presenting with acute pulmonary embolism is associated with improved in-hospital outcomes. A retrospective cohort was extracted from the 2016-2019 National Inpatient Sample database, consisting of 21,730 weighted admissions undergoing CDT acute PE. From the time of admission, the sample was divided into early (<48 h) and late interventions (>48 h). Outcomes were measured using regression analysis and propensity score matching. No significant differences in mortality, cardiac ar
