Surgical treatment for osteochondritis dessicans of the knee.

Osteochondritis dissecans (OCD) of the knee is a disease of the subchondral bone with secondary injury to the overlying articular cartilage. OCD lesions are generally categorized as juvenile-growth plates open-or adult-growth plates closed. This maturity-based classification scheme has a prognostic value in that many juvenile OCD lesions will heal with conservative care while most symptomatic adult OCD lesions need surgical intervention. OCD can result in pain, knee joint effusions, loose body formation, and arthritis. Short-term treatment goals include pain and symptom resolution while the lo

Expression of genes encoding matrilin-3 and cyclin-I during the impairment and recovery of chicken growth plate in tibial dyschondroplasia.

Tibial dyschondroplasia (TD) is a skeletal disease characterized by the disruption ofendochondral bone formation in avian species. The aim of this study was to determine the expression of Matrilin-3 and Cyclin-I genes in chicken growth plate during impairment and recovery from TD. Gene expressions were analyzed by polymerase chain reaction, and proteins by immunohistochemistry and in situ hybridizations. Expression of genes encoding Matrilin-3 and Cyclin-I were diminished with parallel decrease in proteins during TD, with fewer signs of cartilage cell differentiation. In contrast, there was an

Dyspnea in a patient with Mounier-Kuhn syndrome.

Poorly differentiated non-small cell lung carcinoma with a component of sarcoma-like (spindle and/or giant cells) or sarcoma (malignant bone, cartilage, or skeletal muscle) cells are called pleomorphic carcinoma. These carcinoma represent one of the 5 subtypes of rare pulmonary malignancies collectively classified as sarcomatoid carcinoma by the World Health Organization histologic classification of lung tumors. The pathogenesis of sarcomatoid carcinoma remains unclear, and treatment of this malignant tumor is less defined. Very few cases of sarcomatoid carcinoma involving the upper respirator

[Interleukins network in rheumatoid arthritis pathophysiology: beyond proinflammatory cytokines].

Rheumatoid arthritis (RA) is a systemic autoimmune disease characterized by synovitis and progressive destruction of the joint cartilage and underlying bone, together with diverse extra-articular manifestations. Cytokines act as soluble effector mediators of the inflammatory process. Therapeutic neutralization with monoclonal antibodies against the pro-inflammatory cytokines TNF-alpha and interleukin 1 (IL-1) has shown a clear efficacy on inflammation and clinical manifestations of RA, although a percentage of patients do not respond. This review covers new relevant cytokines in the RA physiop

[Mechanism underlying osteoarthritis induced by mechanical stress on joint cartilage].

Osteoarthritis (OA) , one of the most common skeletal disorders characterized by cartilage degradation and osteophyte formation in joints, is induced by accumulated mechanical stress ; however, little is known about the underlying molecular mechanism. Several experimental OA models in mice by producing instability in the knee joints have been developed to apply approaches from mouse genetics. Our mouse genetic approaches found that the induction of a transcriptional activator Runx2 in chondrocytes under mechanical stress contributes to the pathogenesis of OA through chondrocyte hypertrophy. In

Ossified posterior longitudinal ligament with an atypical clinical presentation in an Indian male. A case report.

Ossification of the posterior longitudinal ligament is an uncommon disorder mostly seen in the Japanese population and hence termed by some the "Japanese disease". Ossification of the posterior longitudinal ligament is more common in the cervical spine (2). Clinically it is usually asymptomatic, but serious neurological deficits have been seen in some patients (2). The ossified mass composed of lamellar bone and focal calcified cartilage expands in volume causing spinal canal stenosis and cord compression. Morphologically, four forms of ossification of the posterior longitudinal ligament have

Calcium-containing crystals and osteoarthritis: implications for the clinician.

The clinical implication of articular deposits of calcium-containing crystals (specifically of calcium pyrophosphate dihydrate and hydroxyapatite) in osteoarthritis is unknown. Recent longitudinal studies have suggested that in some instances calcium crystals are direct participants in cartilage damage, while in other situations they are merely markers of joint damage. Better understanding of the mechanisms of crystal formation, especially in relation to inorganic pyrophosphate regulation, has lead to potential avenues for therapeutic intervention. The current treatment of osteoarthritis assoc

Screening for esophageal luminal narrowing--a magnetic disc tablet as a bolus challenge: preliminary report.

Narrowing of the esophageal lumen often exists for months before being noted by the patient. Screening tests for such lesions would be useful, if applicable to office use. A magnetic disc tablet (14 mm in diameter and 5 mm thick) was fashioned by hand using dental acrylic material to enclose the magnet with a smooth watertight sheath. A magnetic detector was held on the anterior abdominal wall 2.5 cm below the xiphoid cartilage and 2.5 cm to its left. A significant change in reading indicated that the tablet had reached the cardia of the stomach. The magnetic tablet was readily detected in 11

A case of multisymptomatic relapsing polychondritis in a 22-year-old woman.

A case of multisymptomatic relapsing polychondritis in a 22-year-old woman. We report a case of a 22-year-old woman with relapsing polychondritis (RP)--a rare and little known systemic autoimmune disease characterised by episodic inflammation of cartilaginous structures (ear, nose, bronchi, trachea, larynx, ribs, cardiovascular system). This patient presents with a seven-year history, initiated by the saddle nose. The patient developed a multitude of symptoms: auricular chondritis, ocular symptoms, recurrent arthritis, respiratory complications (laryngotracheomalacia, bilateral vocal cord pals

Musculoskeletal connective tissue cells with muscle: expression of muscle actin in and contraction of fibroblasts, chondrocytes, and osteoblasts.

The expression of the gene for a muscle actin in certain nonmuscle cells and the contraction of these connective tissue cells has been associated with several important physiological and pathological processes; the contraction of healing skin wounds and the contracture in Dupuytren's disease being two notable examples. Studies in recent years have shown that a much wider variety of connective tissue cells than previously considered, including cells in many of the musculoskeletal tissues, e.g., chondrocytes and osteoblasts, can express the gene for alpha-smooth muscle actin and can display cont

Radiographic imaging: the 'gold standard' for assessment of disease progression in rheumatoid arthritis.

Rheumatoid arthritis (RA) is characterized by a chronic inflammation of the joints, which leads to the destruction of articular cartilage and bone. The degree of joint damage assessed by radiographic imaging represents a key outcome in RA. There are several methods for scoring the joint damage associated with RA. The most widely used are the Sharp and Larsen systems, as well as more recent modifications of each method. Radiographic imaging has several advantages compared with other outcome measures in RA, specifically: X-rays reflect the history of joint pathology, provide a permanent record f

[Repeated rupture of the extensor tendons of the hand due to fluoroquinolones. Apropos of a case].

Fluoroquinolone toxicity on cartilages and tendons has been well known since 1983. Tendon inflammation or rupture has been described. Achilles tendon rupture is the most frequent complication but many other sites of tendon injuries have been reported. This article presents a case of rupture of extensor tendons of the hand in an elderly woman treated by fluoroquinolones. As far as we know, this site of tendon lesion has never been previously described. Histological examination of tendon injuries was possible after surgical treatment. Histological structures were similar to the classical descrip

Pancarpal arthrodesis for treatment of carpal collapse in the adult horse. A report of two cases.

Two mares with multiple carpal bone fractures, malarticulation and degenerative joint disease were successfully treated with unilateral arthrodeses of the antebrachiocarpal, middle carpal, and carpometacarpal joints. Arthrodesis was achieved by removal of articular cartilage, autogenous cancellous bone graft, and double dynamic compression plating. In one horse, wedge ostectomy corrected the severe, acquired angular limb deformity. External coaptation supplemented the internal fixation. Postoperative complications were limited to cast sores that healed with treatment, although protracted in on

Synovial chondromatosis.

Two patients were evaluated for a complaint of long-standing hip pain. No etiology of their progressively disabling symptoms was found by routine diagnostic studies. Hip arthrography and arthrotomography in each case, however, demonstrated multiple intraarticular filling defects consistent with nonradiopaque loose bodies. These were subsequently confirmed at arthrotomy. Synovial chondromatosis is a rare disease in which intrasynovial cartilage metaplasia results in multiple intracapsular loose bodies. Its presence in plain radiographs is uncommon, but has been previously documented. This can a

Advanced necrotizing external otitis treated by suboccipital craniectomy.

Necrotizing external otitis is a destructive infection caused by Pseudomonas aeruginosa. It may spread to surrounding soft tissue, cartilage and bone from the skin of the external canal and cause cranial nerve palsy. Most of the disease occurs in elderly diabetics and is accompanied by high mortality in an extended case in spite of intensive antibiotic therapy. A case of advanced necrotizing external otitis with palsies of VI to XII cranial nerves was presented. The patient, 69-year-old male, took a favorable course after complete debridement by suboccipital craniectomy followed by intensive a

Elevated levels of collagenase and prostaglandin E2 from synovium associated with erosion of cartilage and bone in a patient with chronic Lyme arthritis.

A patient with chronic Lyme arthritis and roentgenographic evidence of bony erosion underwent a synovectomy; proliferative synovium (pannus), containing aggregates of small lymphocytes, was found adherent to eroded cartilage and bone. During 8 days in tissue culture, the synovial cells produced large amounts of collagenase and prostaglandin E2, but only low levels of both neutral and acid proteinases. Sixty-seven percent of the lymphocytes from the synovium were T cells; 19% were B cells. Attempts to identify agent/antigen in the synovial cells were unsuccessful. Thus, the synovium of this pat

The inflammatory microenvironment repurposes BMP-2 signaling to drive pathological osteophyte formation in osteoarthritis.

BACKGROUND: Osteoarthritis (OA) is increasingly recognized as a low-grade inflammatory disease, yet how the inflammatory milieu interacts with regenerative signals remains poorly understood. Bone morphogenetic protein 2 (BMP-2) typically functions as an anabolic factor for cartilage; however, its levels are paradoxically elevated in degenerating joints. This study investigates whether the inflammatory microenvironment acts as a molecular switch that alters the functional outcome of BMP-2 signaling from repair to pathology. METHODS: BMP-2 levels were quantified in human OA cartilage and synovi

Biomechanical optimization of pedicle screw trajectories in osteoporotic lumbar fusion: finite element analysis and validation of robotic-assisted implementation.

Osteoporotic degenerative lumbar disease poses substantial challenges to pedicle screw fixation, with high rates of implant loosening and malposition compromising stability and safety. This study integrated finite element analysis (FEA) for preoperative trajectory optimization with robotic-assisted execution to enhance outcomes in short- and long-segment fusions. A three-dimensional FEA model of the L3-S1 osteoporotic spine was developed from CT data, simulating parallel, 15° medially angulated, and trapezoidal trajectories for two-segment (L4-L5) and three-segment (L3-L5) constructs under phy

Preliminary Study on Grading Diagnosis of Early Knee Osteoarthritis by Shear Wave Elastography.

Источник пометил публикацию как отозванную

Knee osteoarthritis (KOA) is the most common chronic bone joint disease. The WHO points out that KOA has become the fourth most disabling disease in the world, and the main clinical treatment is prevention. At present, the clinical diagnosis of knee osteoarthritis with deformation is mainly made by X-ray and two-dimensional ultrasound, and the preventive treatment effect is not good. Shear wave elastography (SWE) has been widely used in clinical practice for its advantages of noninvasive, simple, rapid, and high accuracy in soft tissue hardness. This study investigates the feasibility of using
