Saphenous vein graft aneurysms: a growing problem? Presentation of two cases and surgical management.

Background Saphenous vein graft aneurysm is a rare complication after coronary artery bypass grafting leading to reoperations. Case Description In two patients with giant aneurysms, the subclavian artery and femoral vein were cannulated. The first patients' operation was combined with aortic valve replacement. The pulsating mass could be removed. The second patients' operation was planned as an off-pump coronary artery bypass grafting. Upon surgical entry, the aneurysm ruptured and the operation was continued as on-pump beating-heart procedure. Conclusion Because of better follow-up and increa

Intestinal obstruction due to dual gastrointestinal atresia in infants: diagnosis and management of 3 cases.

BACKGROUND: Several types of congenital lesions can cause complete or incomplete obstruction of the intestine. Our purpose is to present 3 neonates with dual intestinal type I atresia, i.e., simultaneous obstructive lesions at 2 locations in which the atresia manifested as diaphragm-like tissue. CASE PRESENTATION: All 3 cases were female infants ranging in age from 2 to 14 months. The common symptom in all cases was intermittent persistent vomiting. In some cases the vomitus was bilious, and other symptoms included abdominal distention and delayed meconium passage. Prior surgeries at another

Management of a Peripheral Giant Cell Granuloma in the esthetic area of upper jaw: A case report.

INTRODUCTION: Peripheral Giant Cell Granuloma (PGCG) is the most common giant cell lesion found in the jaws; it originates from the connective tissue of the periosteum or from the periodontal ligament in response to local chronic irritation. Early and accurate diagnosis of these lesions allows for conservative management without compromising adjacent teeth or tissues. PRESENTATION OF CASE: This paper presents a case of PGCG in the esthetic zone of the upper jaw in a 23-year-old female patient. The treatment offered consisted of an excisional biopsy with immediate soft tissue grafting to preve

The use of an intrathecal pump to manage intractable cancer pain in a pediatric patient: a case report.

A 15-year-old girl with combined immune deficiency syndrome, diagnosed with metastatic squamous cell cancer of the anus, had significant pain secondary to vulvar-perianal condyloma. Conventional treatment with oral and intravenous analgesics was limited by significant side effects of mental status changes and urinary retention leading to clinical deterioration that precluded attempts at chemotherapy. An intrathecal pump was implanted in the challenging setting of neutropenia. There was a drastic improvement in her quality of life and the ability to tolerate further chemotherapy. The option of

Aortic stent and renal autotransplantation for the management of renovascular hypertension with Takayasu's arteritis: report of a case.

Takayasu's arteritis (TA) is a rare inflammatory disease affecting the aorta and its major branches. In patients with TA, middle aortic syndrome with aortic and renal artery involvement causes severe hypertension that does not respond well to medical therapy. Currently, the optimal therapeutic options have not been established, and the reported results of different treatments vary widely. We herein present a case of middle aortic syndrome with renovascular hypertension caused by TA in a 12-year-old male treated by an aortic stent and renal autotransplantation as a two-staged procedure.

A case report of obstetrical management of a pregnancy with hypermobile Ehlers-Danlos syndrome and literature review.

We present a case report of a successful pregnancy outcome in a woman diagnosed with Ehlers-Danlos syndrome (EDS) hypermobility type or type III. EDS is a group of connective tissue disorders that has a common genotypic defect, but heterogeneous phenotypic presentations. The variation in EDS manifestations can result in moderate to severe effects on life-expectancy for some types. A number of studies and a review of the literature indicate that generally in pregnant women with EDS, maternal and neonatal outcomes are favourable. However, in EDS type IV, pregnancy can be associated with serious

Management of Unstable Thoraco-Lumbar Fractures with Pedicular Screw Instrumentation: A Series of 30 cases.

INTRODUCTION: Trauma to spine is one of the grave injuries. They cause infinite morbidity and disability to the patient. Lately consensus is evolving across the world for stabilization of unstable spines with fusion and instrumentation even though most authors agree that neurological recovery is independent of treatment modality. Objectives of this study are to evaluate effectiveness of spinal stabilization with pedicular instrumentation in the management of thoraco-lumbar fractures to restore structural stability, early ambulation and improving neurological status of the patient Material and

Management of fibro-osseous lesions of the craniofacial area. Presentation of 19 cases and review of the literature.

INTRODUCTION: Fibro-osseous lesions constitute a rare benign type of pathology with a non-odontogenic lineage that affect the craniofacial area. According to Waldrom's classification, these lesions are divided into: fibrous dysplasia (FD), cemento-ossifying fibroma (COF) and desmoplastic fibroma (DF). MATERIAL AND METHODS: A retrospective study was performed on patients diagnosed with fibro-osseous lesions of the craniofacial area at the Hospital Universitario La Fe, Valencia, during 1987-2009. A total of 19 cases were collected: 15 cases compatible with an FD diagnosis, 3 cases with a COF di

Management of an endodontic infection with an extra oral sinus tract in a single visit: a case report.

Chronic apical periodontitis can drain through a sinus tract, which may be intra-oral or extra-oral, though an intra-oral drainage is more common. Though such cases can be treated in a single visit, there is no report on a single visit endodontic treatment for a chronic apical periodontitis with a draining sinus. This case report demonstrates that the apical periodontitis with a cutaneous sinus tract can be successfully treated endodontically as a single visit procedure, if the patient is in good systemic health.

Management of von Willebrand disease type 3 during pregnancy - 2 cases reports.

Specific phosphorylation/dephosphorylation processes are fundamental for the transduction of external stimuli into physiological responses. A few of these processes appear to be modulated by light in cultured Symbiodinium microadriaticum since the BiP-like protein SmicHSP75 undergoes threonine dephosphorylation upon light stimuli. Several isoforms of the protein are encoded in the S. microadriaticum genome and thus, we identified and heterologously expressed a specific sequence corresponding to the previously identified SmicHSP75 isoform to obtain a highly specific antibody. We then determined

Challenges in managing a mother with a dual diagnosis of peripartum cardiomyopathy and paranoid schizophrenia - a case report.

INTRODUCTION: Psychotic illness has a low incidence in the puerperal period. Peripartum cardiomyopathy as a complication of pregnancy is also rare. CLINICAL CASE: We report a case where the above two conditions occurred simultaneously in a patient and posed significant difficulties in the clinical management. She was diagnosed as having paranoid schizophrenia and peripartum cardiomyopathy. Many of the antipsychotics were contraindicated, and electroconvulsive therapy could not be administered due to the added risks involved with regard to anesthesia. She was therefore managed with clonazepam

HIV case manager preparedness for practice in Ryan White CARE Act funded settings.

While current research on the factors affecting the HIV epidemic within the general population has considered the role of HIV case managers, much remains to be known about case management effectiveness and how it might be enhanced. This article presents the data from a statewide survey of case management professionals in Florida. The study focused on case managers' preparation for practice and barriers to successful practice. The study results reflect a very broad educational preparation in multiple disciplines with highly varied means of case manager training and orientation at entry to pract

Management and prevention of varicella-zoster virus infection in pregnancy: a case report and review of the literature.

Primary infection with varicella-zoster virus (VZV) during pregnancy can lead to devastating outcomes for both the mother and fetus. We describe a case of VZV infection in a pregnant woman who presented at 38 weeks' gestation. We also review the literature regarding management and prevention. Varicella-zoster virus-associated pneumonia in the mother is important to recognize. Outcomes in the newborn largely are dependent on gestational age at the time of infection. Prevention is paramount to management.

Physical therapy management of entrapment of the superficial peroneal nerve in the lower leg: a case report.

This case report describes a 40-year-old male who presented with complaints of pain in the left lower lateral one-third of the leg. Tenderness was elicited 9.7 cm above the lateral malleoli with a positive Tinel's sign at the same site causing radiating pain into the foot (visual analog scale (VAS) score of 6.3 cm). Physical diagnosis for entrapment of the superficial peroneal nerve at the site of the peroneal tunnel was entertained based on clinical examination and three positive provocation tests. Conventionally, treatment for this type of entrapment has been surgical decompression by splitt
