Spontaneous cerebellar strokes. Clinical observations in 60 patients.

This is a retrospective study carried out by clinical analysis of the authors' experience in 60 patients with cerebellar strokes--of which 39 were spontaneous cerebellar hemorrhage and 21 were cerebellar infarction--to compare the clinical presentation, course, and prognosis. Hypertension was found to be the most important etiologic factor. Clinical differentiation of cerebellar hemorrhage from infarction was not possible, but the management was similar in both entities. Computerized tomography was necessary to make diagnosis of the stroke mechanism. In evaluating outcome, the authors conclude

Bilateral medial medullary infarction: magnetic resonance imaging and correlative histopathologic findings.

Bilateral medial medullary infarction is a rare event which clinically presents with flaccid tetraplegia sparing the face, bilateral disturbance of deep sensation, hypoglossal nerve palsy and respiratory failure. We here report a patient with such symptoms in whom magnetic resonance imaging enabled the detection of signal abnormalities in the lower brainstem as soon as 9 h after onset. Results of a control study 3 weeks later correlated well with the extent of infarction that was seen at autopsy. Early lesion detection in the lower medulla by magnetic resonance imaging and the unfavorable prog

Clinical observations in the acute phase of cerebellar hemorrhage and infarction.

Three cases of cerebellar hemorrhage and 5 of cerebellar infarction, diagnosed by brain CT, were examined from the early phase of the onset. All of our cases were mild to moderate in severity and were cured satisfactorily within 3 to 8 weeks of medical treatment. Dizziness and nausea were the commonest symptoms and cerebellar and other CNS signs could be detected only for a short period in some of the cases. Although neuro-otological examinations, especially gaze nystagmus, eye tracking and positional nystagmus tests, were useful for diagnosing the central vestibular lesion, no definite indica

Periventricular leukomalacia: evaluation with MR imaging.

We reviewed 24 magnetic resonance (MR) images of 12 infants diagnosed as having periventricular leukomalacia based on ultrasound results. Abnormalities were identified on the MR images of every patient and were divided into three categories: distribution of low-signal-intensity areas within the cerebral white matter, degree of ventriculomegaly and brain atrophy, and extent of myelination. Follow-up examinations on six patients were also compared for significant changes. Abnormalities noted on MR images that most consistently related to poor outcome for the patient were low-signal-intensity les

Ideomotor apraxia and aphasia: an examination of types and manifestations of apraxic symptoms.

Eighty-eight aphasic patients with the four standard syndromes as well as two control groups, 10 right-sided brain damaged patients and 10 patients without brain damage were examined for ideomotor apraxia by means of 200 tasks. The tasks required oral, arm, leg and bimanual movements, both on verbal command and on imitation. The limb movements were half meaningful, half meaningless. Performances were evaluated according to five response categories: correct, fragmentary, augmentative, perseveratory, other errors. The aim of the investigation was to ascertain whether there are apractic syndromes

Infarctions of the brainstem and cerebellum: a correlation of computed tomography and angiography.

Forty-nine patients were found to have computed tomographic (CT) and clinical evidence for infarction in the brainstem and cerebellum. Seventeen had correlative angiography, of which 15 had a severe occlusive vascular lesion somewhere in the vertebrobasilar system. The correlation between angiographic and CT localization of the infarcts was not good. Inferiorly located infarcts were probably often missed on CT because of basal artifacts. Combined infarctions of the brainstem and cerebellum were common. Nineteen of the 49 patients had an associated mass effect in the posterior fossa; in eight o

Serial computerized tomographic scanning and the prognosis of severe head injury.

Serial (two or three) computerized tomographic (CT) scans were performed on a series of 58 patients with severe head injury. The protocol called for scans to be done upon admission and on Days 3 and 7 after injury. New lesions (i.e., lesions not visualized on the initial CT scans but appearing on subsequent scans) were a frequent finding, occurring in over half of all patients. For the purposes of this study a new lesion was classified as edema involving two or more lobes, extra-axial hematoma, parenchymal hemorrhage, or infarction. Significant correlation was found between good outcome and th

METTL16 Promotes Cerebral Ischemia-Reperfusion Injury via m6A-Dependent Upregulation of TIPARP.

BACKGROUND: Ischemic stroke leads to severe cerebral ischemia/reperfusion (I/R) injury, resulting in neuronal death and neurological deficits. The N6-methyladenosine (m6A) methyltransferase METTL16 has emerged as a key regulator of RNA metabolism, but its specific role and mechanism in ischemic stroke remain unclear. METHODS: A transient middle cerebral artery occlusion (MCAO) model was established in adult male C57BL/6J mice. METTL16 was knocked down via intracerebroventricular injection of shRNA-expressing lentivirus at 6 hours post-reperfusion. Cerebral infarct volume (TTC staining), neuro

Kirenol alleviates cerebral ischemic injury by promoting synaptic plasticity via HDAC2-mediated BDNF expression.

BACKGROUND: Ischemic stroke is an acute cerebrovascular disorder posing various public health challenges; however, the treatment is lacking. Kirenol (Kir), a natural diterpenoid compound with various bioactive properties, demonstrated potential neuroprotective effects on PC12 cells subjected to glutamic acid injury. However, the protective mechanism of Kir remains undefined. OBJECTIVE: This study explored whether Kir could alleviate cerebral ischemic injury by protecting neurons and promoting synaptic plasticity. The molecular mechanisms involved were also elucidated. METHODS: Kir was inject

Propofol attenuates cerebral ischemia-reperfusion-mediated neuronal apoptosis and oxidative damage by regulating the miR-6838-5p/AQP11 axis.

INTRODUCTION: This study aimed to elucidate the mechanism by which propofol (PPF) exerts its effects in cerebral ischemia-reperfusion injury (CI/RI). MATERIAL AND METHODS: A rat model of CI/RI was established via middle cerebral artery occlusion/reperfusion (MCAO/R). MCAO/R rats were pre-treated with PPF (10 mg/kg) via intraperitoneal injection. Additionally, 48 h before PPF administration, miR-6838-5p agomir/antagomir and aquaporin-11 (AQP11) lentiviral overexpression vectors were injected into MCAO/R rats. Infarct size was determined using 2,3,5-triphenyl tetrazolium chloride staining. Neur

Effect of encephalomyosynangiosis procedure on post stroke recovery in a permanent model of ischemic stroke.

Acute ischemic stroke (AIS) remains a leading cause of disability worldwide. Limited treatments for AIS with thrombolytics are further constrained by narrow therapeutic windows and no neuroprotective effects. Damaged or underdeveloped vasculature and poor angiogenesis after AIS remains a major cause of failed neuroprotective therapy after AIS. There is a critical need for alternative approaches that enhance angiogenesis and neuroprotection to improve post-stroke recovery. Here, we use an indirect intracranial bypass technique via encephalomyosynangiosis (EMS), which involves the transposition

Reversible intracranial contrast medium accumulation after embolization of unruptured cerebral aneurysms and its association with transient neurological deficits: A single center experience.

BACKGROUND: Use of iodine-containing contrast medium (CM) is obligatory for endovascular treatment (EVT) of cerebral aneurysms. After EVT, intracranial density increases (DIs) can be detected in cranial computed tomography (CT). Those DI can correspond to subarachnoid hemorrhage (SAH), infarction or reversible CM accumulation (RCMA). The latter can be mistaken for hemorrhage, especially if they are accompanied by neurological deficits. OBJECTIVE: To analyze postinterventional DI after EVT of unruptured cerebral aneurysms and associated clinical symptoms and to identify risk factors for the oc

Effect of Blood Transfusion on Cerebral Hemodynamics and Vascular Topology Described by Computational Fluid Dynamics in Sickle Cell Disease Patients.

The main objective of this study was to demonstrate that computational fluid dynamics (CFD) modeling can be used to study the contribution of covert and overt vascular architecture to the risk for cerebrovascular disease in sickle cell disease (SCD) and to determine the mechanisms of response to therapy such as chronic red blood cell (cRBC) transfusions. We analyzed baseline (screening), pre-randomization and study exit magnetic resonance angiogram (MRA) images from 10 (5 each from the transfusion and observation arms) pediatric sickle SCD participants in the silent cerebral infarct transfusio

Endogenous IFN-β signaling exerts anti-inflammatory actions in experimentally induced focal cerebral ischemia.

BACKGROUND: Interferon (IFN)-β exerts anti-inflammatory effects, coupled to remarkable neurological improvements in multiple sclerosis, a neuroinflammatory condition of the central nervous system. Analogously, it has been hypothesized that IFN-β, by limiting inflammation, decreases neuronal death and promotes functional recovery after stroke. However, the core actions of endogenous IFN-β signaling in stroke are unclear. METHODS: To address this question, we used two clinically relevant models of focal cerebral ischemia, transient and permanent middle cerebral artery occlusion, and two genetic

[Asymptomatic multiple sclerosis - 3 cases (author's transl)].

Multiple Sclerosis (MS) cases found at autopsy in patients who had died from other diseases and in whom no sign or symptom could be related to MS are called "asymptomatic". Three cases are reported. The first patient was a 62 year old man who presented with a slowly progressive disturbance of gait, incontinence and deterioration of intellectual function. A falx meningioma was surgically removed. The patient died 3 years later with an acute respiratory illness. Examination of the brain disclosed evidence of the operation and numerous old plaques disseminated through the cerebral hemispheres (ce
