Default network activity is a sensitive and specific biomarker of memory in multiple sclerosis.

BACKGROUND: Patients with multiple sclerosis (MS) suffer memory impairment but the link between MS-related neuroanatomical changes (brain atrophy) and memory is relatively weak. OBJECTIVE: The purpose of this study was to use functional magnetic resonance imaging (fMRI) to investigate task-induced default network (DN) deactivation as a neurophysiologic biomarker of memory functioning in MS. METHODS: Twenty-eight MS patients underwent high-resolution MRIs to measure brain atrophy (third ventricle width, cerebral gray matter, cerebral white matter, parenchymal fraction, and thalamic, caudate,

Intracranial midline dermoid and epidermoid cysts in children.

OBJECT: Dermoid and epidermoid cysts are rare space-occupying lesions of the central nervous system. Although characterized by a slow growth rate, they are often associated with serious complications. Surgery is the only effective treatment, and radical resection of the entire cyst, whenever possible, generally succeeds in achieving a cure. Authors of large series have only occasionally reported on pediatric cases, and these reports often lack a specific analysis of those cerebral midline lesions. METHODS: The authors report on the treatment of 19 patients (16 with intracranial intradural der

Luciferase as a model for the site of inhaled anesthetic action.

UNLABELLED: The in vivo potencies of anesthetics correlate with their capacity to suppress the reaction of luciferin with luciferase. In addition, luciferin has structural resemblances to etomidate. These observations raise the issues of whether luciferin, itself, might affect anesthetic requirement, and whether luciferase resembles the site of anesthetic action. Because the polar luciferin is unlikely to cross the blood-brain barrier (we found that the olive oil/water partition coefficient was 100 +/- 36 x 10(-7)), we studied these issues in rats by measuring the effect of infusion of lucifer

[Moyamoya disease associated with a false aneurysm--case report and review of the literature].

A 67-year-old male was admitted to our clinic on May 1, 1981, because of mild confusion and a left hemiparesis. About 6 years ago, when he had a left-sided homonymous hemianopia, carotid angiography revealed moyamoya disease. CT scan on the admission showed an intracerebral hematoma in the right temporal lobe, which ruptured to the lateral ventricle. An angiogram demonstrated the net work of moyamoya vessels in the base of the brain and a "newly-appeared" aneurysm at the peripheral portion of the right anterior choroidal artery. This artery was shown on the previous angiogram also but did not

Hypothalamic relapse of a cardiac large B-cell lymphoma presenting with memory loss, confabulation, alexia-agraphia, apathy, hypersomnia, appetite disturbances and diabetes insipidus.

A 37-year-old Hispanic man with a right atrial intracardiac mass diagnosed as diffuse large B-cell lymphoma (DLBCL) was successfully treated with surgery and chemotherapy. During 4 years, several total-body positron emission tomography and MRI scans showed no extracardiac lymphoma. On year 5 after the cardiac surgery, patient presented with sleepiness, hyperphagia, memory loss, confabulation, dementia and diabetes insipidus. Brain MRI showed a single hypothalamic recurrence of the original lymphoma that responded to high-dose methotrexate treatment. Correction of diabetes insipidus improved al

Complications following endoscopic intracranial procedures in children.

BACKGROUND: The significant technological improvement of endoscopic instrumentation has allowed, in the last 10 years, a widespread diffusion of neuroendoscopic procedures. Nevertheless, severe, sometimes life-threatening, complications may occur during neuroendoscopic surgery, and the incidence and age specificity of complications in children have been underdescribed so far. MATERIALS AND METHODS: Complications recorded in a prospectively collected database of pediatric patients undergoing neuroendoscopic procedures were analysed; the medical histories of the patients and the surgical proced

Clinical diagnosis and mutation analysis of four Chinese families with succinic semialdehyde dehydrogenase deficiency.

BACKGROUND: Succinic semialdehyde dehydrogenase (SSADH) deficiency is a rare autosomal recessively-inherited defect of γ-aminobutyric acid (GABA) metabolism. The absence of SSADH, which is encoded by aldehyde dehydrogenase family 5 member A1 (ALDH5A1) gene, leads to the accumulation of GABA and γ-hydroxybutyric acid (GHB). Few cases with SSADH deficiency were reported in China. CASE PRESENTATION: In this study, four Chinese patients were diagnosed with SSADH deficiency in Tianjin Children's Hospital. We conducted a multidimensional analysis with magnetic resonance imaging (MRI) of the head, s

Anatomical determinants of DTI-ALPS: Effects of ROI definition, ventricular morphology, and periventricular deformation.

The diffusion tensor image analysis along the perivascular space (DTI-ALPS) index is increasingly used as a non-invasive MRI biomarker proposed to reflect glymphatic and perivascular function, yet the anatomical validity and measurement stability of the metric remain incompletely characterised. Using diffusion MRI data from 850 healthy young adults and 150 healthy ageing participants from the Human Connectome Project, I systematically evaluated the influence of region-of-interest (ROI) placement and ventricular anatomy on ALPS measurements. Reference ALPS implementations demonstrated substanti

Associations between ventricular boundary shift integral and composite cognitive domains across the alzheimer's disease continuum.

Cognitive impairment is hallmark of Alzheimer’s disease (AD). Although structural MRI has consistently demonstrated widespread brain atrophy in AD, the comparative utility of different imaging sequences for predicting cognitive decline remains uncertain. The Boundary Shift Integral (BSI), a longitudinal measure quantifying brain volume change over time, provides a dynamic alternative to conventional cross-sectional volumetric measures and could more sensitively reflect cognition-related neurodegeneration. This study investigated the associations between BSI-derived atrophy and domain-specific

Predicting ventricular volume reduction and overdrainage in idiopathic normal pressure hydrocephalus after ventriculoperitoneal shunt implantation: a radiological study.

BACKGROUND AND PURPOSE: Radiological evaluation plays a crucial role in assessing the outcomes of ventriculoperitoneal shunt (VPS) surgery in patients with idiopathic normal pressure hydrocephalus (iNPH). Ventricular volume reduction and overdrainage remain critical concerns after shunt implantation. This study aimed to investigate the radiological changes following VPS surgery and identify predictors of ventricular volume change and overdrainage. METHODS AND METHODS: This retrospective study included 75 patients with iNPH who underwent VPS implantation using Medtronic Strata NSC adjustable p

Automated segmentation of ventricular volumes and subarachnoid hemorrhage from computed tomography images: Evaluation of a rule-based pipeline approach.

Changes in ventricular size, related to brain edema and hydrocephalus, as well as the extent of hemorrhage are associated with adverse outcomes in patients with subarachnoid hemorrhage (SAH). Frequently, these are measured manually using consecutive non-contrast computed tomography scans. Here, we developed a rule-based approach which incorporates both intensity and spatial normalization and utilizes user-defined thresholds and anatomical templates to segment both lateral ventricle (LV) and SAH blood volumes automatically from CT images. The algorithmic segmentations were evaluated against two

Lymphoma of the central nervous system originating from the septum pellucidum region: Two case reports with literature review.

RATIONALE: Non-Hodgkin lymphoma affecting the brain, eyes, and cerebrospinal fluid without systemic spread is known as primary central nervous system lymphoma (PCNSL). While intracerebroventricular PCNSL is commonly found in the lateral ventricles and the third and fourth ventricles, the occurrence of PCNSL originating from the septum pellucidum is extremely rare. PATIENT CONCERNS: Two patients presented with recent memory loss and high cranial pressure. DIAGNOSES: Magnetic resonance imaging revealed a clear enhancing lesion in the septum pellucidum region. Pathological examination confirmed

Resection of intra- and paraventricular malignant brain tumors using fluorescein sodium-guided neuroendoscopic transtubular approach.

BACKGROUND: The requirement of brain retraction and difficulty in distinguishing the tumor demarcation are challenging in conventional approaches to intra- and paraventricular malignant tumors (IV-PVMTs). Tubular retractors can minimize the retraction injury, and fluorescein-guided (FG) surgery may promote the resection of tumors. Our aim is to evaluate the feasibility, safety, and effectiveness of fluorescein-guided endoscopic transtubular surgery for the resection of IV-PVMTs. METHODS: Twenty patients with IV-PVMTs underwent FG endoscopic transtubular tumor resection. Fluorescein sodium was

Craniopharyngioma treatment: an updated summary of important clinicopathological concepts.

INTRODUCTION: Craniopharyngiomas (CPs) are benign histological tumors that may develop at different positions along the hypothalamic-pituitary axis. Their close, heterogenous relationship to the hypothalamus makes surgical removal challenging even though this remains the primary treatment strategy. AREAS COVERED: This article presents a critical overview of the pathological and clinical concepts regarding CPs that should be considered when planning treatment. Thus, we have performed a comprehensive review of detailed CP reports published between 1839 and 2020. EXPERT OPINION: CP surgery shou

Intraparenchymal ventricular diverticula in chronic obstructive hydrocephalus: prevalence, imaging features and evolution.

BACKGROUND: Intraparenchymal cavities communicating with the ventricles may appear in patients with chronic obstructive hydrocephalus despite no identifiable surgerical, vascular or traumatic causes. The rate, features, pathogenesis, evolution and clinical impact of intraparenchymal diverticula have not been outlined, yet. METHODS: Brain MRIs of 130 patients (mean age: 11.3 years; age range: 0-67; 60 females) with chronic obstructive hydrocephalus were analyzed. The pathogenesis, neurosurgical treatment, ventricle size, signs of transependymal reabsorption and septum pellucidum integrity of t

Intraventricular Taenia solium cysts presenting with Bruns syndrome and indications for emergent neurosurgery.

Bruns syndrome is an unusual phenomenon, characterized by attacks of sudden and severe headache, vomiting, and vertigo, triggered by abrupt movement of the head. The presumptive cause of Bruns syndrome is a mobile deformable intraventricular mass leading to an episodic obstructive hydrocephalus. Intraventricular tumors have been associated with Bruns syndrome; however, few cases of intraventricular neurocysticercosis have been reported to present with Bruns syndrome. We report the first series of fourth ventricular neurocysticercosis presenting with Bruns syndrome in the United States and revi
