BACKGROUND: Sepsis is a leading cause of neonatal death. Intrapartum azithromycin reduces neonatal nasopharyngeal carriage of potentially pathogenic bacteria, a prerequisite for sepsis. Early antibiotic exposure has been associated with microbiota perturbations with varying effects. This study aims to understand the effect of intrapartum azithromycin intervention on the developing nasopharyngeal microbiota of the child.
METHODS: Using 16S rRNA gene sequencing, we analysed the microbiota of 343 nasopharyngeal samples collected from birth to 12 months from 109 healthy infants selected from a do

Coronavirus disease 2019 (COVID-19) continues to ravage health and economic metrics globally, including progress in maternal and child nutrition. Although there has been focus on rising rates of childhood wasting in the short term, maternal and child undernutrition rates are also likely to increase as a consequence of COVID-19 and its impacts on poverty, coverage of essential interventions, and access to appropriate nutritious foods. Key sectors at particular risk of collapse or reduced efficiency in the wake of COVID-19 include food systems, incomes, and social protection, health care service

BACKGROUND: Disclosure is a difficult but important process for victims of child maltreatment. There is limited research on child maltreatment disclosure. Young people have been reluctant to disclose victimization to adults, but short message service (SMS) crisis services may represent one novel method of engaging young people around sensitive topics.
OBJECTIVE: The purpose of this study was to determine characteristics of child maltreatment disclosure to an SMS-based crisis service.
METHODS: We conducted a content analysis of all conversations (N=244) that resulted in a mandatory report by

BACKGROUND: Formal education can be a nutrition-sensitive intervention that supports the scale-up and impact of nutrition-specific actions. Maternal education has long been linked to child survival, growth, and development while adult earnings and nutrition are tied to years in school as a child. However, less is known about the relationship between maternal education and the micronutrient status of children, women and the general population.
METHODS: Using country-level data and an ecological study design, we explored the global associations between women's educational attainment and: a) ane

BACKGROUND: Portal hypertension in patients with liver cirrhosis causes manifestations such as esophageal varices, ascites and edema. Some studies have been conducted about the role of esophageal varices in the development of esophageal motor disorders and abnormal gastroesophageal reflux in these patients. Ascites could be a factor promoting gastroesophageal reflux and it has been questioned whether reflux would favor the rupture of varices. However there are a few studies using ambulatory esophageal pH recording in the evaluation of these patients.
AIMS: Evaluate gastroesophageal reflux by

OBJECTIVES: Detachment of the septal leaflet of the tricuspid valve from the annulus (TVD) has been used to improve visualization of ventricular septal defects (VSDs), but may be associated with increased operative time, heart block, and the development of tricuspid regurgitation (TR).
METHODS: Patients undergoing VSD closure between 1/1/96 and 31/12/99 were retrospectively reviewed. Follow-up was obtained from the patients' cardiologists.
RESULTS: Transatrial VSD closure was performed in 172 patients with TVD in 36 (21%) at the surgeon's discretion. The leaflet incision was repaired with a

PURPOSE: To evaluate whether adult and pediatric trauma center status, as well as the presence of dedicated child protection teams, influences radiology resident performance in detecting non-accidental trauma on the Emergent/Critical Care Imaging Simulation (WIDI SIM) exam.
MATERIALS AND METHODS: We retrospectively analyzed 639 WIDI SIM exam scores for four pediatric non-accidental trauma cases completed by radiology residents across 33 programs. Residents were stratified by level (R1-R4) and institutional factors, including adult trauma center status, pediatric trauma center status, and chil

IMPORTANCE: Concerns about screen use and early language development are widespread, and prior research has reported mixed findings. Many studies have not distinguished baseline differences from developmental change, leaving uncertainty about whether screen use is associated with actual language development in early childhood.
OBJECTIVE: To examine associations between children's and parental screen use and children's language development from ages 5 to 7 years.

INTRODUCTION: Serotonin (5-hydroxytryptamine [5-HT]) plays an important role in early development, and fetal 5-HT has been reported to arise from placental and maternal sources. Previous human studies have established an association between maternal 5-HT levels and neurodevelopmental outcomes in populations with autism. In this study, we analyze umbilical cord blood and placental 5-HT levels at birth to further investigate the relationship of gestational 5-HT levels with birth outcomes and offspring cognitive development.
METHODS: Participants were enrolled in the Safe Passage Study conducted

Prenatal alcohol exposure (PAE) can affect brain development in early life, but few studies have investigated the effects of PAE on trajectories of white matter tract maturation in young children. Here we used diffusion weighted imaging (DWI) repeated over three time points, to measure the effects of PAE on patterns of white matter microstructural development during the pre-school years. Participants were drawn from the Drakenstein Child Health Study (DCHS), an ongoing birth cohort study conducted in a peri-urban community in the Western Cape, South Africa. A total of 342 scans acquired from 2

INTRODUCTION: Fetal alcohol spectrum disorders (FASD) have an estimated global prevalence of 2-5% of births, but prevalence is reported to be as high as 15.5% for FASD in certain high-risk communities in South Africa. Preclinical studies demonstrate that alcohol consumption during pregnancy interferes with thyroid hormone availability and function and negatively impacts exposed offspring. Very little is currently reported on this phenomenon in humans.
METHODS: This pilot study was embedded in the Drakenstein Child Health Study, a multi-disciplinary longitudinal birth cohort study investigatin

BACKGROUND: Adolescence represents a critical period for the development of overweight that tracks into adulthood. This risk is significantly heightened for adolescents that become pregnant, many of whom experience postpartum weight retention. The aim of this study was to evaluate Balance Adolescent Lifestyle Activities and Nutrition Choices for Energy (BALANCE), a multicomponent obesity prevention intervention targeting postpartum adolescents participating in a national home visiting child development-parent education program.
METHODS: A group randomized, nested cohort design was used with 1

Brain abscess, while rare, confers high mortality, especially in the developing world. The case of a Ugandan child with a polymicrobial brain abscess including infection with Tissierella praeacuta/Clostridium hastiforme requiring repeated drainage and eventual surgical excision is reported. The case demonstrates the importance of considering anaerobic organisms in the treatment of children with brain abscess from the developing world.

Fatal fungal pneumonias developed in two children with ALL during remission induction. Pulmonary and central nervous system signs suggested the L-asparaginase-induced multiple-thromboses syndrome in both children. Fungal infection should be considered-whenever pneumonia develops in a neutropenic child already receiving broad-spectrum antibiotics or when multiple thromboses occur.

BACKGROUND: Despite early identification of deafness through universal newborn hearing screening, deaf and hard of hearing children can still face inequitable challenges in their development and life opportunities. Large scale registries, collecting standardised information on all individuals in a population with the relevant condition, can reveal variations in practices, processes, and outcomes, and identify targets for improvement. For childhood deafness, where care delivery spans multiple service sectors, data linkage is the only feasible way to bring data together. Data linkage also minimi

Maternal and infant mortality remains a major public health concern in sub-Saharan Africa. While male involvement in pregnancy and child birth has proven to be an effective intervention that can significantly reduce these deaths, low-income settings like Cameroon continue to encounter obstacles in engaging men in maternal and child health services. This study seeks to examine and contextualize barriers to male partner involvement in antenatal care in order to inform the development of an intervention aimed at promoting male participation in maternal and child health in Cameroon. We employed a

CONTEXT: Wasting is a key indicator of child survival and well-being. Enhancing practices related to infant and young child feeding (IYCF) is a suggested strategy to mitigate mortality risks and enhance nutritional conditions associated with wasting. Interventions centered on behavioral modifications through IYCF counseling hold promise in increasing the effectiveness of IYCF practices.
OBJECTIVE: In this review we sought to study the impact of intensive IYCF counseling interventions for preventing wasting and nutritional edema among infants and children up to 5 years of age.

BACKGROUND: Childhood malnutrition can impair physical and cognitive development, thereby diminishing productivity at adulthood. Current research recognizes the crucial impact of early childhood nutrition on the long-term health and well-being of children. Ghana has achieved notable progress in lowering childhood malnutrition; nevertheless, it remains a major concern. This study advances current understanding on the socioeconomic drivers and magnitude of child malnutrition in the Ghanaian context.
METHODS: We used data from 2022 Ghana Demographic and Health Survey to analyze inequalities in m
