The role of minimal access surgery in the acute abdomen.

The improvement in surgical decision-making for patients with abdominal pain but an uncertain diagnosis using DL has now been shown to decrease both negative and nontherapeutic laparotomy rates. Once the diagnosis is established, DL can be taken a step further in many cases, as therapeutic intervention via laparoscopy is possible for a number of these conditions without resorting to a laparotomy. Conditions amenable to therapeutic laparoscopy include appendicitis, perforated peptic ulcer, diverticulitis, small bowel obstruction, acute cholecystitis, diaphragmatic rupture, and splenic or hepati

[Videolaparoscopic cholecystectomy. Experience, evolution, and comparison of techniques].

The paper broadly outlines the technique of laparoscopic cholecystectomy used by the authors to treat patients suffering from calculosis of the gallbladder. Basing their comments on the French school, the authors review the literature to identify those innovations introduced by individual operators in elation to the original technique which have enabled this method to be extended to those cases complicated by acute cholecystitis, VBP calculosis or sequelae from abdominal surgery. The most important innovations include the use of a laparoscope with oblique 30-degrees vision, which is extremely

Marked clinical difference between two sibs affected with juvenile metachromatic leukodystrophy.

In a child with enzymatically and histopathologically proven metachromatic leukodystrophy (MLD), the disease pursued a course typical of juvenile MLD characterized by neurological degeneration beginning at age 9 years and ending in death at age 18. A younger brother of the patient was found to have profound deficiency of arylsulfatase A in leukocytes and to excrete five- to 20-fold greater-than-normal amounts of sulfatide in the urine. He was completely free of symptoms attributable to MLD until age 16 when he developed acute cholecystitis caused by sulfatide accumulation in the gallbladder. R

Percutaneous drainage of subphrenic abscess: a review of 62 patients.

Sixty-two patients who presented with subphrenic abscesses had the abscesses drained percutaneously. Diagnosis of a subphrenic collection was usually made with sonography. Initial percutaneous drainage was accomplished with a combination of sonography and fluoroscopic guidance. More recently (in the last 31 cases) most drainages were done with sonographic guidance alone. Successful catheter drainage was achieved in 85% with a complication rate of 4.8%. Failure of catheter drainage (defined as any patient who was not cured by percutaneous drainage alone) occurred in patients with multiple colle

Campylobacter Enteritis.

Campylobacter jejuni (previously called "related vibrio") has recently become recognized as an important cause of acute diarrhoeal disease in many countries. As with other intestinal pathogens, the clinical picture of C. jejuni infection varies from symptomless excretion to severe disease. The incubation period averages two to five days. Fever, abdominal pain and bloody diarrhoea are the usual symptoms of campylobacter enteritis. Although it is normally a self-limiting disease, complications such as cholecystitis, peritonitis, septicaemia and meningitis occasionally arise. The small intestine

Laparoscopic cholecystectomy for gallbladder disease in patients with severe cardiovascular disease.

BACKGROUND: Cardiovascular disease (CVD) and gallstones are reported to be strongly associated because both diseases are frequently part of a metabolic syndrome. Laparoscopic cholecystectomy (LC) has become the standard treatment for gallbladder disease around the world. Cardiovascular disease is considered as an absolute or relative contraindication for LC; however, clinical information regarding the applicability of LC for treating gallbladder disease in CVD patients is lacking. This study aims to assess the suitability of LC for the treatment of gallbladder disease in CVD patients. METHODS

Cholecystectomy vs Endoscopic Retrograde Cholangiopancreatography or No Intervention After Gallstone-Related Acute Pancreatitis.

IMPORTANCE: Same-admission cholecystectomy is recommended by guidelines for mild gallstone-related acute pancreatitis, yet surgery is often deferred. Endoscopic retrograde cholangiopancreatography (ERCP) is often used as an interim strategy, but its effectiveness compared with cholecystectomy is uncertain. OBJECTIVE: To compare cholecystectomy, ERCP only, and no intervention in association with recurrent acute pancreatitis and other gallstone-related complications, accounting for death as a competing event. DESIGN, SETTING, AND PARTICIPANTS: This population-based cohort study used nationwide

AI Teaches Surgical Diagnostic Reasoning to Medical Students: Evidence from an Experiment Using a Fully Automated, Low-Cost Feedback System.

OBJECTIVE: While AI-generated feedback has shown promise in medical education, prior studies have only used AI for feedback, with question design handled by human experts, and the process required human involvement. This study aimed to evaluate the effectiveness of a fully automated AI-based system that generates both multiple-choice questions (MCQs) and personalized feedback, without any human input, on improving diagnostic reasoning in preclinical medical students. DESIGN: A prospective, parallel-group, interventional study. The intervention group (Year-1 students) received AI-generated MCQ

Improving outcomes in emergency general surgery: Construct of a collaborative quality initiative.

BACKGROUND: Emergency general surgery conditions are common, costly, and highly morbid. The proportion of excess morbidity due to variation in health systems and processes of care is poorly understood. We constructed a collaborative quality initiative for emergency general surgery to investigate the emergency general surgery care provided and guide process improvements. METHODS: We collected data at 10 hospitals from July 2019 to December 2022. Five cohorts were defined: acute appendicitis, acute gallbladder disease, small bowel obstruction, emergency laparotomy, and overall aggregate. Proces

Enterococcus thailandicus: Genomic evidence of a potential new player in enterococcal virulence and antimicrobial resistance.

BACKGROUND: Enterococcus thailandicus has been isolated from animals, fermented foods, wastewater, and humans, but its clinical relevance remains unclear. We investigated the pathogenic potential of E. thailandicus by phenotypic and genomic characterization of two human isolates from acute abdominal infections, complemented by a species-wide comparative genomic analysis of all publicly available E. thailandicus genomes. METHODS: Two E. thailandicus isolates were recovered from patients with acute cholecystitis and small bowel perforation. Species identification was performed by MALDI-TOF MS a

Erector Spinae Plane Block as an Analgesic Strategy for Hepatopancreaticobiliary Pain: A Systematic Review.

BACKGROUND: Hepatopancreaticobiliary (HPB) pain is a common cause of emergency department (ED) and intensive care unit (ICU) presentation and is frequently managed with opioid analgesics. In the setting of the ongoing opioid crisis, effective non-opioid analgesic strategies are urgently needed. The erector spinae plane block (ESPB) has emerged as a promising regional anesthesia technique. OBJECTIVES: To systematically review and synthesize existing evidence on the feasibility, effectiveness, and safety of ESPB for HPB-related pain in acute care settings. METHODS: A systematic review was cond

Dengue fever uncovers a late diagnosis of sickle cell disease- a case report.

BACKGROUND: Sickle cell disease (SCD) is a genetically inherited hemoglobinopathy characterized by chronic hemolysis, vaso-occlusive episodes, and end-organ complications. Individuals with SCD are more vulnerable to infections, and co-infections such as dengue fever can lead to severe clinical manifestations. Late diagnosis of SCD in adulthood is rare but possible, particularly in individuals with a milder phenotype or elevated fetal hemoglobin (HbF). CASE PRESENTATION: We present the case of a 28-year-old male with no known chronic illnesses who was admitted with fever, abdominal pain, and m

Intractable Postoperative Wounds Caused by Self-Inflicted Trauma in a Patient with Cutaneous Munchausen Syndrome Presenting as a Pyoderma Gangrenosum-Like Lesion.

A 50-year-old Japanese woman consulted the emergency department of our hospital for bleeding due to an intractable postoperative wound on the lower abdomen; the postoperative wound was owing to a laparoscopic cholecystectomy performed 1 year previously for acute cholecystitis. She presented with a painful ulcer on her right lower abdomen. She also presented with multiple scars, skin grafts on the extremities, and a missing left lower leg, the causes for all of which were unexplained. The results of her blood test were normal, except for the hemoglobin level. Histology of the skin biopsy specim

Hepcidin is an antibacterial, stress-inducible peptide of the biliary system.

BACKGROUND/AIMS: Hepcidin (gene name HAMP), an IL-6-inducible acute phase peptide with antimicrobial properties, is the key negative regulator of iron metabolism. Liver is the primary source of HAMP synthesis, but it is also produced by other tissues such as kidney or heart and is found in body fluids such as urine or cerebrospinal fluid. While the role of hepcidin in biliary system is unknown, a recent study demonstrated that conditional gp130-knockout mice display diminished hepcidin levels and increased rate of biliary infections. METHODS: Expression and localization of HAMP in biliary sys
