Impact of scaling and root planing on salivary and serum plasminogen activator inhibitor-1 expression in patients with periodontitis with and without type 2 diabetes mellitus.

BACKGROUND: Plasminogen activator inhibitor-1 (PAI-1) is significantly enhanced in insulin resistance and inflammation and ascribed as a proinflammatory marker. This study aimed to compare and correlate salivary and serum PAI-1 and alpha 2-macroglobulin (α2MG) in patients with periodontitis with and without type 2 diabetes mellitus (T2DM) and also appraise the consequence of periodontal treatment on these biomarkers. METHODS: Sixty subjects enlisted were split into two groups; Group 1 consisted of 30 systemically healthy subjects with Stage II and III, generalized, Grade B, C periodontitis wh

Epigenetic gene regulation by histone demethylases: emerging role in oncogenesis and inflammation.

Histone N-terminal tails of nucleosomes are the sites of complex regulation of gene expression through post-translational modifications. Among these modifications, histone methylation had long been associated with permanent gene inactivation until the discovery of Lys-specific demethylase (LSD1), which is responsible for dynamic gene regulation. There are more than 30 members of the Lys demethylase (KDM) family, and with exception of LSD1 and LSD2, all other KDMs possess the Jumonji C (JmjC) domain exhibiting demethylase activity and require unique cofactors, for example, Fe(II) and α-ketoglut

[Periodontitis and systemic diseases: from science to clinical practice].

The evidence for an association between systemic diseases and periodontitis is strongest with diabetes mellitus type 2 and cardiovascular disease. There is a moderate association of periodontitis with adverse pregnancy outcomes and rheumatoid arthritis. Periodontal treatment has, on average, a positive effect on reducing systemic infection and improving the condition of the vascular system. For diabetes patients, periodontal treatment can also have a positive effect on metabolic regulation. There is insufficient evidence that periodontal treatment prevents adverse pregnancy outcomes and rheuma

Punica granatum peel extract as adjunct irrigation to nonsurgical treatment of chronic gingivitis.

Pomegranate is one of the most universally studied medicinal plants for its ethnomedical history, with several studies presenting the positive outcome of its use or its extracts in managing inflammation. The objective of the present trial was to investigate the efficiency of the traditionally used 5% of pomegranate peel extract in treating gingival inflammation. Herein, 34 chronic gingivitis patients were randomized in a 1:1 ratio for four weeks in a controlled, double-blind clinical trial to evaluate the effect of the adjunctive use of a pulsating jet irrigator containing 5% pomegranate peel

Arthritis and Periodontitis: An Association Debated for Over Two Centuries.

The chronic diseases, rheumatoid arthritis (RA) and periodontal disease (PD) lead to confined destruction of soft and hard tissues as a result of inflammatory processes. Their pathogenesis is dictated by a network of inflammatory cells and its mediators. They also share some etiological risk factors and therapeutic alternatives. The evolution of focal infection theory is summarized in this review, with special reference to PD and its relationship to RA. Foci of chronic infections exist in the oral cavity and may result in anatomically distant disease in certain individuals. Recent cumulative e

Oral complications of cancer therapy.

The mouth is a frequent site of complications arising from drug or radiation cancer therapy, with mucositis, xerostomia, osteoradionecrosis, and local infections being the most common. From the stand-point of dose limitation, treatment breaks, quality of life, and health economic outcomes, mucositis is the most significant acute oral toxicity. Xerostomia, a chronic side effect of radiation, involves the salivary gland tissue, and results in changes in taste, tissue resilience, and an increased risk of caries and periodontal disease. While the incidence of osteoradionecrosis seems to be decreas

[Thoracic actinomycosis. Presentation of 3 cases].

We have had the chance to follow three patients with thoracic actinomycosis in a relatively short period of time (18 months). They were three male patients, with chronic dental infections. The illness appeared as toxic syndrome and fever. In all three cases diagnosis was made through samples obtained through fibropticbronchoscopy. At the same time all of them evolved favorably to the antibiotic treatment (penicillin and/or tetracyclines) prescribed. We conclude that the diagnosis of thoracic actinomycosis should be suspected in those patients with a compatible clinical and radiological conditi

Reassessing Chronic Lyme Disease and Post-Treatment Lyme Disease Syndrome as Focal Infections.

The complete clinical spectrum of Lyme borreliosis has been recognized for nearly 50 years, yet its diagnosis remains challenging due to the heterogeneity of symptoms. While many symptoms are likely nonspecific, a recurring cluster, including severe fatigue, brain fog, cognitive decline, memory impairment, joint and muscle pain, limb numbness, headaches, and low-grade fever, is often labeled in the scientific literature as post-treatment Lyme disease syndrome (PTLDS), and in the popular media as "chronic Lyme disease." Based on clinical experience and retrospective case analysis, this study hy

Association of periodontitis and tooth loss with cognitive decline in the older adults - a systematic review.

BACKGROUND: The increasing global burden of dementia highlights the importance of identifying factors that may contribute to cognitive decline in later life. Growing evidence suggests that chronic oral conditions, particularly periodontitis (PD) and tooth loss, may be associated with Alzheimer's disease (AD) and related dementias. This study synthesizes current observational evidence on the association between PD, tooth loss, and cognitive impairment (CI) among older adults. METHODS: A comprehensive literature search was conducted in PubMed, the Cochrane Library, Embase, Scopus, and Google Sc

Altered gene expression profile in human gingival keratinocytes treated with Aggregatibacter actinomycetemcomitans and their role in the development of head and neck squamous cell carcinoma.

BACKGROUND: Periodontal diseases have been associated with several systemic conditions, including certain types of cancer. Chronic inflammation, a characteristic feature of periodontal disease, has been hypothesized to contribute to carcinogenesis and tumor progression. However, the specific relationship between Aggregatibacter actinomycetemcomitans and cancer development remains an area of ongoing research. OBJECTIVES: The aim of the study was to investigate the potential association between A. actinomycetemcomitans, a well-established periodontal pathogen, and the development of head and ne

Cemental Tear: A Multicenter Retrospective Case Series.

Cemental tear (CeT) is an often-overlooked condition that may mimic endodontic and periodontal diseases, leading to misdiagnosis and inappropriate management. This multicenter study reports a series of CeT cases characterized using an integrated clinical, cone-beam computed tomography (CBCT)-based imaging, and tissue-based analytical approach. Twenty-nine teeth from 21 patients were diagnosed with CeT based on clinical and CBCT findings. Fragment location, root third distribution, and extent of the root surface and periradicular bone involvement were assessed. Extracted teeth and surgically re
