Efficacy in standard clinical practice of colonoscopic polypectomy in reducing colorectal cancer incidence.

BACKGROUND: Colorectal cancer is one of the leading causes of death from cancer in Western countries. Removal of adenomas is based on the assumption that it could lead to a reduction in the incidence of colorectal cancer, as demonstrated by the National Polyp Study in the USA. A critical issue is whether the benefit observed in clinical trials can also be observed in standard clinical practice. To address the issue, a multicentre Italian collaborative study was organised. METHODS: The study cohort comprised 1693 subjects of both sexes, aged 40-69 years, enrolled between 1980 and 1987 followin

Diverse image generation with diffusion models and cross class label learning for polyp classification.

Pathologic diagnosis is a critical phase in deciding the optimal treatment procedure for dealing with colorectal cancer (CRC). Colonic polyps, precursors to CRC, can pathologically be classified into two major types: adenomatous (malignant potential) and hyperplastic (benign). Various imaging techniques, such as narrow band imaging (NBI) and white light imaging (WLI), are adopted in capturing polyp-specific features for accurate classification and have different advantages. However, the existing classification techniques mainly rely on a single imaging modality and show limited performance due

SparCC co-occurrence networking reveals intracommunity dynamics of the microbiome following colorectal surgery.

The intestinal microbiota plays a critical role in post-surgical wound healing following bowel resection; however, perioperative, prophylactic antibiotic administration may deleteriously affect it. We previously used 16S rRNA amplicon sequencing of stool samples to assess perioperative and longitudinal changes in the microbiome through 6 months in patients undergoing (i) colonoscopy after mechanical bowel prep (MBP) alone, (ii) non-resectional colorectal surgery after MBP with oral antibiotics and prophylactic intravenous antibiotics no longer than 24 h post-operative (surgical bowel prep [SBP

Transmural healing is significantly associated with prevention of clinical relapse in Crohn's disease.

SUMMARY: This multicenter, retrospective study assessed whether endoscopic, histologic, or radiologic remission best predicts outcomes in ileal Crohn's disease (CD). Seventy-one patients who underwent paired colonoscopy with ileal biopsies and magnetic resonance enterography (MRE) were followed for 18 months. While remission by all modalities was associated with reduced relapse on univariate analysis, multivariable analysis showed that only radiologic remission (transmural healing) independently predicted reduced clinical relapse and longer relapse-free survival. Endoscopic and histologic remi

Artificial Intelligence Model for Detection of Colorectal Cancer on Routine Abdominopelvic CT Examinations: A Training and External-Testing Study.

BACKGROUND. Radiologists are prone to missing some colorectal cancers (CRCs) on routine abdominopelvic CT examinations that are in fact detectable on the images. OBJECTIVE. The purpose of this study was to develop an artificial intelligence (AI) model to detect CRC on routine abdominopelvic CT examinations performed without bowel preparation. METHODS. This retrospective study included 3945 patients (2275 men, 1670 women; mean age, 62 years): a training set of 2662 patients from Severance Hospital with CRC who underwent routine contrast-enhanced abdominopelvic CT before treatment between Januar

Patients Prescribed Direct-Acting Oral Anticoagulants Have Low Risk of Postpolypectomy Complications.

BACKGROUND & AIMS: Use of direct-acting oral anticoagulants (DOACs) is increasing, but little is known about the associated risks in patients undergoing colonoscopy with polypectomy. We aimed to determine the risk of post-polypectomy complications in patients prescribed DOACs. METHODS: We performed a retrospective analysis using Optum's de-identified Clinformatics Data Mart Database (2003-2016) (a de-identified administrative database from a large national insurance provider) to identify adults who underwent colonoscopy with polypectomy or endoscopic mucosal resection (EMR) from January 1, 20

Colorectal cancer screening for average-risk adults: 2018 guideline update from the American Cancer Society.

In the United States, colorectal cancer (CRC) is the fourth most common cancer diagnosed among adults and the second leading cause of death from cancer. For this guideline update, the American Cancer Society (ACS) used an existing systematic evidence review of the CRC screening literature and microsimulation modeling analyses, including a new evaluation of the age to begin screening by race and sex and additional modeling that incorporates changes in US CRC incidence. Screening with any one of multiple options is associated with a significant reduction in CRC incidence through the detection an

Analysis of DNA Methylation at Specific Loci in Stool Samples Detects Colorectal Cancer and High-Grade Dysplasia in Patients With Inflammatory Bowel Disease.

BACKGROUND & AIMS: Patients with inflammatory bowel diseases (IBDs), including ulcerative colitis and Crohn's disease, are at increased risk for colorectal cancer (CRC). Analyses of DNA methylation patterns in stool samples have been reported to detect CRC in patients with IBD. We sought to validate these findings in larger cohorts and assess the accuracy of analysis of DNA methylation patterns in stool for detection of CRC and high-grade dysplasia (HGD) normalized to methylation level at ZDHHC1. METHODS: We obtained buffered, frozen stool samples from a US case-control study and from 2 Europ

Cost Analysis of Endoscopic Mucosal Resection vs Surgery for Large Laterally Spreading Colorectal Lesions.

BACKGROUND & AIMS: Large laterally spreading lesions (LSL) in the colon and rectum can be safely and effectively removed by endoscopic mucosal resection (EMR). However, many patients still undergo surgery. Endoscopic treatment may be more cost effective. We compared the costs of endoscopic versus surgical management of large LSL. METHODS: We performed a prospective, observational, multicenter study of consecutive patients referred to 1 of 7 academic hospitals in Australia for the management of large LSL (≥ 20 mm) from January 2010 to December 2013. We collected data on numbers of patients und

Attitudes and beliefs regarding colorectal cancer screening among African American patients of community health centers in North-Central Florida and stakeholder perspectives: a qualitative study.

BACKGROUND: Colorectal cancer (CRC) is one of the leading causes of cancer mortality in the US with African Americans continuing to experience CRC health disparities due to numerous factors. While colonoscopy remains the most utilized test for CRC screening, stool-blood tests are cost-effective and offered by community health centers (CHC) serving un- and underinsured patients. Qualitative research has identified multilevel barriers to CRC screening and can potentially inform the development of tailored interventions to promote stool-based screening. METHODS: In this qualitative study, we exp

Case Report: Endoscopic resection of giant colonic pedunculated polyps in patients with AIDS using a novel nylon rope and titanium clip technique.

The three patients with AIDS were all male, aged between 56 and 67 years. One also had liver cirrhosis. They presented to the Chengdu Public Health Clinical Medical Center with a history of several months of blood in the stool. Colonoscopy revealed large pedunculated polyps in the sigmoid colon, with the largest measuring 5 cm and the smallest 3.5 cm. The polyps nearly completely obstructed the lumen. The long, thick stems were freely mobile within the narrow lumen, making it difficult to capture sufficient tissue with the inner ring of the forceps. The entire colon was examined with the aid o

Case Report: Recurrent colonic metastasis from lung cancer-diagnostic pitfalls and therapeutic challenge of a peculiar case.

Lung cancer (LC) mortality exceeds 20%, and detecting metastases from LC is becoming a challenging step in understanding the real prognostic role of specific localization. We report a case of a patient with lung metastasis to the colon with local recurrence at the anastomosis after radical resection for metastasis. In both cases, the diagnosis was on oncological follow-up, and surgery was offered in consideration of reasonable life expectancy, good control of LC, and high risk of intestinal occlusion. A 67-year-old male, with a history of LC 18 months ago, was referred to our surgical unit aft

Facilitators and barriers to colorectal cancer screening in an outpatient setting.

BACKGROUND: Colorectal cancer (CRC) is the third most common cancer and the second leading cause of cancer-related deaths in the United States. Still, 1 in 3 adults aged 50 years to 75 years have not been screened for CRC. Early detection and management of precancerous or malignant lesions has been shown to improve overall mortality. AIM: To determine the most significant facilitators and barriers to CRC screening in an outpatient clinic in rural North Carolina. The results of this study can then be used for quality improvement to increase the rate of patients ages 50 to 75 who are up to date

Estimating the preferences and willingness-to-pay for colorectal cancer screening: an opportunity to incorporate the perspective of population at risk into policy development in Thailand.

AIMS: Colorectal cancer (CRC) is one of the public health burdens that can be lowered by early detection. This study aims to examine the preferences and willingness-to-pay of a population at risk for CRC screening in Thailand. Understanding the preferences for these individuals at risk would help Thailand, as an example of LMICs, to design effective population-based CRC screening programs. MATERIALS AND METHODS: A discrete choice experiment (DCE) was conducted among screening-naïve adults aged 50-75 years, who were at risk of CRC, in the out-patient department of a tertiary care hospital in T

Diphyllobothrium latum Mimicking Subacute Appendicitis.

Diphyllobothrium latum (D. latum) infection in humans is uncommon in the United States. Although there has been a drastic decline in the report of D. latum infection in this region, physicians should be aware of an uncommon presentation and its clinical relevance. We report a case of 55-year-old female of Ecuadorian/Peruvian origin who presented with an unknown cause of chronic right lower quadrant abdominal pain for 2 months without other particular symptoms. Initial workup revealed evidence of iron deficiency anemia, and stool occult blood test was positive. She was scheduled for a colonosco
