The CCAT2 enigma: pioneering insights into colorectal cancer pathophysiology and therapeutic innovation.

Colorectal cancer (CRC) remains a major global health burden and a leading cause of cancer-related morbidity and mortality. It ranks as the third most commonly diagnosed malignancy and the second leading cause of cancer-related deaths worldwide. These statistics underscore the urgent need for improved diagnostic, prognostic, and therapeutic strategies to combat the disease. The development of CRC is driven by a combination of genetic predisposition, environmental exposures, and lifestyle-related factors. Major risk factors include a family history of CRC, unhealthy diet, smoking, excessive alc

Decoding colorectal cancer lung metastasis: a global research odyssey.

BACKGROUND: Colorectal cancer (CRC) remains a major global health burden, particularly when complicated by pulmonary metastasis (PM), which significantly worsens prognosis and reduces survival despite active treatment. The absence of standardized therapeutic strategies for CRC with PM has driven research into its underlying biological mechanisms, including epithelial-mesenchymal transition, immune evasion, and tumor microenvironment dynamics. METHODS: A bibliometric analysis was conducted on 2,645 publications related to CRC with PM retrieved from the Web of Science Core Collection (1991-2025

DMMR status and synchronous lesions predicts metachronous lesions after curative resection for rectal cancer.

BACKGROUND: There are no established standard colonoscopy surveillance protocols for patients after curative rectal cancer resection. We investigated the predictive factors for colorectal neoplasms during surveillance colonoscopies to identify patients who are at risk of developing metachronous neoplasms in the residual colorectum. METHODS: This was a single-center, retrospective study that included patients with diagnosis of rectal carcinoma who had undergone curative resection from October 2012 to June 2018. Clinicopathological variables were analyzed by logistic regression analysis to iden

Stage-by-stage comparison of clinical and oncologic outcomes in ulcerative colitis-associated versus sporadic colorectal cancer: A propensity-matched analysis.

INTRODUCTION: Patients with ulcerative colitis (UC) have an increased risk of developing colorectal cancer (CRC), which differs in carcinogenesis from sporadic CRC (S-CRC). This study aimed to compare the clinical features and oncologic outcomes between UCCRC and S-CRC groups. PATIENTS AND METHODS: A retrospective cohort of patients who underwent surgery at Asan Medical Center, Seoul, Korea, between January 2010 to December 2020 comprised 46 patients with UCCRC and 9717 patients with S-CRC. Before propensity score matching (PSM), several variables significantly differed between the 2 groups.

Nomogram to predict the prognostic value of tumor deposits for patients with papillary thyroid carcinoma.

BACKGROUND: Tumor deposits (TDs), nodules in the peritumoral adipose tissue with no architectural residue of lymph node, have previously been described in colorectal adenocarcinomas with poor prognosis. However, the significance of TD has not been fully investigated in patients with papillary thyroid carcinoma (PTC). METHOD: We retrospectively enrolled 541 patients undergoing surgery between 2015 and 2021. The patients were classified into two groups according to TD status (TD vs non-TD), and the clinicopathologic characteristics and disease-free survival (DFS) were compared. Associations of

Cancer risks for MSH6 pathogenic variant carriers.

INTRODUCTION: Lynch syndrome (LS) is a hereditary cancer syndrome caused by pathogenic variants (PVs) in DNA mismatch repair genes, including MSH6. Although MSH6-associated LS (MSH6-LS) is known to increase the risk of several cancers, precise risk estimates-particularly for non-colorectal cancers-remain uncertain. This limits personalised clinical guidance for MSH6-LS. This study aims to refine cancer risk estimates for individuals with pathogenic or likely pathogenic MSH6 variants. METHODS: A retrospective cohort study was conducted using data from 360 Dutch families, comprising 1117 MSH6 P

Longitudinal Outcomes of Abdominoperineal Resection Reconstruction: A Single-center Retrospective Review.

BACKGROUND: Reconstruction after abdominoperineal resection (APR) typically utilizes primary closure, locoregional myocutaneous flaps (gracilis or vertical rectus myocutaneous [VRAM]), or omental flaps. Although flap coverage is considered superior to primary closure, no specific flap is preferred, and reconstructive complications can occur in 20%-50% patients. The purpose of this study was to compare outcomes of perineal reconstruction with VRAM, gracilis, and omental flaps. METHODS: A single-institution retrospective cohort review was performed on all adult patients who underwent APR defect

Comparison of Running Versus Interrupted Sutures for Fascial Closure in Ileostomy Takedown.

Temporary loop ileostomies are common after colorectal cancer surgery to reduce anastomotic leak severity. However, ileostomy takedown carries risks like surgical site infections (SSIs) and incisional hernias. The optimal fascial closure technique during takedown remains debated. This study compared these techniques regarding postoperative complications. This retrospective study analyzed data from 69 patients undergoing scheduled loop ileostomy closure between 2016-2020 at two Romanian surgical departments. Data collected included demographics, clinical variables (ASA score, comorbidities), su

Prophylactic Mesh in Parastomal Hernia Prevention: Current Evidence.

INTRODUCTION: Parastomal hernia (PSH) is a common long-term complication following stoma creation. The incidence of PSH exceeds 50% in long-term follow-up of end colostomy patients, while it remains lower in ileostomies and ileal conduit urinary diversions. PSH prevention strategies are of interest due to the poor outcomes and high recurrence rates associated with PSH repair. OVERVIEW OF TECHNIQUES TO PREVENT PSH: Various technical approaches have been explored to reduce the risk of PSH. However, none have shown consistent benefit toward reducing PSH rate without the use of prophylactic mesh.

Peritoneal Cancer Index Dominates Prognosis After CRS-HIPEC for Colorectal Peritoneal Metastases: A Consecutive Single-Centre Cohort with 3-Year Follow-Up.

BACKGROUND: Cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) can cure selected patients with colorectal peritoneal metastases (CPM). Real-world prognostic data, especially for the Peritoneal Cancer Index (PCI) and completeness of cytoreduction (CCR), are limited. METHODS: We retrospectively analysed 75 consecutive patients treated with CRS + HIPEC at a tertiary centre (2014-2022), giving ≥36 months potential follow-up. Overall survival (OS) was assessed by Kaplan-Meier and Cox models. PCI was grouped 0-10, 11-20, >20; CCR was dichotomised (CCR-0 vs. CCR 1/2).

Evaluating transanal irrigation for alleviating symptoms of low anterior resection syndrome post rectal cancer surgery: A meta-analysis.

BACKGROUND: Colorectal cancer is the third most commonly diagnosed cancer and the second leading cause of cancer-related death worldwide. Despite medical advances, approximately 80% of all rectal cancer survivors develop low anterior resection syndrome. Although transanal irrigation holds promise against low anterior resection syndrome, existing evidence is inconclusive. In this study, we evaluated the efficacy and safety of transanal irrigation in managing low anterior resection syndrome. METHODS: Cochrane Library, PubMed, Embase, Chinese Electronic Periodical Services, and ClinicalTrials.go

Functional Outcomes After Rectal ESD: A Retrospective Evaluation of Low Anterior Resection Syndrome.

OBJECTIVE: Endoscopic submucosal dissection (ESD) is increasingly preferred as an alternative to surgery and as an organ-preserving method for the treatment of rectal lesions. However, its impact on bowel function-particularly the development of low anterior resection syndrome (LARS)-remains poorly understood. This study aimed to evaluate the incidence of LARS following rectal ESD and to identify associated clinical and pathological factors. METHODS: Data from 118 patients who underwent rectal ESD between January 2018 and December 2024 were retrospectively analyzed. Demographic characteristic

Comparing Open, Laparoscopic and Robotic Liver Resection for Metastatic Colorectal Cancer-A Systematic Review and Network Meta-Analysis.

Colorectal liver metastases (CRLM) can be surgically managed through open resections (OLR), laparoscopic resections (LLR), or robotic liver resections (RLR). However, there is ongoing uncertainty regarding the safety and effectiveness of minimally invasive approaches like LLR and RLR. This study aims to clarify these issues by conducting a network meta-analysis (NMA) to compare outcomes across OLR, LLR and RLR for patients with CRLM. Following the PRISMA-NMA guidelines, the meta-analysis included 13 studies with a combined total of 6582 patients. Of these, 50.6% underwent LLR, 45.3% underwent

Cytoreductive surgery and hyperthermic intraperitoneal chemotherapy in New Zealand: peri-operative outcomes and service development over a decade.

BACKGROUND: Cytoreductive surgery (CRS) and hyperthermic intraperitoneal chemotherapy (HIPEC) is the standard of care for selected cases of peritoneal surface malignancy. However, due to its morbidity and learning curve, it is only delivered in six centres in Australia and Aotearoa New Zealand (AoNZ). In this study, we report peri-operative morbidity and mortality following CRS/HIPEC at Waikato and Braemar Hospitals, which have treated patients from all regions of AoNZ since 2008. METHODS: We retrospectively reviewed a database of all patients undergoing CRS and HIPEC from 01/01/2008 to 01/11

Double hepatic vein reconstruction during extended anatomical resection of segment 8 for colorectal liver metastasis.

BACKGROUND: Hepatic vein reconstruction (HVR) is occasionally necessary for resecting hepatic malignancies to ensure surgical margins while preserving remnant liver function [1]. Reports of multiple HVR are rare due to the highly technical demanding procedure and high risk of morbidity [2]. We introduce our procedure of double HVR for metastatic liver tumors invading the right hepatic vein (RHV) and middle hepatic vein (MHV). METHODS: The patient was a 66-year-old man with colorectal liver metastasis in segment 8, invading RHV and MHV. Due to impaired liver function, extended right hemihepate

Injection sclerotherapy for the treatment of haemorrhoids in anticoagulated patients.

BACKGROUND: Symptomatic haemorrhoids (SH) are a common condition; however, conventional outpatient treatment, including rubber band ligation, is contraindicated in patients receiving concurrent anticoagulation. Injection sclerotherapy (IST) has been proposed as a treatment option for these patients. METHODS: A retrospective review of case notes was performed in a colorectal surgery department that sits alongside a tertiary cardiothoracic surgical unit. Patients treated with an IST for SH between 1 April 2014 and 30 November 2021 were identified. Anticoagulation was not stopped in these patien
