Cardiocerebral Resuscitation: An Approach to Improving Survival of Patients With Primary Cardiac Arrest.

Out-of-hospital cardiac arrest (OHCA) is a major public health problem. In the United States, OHCA accounts for more premature deaths than any other cause. For over a half-century, the national "Guidelines" for resuscitation have recommended the same initial treatment of primary and secondary cardiac arrests. Using this approach, the overall survival of patients with OHCA, while quite variable, was generally very poor. One reason is that the etiologies of cardiac arrests are not all the same. The vast majority of nontraumatic OHCA in adults are due to a "primary" cardiac arrest, rather than se

Identification and mitigation of a systematic analysis error in a multicenter dual-energy x-ray absorptiometry study.

Background/AimsData integrity in multicenter and longitudinal studies requires implementation of standardized reproducible methods throughout the data collection, analysis, and reporting process. This requirement is heightened when results are shared with participants that may influence health care decisions. A quality assurance plan provides a framework for ongoing monitoring and mitigation strategies when errors occur.MethodsThe Diabetes Control and Complications Trial (1983-1993) and its follow-up study, the Epidemiology of Diabetes Interventions and Complications (1994-present), have chara

Correctional-staff wellness in a statewide jail system: health appraisals, occupational strain and priorities for organizational support.

PURPOSE: Although substantial research has examined correctional officer health, fewer studies have focused on jail staff, and even fewer have done so within a unified, statewide jail system. This study aims to address this gap by examining staff perceptions of health and wellness across Tennessee's county jails, where centralized oversight enables system level insight. DESIGN/METHODOLOGY/APPROACH: Open-ended questionnaire data from 412 correctional personnel were analyzed following survey distribution to staff across all 120 Tennessee county jails. While jails are typically decentralized, Te

Awareness, willingness to use and use of doxycycline post-exposure prophylaxis among gay, bisexual and other men who have sex with men in China: a national cross-sectional survey.

BACKGROUND: Bacterial sexually transmitted infections (STIs) disproportionately impact gay, bisexual and other men who have sex with men (GBMSM) in China. Doxycycline post-exposure prophylaxis (DoxyPEP) can prevent syphilis and chlamydia among GBMSM; however, it has not yet been officially endorsed in China. This study aims to investigate the perceptions and use of DoxyPEP among GBMSM in a nationwide sample. METHODS: We conducted a nationwide online cross-sectional survey in China between November 2024 and February 2025. Participants were recruited through community-based organizations and Ce

Outcomes of Older Adults With Advanced Cancer Who Prefer Quality of Life vs Prolonging Survival: A Secondary Analysis of the GAP70+ Cluster Randomized Clinical Trial.

IMPORTANCE: Health care systems should strive to achieve outcomes that matter to patients. Limited data exist on the outcomes achieved by patients with cancer with different treatment outcome preferences. OBJECTIVE: To describe treatment outcome preferences among older adults with advanced cancer and explore differences in outcomes between patients prioritizing survival vs quality of life (QoL), testing the hypothesis that patients prioritizing survival would live longer, while those prioritizing QoL would have fewer treatment-related adverse effects (TRAEs) and hospitalizations. Prevalence, associated factors, and machine learning classification of low back pain among postmenopausal women attending rehabilitation and physiotherapy facilities in Bangladesh.

BACKGROUND: Low back pain (LBP) is a leading cause of disability worldwide and disproportionately affects women, particularly after menopause. However, evidence on the prevalence and associated factors of LBP among postmenopausal women in low- and middle-income countries remains limited. This study aimed to estimate the past-year prevalence of LBP among postmenopausal women attending selected rehabilitation and physiotherapy facilities in Bangladesh, examine factors associated with LBP, and evaluate the internal classification performance of machine learning models. METHODS: A clinic-based cr

NSAID use and clinical outcomes in COVID-19 patients: a 38-center retrospective cohort study.

BACKGROUND: Non-steroidal anti-inflammatory drugs (NSAIDs) are commonly used to reduce pain, fever, and inflammation but have been associated with complications in community-acquired pneumonia. Observations shortly after the start of the COVID-19 pandemic in 2020 suggested that ibuprofen was associated with an increased risk of adverse events in COVID-19 patients, but subsequent observational studies failed to demonstrate increased risk and in one case showed reduced risk associated with NSAID use. METHODS: A 38-center retrospective cohort study was performed that leveraged the harmonized, hi

Geographic access to buprenorphine prescribers for patients who use public transit.

OBJECTIVE: Urban Medicaid enrollees with opioid use disorder often rely on public transit to reach buprenorphine prescribers. Research has not shown whether public transit provides this population with adequate geographic access to buprenorphine prescribers. We examined travel times to buprenorphine prescribers by car and public transit in urban areas, and determined whether car-based Medicaid regulatory standards produce their intended geographic coverage. METHODS: We obtained data for this study from the Substance Abuse and Mental Health Services Administration's Buprenorphine Practitioner

Home-Based Cardiac Rehabilitation in Brazil's Public Health Care: Protocol for a Randomized Controlled Trial.

BACKGROUND: Coronary artery disease (CAD) is among the main causes of hospitalization and death worldwide, therefore, the implementation of programs to reduce its impact is necessary. Supervised cardiac rehabilitation has been shown to have positive effects on CAD control. However, there are barriers to patient participation in the traditional, face-to-face cardiac rehabilitation programs, mainly in low-resource environments. OBJECTIVE: This study aimed to verify patient compliance to a home-based cardiac rehabilitation program, which includes unsupervised health education and physical exerci

Evaluation of a Custom-Developed Computer Game to Improve Executive Functioning in 4- to 6-Year-Old Children Exposed to Alcohol in Utero: Protocol for a Feasibility Randomized Controlled Trial.

BACKGROUND: Fetal alcohol spectrum disorder (FASD) is one of the most common causes of preventable intellectual disability, and the key associated deficits are in executive function (EF). Aspects of EF can be improved using cognitive training interventions. The highest prevalence of FASD globally (at a rate of 135.1 per 1000) has been found in a South African population in the Western Cape province. There is a shortage of specialized health service personnel, and there are limited remedial services. Computer-based cognitive training, if age and culturally appropriate, could be an effective way

Protocol for the evaluation of the population-level impact of Zimbabwe's prevention of mother-to-child HIV transmission program option B+: a community based serial cross-sectional study.

BACKGROUND: WHO recommends that HIV infected women receive antiretroviral therapy (ART) minimally during pregnancy and breastfeeding ("Option B"), or ideally throughout their lives regardless of clinical stage ("Option B+") (Coovadia et al., Lancet 379:221-228, 2012). Although these recommendations were based on clinical trials demonstrating the efficacy of ART during pregnancy and breastfeeding, the population-level effectiveness of Option B+ is unknown, as are retention on ART beyond the immediate post-partum period, and the relative impact and cost-effectiveness of Option B+ compared to Opt

Dietary calcium and vitamin D intake in an adult Middle Eastern population: food sources and relation to lifestyle and PTH.

Little is known about calcium and vitamin D intakes in Middle Eastern countries, where the prevalence of hypovitaminosis D is high. This study identifies major sources of calcium and vitamin D in the Lebanese diet, examines lifestyle factors that may influence intake of these nutrients and investigates the relationship between nutritional or lifestyle factors and parathyroid hormone (PTH). Three hundred sixteen young healthy volunteers aged 30 to 50 (men, non-veiled and veiled women) were recruited from different rural and urban Lebanese community centers. Food frequency questionnaire was used

Hysterectomy risk in premenopausal-aged military veterans: associations with sexual assault and gynecologic symptoms.

BACKGROUND: Several gynecological conditions associated with hysterectomy, including abnormal bleeding and pelvic pain, have been observed at increased rates in women who have experienced sexual assault. Previous findings have suggested that one of the unique health care needs for female military veterans may be an increased prevalence of hysterectomy and that this increase may partially be due to their higher risk of sexual assault history and posttraumatic stress disorder (PTSD). Although associations between trauma, PTSD, and gynecological symptoms have been identified, little work has been

Emotion-Adaptive Large Language Model-Driven Clinical Decision Support: User Evaluation of the Empathic Clinical Decision Support System Framework for Trust and Explainability.

BACKGROUND: The increasing prevalence of cannabis use has motivated researchers to develop computational behavioral models that predict usage patterns and related health impacts in naturalistic environments. However, the opaque nature of many artificial intelligence (AI) systems limits users' ability to interpret outputs and undermines trust. Existing explainable artificial intelligence techniques often remain overly technical and do not account for the confusion or frustration clinicians may experience when interpreting complex model explanations.
