Augmentation cystoplasty and risk of neoplasia: fact, fiction and controversy.

PURPOSE: We determined if ileal/colonic bladder augmentation performed in patients with congenital bladder abnormalities is an independent risk factor for bladder malignancy. MATERIALS AND METHODS: We reviewed a registry of patients with bladder dysfunction due to neurological abnormalities, exstrophy and posterior urethral valves. Individuals treated with augmentation cystoplasty were matched (1:1) to a control group treated with intermittent catheterization based on etiology of bladder dysfunction, gender and age (±2 years). RESULTS: We evaluated 153 patients with an ileal/colonic cystopla

Electron microscopic findings in levator muscle biopsies of patients with isolated congenital or acquired ptosis.

OBJECTIVE: Systemic mitochondriopathies as chronic progressive external ophthalmoplegia (CPEO) are frequently associated with ptosis. We investigated whether mitochondrial abnormalities in the levator muscle are also found in patients with isolated congenital or acquired ptosis showing no other signs of mitochondrial cytopathy. METHODS: Biopsies of levator muscle were taken during surgery from 24 patients with isolated congenital (group 1) or early-onset acquired ptosis (group 2). All patients were given a thorough clinical examination before and after surgery. Ultrathin muscle sections were

Congenital anomalies are commonly associated with exomphalos minor.

AIM: The objective of this study is to define the incidence of chromosomal and congenital anomalies in neonates with exomphalos major and minor. BACKGROUND: Incidence of major congenital anomalies varies from 35% to 81% in exomphalos. It is unclear whether these malformations are more common with exomphalos major. MATERIAL AND METHODS: The case notes of 82 antenatal diagnoses of exomphalos, made between January 1998 and December 2004, were retrospectively reviewed. Exomphalos major was defined as a defect 5 cm or greater and exomphalos minor a defect less than 5 cm in diameter. RESULTS: The

[A paradigmatic case of partial agenesis of the pericardium].

Complete or partial absence of the pericardium is an uncommon congenital abnormality. Since its first description, several cases have been reported. Although the clinical and instrumental features of this defect are well described, at present it is often overlooked. We report a case of a 19-year old boy referred to our Echocardiography laboratory because of a suspected interatrial septal defect. For this reason it has been recommended to undergo cardiac catheterization. The patient, completely asymptomatic, had a negative physical examination. The echocardiogram excluded the presence of an int

[Congenital ptosis of the upper eyelid: indication for early ophthalmological examination].

OBJECTIVE: Description of the results of clinical investigation and surgical correction in patients with congenital upper eyelid ptosis. DESIGN: Descriptive. SETTING: The Eye Hospital, Rotterdam. METHOD: Evaluation of clinical findings and results of operative treatment in 114 consecutive patients with congenital ptosis. RESULTS: Amblyopia was present in 20% of the 114 patients, and was caused by stimulus deprivation due to the ptotic eyelid (22%), strabismus (48%), and refraction disorder (30%). Treatment of amblyopia did not induce visual improvement in 7% of the patients (8/114). These

Giant congenital melanocytic nevi: brain magnetic resonance findings in neurologically asymptomatic children.

BACKGROUND: Patients with giant or multiple congenital melanocytic nevi occasionally have leptomeningeal melanocytosis, also called neurocutaneous melanosis (NCM). Patients with symptomatic NCM usually have signs or symptoms of increased intracranial pressure and have a poor prognosis. Magnetic resonance (MR) imaging is sensitive in detecting melanin; several recent reports have described the MR findings in neurocutaneous melanosis. OBJECTIVE: The aim of this study is to review the brain MR findings and their potential significance in a group of neurologically asymptomatic children with giant

[Congenital subclavian steal syndrome. Apropos of 14 cases. Review of the literature].

Fourteen cases of the congenital subclavian steal syndrome are presented with a review of 99 cases in the literature. This abnormality is usually asymptomatic and a fortuitous finding (clinical: asymmetric blood pressure; radiological: investigation of an associated cardiovascular malformation). The possible malformations are numerous and may be diagnosed by angiography: however, they can be suspected from three simple investigations, clinical examination indicating the side of lower blood pressure, chest X-ray and barium swallow. Seventy one per cent congenital subclavian steal syndromes are

[Congenital unilateral absence of pulmonary artery: a case report].

Congenital unilateral absence of pulmonary artery (UAPA) is a rare congenital vascular malformation with diverse clinical manifestations and often challenging diagnosis. This article reports a case of a young male with hemoptysis as the main symptom. The uniqueness of this case lies in the previous history of patent ductus arteriosus (PDA) surgery and the confirmation of complete absence of the right pulmonary artery through computed tomography angiography (CTPA), with the right pulmonary artery being supplied by an independent abnormal vessel originating from the aortic arch. The significance

An Update on Diagnosis of Tracheomalacia in Children.

Congenital tracheomalacia is the most common congenital tracheal abnormality and occurs in approximately 1:2,100 children. Tracheomalacia can be isolated or associated with other airway anomalies such as laryngomalacia, bronchomalacia, and large laryngeal clefts. Also, an abnormal division of the embryonic foregut is frequently associated with congenital tracheomalacia; mostly in the form of proximal esophageal atresia with distal tracheoesophageal fistula. In such cases, the ratio between the cartilage ring and the posterior membranous wall drops from the normal ratio of 4 to 5:1 to 2 to 3:1.

Ultrasound biomicroscopic characteristics of the anterior segment in primary congenital glaucoma.

PURPOSE: To evaluate qualitatively and quantitatively ultrasound biomicroscopic (UBM) features of the anterior segment in eyes with primary congenital glaucoma. METHODS: UBM of 45 eyes of patients with primary congenital glaucoma (39 previously operated and 6 unoperated eyes) and 28 control eyes were included in this study. UBM parameters were correlated with ocular biometry. Iris thickness, ciliary body thickness, ciliary body-lens distance, posterior chamber depth, and anterior chamber angle were measured and compared with control eyes. Other features of the anterior segment were qualitativ

Primary parathyroid hyperplasia associated with thyroid hemiagenesis and agenesis of the isthmus.

BACKGROUND: Congenital thyroid anomalies can be caused by abnormal descent or agenesis of part of the thyroid gland. Agenesis of the thyroid gland may involve one or both lobes of the gland, with or without isthmus, and is very rare. Congenital thyroid anomalies have been reported to be associated with thyroid diseases (hyperthyroidism, adenomatous goiter, and cancer) but not with parathyroid diseases. METHODS: Case reports of two patients describe the methods used. RESULTS: We report two patients with hyperparathyroidism and congenital thyroid anomalies. One patient had a left thyroid lobe

Propranolol as a treatment for multiple coronary artery micro-fistulas.

We present a case of congenital multiple coronary artery-left ventricle micro-fistulas, which were treated with propranolol disappearing within 6 months. She had a malformative syndrome associated with a chromosomal abnormality. The treatment for coronary artery fistula includes surgical ligation and transcatheter closure, but they are not indicated in congenital micro-fistulas. We propose propranolol as a treatment in this type of diffuse fistula.

Congenital esophageal stenosis: the differential diagnosis and management.

Congenital esophageal stenosis (CES) is a rare congenital abnormality that is difficult to diagnose and often masquerades as other types of structural esophageal disease. We report three cases of CES with different presenting symptoms. We advocate for balloon dilation as the preferred first approach to therapeutic intervention. CES is an important clinical entity in the evaluation of pediatric esophageal disorders and should be suspected in young infants with dysphagia.

Recurrent vulval congenital lymphangioma circumscriptum--a case report and literature review.

Lymphangioma circumscriptum (LC) is a defect of lymphatics in deep dermis. This abnormal saccular dilatation of lymphatics occurs in dermal papillae visible as vesicles clinically. The entity may be acquired or congenital. The occurrence of congenital variety of LC in vulva is extremely rare, with only few cases reported in the literature. We communicate one such case presenting as recurrent vulval growth along with a review of literature and treatment modalities.

Complex bilateral polysyndactyly featuring a triplet of delta phalanges in a syndactylised digit.

The delta phalanx is a rare congenital skeletal anomaly. An abnormal C-shaped epiphysis is usually responsible for a progressive angular digital deformity observed either in hands or feet. Solitary delta phalanges are usually described. We report a case of bilateral congenital hand malformations featuring a triplet of delta phalanges affecting a single digit on one hand, together with a concealed central polydactyly on the other.

Pyloric diaphragm, antral web, congenital antral membrane--a surgery rarity?

Three cases of pyloric diaphragm in elderly patients are reported. The finding of these patients within a 2-year period (1976-8) suggests that the condition is not as rare as previously thought. Diagnosis was achieved endoscopically and the condition managed surgically. While there has been controversy in the past over whether the disorder is acquired or congenital, the histological evidence of the cases presented here tends to suggest that it is a late presentation of a congenital abnormality.

Epidemiological and Histopathological Characteristics of Fetuses with Congenital Disorders: A Study in Greece.

Congenital malformations constitute a major public health issue. Nonetheless, Greece does not participate in the European network of congenital anomalies. To examine the epidemiology of congenital abnormalities and investigate possible factors associated with their development, autopsies were performed on 649 fetuses referred from three healthcare facilities in Thessaloniki during 1992-2008. Adequate statistical analysis methods were implemented. Birth defects were found in 256 fetuses (39.5%)-primarily related to the musculoskeletal (17.3%), nervous (14.5%), cardiovascular (12.5%) and urinary

Are We Playing It Fast and Loose With the Serofast?

Four infants (one singleton and a set of triplets) born to mothers with serofast reactive plasma reagin at 1:4 to 1:8 were found to have congenital syphilis. Each mother had a history of receiving appropriate treatment for their syphilis stage at the time of diagnosis with benzathine penicillin G 2.4 million units intramuscularly weekly 3 times. Both exhibited a 4-fold decrease in their reactive plasma reagin titer. The singleton was asymptomatic but found to have long bone radiographic evidence for congenital syphilis. Of the triplets, one had laboratory abnormalities and a rash, while all 3

Associated Anomalies in Radial Ray Deficiency.

Radial ray deficiency (RRD) may be isolated, without other congenital anomalies or co-occurring with other, non-RRD, congenital anomalies. The prevalence and the types of co-occurring anomalies are variable in the reported studies. The aim of this study was to obtain the prevalence and the types of co-occurring congenital anomalies among cases with RRD in a geographically well-characterized population of 387,067 consecutive births in northeastern France from 1979 to 2007 including live births, stillbirths and terminations of pregnancy. During the study period 83 cases with RRD were ascertained
