A framework for multidisciplinary management of autonomic dysfunction in Parkinson disease.

Autonomic dysfunction (AD) is present in nearly all people with Parkinson disease (PD), contributing to tremendous morbidity and mortality. Highly variable presentations including cardiovascular, gastrointestinal, urogenital, and thermoregulatory dysfunction can substantially affect daily function, safety, medication tolerance, and quality of life. Although autonomic symptoms are frequently encountered in neurologic practice, their management frequently extends beyond the traditional scope of neurologic care and may require input from multiple disciplines. Despite the increasing complexity of

Arbovirus co-circulation and epidemiological convergence in a changing climate: challenges for surveillance, diagnosis and public health preparedness.

Arboviral diseases are undergoing substantial epidemiological changes driven by climate change, urbanization, global mobility, vector expansion, and evolving ecological conditions. These processes increasingly favor the geographic overlap and co-circulation of multiple arboviruses, including in regions previously considered at limited risk. In this review, we describe this broader process as epidemiological convergence, referring to the progressive overlap of arboviral transmission systems across shared geographic, ecological, vector, host, and public health contexts. Such convergence creates

SMURF: soft-segmentation for single-cell reconstruction and topological analysis of spatial transcriptomic data.

High-resolution spatial transcriptomics requires computational methods to accurately assign transcripts to individual cells. We present SMURF (Segmentation and Manifold UnRolling Framework), a cross-platform soft-segmentation algorithm that uses deep learning to map mRNAs from capture spots to nearby nuclei. SMURF also unrolls complex tissue architectures by projecting cells onto Cartesian coordinates, enabling analyses of cell-type organization and gene expression gradients. We show that SMURF assigns mRNAs to single cells more accurately than existing approaches and robustly unrolls complex

Cleft lip∕palate. From embryology to developmental pathogenesis.

Cleft of the lip associated with or unassociated with cleft of the palate and cleft palate (CLP) represent frequent birth defects, and the mechanisms by which they occur are not completely clear. On the other hand, knowledge of the normal mechanisms underlying lip development and palatogenesis is still incomplete, and therefore a better understanding and consolidation of our knowledge in this field could help to study these deformities. Upper lip and secondary palate formation are complex processes, involving the subtle integration of several biological events, the disruption of which causes t

Cellular and Microcircuit Mechanisms of Anesthetic Disruption of Conscious-State Organization.

The reversible effects of general anesthesia can be used to examine how brain activity changes during transitions into and out of altered conscious states. Such changes are commonly tracked using behavioral responses, electroencephalographic recordings, and measures of functional connectivity. These measures, however, reflect processes occurring at lower levels of organization, including the activity of pyramidal neurons, dendritic integration, interneuron function, thalamocortical signaling, and glial regulation of the extracellular environment. Anesthesia does not affect all neural processes

Nucleoside-modified PVA hydrogel with antibacterial and wet-adhesion properties accelerates healing of diabetic oral ulcers.

Oral ulcers, particularly in diabetic patients, pose significant clinical challenges due to impaired healing conditions and high susceptibility to infection. Considering the highly dynamic, moist, and complex multi-bacterial environment of the oral cavity, there is an urgent need to develop a bioactive formulation with wet-adhesion and antibacterial properties to manage diabetic oral ulcers. Here, we developed a novel multifunctional hydrogel (polyvinyl alcohol-borate-cytidine-Ag+, PBCA) based on the i-motif structure of C-Ag+-C coordination, boronate ester bonds formed between PVA and cytidin

Multidisciplinary Orthodontic Management of Non-syndromic Delayed Tooth Eruption and Impacted Maxillary Canine Complicated by Traumatic Mandibular Fracture in a Healthy Adolescent: A Case Report.

Delayed eruption of multiple teeth in systemically healthy, non-syndromic adolescents represents a diagnostic and therapeutic challenge. When combined with impacted maxillary canines and vertical discrepancies, management becomes more complex. A 14-year-old healthy male presented with delayed eruption (DE) of multiple permanent teeth. Clinical and radiographic examination revealed bilateral DE of mandibular canines and both premolars, a unilateral impacted maxillary canine, and a severe deep bite. The patient had no relevant medical history and showed normal physical development. Orthodontic t

Neuroinflammatory and Motor Alterations in LRRK2*G2019S Transgenic Mice Without Enhanced Vulnerability to Aging or Chronic MPTP-Induced Nigrostriatal Neurodegeneration.

LRRK2 G2019S mutation is the most common genetic cause of Parkinson's disease (PD) producing clinical manifestations similar to sporadic PD patients, hinting at the relevance of this mutation in the pathophysiological mechanisms of the disease. However, its role potentiating nigrostriatal neurodegeneration, particularly in response to mitochondrial dysfunction, remains unclear. Here, using histological, neurochemical and behavioral analyzes, we provide a complete characterization of a mouse model expressing the human LRRK2 G2019S mutation (hLRRK2*G2019S mice), in the context of aging and toxin

From molecules to minds: Integrative multi-omics in psychiatry.

Psychiatric disorders are biologically complex conditions arising from interactions across genomic, epigenomic, transcriptomic, proteomic, metabolomic, and metagenomic layers. Single-omics approaches rarely capture more than a fraction of the variance in complex conditions, underscoring the importance of integrative multi-omics frameworks. This mini-review summarizes key methodologies and their application in psychiatric research, with a focus on systems-level integration of genomic risk scores, transcriptomic networks, and neuroimaging data to advance biological understanding of disorders suc

DNA damage responses in HCMV infection: emerging mechanisms and outstanding questions.

The cellular DNA damage response (DDR) is essential for maintaining genome integrity in the face of exogenous assault, as well as errors and breaks introduced during DNA replication. The DDR is a complex network of cellular processes that detects the type and severity of DNA damage and coordinates downstream actions. These include activating specific DNA repair pathways, including cell cycle arrest to allow time for repair, and-if the damage is irreparable-triggering apoptosis to eliminate cells with compromised genomes. DNA viruses have evolved multiple strategies to evade, suppress, or even

Limb Salvage in Acute Limb Ischemia and Diabetic Foot Gangrene Associated with Pancreatic Acinar Cell Carcinoma-Related Hypercoagulability.

Malignancy-associated hypercoagulability may cause arterial thrombotic events and acute limb ischemia. When combined with diabetic vasculopathy, rapid progression to gangrene and severe infection may occur. We report a patient with type 2 diabetes who developed acute limb ischemia and progressive diabetic foot gangrene. Despite prior endovascular therapy, severe infection with necrotizing soft tissue infection and osteomyelitis developed. A staged limb-salvage approach involving aggressive debridement, removal of infected bone, pedicled flap reconstruction, antimicrobial therapy, anticoagulati

Implementing a Nurse Navigator for Placenta Accreta Spectrum Disorder.

BACKGROUND: Placenta accreta spectrum disorder is a condition that is rare but is becoming more common due to the increase in cesarean rates. Placenta accreta spectrum disorder is known to have high morbidity and mortality rates due to its increased risk of hemorrhage. These surgical cases are complex and need to have a robust multidisciplinary team to decrease the risk of potential complications. Due to the complexity of this diagnosis, the plan of care must involve coordination of several specialties and roles as well as ensuring the patient has adequate education and explanation regarding t

Amino acid chemistry and post-translational modifications underlying marine adhesive proteins: Biochemical insights for designing underwater adhesives.

Underwater adhesion remains a complex challenge due to the interference of water with traditional bonding mechanisms. Marine organisms, such as mussels, sandcastle worms, and barnacles, have evolved specialized adhesive proteins that overcome these limitations through functional amino acid motifs. This review highlights how post-translationally derived amino acid motifs, such as 3,4-dihydroxylphenylalanine (DOPA) and phosphoserine, drive interfacial adhesion and the curing process through interfacial binding, metal coordination, and crosslinking. Alongside these, canonical amino acids, such as

Unveiling the content of frontal feedback in challenging object recognition.

Object recognition under challenging real-world conditions, including partial occlusion, remains an enduring focus of investigation in cognitive visual neuroscience. This study addresses the insufficiently elucidated neural mechanisms and temporal dynamics involved in this complex process, concentrating on the persistent challenge of recognizing objects obscured by occlusion. Through the analysis of human EEG data, we decode feedback characteristics within frontotemporal networks, uncovering intricate neural mechanisms during occlusion coding, with a specific emphasis on processing complex sti

Unraveling the complexity of neurodegeneration: heterogeneous damage patterns of locus coeruleus and substantia nigra in Alzheimer's disease.

INTRODUCTION: Alzheimer's disease (AD) involves complex neurodegeneration in the locus coeruleus (LC) and substantia nigra (SN), critical brain regions affecting cognitive and emotional processes. METHODS: Using the National Alzheimer's Coordinating Center dataset of 11,583 participants, we conducted a multimodal analysis involving descriptive statistics, multifactorial regression, mediation analysis, and receiver operating characteristic curve assessment. RESULTS: LC degenerative changes (18.57%) significantly exceeded SN changes (4.69%). The LC demonstrated stronger correlations with psych

Emergency managers' challenges with wildfires and related cascading hazards in California.

This study investigates the complexities faced by emergency managers in wildfire-prone areas to uncover pressing issues and potential solutions. Four themes are discerned through three focus group discussions with emergency managers from nine counties across California. First, there is unequal access to resources for both risk assessment and response, with counties that have fewer resources facing significant challenges in effectively managing wildfire risks. Second, effective risk communication depends on the available resources and the unique characteristics of each community. Participants s

Forging a path toward kidney transplant equity through multilevel interventions.

PURPOSE OF REVIEW: Persistent disparities in access to kidney transplantation, particularly living donor transplant and preemptive transplant, have gained increasing national attention including efforts to describe their multifactorial root causes. Multilevel structural impediments occurring at the recipient, donor, clinician, and health system level contribute to these disparities. This review overviews these key barriers, as well as recent successful interventions designed to address longstanding disparities. RECENT FINDINGS: Several systems-level interventions including the Systems Interve

Generation and Reactivity of a High-Spin Iron(IV)-Oxo Complex That Is Stable at Ambient Temperatures.

Nature operates a variety of challenging oxidation reactions through intermediates bearing tetravalent iron centers bound to a terminal oxo ligand. The high-spin (S = 2) electronic configuration is believed to be particularly important in C-H activation reactions mediated by iron(IV)-oxo species. Coordination environments that promote high-spin ground states obviate the need for spin-state crossing to access this state and can promote rapid oxidation reactivity. As a result, however, synthetic iron(IV)-oxo species with S = 2 ground states tend to exhibit poor thermal stabilities, which has ham

Catalytic transfer zincation using ammonium cations.

C-H zincation is an efficient route to useful organozinc compounds. To date, most approaches effect aryl C-H zincation with selectivity controlled by substrate pKa. Herein, we report a heteroaryl C-H zincation method that uses an easy to access (β-diketiminate)Zn-Me complex and [(R3N)H][Anion] and proceeds under electronic control. This catalytic process is anion dependent; the less coordinating anion [B(C6F5)4]- proved superior to [CHB11H5Br6]-, with the latter forming the intimate ion pair [(β-diketiminate)Zn(CHB11H5Br6)]. The Zn-Me complex and the [(R3N)H]+ salt can also initiate catalytic
