Regional, multimodal, and opioid-sparing strategies after trauma: a review.

Effective analgesia is fundamental in the management of trauma, critical care, and emergency surgical patients. However, historical reliance on opioid-based pain management strategies can be associated with adverse effects, including respiratory depression, delirium, and prolonged opioid exposure after discharge, leading to dependence. In response, multimodal analgesia strategies emphasizing non-opioid and regional techniques have become an important adjunct in trauma care pathways. This review summarizes current evidence supporting neuraxial anesthesia, peripheral and truncal nerve blocks, an

Atypical clinical presentation of oncocytic adrenocortical carcinoma with decompensated metabolic syndrome and psychotic outbreak.

Adrenal incidentalomas, mostly adrenal adenomas, affect 3%-10% of the global population. Adrenocortical carcinoma (ACC) is rare, with an incidence of 0.7-2 cases per million. Adrenocortical oncocytic neoplasms (ACONs) account for about 10% of ACC cases, often discovered incidentally, with 17-34% being functionally active. We report a case of a woman in her 60s with treatment-resistant hypertension, diabetes and psychotic delirium. Imaging revealed a 6 cm left adrenal mass with marginally elevated metanephrines. Laparoscopic adrenalectomy was performed. Histology confirmed ACON. Positive margins

Enhancing Clinical Decision Support: A Heuristic Evaluation of Explainable AI in Healthcare Dashboards.

Explainable Artificial Intelligence (XAI) is crucial for enhancing transparency, interpretability and actionability of AI systems, particularly in healthcare. The SAD XAI Dashboard, a clinical decision support (CDS) tool for sepsis-associated delirium (SAD), assists physicians in understanding AI-driven predictions for SAD. Our study aimed to evaluate the XAI dashboard's compliance with an XAI usability heuristics checklist. Three experts (human factors and health informatics) applied the heuristics checklist to the dashboard. The evaluation identified several usability issues, including uncle

Neurologic Complications of Babesiosis, United States, 2011-2021.

Babesiosis is a globally distributed parasitic infection caused by intraerythrocytic protozoa. The full spectrum of neurologic symptoms, the underlying neuropathophysiology, and neurologic risk factors are poorly understood. Our study sought to describe the type and frequency of neurologic complications of babesiosis in a group of hospitalized patients and assess risk factors that might predispose patients to neurologic complications. We reviewed medical records of adult patients who were admitted to Yale-New Haven Hospital, New Haven, Connecticut, USA, during January 2011-October 2021 with la

Sepsis and the brain: a review for acute and general physicians.

Sepsis-associated encephalopathy (SAE) describes acute cognitive dysfunction secondary to systemic or peripheral infection occurring outside of the central nervous system (CNS). Symptoms can range from mild confusion to coma and may precede the clinical signs of sepsis. Recognition that SAE is a potential differential diagnosis in patients presenting with delirium is important, as SAE is a diagnosis of exclusion. Physicians should also be aware that severe SAE is associated with a high mortality. Although mortality is often secondary to multiorgan failure rather than neurological sequelae, lon

Weakness in the intensive care unit.

When asked to assess patients in an intensive care unit (ICU) who have respiratory muscle weakness, oropharyngeal weakness and a vulnerable airway, our immediate thought may be of Guillain-Barré syndrome or myasthenia gravis, but there are many other possible causes. For example, previously unrecognised chronic neurological conditions may decompensate and require ICU admission. Clinicians can use various clinical clues to help recognise them and need to understand how patterns of weakness reflect differing causes of reduced consciousness on ICU. Additionally, patients admitted to ICU for any r

Mild to Severe Neurological Manifestations of COVID-19: Cases Reports.

The main focus of Coronavirus disease 2019 (COVID-19) infection is pulmonary complications through virus-related neurological manifestations, ranging from mild to severe, such as encephalitis, cerebral thrombosis, neurocognitive (dementia-like) syndrome, and delirium. The hospital screening procedures for quickly recognizing neurological manifestations of COVID-19 are often complicated by other coexisting symptoms and can be obscured by the deep sedation procedures required for critically ill patients. Here, we present two different case-reports of COVID-19 patients, describing neurological co

Perioperative Outcomes of Cemented vs Cementless Total Hip Arthroplasty: A National Inpatient Sample Study of 81,668 Elective Procedures.

Background: Cemented and cementless fixation techniques in total hip arthroplasty (THA) each present distinct biomechanical properties and perioperative risk profiles. While cementless fixation has gained increasing popularity, large-scale nationally representative comparisons of perioperative outcomes between cemented and cementless elective THA remain limited. This study aimed to compare complication rates, healthcare utilization, and temporal trends between cemented and cementless elective THA using the National Inpatient Sample. Methods: A retrospective cohort study was conducted using the

Association between risk factors and 1-year mortality of physical and/or cognitive components of post-intensive care syndrome in patients with sepsis.

Survivors of sepsis are at high risk of developing post-intensive care syndrome (PICS), which encompasses physical, mental, and cognitive impairments after critical illness. Among these domains, physical and cognitive impairments have been linked to increased long-term mortality; however, their specific associations with mortality among patients with sepsis remain unclear. This study aimed to examine the relationship between PICS and subsequent survival, with a particular focus on physical and cognitive domains. In this retrospective cohort study, we included patients with sepsis who were admi

Drug-Induced Psychosis: A Drastic Turn After a Kidney Transplant.

Drug-induced psychosis is an uncommon but clinically significant complication in solid-organ transplant recipients, particularly those receiving calcineurin inhibitors such as tacrolimus. Neuropsychiatric toxicity can occur even at therapeutic drug levels, and concurrent corticosteroid use may further increase vulnerability. Early recognition is essential, as symptoms may be mistakenly attributed to metabolic disturbances, primary psychiatric illness, or postoperative delirium, thereby delaying appropriate intervention. We report the case of a previously healthy 34-year-old man with end-stage

Two-year trajectory of cognitive decline and neurological sequelae in COVID-19 survivors with acute neurological symptoms.

INTRODUCTION: The COVID-19 pandemic caused by SARS-CoV-2 has profound implications for global public health. It significantly affects the nervous system and cognitive function. The emergence of long COVID has raised concerns regarding long-term cognitive impairment, particularly among patients who experienced neurological symptoms during the acute phase of infection. AIM: This study investigated the impact of neurological symptoms during acute SARS-CoV-2 infection on subsequent cognitive change and neurological sequelae over a 2-year period. METHODS: We enrolled 3,419 hospitalized patients w

Impact of Valsalva maneuver duration on brain function in patients undergoing high-intensity focused ultrasound liver ablation: a randomized controlled trial.

BACKGROUND: Intermittent Valsalva maneuver (VM) is commonly used to facilitate high-intensity focused ultrasound (HIFU) ablation. However, the optimal duration of a VM in terms of adverse impacts on brain function is unknown. This prospective study explored the impact of different VM durations on brain function in patients undergoing HIFU ablation surgery for hepatic cancer. METHODS: Adult patients scheduled for HIFU ablation for liver cancer under general anesthesia were randomized into a control group, ≤5.2 min per VM episode (short VM group), or >5.2 min per VM episode (long VM group). Clinical outcomes and patient-reported outcome measures among patients undergoing posterior lumbar fusion procedures with varying insurance payor status: a propensity score-matching study.

OBJECTIVE: Posterior lumbar fusion (PLF) is a routinely used procedure for treatment of spinal pathology. Several studies have highlighted disparities in reoperation and postoperative complications and demonstrated associations between differing insurance providers, complication rates, and hospital resource utilization in spine surgery. Previous studies have examined broad spinal procedures but have not extended to uninsured patients, or adjusted for sociodemographic factors or comorbidity history. Understanding relationships between payor status and outcomes following fusion procedures is vit
