A case of persistent hypotony following 23-gauge vitrectomy.

We report here a case in which ciliary detachment related to the preparation of a scleral wound was suspected as the cause of persistent hypotony following 23-gauge vitrectomy for proliferative diabetic retinopathy. Ultrasound biomicroscopy (UBM) was performed following injection of a viscoelastic substance into the anterior chamber to carefully investigate the treatment and cause of persistent hypotony. Deepening the anterior chamber by means of the viscoelastic substance enabled UBM identification of ciliary detachment that had not been detected in the shallow anterior chamber. The extent of

Charles Bonnet syndrome.

BACKGROUND: Charles Bonnet syndrome (CBS) involves nonthreatening hallucinations in patients who have no neurological and no psychological abnormalities but with significant visual impairment secondary to ocular disease, such as macular degeneration and diabetic retinopathy. Because of the fear of a mental illness being diagnosed, patients are often reluctant to discuss these hallucinations. CASE REPORTS: Three cases are presented of patients who experienced CBS caused by decreased vision. Each patient had decreased vision and related visual hallucinations that were consistent with CBS. The f

Intravitreal bevacizumab treatment for neovascular glaucoma: histopathological analysis of trabeculectomy specimens.

BACKGROUND: Neovascular glaucoma (NVG) is a serious complication for patients with proliferative diabetic retinopathy (PDR). Bevacizumab is a full-length humanized monoclonal antibody that binds all isoforms of vascular endothelial growth factor (VEGF). Recently, encouraging results regarding the off-label use of intravitreal bevacizumab (IVB) for the treatment of NVG have been reported. We evaluated the histology of bevacizumab-treated trabeculectomy specimens to clarify IVB's biological effects on angle neovascularization. METHODS: We retrospectively reviewed the charts of a consecutive ser

The importance of mitochondria in age-related and inherited eye disorders.

Mitochondria are critical for ocular function as they represent the major source of a cell's supply of energy and play an important role in cell differentiation and survival. Mitochondrial dysfunction can occur as a result of inherited mitochondrial mutations (e.g. Leber's hereditary optic neuropathy and chronic progressive external ophthalmoplegia) or stochastic oxidative damage which leads to cumulative mitochondrial damage and is an important factor in age-related disorders (e.g. age-related macular degeneration, cataract and diabetic retinopathy). Mitochondrial DNA (mtDNA) instability is a

The vitreous gel: more than meets the eye.

PURPOSE: To reexamine the role of the vitreous gel in ocular health and disease based on recent information in the ophthalmic literature. DESIGN: Perspective. METHODS: Review, analysis, and discussion of the implications of selected pertinent literature. RESULTS: A new understanding of the vitreous gel is emerging, placing it central to many disease processes affecting the eye, including diabetic retinopathy, retinal vein occlusion, age-related macular degeneration, nuclear sclerotic cataract, and primary open-angle glaucoma. The vitreous gel recently has been found to have the important fu

Circadian rhythm dysfunction in glaucoma: A hypothesis.

The absence of circadian zeitgebers in the social environment causes circadian misalignment, which is often associated with sleep disturbances. Circadian misalignment, defined as a mismatch between the sleep-wake cycle and the timing of the circadian system, can occur either because of inadequate exposure to the light-dark cycle, the most important synchronizer of the circadian system, or reduction in light transmission resulting from ophthalmic diseases (e.g., senile miosis, cataract, diabetic retinopathy, macular degeneration, retinitis pigmentosa, and glaucoma). We propose that glaucoma may

Topical apraclonidine testing discloses pupillary sympathetic denervation in diabetic patients.

BACKGROUND: Autonomic denervation is common in diabetes mellitus (DM). Pupillary sympathetic denervation (PSD) has been found in Horner syndrome following instillation of apraclonidine 0.5%. We have applied this technique to investigate the prevalence of PSD in DM. METHODS: Apraclonidine 0.5% was instilled in the eyes of 50 patients with DM and 30 age-matched and gender-matched subjects without DM (control subjects). Pupil diameters (PD) were measured before and 60 minutes after instillation. The duration of DM and the degree of diabetic retinopathy (DR) were recorded for each patient. RESUL

Non-arteritic ischaemic optic neuropathy (NAION) in patients under 50 years of age.

PURPOSE: The goal of our study was to compare the case histories and clinical findings in three young patients (aged<50 years) who had undergone their first attack of non-arteritic ischaemic optic neuropathy (NAION). We intended to consider whether NAION at a relatively young age might comprise a separate pathological and diagnostic entity. RESULTS: All three cases revealed some common characteristics. All of them experienced recurrent attacks with bilateral manifestations that led to severe loss of vision and visual field defects. The patients had also suffered from diabetes mellitus for a l

Global data on visual impairment in the year 2002.

This paper presents estimates of the prevalence of visual impairment and its causes in 2002, based on the best available evidence derived from recent studies. Estimates were determined from data on low vision and blindness as defined in the International statistical classification of diseases, injuries and causes of death, 10th revision. The number of people with visual impairment worldwide in 2002 was in excess of 161 million, of whom about 37 million were blind. The burden of visual impairment is not distributed uniformly throughout the world: the least developed regions carry the largest sh

Humanization of an anti-vascular endothelial growth factor monoclonal antibody for the therapy of solid tumors and other disorders.

Vascular endothelial growth factor (VEGF) is a major mediator of angiogenesis associated with tumors and other pathological conditions, including proliferative diabetic retinopathy and age-related macular degeneration. The murine anti-human VEGF monoclonal antibody (muMAb VEGF) A.4.6.1 has been shown to potently suppress angiogenesis and growth in a variety of human tumor cells lines transplanted in nude mice and also to inhibit neovascularization in a primate model of ischemic retinal disease. In this report, we describe the humanization of muMAb VEGF A.4.6.1. by site-directed mutagenesis of

The complications of perfluoropropane gas use in complex retinal detachments.

BACKGROUND: Perfluoropropane (C3F8) is a valuable adjunct in the management of complex rhegmatogenous retinal detachments, and it has become the most commonly used perfluorocarbon gas. However, there have been few large reports about its complications. This retrospective study was undertaken to evaluate the complications of the use of perfluoropropane gas. METHODS: Perfluoropropane gas was used in the repair of a complex retinal detachment in 157 consecutive eyes. Only eyes that had a follow-up of 6 months or greater were included. Any eyes that had undergone previous vitrectomy and vitreous

Acute anterior and posterior ischemic optic neuropathy.

Criteria for diagnosis of acute ischemic optic neuropathy (acute ION) were proposed: acute ION was divided into two groups, ie, acute anterior ischemic optic neuropathy (acute AION) and acute posterior ischemic optic neuropathy (acute PION). Acute AION is characterized by pale or sometimes hyperemic papilledema in the early stage, followed by postneuritic optic atrophy. In contrast, acute PION shows normal optic disk in the early stage followed by development of simple optic atrophy. We analyzed 31 eyes of 24 cases with acute AION and 20 eyes of 18 cases with acute PION. The diagnostic criteri

Pathology of anterior chamber angle in diabetic neovascular glaucoma: extension of corneal endothelium onto iris surface.

The purpose of this study is to elucidate the extension of the corneal endothelium onto the iris surface, a condition which has been found in the eyes with rubeosis iridis in various ocular disorders. Histopathology of the anterior chamber angle was studied by electron microscopy in 6 eyes with neovascular glaucoma occurring in the advanced stage of diabetic retinopathy. Histopathological findings embraced extensive peripheral anterior synechiae occluding the filtration angle and fibrovascular tissue on the anterior surface of the iris. In addition, formation of a continuous layer of endotheli
