Development and Validation of an Acute Large Animal Model for Type A Aortic Dissection.

BACKGROUND: Animal models are essential for translating diagnostic and therapeutic strategies into clinical practice and offer valuable insights into the pathophysiology of diseases such as aortic dissection. This study presents a novel acute in vivo large animal model of Stanford type A aortic dissection, combining open surgical access with endovascular techniques to leverage the advantages of both. The model aims to reproducibly simulate acute dissections in swine, providing a standardized platform for evaluating diagnostics, disease mechanisms, and treatment strategies. METHODS: Six pigs u

Bilateral Vertebral Artery Dissections Associated With Postpartum Preeclampsia: A Case Report.

Postpartum vertebral artery dissections (VADs) are infrequently reported; although data are limited, several case reports describe VAD occurring in the setting of postpartum preeclampsia. We present a 39-year-old G3P3003 (P1 term stillbirth) who developed bilateral VADs in the setting of postpartum preeclampsia and a persistent, nonfocal occipital headache. Six days after an uncomplicated cesarean delivery, the patient developed an unrelenting occipital headache accompanied by severe-range blood pressures. She met diagnostic criteria for postpartum preeclampsia. Despite treatment with magnesiu

Dissecting NO Association and Dissociation Dynamics of Myoglobin, Hemoglobin, and Heme-Model Compounds.

Nitric oxide (NO) binding to heme-proteins is a physiologically relevant molecular process, contributing to regulate blood pressure and flow to provide an adequate O2 supply to all body compartments. This appears to be relevant for energy-demanding tissues, characterized by high O2-consuming rates, such as the eye's retina and striated muscles under physical effort. The reaction of NO with the ferrous heme-Fe atom (heme-Fe(II)) is characterized by very fast association rate constants and very slow dissociation kinetics, which drastically differ from those observed for other gaseous ligands, su

Assessing transfusion need in patients with type A aortic dissection with multiplate aggregometry.

BACKGROUND: Acute type A aortic dissection (ATAAD) represents a life-threatening surgical emergency. Use of antiplatelet drugs and platelet dysfunction exacerbate the already high rates of postoperative morbidity and mortality. The objective of this study was to determine the relationship between preoperative platelet function evaluated through Multiplate® aggregometry and transfusion requirement. METHODS: This observational retrospective study included 180 patients who underwent emergency surgical repair of ATAAD over a two-year period (2019-2021). Platelet function was evaluated preoperativ

Dissecting the Structural Organization, Recruitment and Activation Mechanisms of Centrosomal γ-TuRCs.

Visualizing human centrosomes using cryo-electron tomography revealed the native structure and molecular organization of γ-tubulin ring complexes (γ-TuRCs). γ-TuRCs localized to two distinct centrosomal pools, one in the pericentriolar material (PCM) and another in the centriole lumen, which is released during mitosis. All detected γ-TuRCs were associated with the tetrameric adaptor protein NEDD1. Within the PCM, binding to the centrosomin (CM1) motif of the microcephaly protein CDK5RAP2 in different patterns correlates with conformational changes of γ-TuRCs. In the centriole lumen, the augmin

Methadone and the Kidney: Dissecting Gender Differences in Inflammation and Oxidative Stress Responses.

BACKGROUND: This study explored the gender-specific effects of methadone, a synthetic opioid receptor agonist commonly used in opioid addiction treatment, on renal tissue and function. We aimed to elucidate the underlying mechanisms involving inflammatory pathways and redox system activity. METHODS: Forty-two Wistar rats (200-250 g) were allocated into six groups: three males and three females, each comprised of control, and methadone-treated 5 mg/kg and 20 mg/kg. Over eight weeks, animals received either saline or methadone syrup orally. Blood urea nitrogen (BUN) and serum creatinine (sCr) w

[Multiple spontaneous coronary artery dissections: a multidisciplinary challenge in diagnosis and treatment].

Spontaneous coronary artery dissection (SCAD) is a rare but increasingly recognized cause of acute coronary syndrome, primarily affecting young women without traditional cardiovascular risk factors. We report the case of a 57-year-old woman with obesity and a history of smoking, admitted for acute respiratory failure due to pneumonia, complicated by cardiac arrest. Elevated troponin levels, along with subsequent ECG and echocardiographic findings suggestive of previous myocardial ischemia, led to the indication for coronary angiography. The angiogram revealed severe three-vessel coronary disea

DISSECTING THORACIC AORTIC ANEURYSM WITH CLASSIC IMAGING FINDINGS AND REVIEW OF LITERATURE.

BACKGROUND: Thoracic aortic aneurysm is an increasingly recognized condition that is sometimes diagnosed incidentally on imaging examinations performed to evaluate other unrelated conditions. Symptomatic presentations of thoracic aortic aneurysms may be due to mass effect on the airway, esophagus, recurrent laryngeal nerve and the thoracic spine. Alternatively, they may present with the dreaded complication of aortic dissection or rupture.Occasionally abnormalities of the aortic contour or size can be detected on routine chest x-ray. However, it is difficult to confidently diagnose thoracic ao

Surgical treatment of intramyocardial dissecting hematoma-a case report and literature review.

BACKGROUND: Intramyocardial dissecting hematoma (IDH) is a rare but potentially life-threatening complication, most commonly linked to myocardial infarction (MI) but also described in other settings such as cardiac interventions or severe chest trauma. Owing to the limited data on its diagnosis, management, and outcomes, treatment strategies for IDH remain controversial. CASE PRESENTATION: We present a case of a 53-year-old female who was initially misdiagnosed with Takotsubo cardiomyopathy and later found to have an IDH complicated by anticoagulation dilemmas and infection. Despite initial c

Safety and feasibility of aortic crossclamp for thrombosed acute type A aortic dissection.

OBJECTIVE: Aortic crossclamping in acute type A aortic dissection surgery facilitates proximal procedures during systemic cooling and reduces cardiopulmonary bypass time. However, concerns exist about embolization risk when crossclamping patients with thrombosed false lumen. We compared safety and feasibility of ascending aortic crossclamping between thrombosed and patent false lumen in acute type A aortic dissection. METHODS: This single-center retrospective study analyzed 1550 consecutive acute type A aortic dissection patients undergoing surgery between January 2004 and December 2023. Of t

Dissecting Daboia: Investigating synergistic effects of Russell's viper venom toxins.

Snakebite envenomation remains a life-threatening public health challenge in developing regions of the world. In India, Russell's viper (Daboia russelii) is responsible for the highest number of snakebite-inflicted mortality and morbidity. Diverse toxins have been reported in D. russelii venoms. However, the precise role of these toxins and whether they function independently, additively or synergistically remains unclear. To bridge this knowledge gap, we resolved D. russelii venom into distinct fractions containing major toxins and subjected them to various in vitro and in vivo experiments. O

Impact of spontaneous recanalization of occlusive cervical artery dissection on risk of stroke.

INTRODUCTION: Occlusive cervical artery dissection (CeAD) is associated with worse patient outcome. The net clinical benefit of acute revascularization measures has to be weighed against the likelihood of spontaneous recanalization. Our aim was to assess the hitherto un-addressed impact of spontaneous recanalization on stroke risk in patients with occlusive CeAD. METHODS: MRI verified CeAD patients with initially occlusive CeAD within cohort study that did not undergo acute revascularization measures were assessed. Follow-up data derived from clinical routine and study specific assessments. O

Mortality and burden related with aortic aneurysms and dissections. The importance of information and education.

BACKGROUND: In this study we correlated changes of risk factors for cardiovascular diseases with trends of age standardized mortality rates and burden for aortic aneurysms and dissections. METHODS: We analyzed data from the Global Burden of Diseases and EUROSTAT. FINDINGS: There was a significant increase of expenditure for health from 1980 and 2019. In the period 1980-2000, despite higher health spending, age standardized mortality rates increased in almost all European countries. During the period 2000-2019, in Western European Countries and in Poland, Estonia, Latvia, Slovenia there was a
