Drug resistance in glioblastoma: Challenges, mechanisms and therapeutic strategies (Review).

Glioblastoma multiforme (GBM), the most aggressive and prevalent type of brain cancer, presents a formidable therapeutic challenge due to its high degree of drug resistance. Despite advances in chemotherapy, targeted therapy and immunotherapy, patient outcomes remain dismal, with a 5-year survival rate of only 7%. Resistance mechanisms are multifactorial, including the restrictive nature of the blood-brain barrier (BBB), tumor heterogeneity and adaptive responses within the tumor microenvironment (TME). The BBB limits drug delivery, while efflux transporters further reduce therapeutic efficacy

Nanotechnology-based approaches for treating lysosomal storage disorders, a focus on Fabry disease.

Lysosomal storage disorders (LSDs) are a group of rare diseases in which the defect of a lysosomal protein results in a pathogenic accumulation of nonmetabolized products within the cells. The main treatment for LSDs is enzyme replacement therapy (ERT), consisting in the exogenous administration a recombinant protein to replace the defective one. Although several diseases such as Gaucher, Fabry, and Pompe are treated following this approach, ERT is limited to LSDs without severe neuronal affectation because recombinant enzymes do not cross the blood-brain barrier. Moreover, ERT shows additiona

Tuning H2S Release by Controlling Mobility in a Micelle Core.

Drug delivery from polymer micelles has been widely studied, but methods to precisely tune rates of drug release from micelles are limited. Here, the mobility of hydrophobic micelle cores was varied to tune the rate at which a covalently bound drug was released. This concept was applied to cysteine-triggered release of hydrogen sulfide (H2S), a signaling gas with therapeutic potential. In this system, thiol-triggered H2S donor molecules were covalently linked to the hydrophobic blocks of self-assembled polymer amphiphiles. Because release of H2S is triggered by cysteine, diffusion of cysteine

Liposomal Delivery Systems for Allergen-Specific Immunotherapy: From Molecular Design to Clinical Application.

Liposomal systems are commonly used in drug delivery due to their low toxicity, biocompatibility and biodegradability. In this review, the influence of physicochemical parameters of liposomes, namely size, surface charge, and lipid composition, on biodistribution, dendritic cell uptake, and the character of the induced immune response is examined. The potential of strategies such as targeting C-type lectin receptors, the combined use of toll-like receptor agonists and tolerogenic molecules, and an approach based on high-affinity antigen binding to liposomes via coiled coil-forming peptides is

System-agnostic prediction of pharmaceutical excipient miscibility via computing-as-a-service and experimental validation.

We applied computing-as-a-service to the unattended system-agnostic miscibility prediction of the pharmaceutical surfactants, Vitamin E TPGS and Tween 80, with Copovidone VA64 polymer at temperature relevant for the pharmaceutical hot melt extrusion process. The computations were performed in lieu of running exhaustive hot melt extrusion experiments to identify surfactant-polymer miscibility limits. The computing scheme involved a massively parallelized architecture for molecular dynamics and free energy perturbation from which binodal, spinodal, and mechanical mixture critical points were det

Subcutaneous administration of antibody-drug conjugates: current challenges, emerging opportunities, and future perspectives.

Antibody-drug conjugates (ADCs) are a rapidly growing class of targeted therapeutics designed to deliver potent payloads while minimizing systemic toxicity. Despite significant progress, all approved ADCs are administered intravenously (IV). In contrast, subcutaneous (SC) delivery is well established for antibodies, offering shorter administration time, improved convenience, and the potential for outpatient or home-based treatment. Extending SC delivery to ADCs is therefore attractive but introduces unique challenges related to structural complexity, linker-payload instability, and cytotoxic p

[Heart-brain interaction: mechanisms and therapeutic strategies from integrated TCM and western medicine].

Cardiovascular and cerebrovascular diseases have a high global incidence and are major causes of disability and mortality. This review integrates perspectives from both TCM and modern medicine. Based on the TCM theory of "heart-brain homology", it comprehensively analyzes the effects of cardiovascular risk factors and cerebral lesions on the heart-brain axis. A three-dimensional regulatory axis is proposed, encompassing metabolic reprogramming, inflammatory-oxidative cascade reactions, and epigenetic regulation mediated by ten-eleven translocation methylcytosine dioxygenases(TET enzy

Progress on the Application of Bortezomib and Bortezomib-Based Nanoformulations.

Bortezomib (BTZ) is the first proteasome inhibitor approved by the Food and Drug Administration. It can bind to the amino acid residues of the 26S proteasome, thereby causing the death of tumor cells. BTZ plays an irreplaceable role in the treatment of mantle cell lymphoma and multiple myeloma. Moreover, its use in the treatment of other hematological cancers and solid tumors has been investigated in numerous clinical trials and preclinical studies. Nevertheless, the applications of BTZ are limited due to its insufficient specificity, poor permeability, and low bioavailability. Therefore, in r

An Overview of the Activities of Cefiderocol Against Sensitive and Multidrug- Resistant (MDR) Bacteria.

The need for new therapeutics and drug delivery systems has become necessary owing to the public health concern associated with the emergence of multidrug-resistant microorganisms. Among the newly discovered therapeutic agents is cefiderocol, which was discovered by Shionogi Company, Japan as an injectable siderophore cephalosporin. Just like the other β-lactam antibiotics, cefiderocol exhibits antibacterial activity via cell wall synthesis inhibition, especially in Gram negative bacteria (GNB); it binds to the penicillin-binding proteins, but its unique attribute is that it crosses the peripl

Actively Targeted Magnetothermally Responsive Nanocarriers/Doxorubicin for Thermochemotherapy of Hepatoma.

Nanodrug-delivery systems modified with targeting molecules allow antitumor drugs to localize to tumor sites efficiently. CD147 protein is expressed highly on hepatoma cells. Firstly, we synthesized magnetothermally responsive nanocarriers/doxorubicin (MTRN/DOX) which was composed of manganese zinc (Mn-Zn) ferrite magnetic nanoparticles, amphiphilic and thermosensitivity copolymer drug carriers together with DOX. Then CD147-MTRN/DOX was formed with MTRN/DOX and monoclonal antibody that specifically binds to CD147 protein. It could target hepatoma cells actively and improve the DOX concentratio

Pathways to collaboration: exploring values and collaborative practice between child welfare and substance abuse treatment fields.

Although recent research has highlighted the importance of "bridging the gap" between child welfare and substance abuse treatment delivery systems, few studies examine specific factors that may facilitate such collaboration. This study examined similarities and differences in values and perceived capacity for collaboration between substance abuse and child welfare fields based on survey data from more than 350 respondents in 12 California counties. Although respondents across disciplines held similar values in some areas, such as priorities for services, significant differences between respond

Recent trends in antifungal therapy: focus on systemic mycoses.

Amphotericin B (AmB) remains the 'gold standard' for systemic antifungal therapy and newer drug delivery systems have been effectively developed for this drug. The newer azoles e.g. itraconazole, voriconazole and their derivatives are viable alternatives to AmB, and are suitable for oral administration. Combination antifungal therapy has been tried successfully. For instance, AmB plus flucytosine is preferred in cryptococcal meningitis and candidemia. A liquid preparation of itraconazole and cyclodextrin is pharmacokinetically superior to the conventional dosage form of itraconazole, and is pa

Folic Acid-Functionalized Liposomes Co-Loaded with Doxorubicin and Indocyanine Green for Combined Chemophotothermal Therapy of Cervical Cancer.

The development of multimodal combined treatment strategies employing nanomedicine delivery systems effectively addresses the drawbacks of traditional tumor treatment, such as severe side effects, poor therapeutic effects, and high susceptibility to drug resistance. Herein, a folic acid (FA)-functionalized nanoplatform (FA-LP-ICG/DOX) was designed and fabricated for combined chemophotothermal therapy of cervical cancer. Anticancer drug doxorubicin (DOX) and photothermal agent indocyanine green (ICG) are co-loaded in the core of the liposome, followed by the modification of folic acid as the ta

Programmable Metal-Organic Framework Biointerfaces Against Pathogens.

Emerging viral diseases continue to pose major challenges to global health, creating demand for materials that can support pathogen control, diagnosis, and therapy. Owing to their tunable structures and versatile biointerfaces, metal-organic frameworks (MOFs) have attracted increasing attention in anti-pathogen applications. While previous studies have often focused on individual functions such as catalysis, biosensing, or drug delivery, a broader perspective on the functional development of MOF-based systems remains limited. In this Review, we summarize recent advances in MOF-enabled pathogen

Zn(ii) and Cu(ii) complexes containing bioactive O,O-chelated ligands: Homoleptic and heteroleptic metal-based biomolecules.

Historically, many useful drugs have been developed from natural sources and their mechanisms of action deeply investigated for therapeutic applications. Recently, the interaction between pharmacologically active biomolecules and transition metal ions has opened the way to the construction of new drugs, where the unique properties of metal complexes are combined with the specific mechanisms of action of the coordinated bioligands of natural extraction. In this context, this perspective summarizes some recent research studies devoted to the development of new metal-based drugs containing Zn(ii)

Intraocular sustained drug delivery using implantable polymeric devices.

Vitreoretinal diseases involving age-related macular degeneration (AMD) are refractory to most topical or systemic drugs. The retina and the vitreous cavity have a unique position regarding pharmacokinetics in that the inner and outer blood retinal barriers separate the retina and vitreous from the systemic circulation. Eye drops achieve minimal therapeutic concentrations in the vitreoretinal tissue. Drug delivery systems are a strategy to address this. Intraocular sustained drug release using implantable devices has been investigated to treat vitreoretinal diseases. Possible targeted diseases
