Systematic post-translational modification genome wide identifies therapeutic targets for Alzheimer's disease: evidence from multi-cohort analysis.

BACKGROUND: The rapid increase in the incidence of Alzheimer's disease (AD) has raised concerns, given its profound effects on both society and the economy. Despite extensive research efforts in this area, there are no existing treatments that have the ability to change the progression of the disease. METHODS: To identify the distinct subtypes of AD, consensus clustering was employed. Following this, module genes were identified through the implementation of WGCNA. In addition, the investigation included the identification of hub genes through the application of machine learning. Ultimately,

CBr4-Catalyzed Substituent-Dependent Michael Addition/Paal-Knorr Cyclization of Indole with α,β-Unsaturated Ketones.

We report CBr4 catalyzed Michael addition of indole to α,β-unsaturated ketones for the synthesis of β-indolylketones through halogen bonding catalysis. This reaction is compatible with a diverse range of chalcones, including drug-derived chalcones containing sensitive functional groups such as amides, yielding the addition products in good yields. Additionally, 3-indolyl furanoid motifs have been synthesized through the Michael addition followed by Paal-Knorr cyclization by utilizing various unsymmetrical 1,4-enediones in a one-pot process with good yields.

Reducing the frequency of COPD exacerbations with antimicrobial therapy.

Chronic obstructive pulmonary disease (COPD) exacerbations are a leading cause of hospitalization in the United States, incurring a large financial burden on the healthcare system and placing significant strain on patient quality of life. Adding empiric antimicrobial agents, specifically macrolides, to the medical regimen of patients with severe COPD has been shown to reduce the frequency of COPD exacerbation. Although prophylactic macrolide therapy may relieve COPD burden, providers should consider the consequences of long-term antimicrobial utilization before prescribing these drugs.

Poly-L-Lactic Acid Reconstitution Technique to Reduce Needle Obstruction.

Facial lipoatrophy in patients with HIV is a well-established adverse effect of highly active antiretroviral therapy (HAART), further augmenting stigma and decreasing treatment adherence. Poly-L-lactic acid (PLLA) is approved by the US Food and Drug Administration for the correction of facial fat loss in this population. Unfortunately, needle obstruction from microparticles is a common obstacle that clinicians may encounter during this procedure. We describe a simple and effective way to mitigate this problem by utilizing a water bath to warm the filler prior to injection.

Re-exposure causing rapid onset of vancomycin induced thrombocytopenia.

Vancomycin is used for the treatment of gram-positive infections. The increasing prevalence of methicillin-resistant Staphylococcus aureus since the 1980 s has led to a significant increase in the clinical utility of vancomycin in the United States. The diagnosis of vancomycin induced thrombocytopenia (VIT) poses a challenge since it requires the exclusion of other etiologies of thrombocytopenia, such as infectious or immunologic diseases and alternative drugs. Physicians should be aware that VIT is a rare but important complication of IV vancomycin therapy and failure to recognize this diagno

Modeling Candida auris Colonization on Porcine Skin Ex Vivo.

Candida auris spreads person to person in hospitals and other healthcare facilities. The heightened capacity for C. auris to colonize skin contributes to the difficulty in eradicating this drug-resistant and deadly pathogen in nosocomial settings. Models for the study of C. auris skin colonization are critical for understanding this virulence trait. In light of the similarities between the skin properties of humans and pigs, pigs represent an ideal model for the investigation of skin-C. auris interactions. Here, we describe how to utilize porcine skin for ex vivo studies of C. auris colonizati

CuH-Catalyzed Regio- and Enantioselective Hydrocarboxylation of Allenes: Toward Carboxylic Acids with Acyclic Quaternary Centers.

We report a method to prepare α-chiral carboxylic acid derivatives, including those bearing all-carbon quaternary centers, through an enantioselective CuH-catalyzed hydrocarboxylation of allenes with a commercially available fluoroformate. A broad range of heterocycles and functional groups on the allenes were tolerated in this protocol, giving enantioenriched α-quaternary and tertiary carboxylic acid derivatives in good yields with exclusive branched regioselectivity. The synthetic utility of this approach was further demonstrated by derivatization of the products to afford biologically impor

Simple and Clean Photoinduced Aromatic Trifluoromethylation Reaction.

We describe a simple, metal- and oxidant-free photochemical strategy for the direct trifluoromethylation of unactivated arenes and heteroarenes under either ultraviolet or visible light irradiation. We demonstrated that photoexcited aliphatic ketones, such as acetone and diacetyl, can be used as promising low-cost radical initiators to generate CF3 radicals from sodium triflinate efficiently. The broad utility of this strategy and its benefit to medicinal chemistry are demonstrated by the direct trifluoromethylation of unprotected bidentate chelating ligand, xanthine alkaloids, nucleosides, an

[Effective "home remedies" in orthostatic hypotension. A glass of water before getting up and a brick under the bed].

The symptoms of orthostatic hypotension can be considerably improved by non-pharmacological treatment, and patients with such symptoms should not initially be given medication. Many patients, for example, show a pronounced increase in blood pressure shortly after drinking water, and this phenomenon can be utilized to ameliorate symptoms. In patients who respond inadequately, treatment with pharmacological agents is indicated. The most appropriate drug needs to be selected individually on the basis of its action and tolerability. All medications suitable for treating orthostatic hypotension can

Mycobacterium fortuitum epidemics after open-heart surgery.

We present the clinical and epidemiological features of Mycobacterium fortuitum epidemics involving 19 patients who underwent open-heart surgery. The source of the infection could not be identified. However, bone wax and homografts utilized at that time have been suspected. The infected patients responded poorly to antibiotic management and their courses in most cases were influenced beneficially by total sternectomy and transplantation of the omentum into the mediastinum. The emergence of M. fortuitum may represent an aggressive bacterial strain resistant to presently used broad-spectrum anti

Aberrant PI3Kδ splice isoform as a potential biomarker and novel therapeutic target for endocrine cancers.

INTRODUCTION: PI3K/AKT signaling pathway is upregulated in a broad spectrum of cancers. Among the class I PI3Ks (PI3Kδ/β/δ isoforms), PI3Kδ has been implicated in hematologic cancers and solid tumors. Alternative splicing is a post-transcriptional process for acquiring proteomic diversity in eukaryotic cells. Emerging evidence has highlighted the involvement of aberrant mRNA splicing in cancer development/progression. METHODS: Our previous studies revealed that PIK3CD-S is an oncogenic splice variant that promotes tumor aggressiveness and drug resistance in prostate cancer (PCa). To further e

An Integrative Approach for Improved Assessment of Cardiovascular Safety Data.

Cardiovascular adverse effects in drug development are a major source of compound attrition. Characterization of blood pressure (BP), heart rate (HR), stroke volume (SV), and QT-interval prolongation are therefore necessary in early discovery. It is, however, common practice to analyze these effects independently of each other. High-resolution time courses are collected via telemetric techniques, but only low-resolution data are analyzed and reported. This ignores codependencies among responses (HR, BP, SV, and QT-interval) and separation of system (turnover properties) and drug-specific prope
