Clinical and therapeutic features of 13 adult patients, diagnosed of Pneumonia Varicellosa are retrospectively reviewed, for the last ten years period, at Valle de Hebron General Hospital, Barcelona. Most of them had a cigarette consumption of over 20 per day, three of them showed simple chronic bronchitis criteria, and two had antibodies against HIV. Respiratory symptomatology, and dry cough, was present in 9 (75%), dyspnea in 7 (53%) and pleuritic pain in 6 (46%). Thorax radiology showed a bilateral interstitial pattern. IV aciclovir treatment was begun in patients with respiratory symptoms

Two male patients ages 54 and 58 years had persisting pneumonia with dry cough, dyspnea, weight loss, and fever up to 39 degrees C that did not respond to erythromycin treatment. There was extensive restrictive impairment of ventilation and loss of diffusing capacity for carbon monoxide. Histologic examination of the basal pulmonary infiltrates showed fibrosing alveolitis. Serologic titers indicated that the patients had suffered from Legionella pneumophila infection. We believe that Legionella had caused the fibrosing alveolitis since there was absence of any other causative agents or factors

A 68-year-old woman was admitted with cough and dyspnea. Her chest X-ray showed right pleural effusion and a mediastinal tumor. She underwent mediastinotomy following a preoperative diagnosis of invasive thymoma. A tumor originating from the thymus had invaded the right middle lobe and pericardium, and multiple pleural dissemination was also found. Therefore, considering the patient's age and pulmonary function, we performed only subtotal resection of the tumor. The pathological diagnosis was poorly differentiated squamous cell carcinoma. The patient received irradiation and chemotherapy inclu

We describe the cases of three women with pulmonary involvement in the eosinophilia-myalgia syndrome. The illness was characterized by elevated peripheral blood eosinophil counts, myalgias, fatigue, and dyspnea. Two of three patients had bilateral infiltrates on chest roentgenograms. All three had markedly decreased carbon monoxide diffusing capacities and pulmonary hypertension. High-dose prednisone therapy provided only partial resolution of the pulmonary symptoms. Open lung biopsy specimens showed chronic interstitial and perivascular infiltrates in two of the patients and moderate fibroint

A 65-year-old man was admitted to our hospital complaining of productive cough, dyspnea and stridor. Chest X-ray disclosed overinflation with micronodular infiltrates. Blood examination showed mild eosinophilia and IgE elevation. Pulmonary function test disclosed severe airway obstruction and diffusion capacity impairment. Although clinical improvement was achieved after bronchodilator therapy, laboratory abnormalities continued. Open lung biopsy demonstrated mononuclear cellular and eosinophilic infiltration at alveolar lumen and vessel walls without prominent fibrosis, which was compatible f

It is known that patients suffering from severe cardiomyopathy may develop cyclic changes in breathing (Cheyne-Stokes-breathing) (2, 3). Coughing and dyspnea may be linked to periodic breathing. Specific detailed polysomnographic studies of sleep architecture and oxygen saturation have not been published. Eight patients suffering from dilatative cardiomyopathy (NYHA III-IV) were studied by pulse oximetry and polysomnography. Six of eight patients had severe breathing irregularities. These disturbances became manifest partially as Cheyne-Stokes breathing, partially as central sleep apnea. Durin

A case of chronic eosinophilic pneumonia (CEP), confirmed by transfibroptic bronchoscope lung biopsy, was reported. CEP is a chronic interstitial lung disease characterized by cough, dyspnea, malaise, fever, night sweats, weight loss, variable degree of blood eosinophilia and film revealing peripheral nonsegmental nonmigratory infiltrates. It usually responds dramatically to corticosteroids. The authors suggested that a tentative diagnosis is often made on clinical grounds only without biopsy confirmation and corticosteroid therapy is instituted. It is necessary to perform lung biopsy for esta

Pneumomediastinum cases admitted to our Respiratory Intensive Care Unit during the past 15 years are reviewed. After excluding secondary and iatrogenic pneumomediastinum, spontaneous pneumomediastinum was diagnosed in seven patients being all of them males. The causative conditions were status asthmaticus in four patients, intense cough in two and Valsalva manoeuver in one patient. The most frequent symptoms were dyspnea, chest pain and odynophagia. Subcutaneous emphysema appeared in six patients and there was associated pneumothorax in two cases. ECG was normal in six patients. All cases of s

Although much research has focused on the pathophysiology of panic attacks, little work has been done to describe the phenomenon itself. Twenty-one patients with panic attacks were asked to sequence the panic-related symptoms during an attack in an attempt to clarify the phenomenon. Overall, panic symptoms could be grouped into three categories: early symptoms--consisting of dyspnea, palpitations, chest discomfort, and hot flashes; intermediate symptoms--including shaking, choking, feelings of unreality, sweats, faintness, and dizziness; late symptoms-consisting of fear and paresthesias. A diabetic patient presented with symptoms and laboratory findings compatible with atypical pneumonia. Despite appropriate antibiotic therapy, his dyspnea, arterial oxygenation, and chest roentgenographic findings worsened. Because the patient had a history of homosexual contact, an open lung biopsy was obtained to morphologically define the tissue reaction and to search for a specific etiology. Histologic examination showed bronchiolitis obliterans but did not demonstrate a cause. Legionnaires' disease was subsequently diagnosed on the basis of a fourfold rise in indirect fluorescent antibody

Of 78 apparently healthy subjects with positive serological tests for Chagas disease, 29 had EKG abnormalities. On direct questioning only two referred dyspnea, one had palpitations and another one dizziness. The most frequent EKG abnormalities were: peaked T waves in 10 cases; ventricular premature beats in 8 patients, right bundle branch block in 7, left bundle branch block in 5, abnormal ST-T segment in 4, atrial fibrillation in 3 cases, left ventricular hypertrophy in 3, and left anterior hemiblock in 3. As has been previously recognized, the EKG changes plus the positive serological tests

The adult respiratory distress syndrome (ARDS) is an extreme form of noncardiogenic pulmonary edema associated with alveolar-capillary damage. Clinical features include acute respiratory distress, dyspnea and tachypnea, severe hypoxemia refractory to oxygen therapy, and diffuse bilateral pulmonary infiltrates. Any number of serious disorders can cause ARDS, but the processes leading to the alveolar permeability defect are not understood. Therefore, therapy remains nonspecific and supportive. Treatment includes positive end-expiratory pressure, careful fluid management, steroid therapy, and ade

Histoplasma mediastinitis with complete or partial pulmonary artery obstruction due to compression and/or intraluminal granuloma was diagnosed in five patients and surgically verified in two. The patients, ages 12-27 years, had cough, dyspnea (four cases), and hemoptysis (two cases). Radionuclide imaging showed unilateral absence of pulmonary perfusion and minimal diminution of ventilation. Differentiation of this inflammatory process from other causes of ventilation-perfusion mismatch, for example, congenital hypoplasia and certain acquired causes of arterial obstruction, especially thromboem

A case of Bjork-Shiley mitral valve dysfunction is presented. The patient has not responded to anticoagulant therapy and had hypotension, dyspnea, chest pain, and a pulse deficit but normal sinus rhythm. Simultaneous echocardiogram, ECG, and arterial pulse tracing were used as noninvasive means of monitoring. Nonsurgical correction of a clinical emergency restored the patient to prior normal baseline cardiovascular function. This case illustrates the possibility of restoring normal prosthetic function by supporting the patient medically while undertaking diagnostic testing and arranging surgic

Silicosis is an occupational pulmonary disease caused by prolonged inhalation of crystalline silica particles. Although it predominantly affects thoracic structures, extrapulmonary or systemic manifestations are rare. We report a 60-year-old male who worked as a grinder in a quartz mill for 7 years and presented with advanced dyspnea and chronic cough. Imaging revealed bilateral progressive massive fibrosis and radiodense nodules in the spleen. Splenic involvement, also known as splenic silicosis, is an exceptionally rare entity and was detected incidentally during radiological evaluation. Thi

Pneumopericardium is an uncommon condition characterized by the presence of air within the pericardial sac and may rarely progress to cardiac tamponade. A patient with a history of Roux-en-Y gastric bypass presented with chest pain, dyspnea, and hemodynamic instability. Imaging demonstrated a large pneumopericardium with tamponade physiology. Further evaluation revealed a rare jejunopericardial fistula as the underlying etiology, resulting in communication between the gastrointestinal tract and pericardial space. Emergent pericardial decompression was performed with clinical improvement. This

Pulmonary ossification is the histologic finding of mature bone with or without marrow elements within pulmonary tissue. The mechanism of formation is incompletely understood but thought to include metaplastic transformation of pulmonary fibroblasts to osteoblasts secondary to some form of chronic insult and the resulting inflammation from this insult. There are two classifications of pulmonary ossification, nodular and diffuse, based on histologic features. Management of pulmonary ossification is typically supportive, with treatment of the underlying process if one is identified. Here, we des

Atrioventricular septal defect (AVSD) with cor triatriatum sinister (CTS) is a rare congenital anomalous combination. Few cases of AVSD and CTS have been reported in the literature. However, an isolated case of CTS with partial AVSD, left superior vena cava, and unroofed coronary sinus in an adult has not been reported in the literature. A 20-year-old male, a known case of partial AVSD with CTS, presented with dyspnea on exertion. The patient was surgically managed after a thorough preoperative evaluation and had an uneventful postoperative course. Meticulous evaluation with multiple imaging i

We report the case of a 68-year-old man who presented with pleural effusion and dyspnea during treatment for alcoholic liver cirrhosis. The pleural effusion was transudative and it was diagnosed to be hepatic pleural effusion. Thoracic drainage was continued, but the amount of pleural fluid drainage did not decrease. The patient was strongly positive for antinuclear antibodies and anti-SS-A and anti-SS-B antibodies, thus leading us to suspect pleurisy caused by Sjögren's syndrome. Steroid administration decreased pleural drainage; however, cirrhosis progressed and the patient eventually died.

BACKGROUND: Achondroplasia is the most common short stature skeletal dysplasia, but there are few reports of cardiac surgical intervention in these patients.
CASE SUMMARY: We present a case of mitral valve (MV) repair for mitral regurgitation in a patient with achondroplasia. A 55-year-old man with achondroplasia and obstructive sleep apnea presented with acute-onset dyspnea. Echocardiography demonstrated severe mitral regurgitation with a flail posterior leaflet. He underwent uncomplicated MV repair with artificial cords and an annuloplasty.
DISCUSSION: MV repair is feasible and can be stra
