An epidemic of occupational allergy to latex involving health care workers.

IgE-mediated sensitivity to natural rubber latex is being recognized more frequently among health care workers. Between January 1990 and June 1993, we evaluated 342 consecutive Mayo Medical Center employees who reported symptoms suggestive of latex allergy. All were interviewed and underwent puncture skin testing with extracts of rubber gloves. In some cases, latex-specific IgE antibodies were measured by immunoassay. One hundred four of the 342 employees evaluated (30%) were latex-allergic. Risk factors for sensitization included frequent use of disposable gloves, presence of prior atopic dis

A Very Early Rehabilitation Trial after stroke (AVERT): a Phase III, multicentre, randomised controlled trial.

BACKGROUND: Mobilising patients early after stroke [early mobilisation (EM)] is thought to contribute to the beneficial effects of stroke unit care but it is poorly defined and lacks direct evidence of benefit. OBJECTIVES: We assessed the effectiveness of frequent higher dose very early mobilisation (VEM) after stroke. DESIGN: We conducted a parallel-group, single-blind, prospective randomised controlled trial with blinded end-point assessment using a web-based computer-generated stratified randomisation. SETTING: The trial took place in 56 acute stroke units in five countries. PARTICIPANT

Analysis and comparative study on the influencing factors of bronchopulmonary dysplasia in premature infants with gestational age ≤32 weeks based on logistic regression and decision tree models.

BACKGROUND: Bronchopulmonary dysplasia (BPD), as a common and serious complication in premature infants, has a high mortality rate, and the prognosis of most premature infants with BPD is poor. Early prediction and intervention treatment can improve the prognosis of children to a certain extent. This study aimed to explore the risk factors affecting BPD in premature infants with gestational age ≤32 weeks, provide scientific basis for the prevention and control of BPD. METHODS: We retrospectively collected clinical data from 478 premature infants with gestational age ≤32 weeks admitted to the

Supervised Machine Learning Models for Predicting Sepsis-Associated Liver Injury in Patients With Sepsis: Development and Validation Study Based on a Multicenter Cohort Study.

BACKGROUND: Sepsis-associated liver injury (SALI) is a severe complication of sepsis that contributes to increased mortality and morbidity. Early identification of SALI can improve patient outcomes; however, sepsis heterogeneity makes timely diagnosis challenging. Traditional diagnostic tools are often limited, and machine learning techniques offer promising solutions for predicting adverse outcomes in patients with sepsis. OBJECTIVE: This study aims to develop an explainable machine learning model, incorporating stacking techniques, to predict the occurrence of liver injury in patients with

Identification of prognostic factors associated with contemporary lifestyles in community-dwelling young stroke survivors.

BACKGROUND: Over the past few decades, the global incidence of stroke among young adults (aged 18-50 years) has risen significantly. Given their prolonged post-stroke life expectancy, this trend has resulted in a substantial burden of long-term disability. Beyond traditional vascular prognostic factors, unhealthy modern lifestyles are increasingly contributing to this trend. METHOD: This study enrolled community-dwelling adults from three communities in Guangzhou, who were stratified into a young adult group (aged 18-49 years) and an older adult group (aged ≥50 years). Baseline questionnaires

Navigating Adulthood with Maple Syrup Urine Disease: Patient and Caregiver Perspectives on Healthcare Transition and Independent Living.

BACKGROUND: Medical advances in diagnosis and management have increased the life expectancy of many pediatric-onset rare diseases, including inherited metabolic disorders (IMDs). Historically fatal childhood conditions are now manageable chronic illnesses, with over 90% of individuals with IMD reaching adulthood. As more individuals with IMDs reach adulthood (age 18 years), there is a new imperative for health care transition planning that promotes developmentally-appropriate skills for medical self-management and independent living. Knowledge gaps on the long-term disease-related outcomes lim

From Adolescence to Adulthood: Understanding Care Trajectories in an Early Detection and Intervention Centre in France.

BACKGROUND: Psychiatric disorders often emerge during adolescence or young adulthood, leading to significant disability among youth. The transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Mental Health Services (AMHS) is critical for individuals experiencing emerging psychopathology, with delayed access to care negatively impacting long-term outcomes. Accessing mental health services for adolescents and young adults is often complex and delayed due to challenges in service visibility, accessibility and appropriateness. METHODS: This study examines the care trajectori

Unmasking the Rarity: A Case Report on Platypnea-Orthodeoxia Syndrome With Successful Resolution Through Patent Foramen Ovale Closure.

Platypnea-orthodeoxia syndrome (POS) is an uncommon yet clinically significant medical phenomenon characterized by dyspnea, a distressing symptom manifesting as breathlessness upon assuming an upright position, which notably improves upon reclining. In stark contrast to orthopnea, where dyspnea worsens in a supine position, POS uniquely presents with decreased blood oxygen saturation upon transitioning from lying down to standing up. This syndrome poses diagnostic challenges due to its subtle symptomatology and requires a high index of clinical suspicion for accurate identification. Herein, we

A Rare Presentation of Hepatolithiasis and Acute Cholangitis: A Case Report Highlighting Heterotaxy Syndrome With Left Isomerism.

Heterotaxy syndrome (HS) is a rare congenital disorder characterized by the atypical arrangement of thoracic and abdominal organs. This complex condition is challenging to diagnose due to its ambiguous classification system and the wide spectrum of clinical presentations. HS can be subcategorized into left or right isomerism, with left isomerism typically associated with less severe cardiac malformations but still carrying significant morbidity due to other organ system anomalies. We present the case of a 50-year-old male with a history of diabetes mellitus, hypertension, chronic kidney diseas

Feasibility of implementing a mobile phone-based telemonitoring programme to support pregnant women at high-risk for pre-eclampsia in Karachi, Pakistan: protocol for a mixed-methods study.

INTRODUCTION: High maternal mortality from pre-eclampsia/eclampsia results from lack of early identification and management of pregnant women at high-risk for pre-eclampsia. A potential tool to support pregnant women at high-risk for pre-eclampsia is telemonitoring. There is limited evidence on the use and effectiveness of telemonitoring for pregnant women in low-income and middle-income countries (LMICs) which limits the understanding of the process and mechanisms through which the intervention works in LMICs. This study will explore the feasibility of implementing a mobile phone-based telemo

The Ubiquitous Premature Ventricular Complex.

Premature ventricular complexes (PVCs) are one of the most commonly encountered arrhythmias and are ubiquitous in clinical practice, both in the outpatient and inpatient settings. They are often discovered incidentally in asymptomatic patients, however, can cause myriad symptoms acutely and chronically. Long thought to be completely benign, PVCs have been historically disregarded without pursuing any further evaluation. Newer data have revealed that a high burden of PVCs with specific characteristics can significantly increase a patient's risk of developing PVC-induced cardiomyopathy. The aim
