Stimulation threshold greatly affects the predictive value of intraoperative nerve monitoring.

OBJECTIVES/HYPOTHESIS: Using a standardized, graded, intraoperative stimulation protocol, we aimed to delineate the effects of various stimulation levels applied to the recurrent laryngeal nerve on the postoperative predictive value of intraoperative nerve monitoring. STUDY DESIGN: A total of 917 nerves at risk were included for analysis. Intraoperatively, patients underwent stimulation of the recurrent laryngeal nerve at 0.3, 0.5, 0.8, and 1.0 mA followed by postoperative laryngoscopy for correlation with intraoperative findings. METHODS: Sensitivity, specificity, positive predictive value,

Neorectal hyposensitivity after neoadjuvant therapy for rectal cancer.

BACKGROUND AND PURPOSE: Preoperative radiotherapy for rectal cancer has a detrimental effect on long-term anorectal function and quality of life, additional to that observed after rectal resection. The exact physiological mechanisms for the excess impairment remain unknown. We aimed to investigate neorectal and anal sphincter properties in patients treated with neoadjuvant therapy (NT) prior to total mesorectal excision (TME). MATERIAL AND METHODS: Sixteen patients (NT+ patients) were examined by multimodal neorectal stimulation and standard anorectal physiological testing. Data were compared

[New data for the pathomechanism of neuropathic pain: therapeutic evidences].

The present work is based on literature data from PubMed. Neuropathic pain is caused by a lesion or disease of the somatosensory system. Peripheral and central sensitization play a crucial role in its pathomechanism. The clinical symptoms are mainly characterized by burning and throbbing pain and sensory disturbances like hyperalgesia and allodynia. Therapeutic recommendations are antidepressants, antiepileptics, opioids and neuro-stimulation methods.

Exploratory study of perceived quality of life with implanted standing neuroprostheses.

Individuals with spinal cord injury (SCI) need options for negotiating architectural barriers, completing essential transfers, and accessing items on high shelves or in cupboards that cannot be reached from the wheelchair or safely managed with reachers. Case Western Reserve University (CWRU) and the Department of Veterans Affairs (VA) have developed an assistive technology device to assist individuals with SCI to stand and transfer. The 8-channel implanted CWRU-VA system enables persons with SCI to exercise, stand, and maneuver in the vicinity of their wheelchairs. Interventions that decrease

Reinnervation of urethral and anal sphincters with femoral motor nerve to pudendal nerve transfer.

AIMS: Lower motor neuron damage to sacral roots or nerves can result in incontinence and a flaccid urinary bladder. We showed bladder reinnervation after transfer of coccygeal to sacral ventral roots, and genitofemoral nerves (L1, 2 origin) to pelvic nerves. This study assesses the feasibility of urethral and anal sphincter reinnervation using transfer of motor branches of the femoral nerve (L2-4 origin) to pudendal nerves (S1, 2 origin) that innervate the urethral and anal sphincters in a canine model. METHODS: Sacral ventral roots were selected by their ability to stimulate bladder, urethra

Thiamylal antagonizes the inhibitory effects of dorsal column stimulation on dorsal horn activities in humans.

In humans, peripheral somatosensory information converges upon dorsal horn neurons in the spinal cord, which can be recorded from the dorsal epidural space as spinal cord potentials (SCPs) following segmental dorsal root stimulation (SS) employing epidural catheter electrodes. Antidromic action potentials and descending inhibition from the dorsolateral funiculus may contribute to SCPs following dorsal column stimulation (DCS). Effects of thiamylal (2.5-7.5 mg/kg, i.v.) on SCPs evoked by independent DCS or SS were compared with those evoked by simultaneous DCS and SS (DCS/SS). DCS- and SS-evoke

What is the evidence on efficacy of spinal cord stimulation in (subgroups of) patients with critical limb ischemia?

The use of spinal cord stimulation (SCS) has been advocated for the management of ischemic pain and the prevention of amputations in patients with inoperable critical limb ischemia (CLI), although data on benefit are conflicting. Several reports described apparently differential treatment effects in subgroups. The purpose of this study was to analyze the data on the efficacy of SCS and to clarify preselection issues. Five randomized trials have been performed with a total number of 332 patients. Primary outcome measures were mortality and limb survival. In the largest multicenter randomized tr

Spinal cord stimulation: "neural switch" in complex regional pain syndrome type I.

INTRODUCTION: Complex regional pain syndrome type I (CRPS I) is a neuropathic pain disorder of unclear etiology. It commonly follows a trivial injury and is characterized by spontaneous pain manifesting regionally that is disproportionate to the inciting event. Associated signs and symptoms include allodynia, hyperalgesia, edema, sudomotor, vasomotor abnormalities, and trophic changes. Although multiple modalities exist to treat CRPS I, significant disability, diminution in quality of life, and reduction in overall health often accompany the syndrome. CASE: A case of a 57-year-old man with CR

Epidural spinal cord stimulation plus quipazine administration enable stepping in complete spinal adult rats.

We hypothesized that epidural spinal cord stimulation (ES) and quipazine (a serotonergic agonist) modulates the excitability of flexor and extensor related intraspinal neural networks in qualitatively unique, but complementary, ways to facilitate locomotion in spinal cord-injured rats. To test this hypothesis, we stimulated (40 Hz) the S(1) spinal segment before and after quipazine administration (0.3 mg/kg, ip) in bipedally step-trained and nontrained, adult, complete spinal (mid-thoracic) rats. The stepping pattern of these rats was compared with control rats. At the stimulation levels used,

The potential of electrical stimulation to promote functional recovery after peripheral nerve injury--comparisons between rats and humans.

The declining capacity for injured peripheral nerves to regenerate their axons with time and distance is accounted for, at least in part, by the chronic axotomy of the neurons and Schwann cell denervation prior to target reinnervation. A largely unrecognized site of delay is the surgical suture site where, in rats, 4 weeks is required for all neurons to regenerate their axons across the site. Low frequency stimulation for just 1 h after surgery accelerates this axon crossing in association with upregulation of neurotrophic factors in the neurons. We translated these findings to human patients

Effects of somatosensory electrical stimulation on neuronal injury after global hypoxia-ischemia.

Electrical stimulation (ES) is used after cardiac arrest (CA) for diagnostic and therapeutic purposes. The effects of ES on brain damage induced by hypoxic-ischemic brain injury (HI) has not been investigated. Stimulation of afferent pathways by ES may increase neural injury by releasing excitatory neurotransmitters (glutamate) and thereby exacerbating excitotoxicity. To test this hypothesis, ES was applied to the median nerve (2 h) of adult male Wistar rats after 5 min of asphyxic CA and cardiopulmonary resuscitation. Control animals received no ES. Assessment of neuronal damage in five regio

Initiating extension of the lower limbs in subjects with complete spinal cord injury by epidural lumbar cord stimulation.

We provide evidence that the human spinal cord is able to respond to external afferent input and to generate a sustained extension of the lower extremities when isolated from brain control. The present study demonstrates that sustained, nonpatterned electrical stimulation of the lumbosacral cord--applied at a frequency in the range of 5-15 Hz and a strength above the thresholds for twitches in the thigh and leg muscles--can initiate and retain lower-limb extension in paraplegic subjects with a long history of complete spinal cord injury. We hypothesize that the induced extension is due to toni

Epidural spinal cord stimulation in the treatment of severe peripheral arterial occlusive disease.

Epidural spinal cord stimulation (ESCS) has been suggested to improve microcirculatory blood flow and reduce amputation rates in patients with severe peripheral arterial occlusive disease (PAOD). Pain relief, limb salvage, and skin circulation were studied in 177 patients with ischemic pain caused by nonreconstructible PAOD who were receiving ESCS. Medical or surgical therapy had failed and vascular reconstruction was impossible in all cases. Clinical status was classified as Fontaine's stage III (chronic ischemic rest pain) in 114 patients and Fontaine's stage IV (ischemic pain and ulcers or

Overuse syndromes in baseball, tennis, gymnastics, and swimming.

The shoulder is the "center of action" for most sports including those discussed in this article. Overuse problems of the shoulder are the most common group of injuries affecting the athlete involved in baseball, tennis, swimming, and gymnastics. Most of the injuries can be directly related to the mechanics of throwing, swimming, or gymnastics. After diagnosis, treatment is directed at "selective rest" of the injured shoulder and modalities and medications intended to decrease the inflammatory response of the body to stress.

Clinical efficacy and mechanisms of transcutaneous auricular vagus nerve stimulation targeting the gut-brain axis for postoperative complications of aortic dissection: study protocol for a randomized controlled trial.

BACKGROUND: Aortic dissection (AD) is a life-threatening cardiovascular emergency characterized by rapid onset and high mortality. While surgery intervention, the primary treatment, improves short-term survival, it frequently leads to postoperative complications including systemic inflammatory response syndrome, gastrointestinal dysfunction, and anxiety/depression. These complications may be exacerbated by dysregulation of the gut-brain axis (GBA). Transcutaneous auricular vagus nerve stimulation (taVNS) is a non-invasive neuromodulation technique known to exert anti-inflammatory, prokinetic,

Efficacy of community-based respiratory neuromuscular electrical stimulation on exercise capacity and quality of life in stable COPD: a randomized multicenter parallel-controlled trial protocol.

BACKGROUND: Chronic Obstructive Pulmonary Disease (COPD) is a common and prevalent condition that poses a significant threat to human health. Respiratory muscle fatigue is one of the common clinical manifestations of COPD. Currently, no effective treatment has been proposed to alleviate COPD symptoms. Respiratory neuromuscular electrical stimulation (RNES) enhances diaphragmatic contraction, lung volume, and ventilation through selective activation of type II muscle fibers, as evidenced in neurological and respiratory critical care settings. Although many COPD patients are managed through comm
