Effectiveness and Safety of a Supplement Containing a Pharmacologically Active Basidiomycete Mushroom for Chronic Fatigue and Post-COVID-19 Fatigue Syndrome: Protocol for a Randomized Controlled Trial.

BACKGROUND: Chronic fatigue syndrome, or myalgic encephalomyelitis, is characterized by persistent, unexplained exhaustion unalleviated by rest, with a pathophysiology distinct from underlying medical conditions. It is diagnostically complex due to symptoms that overlap with other disorders and the absence of definitive biomarkers, contributing to limited therapeutic options in current medicine. Lingzhi, a pharmacologically active basidiomycete mushroom, has been empirically used in traditional Chinese medicine for two millennia. This study has a dual focus: (1) systematically evaluating the e

The consultant's guide: evidence-based digital transformation framework for healthcare institutions in developing and resource-constrained economies.

BACKGROUND: Digital transformation is increasingly reshaping healthcare delivery worldwide; however, the adoption of digital health systems in developing and resource-constrained economies remains constrained by infrastructural limitations, poor alignment with clinical workflows, inadequate user involvement, and resistance to organizational change. Many digital healthcare initiatives in these settings continue to experience implementation and sustainability challenges, partly due to the absence of context-specific and evidence-based transformation models. Consequently, this study aimed to deve

Psychopathological aspects of dysphagia: a systematic review on correlations with eating disorders and other psychiatric conditions.

BACKGROUND: The effect of psychopathology on swallowing ability tends to be an overlooked issue in the assessment of dysphagic patients, possibly overshadowed by the given prominence to organic pathologies and the difficulties on the management of these patients. In addition, it should also be kept in mind that a great number of psychotropic drugs can affect swallowing adding problematic clinical issues in this area. Despite this, assessment of dysphagia should be considered as an extremely important issue, due to its impact on basic symptomatology, course of illness and quality of life. OBJE

"You have to keep your nerve on a DMC." Challenges for Data Monitoring Committees in neonatal intensive care trials: Qualitative accounts from the BRACELET Study.

BACKGROUND: Data Monitoring Committees (DMCs) are essential to the good conduct of many trials. Typically they comprise a small expert group which monitors safety, efficacy, progress and early outcome data as trials recruit. DMCs can recommend protocol revisions and early stopping of a trial. As DMC meetings usually consider unblinded interim data confidentially, their deliberations are seldom exposed to research scrutiny. Although there have been some case studies from trials from mixed specialties which offer insights into some of the common issues faced by DMCs, we have, however, little emp

Divisible Atoms or None at All? Facing the European Contributions to Developments of Chemistry and Physics in China.

One of the most important Mid-European professor with more than six thousand academic descendants was the leading Slovenian erudite Jurij Vega. In broader sense, Vega's and other applied sciences of the south of Holy Roman Empire of German Nationality were connected with the mercury mine of Idrija during the last half of millennia. The Idrija Mine used to be one of the two top European producers of mercury, the basic substance of atomistic alchemists. Idrija Mine contributions to the history of techniques, their examinations and approbations is comparable to the other Mid-European achievements

[Dissociative disorders: from Janet to DSM-IV].

I reviewed the literature on dissociation and dissociative disorders from Pierre Janet to DSM-IV, and examined the current trends in research. Janet's theory on hysteria is multifaceted, and is based on three psychological models. Based on a hierarchical model, Janet related hysteric symptoms to the activities within the lower strata of mental hierarchy (automatisms psychologiques), which were demonstrably shown in somnambulism. A second model was based on the concept of a psychological system, which was hypothetically composed of ideas, images, feelings, sensations, and movements. According t

Stratified cities, unequal education: social class, segregation, and educational trajectories in metropolitan Bogotá.

INTRODUCTION: Education in Latin America operates within deeply stratified socio-spatial contexts. Despite extensive research on educational inequality, a critical gap persists in understanding how multidimensional social class - structured through economic, cultural, and social capital - interacts with residential and school segregation to produce differentiated educational trajectories across the full schooling cycle. Drawing on Bourdieusian field theory, this study examines metropolitan Bogotá as a paradigmatic case of a highly stratified Latin American metropolis in which class and territo

Quantification of Mouse Total Body Surface Area: Implications for Preclinical Burn Research.

Clinically, burn severity is reported as the size (and depth) of burn wounds relative to total body surface area (TBSA). This nomenclature is also often used in rodent models of burns. Accordingly, accurate determination and reporting of rodent TBSA is required to ensure the rigor and reproducibility of preclinical burn research. Rodent TBSA is typically estimated indirectly as a function of body mass. Further, empirical quantification of rodent TBSA through pelt dissection does not consider differences in rodent and human anatomy, making comparison of relative burn size in rodents and humans

Clinical Decision-Making Case: Febrile Infant.

AUDIENCE: This clinical decision-making (CDM) case is intended for emergency medicine residents of all levels, medical students, and fellows preparing for standardized oral board exams. INTRODUCTION: Fever in a neonate (infant <28 days old) is a medical emergency due to the high risk of serious bacterial infections (SBIs) like meningitis, sepsis, or urinary tract infections (UTIs).1-3 Compared with older infants and children, neonates have immature immune responses, reduced ability to localize infection, and limited physiologic reserve, which contribute to rapid clinical deterioration and inc

[Evaluation of the Epidemiological, Clinical and Radiological Features of Pneumonia Cases Caused by Mycoplasma pneumoniae in Childhood].

The current study aimed to investigate the clinical, laboratory and radiological findings of the pneumonia cases in children that were confirmed as M.pneumoniae by polymerase chain reaction (PCR) testing and to reveal the factors that can be decisive in the diagnosis. Seventy-seven children were included in this study. The median age of the patients was 31 months (1 month-17 years 4 months). The 63.6% of the patients were younger than five years of age, 53.2% were girls and 46.8% were boys. During the eight-year research period, the frequency of M.pneumoniae in the patients hospitalized with t

Bereavement care guidelines used in health care facilities immediately following perinatal loss: a scoping review.

OBJECTIVE: The objective of the scoping review was to explore the evidence and describe what is known about perinatal bereavement care guidelines provided within health care facilities prior to discharge. Additionally, the review sought to identify what is known about parents' mental health outcomes, and map these outcomes to the characteristics of the bereavement care guidelines. INTRODUCTION: Perinatal loss poses a serious risk of emotional trauma and mental health sequelae. Conflicting evidence for international bereavement care guidelines and inconsistent implementation, a lack of experim

Promoting science-based prevention in communities.

In the past decade, prevention science has emerged as a discipline built on the integration of life course development research, community epidemiology, and preventive intervention trials [Am. Methodological quality of systematic reviews and meta-analyses of rheumatoid arthritis pharmacotherapy: A protocol for a meta-epidemiological study using AMSTAR 2.

BACKGROUND: With the exponential growth of systematic reviews and meta-analyses on RA pharmacotherapy, a comprehensive and up-to-date assessment of methodological quality is urgently needed to gauge the credibility of this evidence base. OBJECTIVE: This protocol describes a meta-epidemiological study that will systematically appraise the methodological quality of published systematic reviews and meta-analyses of RA pharmacotherapy (csDMARDs, bDMARDs, and tsDMARDs) using the AMSTAR 2 instrument and explore study-level factors associated with methodological quality. METHODS: We will systematic

Prevalence and determinants of inappropriate endotracheal cuff pressure in adult surgical patients undergoing general anesthesia: a prospective observational study.

BACKGROUND: Endotracheal tube (ETT) cuff pressure should be maintained between 20 – 30 cmH₂O to prevent air leak, aspiration, and tracheal injury. Cuff inflation is frequently guided by clinical judgement rather than manometry. Data on inappropriate cuff pressure prevalence and determinants in Asian surgical populations are limited. METHODS: We prospectively studied adult patients undergoing elective surgery with endotracheal intubation at a tertiary hospital. After intubation, anaesthesia providers inflated the ETT cuff per usual practice based on clinical judgement without manometry. The res

Personality Traits, Emotional Competencies and Rumination: Measuring Individual and Joint Effect on Psychological Well-Being.

Although a large body of research suggests, that rumination is a strong and consistent predictor for poor well-being, data gathered from other studies underlie possible adaptive mechanisms of rumination and speak of positive aspects of repetitive thinking. Using three-dimensional model of rumination (brooding, reflective pondering and depression-related rumination) the study explores adaptive and maladaptive styles of rumination and their psycho-emotional correlates. The present study utilizes response styles theory (RST) and explores adaptive and maladaptive rumination styles (brooding, refle

Role of baseline emotion dysregulation and perceived daily stress in adherence to and completion of narrative exposure therapy among forced migrants.

BACKGROUND: Forced migrants are at risk of developing post-traumatic stress disorder (PTSD). Narrative exposure therapy (NET) is an empirically supported treatment for forced migrants with PTSD. However, multiple barriers can make it difficult for them to consistently engage in and complete mental health treatment. Identifying factors contributing to the feasibility of NET could enhance its treatment outcomes. OBJECTIVES: The first aim was to examine whether baseline levels of perceived daily stress and emotion dysregulation were associated with NET completion. The second aim was to test whet

Toward Quantum-Informed Addiction Medicine: Coherence, Entangled-Photon Imaging, and a Theranostic Hypothesis.

BACKGROUND: Quantum biology has shown that selected biological processes depend on quantum effects, including coherence, tunnelling, and spin-dependent reactions. In parallel, quantum imaging now uses entangled photons to extract biological information at low photon flux. Early evidence that biological tissue can alter photon entanglement in disease-associated ways raises a translational question: whether decoherence itself can become a diagnostic signal. RATIONALE: Addiction is a disorder of reward-circuit function in which current imaging can define anatomy and network activity but cannot r
