Foreign body in the esophagus is not an unusual finding. Coin in children and bone fragments in adults are commonly observed foreign bodies. Usually foreign bodies are removed within few hours to few days. Forgotten or unattended foreign bodies are seen in either mentally retarded patients or in children. We are reporting here a case, having unusual foreign body in oesophagus, i.e. a thorn of babul tree since one month, presented as perioeso-phagitis with dysphagea in a mentally retarded child of 12 years.

Although adenocarcinoma of the stomach has decreased in incidence over the past several decades, cancer of the gastric cardia has increased rapidly over this time frame. There are several differences between adenocarcinoma of the cardia and distal stomach with respect to epidemiology, risk factors, and prognosis. In addition, recent data raise questions with regard to possible associations of cardia cancer with Barrett's esophagus, intestinal metaplasia of the cardia, and Helicobacter pylori. This article will review the current literature with regard to this important tumor and explore these

An unusual case of intravascular lymphomatosis caused by small noncleaved, non-Burkitt's lymphoma, which presented with adult respiratory distress syndrome, is described. Extensive invasion of the small- and medium-size blood vessels of the lung, liver, spleen, kidneys, heart, esophagus, stomach, small and large intestines, bladder, and brain-but not the bone marrow or peripheral blood-is documented. The possible mechanism and the unusual features of this case are discussed in comparison with previously reported cases. The pertinent literature is reviewed. The problem of diagnosing this pathol

The authors report a case of a two-year-old child with esophageal stricture -- caused first by caustic ingestion and by the end-to-end anastomosis, performed after the excision of the stenotic esophageal segment-- that required repeated dilations. These manouvres unfortunately led to bacteremia and a serious complication of a brain abscess, in the right fronto-parietal area, that was promptly removed. A few months later the child was subjected to an esophageal substitution with a colonic transposition. At present he is in good condition and the follow-up showed normal function of the neo-esoph

A 17-year-old young man presented with a highly unstable fracture dislocation of the third and fourth thoracic vertebrae with neurological deficit, in which the fractured spine had perforated the thoracic esophagus. Open reduction and internal fixation of the spinal fractures in combination with aggressive treatment of the mediastinitis caused by esophageal perforation, consisting of two re-thoracotomies, was performed. Two years after the accident, the patient had recovered well. The neurological deficit had recovered, and there were no difficulties with swallowing.

There is some evidence from anglo-american clinical and experimental studies that gastro-esophageal reflux is more common in patients with laryngitis. Within the framework of an open study, 32 patients with reflux esophagitis and laryngitis were treated with 20 mg omeprazole daily. After 4 weeks at the latest, in all cases inflammation of the esophagus and larynx had healed completely and the patients were without complaints. Suggesting that reflux is the underlying etiology patients with laryngitis seem to benefit from omeprazole.

Out of 81 patients with suspected caustic injury 27% showed esophageal lesions. In 5% the lesions were severe and strictures developed and dilatations were necessary. The diagnosis has to be made by endoscopy in each suspected injury to rule it out or to control its steroid therapy. Even without oropharyngeal lesions caustic injury of the esophagus is possible. Endoscopy and dilatation were made under sedation and did not hurt the patients. No complications occurred. Steroids seem to be useful only in severe caustic ingestions.

Anatomical investigations of the muscular layer of the cardia in corpses of adults (30-70 years of age) have shown the presence in the cardia of a 25-35 mm long sphincter disposed at an angle to the horizontal plane. Its formation proceeds with the participation of both the esophageal musculature (circular layer) and gastric musculature (gastro-esophageal fibers of the oblique muscular layer). As a whole, myo-architectonics of the cardia is dependent on the character of interrelation of the muscular layers of the esophagus and stomach which is responsible for the opening and closure of the gas

The authors consider that predisposing factors in the development of the Mallory-Weiss syndrome include restriction of mobility of the mucous and submucous layers of the cardia in relation to one another, uncoordinated and oppositely directed contractions of differently oriented muscle layers of the lower third of the esophagus and cardial portion of the stomach, incompetence of the cardial closing apparatus. Rupture of the wall of the stomach and duodenum is associated with an immediate dramatic change of the intragastric pressure. The endoscopic method of examination is thought to be the lea

A case of distal volvulus of the stomach as a cause of acute intestinal obstruction in a paraesophageal hernia is presented. The patient, an old woman aged 82, had been suffering from abdominal pain and vomiting for about 48 hours. She successfully underwent emergency operation for the reduction of hernia and plasty of the hiatus anterior the esophagus. On the basis of personal experience and review of literature data, stress is laid on the high incidence of paraesophageal hernia complications and the importance of early diagnosis and surgical repair is underlined.

An estimation of functional results of gastrectomy for gastric cancer in 183 patients has been made. Radical operations with the formation of Y-shaped interintestinal anastomosis and an artificial food reservoir in the area of the small intestine portion anastomozed with the esophagus were performed in 102 patients. Macroscopic signs of reflux-esophagitis were noted in 2 patients (1.96%), dumping-syndrome in 4 patients (3.9%) and hyperproteinemia in 1 patient (0.98%), i. e. the operations performed gave favourable functional results.

With electronic miniaturization, disc batteries have become increasingly popular; hence, they are ingested more frequently by children. Their small size and benign appearance belie their caustic nature. This case report illustrates the principles of management, and is the eighth report of major complications from disc battery impaction in the esophagus. We believe our patient is the youngest child reported, to date, to have suffered this injury. The pathophysiology and surgical management are discussed, and a protocol for the management of suspected disc battery ingestion is presented.

A 41-year-old man was admitted for evaluation of hemoptysis, dysphagia, and pleuritic chest pain associated with a mediastinal mass. Esophagography demonstrated a fistula between the mass and the esophagus. Results of histoplasmosis complement fixation serologic testing suggested an active infection. A methenamine silver stain of a lymph node obtained at mediastinoscopy revealed Histoplasmosis capsulatum. The patient was successfully treated with amphotericin B. This is believed to be the first reported case of an esophageal fistula as a complication of mediastinal histoplasmosis successfully

The transmural granulomatous inflammation of chronic nature which characterizes the Crohn's disease involves primarily the terminal ileum and colon. However, the upper gastrointestinal tract may also be affected by the same histopathological process, usually co-existent with or following Crohn's disease of the ileocecal region. Such cases can present a formidable diagnostic challenge if the clinical, endoscopic and radiographic findings are not correlated. This article describes and illustrates the spectrum of radiological manifestations of Crohn's disease and its associated complications invo

The authors used combined modality therapy including clinical, laboratory, x-ray, radiological, immunological and histological methods to diagnose autoimmune thyroiditis. Enlargement of the thyroid up to the 3-4 th degree, signs of compression or shift of the trachea and esophagus, the presence of nodes, progressive goiter growth and suspected malignant transformation served as indications for surgical intervention. Subfascial extirpation of the thyroid can be an operation of choice in surgical treatment of Hoshimoto's thyroiditis. A question of the time of surgical intervention cannot be cons

From January 1983 to December 1984 in the Division of Pediatric Surgery of Parma, 112 endoscopies of upper gastro-intestinal tract (UGT) were done. Besides the authors refer 49 endoscopies of the colon. Endoscopic indications of the UGT included evaluation of the esophagus in gastro-esophageal reflux, achalasia, lye ingestion, gastro-duodenal ulcer, bleedings of UGT, removal of foreign bodies. The indication of the colonoscopy included bleedings and all symptoms of colitis. The endoscopic procedures were done under sedation or in general anesthesia.

The excellent experiences with the intravascular application of Grüntzig's double-lumen balloon catheter shows his potential use in thin-walled organs. We report our experiences with the dilatation of bile-duct stenoses (end-to-end anastomosis of the Ductus choledochus, sclerosis of the Papilla Vateri, intra- and extrahepatic fibrotic stenosis), stenosis of the esophagus after acid corrosion and stenosis of the ureter anastomosis after Anderson-Hynes operation. These results demonstrate, that in selected cases the balloon dilatation may be an alternative to surgical intervention.

The author presents an analysis of the data on 94 patients with cancer of the gastric cardiac portion with and without the involvement of the esophagus, in whom radical operations (proximal gastric resection and gastrectomy) were followed by various complications. The main complications and causes of patients' death were pleuropulmonary complications and peritonitis. Peritonitis was more frequently due to the anastomosis suture incompetency. Peritonitis resulted in the postoperative mortality totally in 50% of patients. Suture incompetency of entero-esophageal anastomoses was as frequent as th

IgA deposits were observed by direct immunofluorescence in linear distribution along the basement membrane zone in a case of dermatitis herpetiformis (Duhring). In addition, in the serum of the same patient circulating IgA antibasement membrane zone antibodies were detected by indirect immunofluorescence, utilizing normal human skin and monkey esophagus as substrates. The ultrastructural localization of in vivo-bound IgA and circulating IgA antibasement membrane zone antibodies fixed to substrate tissue in vitro was found to be in the uppermost strata of the dermis below the basal lamina.

Congenital stridor was observed in a newborn due to left pulmonary artery sling. Clinical signs and the course of the disease were determined by an extreme tracheal stenosis. In newborns and infants with apneic episodes, inspiratory and expiratory wheezing and cyanosis a pulmonary sling should be considered. Unilateral hyperexpansion of the lung and an anterior indentation of the esophagus are helpful diagnostic signs. Surgical treatment for pulmonary sling is possible but remains unsatisfactory. Patients with associated tracheobronchial stenosis show a high over-all mortality rate.
