[Infections by Streptococcus pyogenes: new aspects of diagnosis, epidemiology, clinical practice, and therapy].

Essential procedures for the cultural and serological laboratory diagnosis of Streptococcus pyogenes are described. A reliable and rapid species identification is achieved by serologic or biochemical tests. Direct antigen detection tests from throat swabs have broadened the diagnostic possibilities; however, they are often not satisfactory in terms of sensitivity. Typing of strains (either by classic serologic techniques or by molecular methods) is of high value for the study of clinical and therapeutic questions. In recent years, the epidemiology of the prevalent group A streptococcal strains

Diarrhea and monkeypox: a consideration.

In addition to the well-known pox infections, novel zoonotic pox diseases have emerged as a severe concern in infectious medicine. In May 2022, monkeypox has already been reported in many countries in Europe, creating a significant public health risk possibility. Human-to-human transfer is being researched right now. As the number of reported cases in various countries climbs, the medical community is concerned, and proper preparation is required.

A Case of Fasciolosis in a 11-Year-Old Syrian Immigrant Boy.

Fasciolosis is a chronic parasitic disease caused by two species of liver trematodes, Fasciola hepatica and Fasciola gigantica, which infest ruminants and rarely humans. The disease is spread all over the world, but more common in Asia in areas with extensive livestock farming and can be imported cases in Europe and America. In this case, we describe imported infection with F. hepatica in Bulgaria from 11-year-old Syrian immigrant boy.

Validation of an Aptima-format Finnish new variant of Chlamydia trachomatis (FI-nvCT) surveillance assay, 2019.

The Finnish new variant of Chlamydia trachomatis (FI-nvCT) is escaping diagnostics in Finland, Norway and Sweden. We have developed and validated an Aptima-format nucleic acid amplification test (NAAT) designed specifically to detect the FI-nvCT. This NAAT has high sensitivity (100%) and specificity (100%) for the FI-nvCT strain, enabling further investigation of the geographic distribution, prevalence and transmission of this diagnostic-escape mutant in screening populations in Europe.

Neisseria meningitidis Serogroup X ST-5799 (ST-22 complex) in Turkey: A unique pediatric case.

Although outbreaks of Neisseria meningitidis serogroup X occured in a couple of African countries, a limited number of serogroup X meningococcal cases were reported in America and Europe as well as Turkey. Additionally, serogroup X is still not represented in current conjugated meningococcal vaccines. Here, we describe the first pediatric case with meningitis caused by Neisseria meningitidis serogroup X ST-5799 (ST-22 complex) that formed a distinct lineage.

Relapsing fever in asylum seekers from Somalia arriving in Belgium in August 2015.

Current population displacements favor the re-emerging of diseases, nowadays very rare in Europe. We report two cases of 'imported' relapsing fever in two Somali asylum seekers occurring shortly after their arrival in Belgium. A diagnosis of relapsing fever should be considered in all migrants coming from endemic regions or having traveled in low hygiene and crowded conditions, presenting with recurrent fever, especially if no malaria parasites are found.

Epizootic of vesicular disease in pigs caused by coxsackievirus B4 in the Soviet Union in 1975.

Swine vesicular disease virus (SVDV) emerged around 1960 from a human enterovirus ancestor, coxsackievirus B5 (CVB5), and caused a series of epizootics in Europe and Asia. We characterized a coxsackievirus B4 strain that caused an epizootic involving 24 488 pigs in the Soviet Union in 1975. Phylogenetic evidence suggested that the swine virus emerged from a human ancestor between 1945 and 1975, almost simultaneously with the transfer of CVB5.

Pseudo-tumor of the scrotum, a rare clinical presentation of dirofilariasis: a report of two autochtonous cases due to Dirofilaria repens.

In Europe, human dirofilariasis refers to a group of autochtonous parasitic infections caused by tissue nematodes of the genus Dirofilaria, responsible for two distinct clinical presentations: Dirofilaria immitis usually presenting as pulmonary lesions and Dirofilaria repens as subcutaneous nodules. Rare in humans, genital involvement manifests itself as pseudotumor nodules affecting the scrotum, epididymis, or spermatic cord. We report on two cases of Dirofilaria repens infections, involving the spermatic cord and epididymis.

[International bone marrow donor exchange].

More than 200 bone marrow transplants utilizing unrelated donors were done worldwide. In patients with chronic myelogenous leukemia survival rates up to 50% were published. Due to the low number of HLA-A, -B, -DR-typed donors in Europe donor search will be successful in only 13% of patients. Efforts should be directed to make an international donor search easier and to increase the donor pool size in each country.

Two cases of possible transmitted HIV drug resistance to all currently available integrase inhibitors, the Netherlands, 2025.

Transmitted integrase inhibitor resistance in newly diagnosed HIV remains rare. We report two cases with baseline resistance to all currently available integrase inhibitors in the Netherlands in 2025. Clinical history and laboratory findings indicate possible transmitted resistance. The cases are not phylogenetically linked, representing two independent introductions of integrase inhibitor resistant HIV into the Dutch population. These observations highlight the need for strengthened surveillance and prevention efforts in Europe and globally.

Challenges in palliative care for refugees.

The ongoing conflict in Ukraine since February 2022 has led to a significant refugee crisis, with many Ukrainians seeking protection across Europe. Among these refugees are individuals with pre-existing serious or chronic illnesses requiring continued medical care in their host countries. This case report highlights the challenges faced by healthcare providers in Switzerland when treating Ukrainian refugees, particularly in the context of palliative care for advanced cancer patients.

'Khatatonia' - cathinone-induced hypertensive encephalopathy.

Khat consumption is an under-recognised cause of hypertensive encephalopathy and intraparenchymal brain haemorrhage. We report the radiological findings of extensive periventricular, subcortical and brain stem white matter pathology of a patient who had consumed excessive amounts of Khat. The Khat plant contains cathinone, an amphetamine-like alkaloid which has been associated with chronic hypertensive end-organ damage, but is seldom considered a cause of cerebrovascular events in northern Europe.

Abducens nerve palsy following a tick bite: a case report.

Neuromuscular paralysis caused by salivary proteins of ticks is a well-known complication after tick bites in Australia, North America, and South Africa. Symptoms may include general weakness, difficulty walking, ascending paralysis, and bulbar paralysis with diplopia, culminating in respiratory failure. In Europe, toxin-mediated paralysis has rarely been noted. We report a case of cranial nerve paralysis with delayed onset after a tick bite in northern Germany.

Endogenously acquired deep sternal wound infection after congenital cardiac surgery.

A 10-month-old child from the Middle East received complete correction for tetralogy of Fallot. Despite an initially uneventful postoperative course, he developed severe deep sternal wound infection after 7 days. This complication was endogenously acquired caused by a resistant and rarely present in Europe, Streptococcus pneumonia. Following surgical debridement, betadine rinsing for 3 days and with a course of specific antibiotic therapy, the patient was discharged in good health.

Relaxing School Suncreen Restrictions.

(1) Utah, Delaware, Vermont, Minnesota and Idaho have the highest skin cancer rates of all U.S. states, according to the Centers for Disease Control and Prevention. (2) Sunscreen is regulated by the U.S. Food and Drug Administration because it makes a "drug claim"--that it helps prevent sunburn and decreases the risks of skin cancer and early skin aging caused by the sun. (3) In Europe and some other countries, sunscreens are regulated as cosmetics, not as drugs.

Molecular Characterization and Serology of Leptospira kirschneri (Serogroup Grippotyphosa) Isolated from Urine of a Mare Post-Abortion in Brazil.

A strain of Leptospira kirschneri (serogroup Grippotyphosa) was cultured from urine of a mare post-abortion in Brazil and characterized by serogrouping, multiple-locus variable-number tandem repeat analysis, PGFE, and sequencing of genes rrs and secY. Strains of L. kirschneri have apparently never been recovered from horses in tropical area, only in Europe and USA. Knowledge of local epidemiology is important to interpret genetic profiles of leptospires circulating in an area.

[Ossification of posterior longitudinal ligament].

Ossification of posterior longitudinal ligament (OPLL) is a hyperostotic disease of the spine associated with myelopathy which is occurred by an anterior compression to the spinal cord. OPLL was first reported by Key GA in 1838, and was previously considered specific to east Asian people, especially Japanese. However, now OPLL is recognized as a subtype of diffuse idiopathic skeletal hyperosteosis, which is detected in Europe and the United States. We discuss the etiology and natural history of OPLL in this review.
