Ocular toxoplasmosis: the role of cellular immune defense in the development of recurrences. Results from animal experiments.

Five toxoplasmosis-free animals and ten animals that had been immunized intraocularly against toxoplasma organisms were used in a rabbit model of ocular toxoplasmosis. Five of the ten immune animals were immunosuppressed by cyclosporin A at a cellular level. Experimental toxoplasmic retinochoroiditis occurred in all animals, a process that in complete contradiction to experience reported previously. In the rabbit model, immunity after a past history of toxoplasma infection has thus far appeared to be a safe form of protection from experimental ocular toxoplasmosis. The concept of "immunologica

[Cataract surgery 220 years ago].

The author reports on the first 14 cataract extractions performed by Karl Kaspar von Siebold, a Würzburg surgeon, in 1766/67. He used Daviel's method. At that time there was no anesthesia, no sterility or even antisepsis, and no optical correction of aphakia for his patients. Four eyes were lost due to infection and another four had much poorer vision after surgery due to irregular corneal scarring. In six cases both patient and physician were satisfied with the result. Siebold's case reports exhibit a fascinating degree of self-criticism and frankness.

A simple method for assessment of association between synanthropic flies and trachoma.

In some areas of endemic trachoma, muscoid flies may play a significant role in transmission of this serious eye infection. A simple and practical method of quantifying the relative household density of synanthropic flies has been developed for use in epidemiological field surveys. The method uses two boards moistened with a 10% sucrose solution. The fly scores showed good reproducibility at the same sampling site during the day and on different days. The only flies identified were Musca sorbens, showing their local density and a significant association of this fly with the presence of inflamm

Whipple's disease.

Whipple's disease is a systemic bacterial infection that once was uniformly fatal and now is treatable with several different antibiotics in most cases. The exact nature of the Whipple's bacillus is unknown, since the organism cannot consistently be cultured. There is also controversy concerning the role of immunologic dysfunction in patients with Whipple's disease. In addition to the small intestine, Whipple's disease can involve the remainder of the gastrointestinal tract, as well as the lymph nodes, joints, nervous system, heart, eyes, hematopoietic system, lungs, liver, and other organs. Infection with Chlamydia trachomatis immunotype J associated with trachoma in children in an area previously endemic for trachoma.

Chlamydia trachomatis immunotype J was isolated from the eye of a five-year-old Navajo child with trachoma. This is the first such isolate from a patient in an area previously endemic for trachoma. Chlamydial infection of the genitals among the Navajo Indians is common, and we have isolated immunotype J from the genital tract of women in this area. This most recent case points to the occurrence of "genital" trachoma. This occurrence adds evidence to the assertion that trachoma may be caused by the "genital" immunotypes of C. trachomatis. Therefore, the traditional distinction between ocular an

Multifocal retinopathy in Borna disease virus infected rabbits.

Experimental infection of rabbits with Borna disease virus led in all cases to a multifocal retinopathy that paralleled the clinical neurologic symptoms. The retinal changes always became evident first in the lower anterior quadrant of the eye. Infectious virus and antigen were detected in altered and unaltered regions of the retina. Individual chorioretinal lesions showed destruction of the pigment epithelium and the photoreceptors and perivascular inflammation close to small choroidal veins. Because of maximal antigen accumulation and the focal destruction of the retinal pigment epithelium w

Neuroretinitis associated with herpes simplex encephalitis in an adult.

A 48-year-old man who died of herpes simplex encephalitis had a bilateral papillitis at autopsy. Intranuclear inclusion bodies were especially numerous within the ganglion cells and inner nuclear layer of the macula and consisted of typical virions by electron microscopy. Contiguous spread from the brain to the eyes may have occurred via the optic nerves. Clinically, the disk and retinal changes were misinterpreted as being caused by papilledema. Papillitis should be included in the differential diagnosis of disk swelling in adults with suspected Herpesvirus hominis infection of the central ne

Puncture wound of the eyelid causing brain abscess.

A case of brain abscess following an apparently trival puncture wound to the eyelid is presented to emphasize the importance of meticulous examination of eye wounds in children to look for penetration into the cranial vault. Any suggestion of such penetrating injury either by history, examination, or X-ray should dictate neurosurgical consultation, and immediate local culture and debridement of the wound. If nonoperative management is elected, a careful watch must be kept for signs of infection, which dictate early exploration, and broad-spectrum antibiotic coverage of Gram-positive and Gram-n

Trachoma and ocular Chlamydia trachomatis rates in children in trachoma-endemic communities enrolled for at least three years in the Tanzania National Trachoma Control Programme.

Trachoma, a blinding eye disease caused by repeated and prolonged infection with Chlamydia trachomatis, is a significant public health problem for sub-Saharan Africa. Tanzania has had a National Trachoma Task Force since 1999, working on trachoma control in endemic districts. The objective of this study was twofold: first, to determine the current status of infection and clinical trachoma in these districts in Tanzania, and second, to determine if a combination of clinical signs could be used as a surrogate for infection. We conducted a survey for trachoma and infection with C. trachomatis in

Delayed infectious keratitis in a patient with radial keratotomy and floppy eyelid syndrome treated with simple interrupted stitches.

A 55-year-old male presented with an infectious keratitis in his left eye, affecting one of the incisions of the radial keratotomy he had undergone thirty-four years ago. Suturing the incision with two simple interrupted stitches was key to the resolution of the infection. Floppy eyelid syndrome was also found in this patient. Could this act as a risk factor for infectious keratitis in radial keratotomy? To our knowledge, this is the first reported case describing the association between both conditions, and the second reported case where sutures have been used as an adjuvant treatment in thes

Human Thelaziasis: Emerging Ocular Pathogen in Nepal.

Thelaziasis is an ocular arthropod-borne, zoonotic disease of the eye infecting the conjunctival sac, lacrimal duct, and lacrimal gland caused by a nematode of the genus Thelazia. We report the first case of human ocular thelaziasis in Nepal in a 6-month-old child from a Rukum district, Nepal. The infant presented with conjunctivitis, and his visual acuity and dilated fundal examination were normal. A total of 6 worms were removed for identification. Collected nematodes were identified based on morphological keys as Thelazia callipaeda. The patient's symptoms improved after removal of the nema

Trachoma control: the SAFE strategy.

As a contagious bacterial infection that affects the conjunctival covering of the eye, the cornea and the eyelids, trachoma is controlled by an endorsed integrated strategy consisting of surgery for trichiasis, antibiotic therapy, facial cleanliness and environmental improvement, namely, the SAFE strategy developed by World Health Organization. Developed based on evidence from previous field trials and constantly modified in practice, SAFE strategy has greatly boosted the progress in trachoma control. Regardless of the fact that there are still many pending questions, national program coordina

Pigmented free floating vitreous cyst in a 10 years old child.

Pigmented free-floating vitreous cyst in retrolental space is rare. It can represent its congenital origin after spontaneous detachment from the ciliary body epithelium or after trivial injury. We report a case of pigmented, free floating non-infective vitreous cyst in a 10 year old child who presented with compliant of transient blurring of vision three days after she joined swimming class. A thorough eye examination with ultrasound B scan and ultrasound biomicroscopy was done to rule out any other associated conditions.

A Case of Keratomycosis Caused by Fusarium Solani at Rural Tertiary Care Center.

Keratomycosis is defined as invasive infection of corneal stroma caused by variety of fungi. Most of these cases occur in agricultural workers with history of corneal trauma with vegetative matter contaminated by fungi. Fusarium species which causes keratomycosis receives occasional mention in literature. We report a case of a 27-year-old male, farmer by occupation presented with complains of foreign body sensation, itching and discharge from the right eye and developed white patch on cornea which was progressed gradually to size of diameter of pea which was later diagnosed as keratitis caused

Zika Virus: The Agent and Its Biology, With Relevance to Pathology.

Once obscure, Zika virus (ZIKV) has attracted significant medical and scientific attention in the past year because of large outbreaks associated with the recent introduction of this virus into the Western hemisphere. In particular, the occurrence of severe congenital infections and cases of Guillain-Barré syndrome has placed this virus squarely in the eyes of clinical and anatomic pathologists. This review article provides a basic introduction to ZIKV, its genetics, its structural characteristics, and its biology. A multidisciplinary effort will be essential to establish clinicopathologic cor

Preventing blindness and saving lives: the centenary of vitamin A.

Within 20 years of its discovery 100 years ago, vitamin A was recognized as critical to normal eyes, growth, and survival. Clinical interest subsequently contracted to its importance in preventing xerophthalmia, until this ophthalmologist stumbled, quite accidently, on its role in fighting life-threatening infections. Repeated, large-scale randomized clinical trials eventually convinced (and reminded) the pediatric and nutrition communities of its importance for child survival. Vitamin A distribution programs are now credited with saving the sight and lives of nearly half a million children ev

Orbital cellulitis as a complication of mandibular odontogenic infection.

A 30-year-old man presented with an acute left eye proptosis, pain, and decreased vision. He had been having a dental infection since 3 days. Orbital CT scan showed abscess in the left orbit. On dental X-ray a periapical radiolucency of the mandibular left-second molar was observed, and no obvious sinus involvement was noted. Intravenous antibiotic therapy did not lead to any significant improvement. Therefore, the patient underwent combined dental and orbital surgery to drain the abscess. Following the surgery, the patient's clinical signs and symptoms resolved completely.

Cefixime-induced oculogyric crisis.

Oculogyric crisis is a neurologic adverse event characterized by bilateral dystonic, usually upward, conjugate eye deviations. Cefixime is a third-generation cephalosporin and is widely used in clinical practice in childhood. Confusion, encephalopathy, coma, myoclonus, nonconvulsive status epilepticus, and seizures have been described with the use of cephalosporins. We presented a cefixime-induced oculogyric crisis in a 7-year-old boy during the treatment of urinary tract infection, and this is the first case of cefixime-induced oculogyric crisis whose ocular symptoms gradually disappeared wit

Racemose cysticercosis presenting as cystic neck swelling.

Cysticercosis is a parasitic infection caused by the larvae of the tapeworm Taenia solium which occurs in neural and extraneural forms. Latter commonly involves subcutaneous tissue, skeletal muscles, and eyes. Head and neck sites commonly involved are buccal mucosa, tongue, and lips. It presents as single or multiple submucosal/cutaneous firm nodules. We report a case of cysticercosis of the supraclavicular region of the neck where it presented as a large soft cystic swelling. The diagnosis was made by sonography and confirmed by gross and microscopic examination to be racemose cysticercosis.

Neuro and intraocular cysticercosis: A clinicopathological case report.

Cysticercosis is one of the most common parasitic infestations in humans. Neurocysticercosis (NCC) is the commonest parasitosis of the central nervous system (CNS), endemic in developing countries and is also seen in developed nations with high rates of immigration from prevalent areas. Co-infection of ocular tissue is also significant and often ophthalmologists come across these lesions in their clinical practice. The mode of treatment in NCC and that of intraocular cysticercosis is somewhat different as far as antiparasitic treatment is concerned. This case report highlights the proper manag
