Granulomatosis with polyangiitis. A diagnostic challenge in pediatric patients.

Granulomatosis with polyangiitis is a systemic autoimmune disease characterized by the presence of granulomatous necrotizing vasculitis with kidney, respiratory, and vascular involvement, whose peak incidence is in the fifth and sixth decades of life. Its localized presentation without systemic involvement is infrequent. We present the case of a 10-year-old patient with recurrent eyelid edema as the initial symptom in the setting of granulomatosis with systemic polyangiitis. Although rare, this disease can affect children, as in the case we present, and given its low incidence and atypical pre

Clinical Assessment, Diagnosis, and Management of Infraorbital Wrinkles and Pigmentation.

Lower eyelid skin is unique and different from that of other areas. In addition to being an area of high exposure to the sun and elements, there are anatomic considerations and specific histologic characteristics that can cause the skin in this area to be more sensitive. These attributes can readily cause under-eye wrinkling and pigmentation. This review aims to present an updated overview of the current knowledge regarding the clinical characteristics, diagnosis, and management of wrinkles and pigmentation in this area. These disorders are usually caused by different factors, such as genetics

Diagnostic challenge: Diffuse large B-cell lymphoma masquerading as orbital cellulitis.

We report a clinical case of orbital lymphoma masquerading as orbital cellulitis. Orbital lymphoma and lymphoma of the orbital adnexa are relatively rare types of lymphoma. They represent the malignant end of a spectrum of lymphoproliferative lesions that occur in the orbit, conjunctiva and eyelid. Diffuse large B-cell lymphomas (DLBCL) with variable clinical presentations have a relatively poor prognosis with rapid visual loss. Therefore, thorough evaluation and assessment with a multi-specialty effort are warranted for earlier diagnosis and initiation of treatment to save life and prevent si

Modified Selective Neurectomy: A New Paradigm in the Management of Facial Palsy with Synkinesis.

All patients with postparalytic facial paralysis are at risk of developing synkinesis due to aberrant nerve regeneration. Synkinesis can result in smile dysfunction, tension, and eyelid aperture narrowing due to overactive and uncoordinated muscle activity. When the synkinesis causes an asymmetric smile, there are several treatment modalities including neurotoxin, neuromuscular retraining, and surgery. Modified selective neurectomy of the facial nerve is a treatment option that potentially can improve the smile mechanism by reducing the activity of counterproductive facial muscles while preser

Ocular manifestations in patient with congenital erythropoietic porphyria.

We present the case of a 52-year-old woman referred to our service because of extreme ocular surface dryness. The patient showed corneal, conjunctival, and eyelid manifestations of ocular congenital erythropoietic porphyria (CEP). We started treatment with autologous serum, topical steroids, and cyclosporine twice a day, topical retinoids, and intense corneal lubrication. The patient referred significant improvement of ocular bothering and less discomfort since treatment was initiated. We describe the management of the herewith presented case of ocular CEP.

Mantle Cell Lymphoma of the Lacrimal Gland.

The authors present 2 female patients of mantle cell lymphoma (MCL) involving the lacrimal gland. The patients include a 75-year-old female and a 59-year-old female. Both patients presented with palpable upper eyelid masses. The first patient was previously diagnosed with gastric and ileocecal MCLs and underwent chemotherapy with remission. The second patient had distant lesions in the muscular limbs that were confirmed to be MCL after the diagnosis of lacrimal gland MCL. Both patients underwent chemotherapy with complete remission.

Pourfour du Petit Syndrome: A Rare Association With Cluster Headache.

Pourfour du Petit syndrome is an uncommon cause of eyelid retraction, associated with unilateral mydriasis and hemifacial hyperhidrosis. This syndrome is caused by hyperactivity of the ipsilateral oculosympathetic pathway and needs to be recognized because it has an opposite clinical presentation but the same topographic and diagnostic value as Horner syndrome. The authors report a rare case of Pourfour du Petit syndrome associated with cluster headache and discuss pathophysiological hypotheses, clinical presentation, complementary exams, pharmacologic testing, treatment options, and prognosis

Developments in the treatment of benign essential blepharospasm.

PURPOSE OF REVIEW: To review new developments in the medical and surgical treatment options for benign essential blepharospasm (BEB). RECENT FINDINGS: Botulinum toxin injections remain the mainstay treatment for BEB with several formulations currently commercially available. Reports in the medical literature support photochromatic modulation for the symptoms of photophobia, as well as oral medications and surgical myectomy for control of the motor signs of eyelid protractor spasm. SUMMARY: Although there remains no cure for BEB, several treatment options are available to effectively manage t

Conjunctival Chemosis Caused by Exposure of the Lacrimal Caruncle: A Case Report.

An 84-year-old woman presented with a 3-month history of conjunctival chemosis in the left eye. At the first examination, the chemosis neighbored the lacrimal caruncle and was localized in the inferomedial region of the conjunctiva. During eyelid closure, only the left lacrimal caruncle was exposed. One month later, the chemosis further extended to the inferolateral region. We debulked the lacrimal caruncle to prevent the exposure of the caruncle. One month after the surgery, conjunctival chemosis had resolved completely. At the postoperative 6-month follow-up, the patient showed no recurrence

Primary cutaneous CD30+ anaplastic large cell lymphoma treated with radiotherapy and methotrexate with development of xanthomas at the sites of prior disease.

Primary cutaneous anaplastic large cell lymphoma is a rare type of cutaneous T-cell lymphoma, and the involvement of the ocular adnexa is extremely rare. Secondary xanthoma-like changes after radiation therapy or chemotherapy have been rarely reported in association with large-cell T-cell anaplastic lymphoma. We report one case of a primary C-anaplastic large cell lymphoma affecting the eyelid with fast progression with multiple nodules in various anatomic sites and development of xanthoma-like lesions after treatment.

[Bernard-Horner syndrome after accidental lesion of carotid artery: case report].

Among the complications of internal jugular vein insertion there is the lesion of the cervical sympathetic trunk with the onset of Bernard-Horner syndrome, consisting of miosis, eyelid ptosis, enophthalmos and anhidrosis on the same side of the lesion. The neurological damage can be caused by the direct puncture of the trunk or by the irritating and compressive action of a hematoma during the puncture of the internal jugular; the clinical picture, when reversible, resolves in a few months. The case we report is about the onset of the syndrome after accidental puncture of carotid artery, follow

[MALT (mucosa-associated lymphoid tissue) lymphoma of the lacrimal gland].

MALT lymphoma of the lacrimal gland is rare. Orbital involvement is usually located on the anterior orbit. We report the case of a 65-year-old man consulting for an eyelid tumor located on the right upper and lateral lid, which clinically recalled lacrimal gland involvement. The gland biopsy and the immunohistochemical analysis concluded in the diagnosis of B and MALT lymphoma. The assessment of the ganglion and extra-ganglion enlargement of these lymphomas is of capital importance in treatment. A multidisciplinary analysis, especially oncological, is important so that the best therapeutic pla

Corneal melt associated with idiopathic dacryoadenitis.

A 15-year-old boy presented with left-sided ptosis, upper eyelid swelling, proptosis, conjunctivitis, and a central corneal epithelial defect with thinning. MRI demonstrated an enhancing orbital mass associated with the lacrimal gland. Biopsy findings were consistent with orbital inflammatory syndrome. Topical cyclosporine and 1% methylprednisolone eliminated the anterior segment inflammation; subsequent treatment with systemic prednisone reduced the ptosis and edema. To our knowledge, this is the only reported case of orbital inflammatory syndrome associated with a corneal epithelial defect a

The eye in sleep apnea syndrome.

Sleep apnea syndrome (SAS) is a disease characterized by recurrent complete or partial upper airway obstructions during sleep. The majority of patients with SAS demonstrate this obstruction either at the nasopharynx or the oropharynx. Risk factors for SAS include obesity, male gender, upper airway abnormalities, alcohol use, snoring, and neck girth of more than 17 in. in men or 16 in. in women. Reported ophthalmic findings in patients with SAS include floppy eyelid syndrome (FES), glaucoma, and non-arteritic anterior ischemic optic neuropathy (NAION).

Oral myiasis in a child by the reindeer warble fly larva Hypoderma tarandi.

We describe a case of human oral myiasis by a first-stage larva of the reindeer warble fly, Hypoderma (Oedemagena) tarandi (L.) (Diptera: Oestridae), in a 12-year-old girl. In September the patient complained of erythema, swelling and conjunctivitis of the right eyelid. Symptoms subsided spontaneously but re-occurred in October. In December she presented with acute swelling of the right corner of the mouth. Later that day a living larva protruded from this swelling. The patient had been on vacation in Norway during the previous summer months.

Orbital aspiration as treatment of microphthalmos with orbital cyst: a case report.

A 6-month-old girl came to the hospital with swelling of the right lower eyelid, exophthalmos, chemosis and upward deviation of the eyeball--all of which had been present since birth. Iris, optic disc, and chorioretinal coloboma were also apparent. Magnetic resonance imaging revealed a small globe with a large cystic lesion in the orbit of the right eye. Pre- and post-operative photographs and magnetic resonance imaging indicated a safe, simple single orbital aspiration as an alternative treatment for mild microphthalmos with an orbital cyst.

Use of the orbicularis oculi muscle flap for severe Marcus Gunn ptosis.

The authors present a 22-year-old man with severe unilateral congenital blepharoptosis associated with Marcus Gunn (jaw-winking) syndrome. The best conventional treatment was levator excision to eliminate the synkinetic reflex and fascia lata brow suspension. However, the previous surgery had some disadvantages. Therefore, the authors report the use of the orbicularis oculi muscle flap to elevate dynamically the ptotic eyelid and to eliminate the synkinetic reflex without levator resection. The postoperative result was both functionally and cosmetically satisfactory after 1 year of follow-up.

Prevalence of primary blepharospasm in a community of Puglia region, Southern Italy.

The authors ascertained the prevalence of primary blepharospasm (BSP) in a community located in Puglia, a region in Southern Italy, by focusing on neurologists and ophthalmologists. The crude prevalence rates were 133 per million (95% CI, 61--153) for both focal and segmental BSP, 74 per million (95% CI, 24--173) for focal BSP, and 59 per million (95% CI, 16--151) for segmental BSP. Prevalence rate increased with age. Apraxia of eyelid opening and BSP coexisted in one third of cases.

Bilateral Kimura's disease with devastating visual outcome.

Kimura's disease is a chronic inflammatory disease involving connective tissues. We treated a patient with bilateral periocular Kimura's disease causing eyelid deformity and exophthalmos for over 20 years. She has been operated on 23 times and biopsies were taken and studied histologically. The disease continued despite the multiple surgical excisions. The consequent exophthalmos and lid changes caused lagophthalmos that resulted in severe visual impairment due to chronic bilateral corneal exposure. Kimura's disease may have a devastating effect on visual function.

[Acute trichinosis in children. Intrafamilial epidemic outbreak in Mexico City].

Four cases of trichinosis found in a family living in Iztapalapa, Mexico, D.F. are reported. The family reported to frequently eat raw sausage originally from Villanueva, Zacatecas from which Trichinella spiralis larvae were isolated. Diagnosis was difficult in the first case since no epidemiological history was associated with the patient, besides the fact that the patient showed no signs of eosinophilia and blood tests were negative. The diagnosis in this case was confirmed through muscular biopsy. The predominating symptomatology consisted in the trilogy of fever, eyelid edema and myalgias.
