Gut dysbiosis-mediated platelet activation as MASLD and cardiovascular disease link.

Metabolic dysfunction-associated steatotic liver disease (MASLD) is characterized by liver disease progression and cardiovascular disease (CVD). The mechanism linking MASLD with CVD is not fully understood. Here, we report experimental and clinical studies suggesting that platelet activation may be implicated in the pathogenesis of MASLD and CVD.

Ethanolamine phosphate phospholyase in NAFLD: Emerging mechanisms, pathways, and therapeutic opportunities.

Nonalcoholic fatty liver disease (NAFLD) represents a growing global health burden with limited pharmacological options. Ethanolamine Phosphate Phospholyase (ETNPPL) is a liver enzyme involved in lipid metabolism, predominantly expressed in the mitochondrial matrix. Recent studies have confirmed a potential association between ETNPPL dysregulation and insulin resistance, ferroptosis, and macrophage reprogramming. These findings partially overlap with the pathogenesis of NAFLD. However, a comprehensive synthesis of these mechanisms remains lacking. On this basis, we propose an evidence-stratifi

[Untargeted metabolomics-based exploration of potential metabolic biomarkers for subclinical atherosclerosis in metabolic-associated fatty liver disease].

OBJECTIVES: Cardiovascular disease is the most common extrahepatic complication and leading cause of death in metabolic dysfunction‑associated fatty liver disease (MAFLD). However, early metabolic biomarkers for subclinical atherosclerosis (SA) in MAFLD patients remain unclear. This study used untargeted metabolomics to investigate serum metabolite and pathway changes associated with SA in MAFLD patients and to screen for potential serum metabolic biomarkers. METHODS: From December 2023 to December 2024, 64 MAFLD patients with SA enrolled in the "Pan-Vascular Cohort Study" at the Health Manag

Integrating Phosphatidylethanol to Enhance Alcohol Quantification in Steatotic Liver Disease and Clinical Trials.

Accurate quantification of alcohol intake is essential for the diagnosis, subclassification, and management of steatotic liver disease (SLD). In everyday clinical practice, alcohol histories rely primarily on physician-assisted self-report, with validated questionnaires used variably. Both provider-level inconsistencies in documentation and patient-level factors (e.g., recall and social-desirability biases) can contribute to misclassification of SLD subtypes. Phosphatidylethanol (PEth), a direct alcohol biomarker detectable for ~2-4 weeks, has emerged as a promising objective tool that can com

Early-life 6:2 diPAP exposure induces SQSTM1/p62-associated NAFLD-like liver injury in juvenile zebrafish.

Per- and polyfluoroalkyl substances (PFASs) are persistent environmental contaminants closely linked to metabolic liver disease; however, the transgenerational hepatotoxicity and underlying mechanisms of emerging PFAS substitutes remain poorly understood. Herein, we focused on 6:2 polyfluoroalkyl phosphate diester (6:2 diPAP), a prominent PFAS substitute, to evaluate its developmental hepatotoxicity. By integrating zebrafish phenotyping, transcriptomic feature selection, virtual gene knockout, and experimental validation, we characterized the candidate molecular events through which 6:2 diPAP

The Role of Dietary Protein in Mitigating the Risk of Nonalcoholic Fatty Liver Disease.

Nonalcoholic fatty liver disease (NAFLD) is a prevalent liver disease, mainly associated with excessive accumulation of fat in the liver. It has become a global health concern. The diagnosis of NAFLD is often done through liver biopsy; however, noninvasive methods have their own advantages. Dietary intervention, especially increased dietary protein alongside managing overall body weight, have been shown to be a promising strategy to lessen the impact of NAFLD. Dietary protein has been shown to reduce fat accumulation in the liver by increasing liver metabolism, eliciting satiety, improving ins

Machine learning-based integration reveals reliable biomarkers and potential mechanisms of NASH progression to fibrosis.

Non-alcoholic fatty liver disease (NAFLD) affects about 25% of adults worldwide. Its advanced form, non-alcoholic steatohepatitis (NASH), is a major cause of liver fibrosis, but there are no non-invasive tests for diagnosing or preventing it. In our study, we analyzed data from multiple sources to find crucial genes linked to NASH fibrosis. We built diagnostic models using 103 machine learning algorithms and validated them with two external datasets. All models performed well, with the best one (RF + Enet[alpha = 0.6]) achieving an average AUC of 0.822. This model used five key genes: LUM, COL

Review of roles of RNA-binding proteins on NAFLD and the related pharmaceutical measures.

Nonalcoholic fatty liver disease (NAFLD) is the most common chronic liver disease worldwide and poses a serious threat to public health. NAFLD is considered a risk factor for metabolic syndrome (MS) and is closely associated with type 2 diabetes mellitus (T2DM), obesity, dyslipidemia, and cardiovascular disease. Recently, increasing attention has been paid to the role of RNA-binding proteins (RBPs) in the pathogenesis of NAFLD. A growing body of research has linked RBPs-such as human antigen R (HuR), sequestosome 1 (p62), polypyrimidine tract-binding protein 1 (PTBP1), and heterogeneous nuclea

The Role of Traditional Chinese Medicine in the Management of Nonalcoholic Fatty Liver Disease: Targeting Gut Microbiome.

The incidence of non-alcoholic fatty liver disease (NAFLD) has increased and become a serious global public health problem in recent years. The currently generally used clinical treatments have disadvantages such as side effects, limitations, and poor patient compliance. Traditional Chinese medicine (TCM) has a pharmacological effect with multiple components, multiple targets, and multiple pathways, emphasizing a "holistic concept" and "differential diagnosis and treatment," which is compatible with the complex pathogenesis of GM and NAFLD. Previous studies have demonstrated a close relationsh

Diagnostic accuracy of artificial intelligence models for imaging detection of hepatic steatosis through systematic review and meta analysis.

Non-alcoholic fatty liver disease (NAFLD) is an increasingly prevalent chronic liver condition affecting nearly 30% of the global population. Characterized by hepatic steatosis in the absence of significant alcohol intake, NAFLD can progress to non-alcoholic steatohepatitis (NASH), fibrosis, cirrhosis, and hepatocellular carcinoma. Although liver biopsy is the diagnostic gold standard, its invasiveness, cost, and associated risks limit widespread application. Artificial intelligence (AI) offers promising non-invasive alternatives by leveraging large datasets to enhance diagnostic precision. To

Hydroxytyrosyl Eicosapentaenoate as a Potential Antioxidant for Omega-3 Fatty Acids: Improved Synthesis and Comparative Evaluation with Other Natural Antioxidants.

Hydroxytyrosol (HT), the primary phenolic compound in virgin olive oil, has notable cardiovascular benefits, particularly in preventing low-density lipoprotein (LDL) oxidation. However, its hydrophilicity limits its solubility and integration into lipid-based formulations. This study aimed to enhance its lipophilicity by synthesizing hydroxytyrosyl eicosapentaenoate (HT-EPA), a derivative of HT and eicosapentaenoic acid (EPA), using a one-step enzymatic catalysis with lipase B from Candida antarctica (CALB). The reaction, performed as a suspension of HT in ethyl eicosapentaenoate (Et-EPA) (1:9

Neddylation and MASLD: From Pathophysiology to Therapy.

Metabolic dysfunction-associated steatotic liver disease (MASLD), formerly known as non-alcoholic fatty liver disease (NAFLD), is intimately linked to the dysregulation of key transcription factors or enzymes involved in hepatic metabolism. Neddylation, a post-translational modification, refers to the covalent attachment of neuronal precursor cell-expressed developmentally down-regulated 8 (Nedd8) to specific substrates and plays a significant role in regulating protein conformation, localisation and activity. The connection between neddylation and MASLD is a burgeoning area of research that i

Metabolic dysfunction associated fatty liver disease and type 2 diabetes: pathophysiological links, epidemiological trends, and clinical implications.

Metabolic dysfunction-associated steatotic liver disease (MASLD), previously known as non-alcoholic fatty liver disease (NAFLD), has recently been recognized as a public health issue because it is closely linked to metabolic disorders, including type 2 diabetes mellitus (T2DM). This review aims to discuss the bidirectional relationship between MASLD and T2DM and the similarities in their pathophysiology, which include insulin resistance (IR), lipogenesis, inflammation, and alterations in the gut microbiota. The incidence of MASLD has increased concomitantly with the obesity and diabetes pandem

Diabetes and the liver.

Type 2 diabetes mellitus (T2D) and metabolic dysfunction-associated steatotic liver disease (MASLD), formerly termed non-alcoholic fatty liver disease (NAFLD), are increasingly prevalent conditions that are closely associated. The shared pathophysiology of insulin resistance, chronic inflammation and increased lipid deposition means that there is a bidirectional relationship between the two conditions. MASLD is now a leading cause of cirrhosis and hepatocellular carcinoma (HCC) and has a high prevalence in patients with T2D. This review explores the synergy between MASLD and T2D, while also ou

Optimization of Cyclophilin B-Targeted Tri-vector Inhibitors for Novel MASH Treatments.

Cyclophilins have been implicated in the pathophysiology of metabolic dysfunction-associated steatohepatitis (MASH). Pharmacological inhibition of the cyclophilin B isoform has the potential to attenuate liver fibrosis in MASH, but current cyclophilin inhibitors in clinical trials lack isoform selectivity. We previously reported the novel tri-vector small-molecule inhibitor 1 that exhibited improved subtype selectivity by simultaneously engaging three pockets on the surface of cyclophilins. Here, we present structure-activity relationships that address genotoxicity concerns, enhance subtype se

An insight on the additive impact of type 2 diabetes mellitus and nonalcoholic fatty liver disease on cardiovascular consequences.

BACKGROUND: Nonalcoholic fatty liver disease (NAFLD) and type 2 diabetes mellitus (T2DM) are associated with a multifactorial complicated aetiology that is often coexisting and has a strong and distinct connection with cardiovascular diseases (CVDs). In order to accomplish effective and appropriate therapeutic strategies, a deeper understanding of the bidirectional interaction between NAFLD patients, NAFLD patients with T2DM, and NAFLD patients with CVDs is required to control the concomitant rise in prevalence of these conditions worldwide. This article also aims to shed light on the epidemio

Sarcopenia as a Risk Factor for Mortality in NAFLD: How Should We Diagnose It?

OBJECTIVES: Sarcopenia increases the risk of nonalcoholic steatohepatitis (NASH) and cirrhosis in patients with nonalcoholic fatty liver disease (NAFLD). Subsequently, poorly managed NAFLD can result in adverse health outcomes. Lifestyle interventions are effective for both NAFLD and sarcopenia; however, diagnosis of sarcopenia in this population is not well defined. This review aimed to examine current methods to diagnose sarcopenia in NAFLD patients. METHODS: MEDLINE, EMBASE, and CINAHL databases were searched for articles published until July 2023 using the terms "Non-alcoholic fatty liver
