Obesity and Obesity Shape Markedly Influence Spine Biomechanics: A Subject-Specific Risk Assessment Model.

Underlying mechanisms of obesity-related back pain remain unexplored. Thus, we aim to determine the effect of obesity and its shapes on the spinal loads and the associated risks of injury. Obesity shapes were initially constructed by principal component analysis based on datasets on 5852 obese individuals. Spinal loads, cycles to vertebral failure and trunk stability margin were estimated in a subject-specific trunk model taking account of personalized musculature, passive ligamentous spine, obesity shapes, segmental weights, spine kinematics and bone mineral density. Three obesity shapes (mea

Unusual circumstances to diagnose cervical Pott's disease.

Tuberculous spondylodiscitis usually affects the dorso-lumbar spine, and its cervical location is a rare condition that can mimic other diseases and consequently cause treatment delays. We report a case of tuberculous spondylodiscitis of the lower cervical spine discovered under unusual circumstances in a patient with severe polytrauma involving a cranio-cerebral trauma, a non-displaced fracture of the two laminæ and the spinous process of the C6 vertebrae as well as fibular and tibial shaft fractures. The patient underwent static tibial nailing, and a collar with occipital and chin supports w

Pathogenesis and clinical aspects of pain in patients with osteoporosis.

Bone pain is one of the most frequent kinds of chronic pain, mainly in elderly patients. It causes a significant worsening of functional capacity and deterioration in the quality of life in people affected. Mechanisms of pain in osteoporosis are poorly known and often extrapolated by other pathologies or other experimental model. One of principal causes would be a "hyper-remodeling" of bone, that involves osteoclasts activity and pathological modifications of bone innervation. Several studies show that osteoclasts play a significant role in bone pain etiology. Pain in osteoporosis is mainly no

Masticatory tensile developed in upper anterior teeth with chronic apical periodontitis. A finite-element analysis study.

Commonly is accepted that a non-vital tooth has a higher risk of root fracture but there is a relatively little knowledge about the specific biomechanical behavior of non-vital frontal teeth with chronic apical periodontitis. The aim of our study was to evaluate the deformation and tensile generated in these teeth while vertically loading them because it is crucial to assess the moment when the absorbed occlusal forces exceed the elasticity of root dentine. Using the method of finite-element analysis, we highlighted the distribution patterns of the compressive and tension tensile, as well as t

[Development and clinical application of elastic trephine through pedicle of fractured vertebra].

OBJECTIVE: To evaluate the feasibility and surgical procedure of bone grafting with elastic trephine. METHODS: A total of 42 cases of thoracolumbar fracture received pedicle screw fixations by combining bone grafting with elastic trephine through pedicle of fractured vertebra. The ratios of the altitude of traumatic vertebra to normal altitude, kyphosis Cobb angle and sagittal index were measured by preoperative and postoperative X-ray images. RESULTS: A follow-up visit of 12-26 months (average: 14.8) showed that the dislocations of vertebrae were corrected and fusions completed. Proportion

[Biomechanical study of four palmar locking plates and one non-locking palmar plate for distal radius fractures: stiffness and load to failure tests in a cadaver model].

Five different palmar fixation plate designs were compared in a distal radial osteotomy cadaver model with regard to their biomechanical properties. A metaphyseal osteotomy gap of 1 cm was performed and the osteosynthesis was plated according to the manufacturer's instructions. Axial load was applied to the construct by a pneumatic material testing machine. Five implant groups with eight cadavers each were tested concerning stiffness. None of the constructs developed deformity and movement of the fracture gap larger than 2 mm with a load of 100 N. Increasing the load to 250 N revealed signific

Prediction of bone mechanical properties using QUS and pQCT: study of the human distal radius.

The objective was to compare the prediction of bone mechanical properties provided by axial transmission to that provided by peripheral quantitative computed tomography (pQCT) at the distal radius. The distal radius is the location for Colles' fractures, a common osteoporosis related trauma situation. Measurements of the radial speed of sound were performed using three axial transmission devices: a commercial device (Sunlight Omnisense, 1.25 MHz), a bi-directional axial transmission prototype (1 MHz), both measuring the velocity of the first arriving signal (FAS), and a low frequency (200 kHz)

Optimizing bisphosphonate therapy in patients with breast cancer on endocrine therapy.

Deterioration of bone health is a major concern during progression and treatment of patients with breast cancer, especially in postmenopausal women. Disease- and treatment-associated skeletal-related events include fractures, spinal compression, bone pain, and hypercalcemia of malignancy. Bisphosphonates, which inhibit osteoclastic bone resorption, are important new agents in the management of skeletal-related events, and their impact on breast cancer-related bone metastases and on bone loss during long-term estrogen deprivation therapies such as aromatase inhibitors is reviewed. Intravenous p

Computed tomography-guided percutaneous transpedicular biopsy of the thoracic spine.

BACKGROUND: Despite the recent, widespread use around the world of computed tomography (CT)-guided spinal biopsy for presurgical evaluation, its use has remained uncommon in Taiwan. Using 9 patients, we attempted to evaluate the safety of its use for the thoracic spine biopsy. METHODS: In the past 3 years (May 1996 to May 1999), 9 patients with known thoracic or multiple spinal lesions underwent CT-guided biopsy. The patients comprised 4 men and 5 women; with ages ranging between 28 and 85 years. For the transpedicular approach, patients were laid prone on the CT table. A trephine bone biopsy

Treatment of mandibular osteoradionecrosis by cancellous bone grafting.

PURPOSE: This study was undertaken to evaluate a new method in the treatment of mandibular osteoradionecrosis. PATIENTS AND METHODS: Eight patients, seven male and one female, with a mean age of 64 years (range, 43 to 67 years), suffering from osteoradionecrosis of the mandible, two bilaterally and six unilaterally, were treated. Five initially had hyperbaric oxygen (HBO) followed by sequestrectomy, and three had sequestrectomy alone. Because healing failed to occur, all patients were treated by removal of the necrotic bone in the affected part of the mandible and filling the defect with comp

Experimental Investigation on Hybrid Steel-Grout Vertical Joints with Steel Anchor Rings for Precast Concrete Shear Walls.

Vertical joints are commonly required in precast concrete shear walls when large wall panels are divided for fabrication, transportation, and erection. To reduce on-site work and improve assembly efficiency, this study proposes and investigates a hybrid steel-grout vertical joint, namely a steel-anchor-ring grouted connection, which uses embedded threaded sleeves, steel anchor rings, an inserted vertical bar, and high-strength grout to transfer shear between adjacent wall panels. Monotonic direct shear tests, cyclic shear tests under normal tension and compression, and cyclic loading tests on

In Situ Crosslinked Biodegradable Hydrogels Based on Poly(Ethylene Glycol) and Poly(ε-Lysine) for Medical Application.

Hydrogels have emerged as promising biomaterials due to their excellent performance; however, their biocompatibility, biodegradability, and absorbability still require improvement to support a broader range of medical applications. This paper presents a new biofunctionalized hydrogel based on in situ crosslinking between maleimide-terminated four-arm-poly(ethylene glycol) (4-arm-PEG-Mal) and poly(ε-lysine) (ε-PL). The PEG/ε-PL hydrogels, named LG-n, were rapidly formed via amine/maleimide reaction by mixing 4-arm-PEG-Mal and ε-PL under physiological conditions. The corresponding dry gels (DLG-

Study on triaxial compression performance and damage characteristics of fiber-reinforced ecological matrix cementing gangue gypsum fill material.

Polypropylene fiber was equally mixed into alkali-activated slag fly ash geopolymer in order to ensure the filling effect of mine goaf and improve the stability of cemented gangue paste filling material with ecological matrix. Triaxial compression tests were then conducted under various conditions. The mechanical properties and damage characteristics of composite paste filling materials are studied, and the damage evolution model of paste filling materials under triaxial compression is established, based on the deviatoric stress-strain curve generated by the progressive failure behavior of sam

Reperfusion Injury (RPI)/White Cord Syndrome (WCS) Due to Cervical Spine Surgery: A Diagnosis of Exclusion.

BACKGROUND: Following acute cervical spinal cord decompression, a subset of patients may develop acute postoperative paralysis due to Reperfusion Injury (RPI)/White Cord Syndrome (WCS). Pathophysiologically, this occurs due to the immediate restoration of normal blood flow to previously markedly compressed, and under-perfused/ischemic cord tissues. On emergent postoperative MR scans, the classical findings for RPI/ WCS include new or expanded, and focal or diffuse intramedullary hyperintense cord signals consistent with edema/ischemia, swelling, and/or intrinsic hematoma. To confirm RPI/WCS, M

A biomechanical comparison of closed-reduction osteosynthesis techniques for fractures of the ulnar styloid.

BACKGROUND: Ulnar styloid fractures (USF) are frequently underestimated and can contribute to clinically relevant wrist instability and impaired functional outcomes. Several osteosynthesis techniques are available for the treatment of unstable USF. This study aims to compare the biomechanical stability of fixation methods applicable to closed reduction - specifically K-wire and headless compression screw fixation - to support clinical decision-making. METHODS: This biomechanical in vitro study was conducted using 30 specimens of artificial ulnae. USF were simulated through a standardized oste
