[Position and mobility of the articular disk after surgical management of diacapitular and high condylar dislocation fractures of the temporomandibular joint].

Magnetic resonance imaging (MRI) assessment of traumatized temporomandibular joints (TMJ) usually focuses on disc position, defining regular joint function by normal, excentric or displaced disc position. So far, there are only few reports regarding disc position after open reduction of diacapitular or high condylar fractures of the TMJ with dislocation. The aim of the present study was to evaluate the role of the disc as regards postoperative functional outcome by electronic axiographic recordings of condylar movements and MRI, displacement of the disc and lesions of TMJ soft tissues being fr

Improving the Diagnosis of Ipsilateral Femoral Neck and Shaft Fractures: A New Imaging Protocol.

BACKGROUND: Despite increased awareness of ipsilateral femoral neck fractures in patients with high-energy femoral shaft fractures and advanced imaging with thin-cut high-resolution computed tomography (CT), failure of diagnosis remains problematic. The purpose of the present study was to determine if the preoperative diagnosis of ipsilateral femoral neck fractures in patients with high-energy femoral shaft fractures can be improved with magnetic resonance imaging (MRI) compared with radiographic and CT imaging. METHODS: In response to delayed diagnoses of femoral neck fractures despite thin-

Evaluation and Management of Pediatric Humeral Shaft Fractures.

Diaphyseal humerus fractures in the pediatric population have dynamic treatment strategies that depend on injury pattern, mechanism, patient skeletal maturity, and size. Treatment strategies include closed reduction with various immobilization techniques, flexible nails, uniplanar or multiplanar external fixation, or plate osteosynthesis. Accepted parameters for sagittal and coronal alignment vary based on age and potential for remodeling, and the multiplanar motion of the shoulder joint can accommodate for greater variations in alignment than many other long bone fractures. Complications such

Delayed Reconstruction Following Incarceration of the Medial Epicondyle in the Elbow Joint: A Case Report and Review of the Literature.

CASE: Neglected medial epicondylar fractures with intra-articular incarceration are uncommon. At 6 weeks after an elbow dislocation, an 11-year-old child was referred to our unit with painless, restricted range of motion. Clinical assessment and radiographic imaging revealed an incarcerated medial epicondylar fragment in the ulnohumeral joint. We performed open reduction and internal fixation (ORIF) and transposed the ulnar nerve. The clinical and radiographic outcomes at the final follow-up were excellent. CONCLUSION: The management of neglected pediatric medial epicondylar fractures with in

The modified Ravitch approach for the management of severe anterior flail chest with bilateral sternochondral dislocations: a case report.

The management of flail chest continues to evolve as scientific evidence and surgical experience accumulates. Flail chest injuries that span the sternum present a rare and complicated injury pattern that can be challenging to manage both medically and surgically. Our patient is a 69-year-old involved in a high-speed motor vehicle crash with respiratory failure secondary to an anterior flail chest. Tomographic examination confirmed a sternal fracture with bilateral sternochondral dislocations and multiple rib fractures. The rib fractures created a lateral flail segment which extended towards th

Displaced, unstable fractures of the radial head: fixation vs. replacement--what is the evidence?

Unstable, displaced fractures of the radial head nearly always occur in association with other fractures or ligament injuries. Radiocapitellar contact is important to elbow and forearm stability and should be restored in the context of such injuries. Open reduction and internal fixation of the radial head is associated with early failure, nonunion, and poor forearm motion. Replacement of the radial head with a metal prosthesis may be preferable. The major pitfall of a metal prosthesis is insertion of a prosthesis that is too large in longitudinal length, which can lead to subluxation, capitell

Management of distal humerus fractures.

Fractures of the distal humerus are complex injuries that can be effectively treated with open reduction and internal fixation (ORiF). Exposure of a complex intra-articular fracture may best be achieved through a posterior approach with osteotomy of the olecranon process. The ulnar nerve must be identified and protected, the articular surface must be reduced anatomically, and rigid fixation must be applied to both the medial and lateral columns of the distal humerus. Range of motion should be initiated as soon as possible postoperatively. Complications such as ulnar neuropathy, elbow stiffness

Management of comminuted radial head fractures with replacement arthroplasty.

Radial head arthroplasty is indicated for displaced comminuted radial head fractures that cannot be managed reliably with open reduction and internal fixation and that have an associated elbow dislocation. Replacement also is indicated in patients with comminuted radial head fractures that have or are likely to have a disruption of the medial col-lateral, lateral collateral, or interosseous ligaments. Biomechanical studies have demonstrated that metallic implants restore elbow stability similar to the native radial head. The early and midterm clinical experience with metallic radial head arthr

Internal fixation of valgus-impacted humeral head fractures using the screw-wiring technique.

Between February 1996 and July 2000 eight patients (ages 35-74 years) with nine severely impacted humeral head fractures were operated on with the aim of preserving the humeral heads. Patients were treated surgically with open reduction, autologous bone grafting, and internal fixation with a screw-wiring technique. There were no complications and all patients were satisfied with the results. After 10-63 months there was no evidence of nonunion, arthritis, avascular necrosis, or loosening of implants. The mean Constant score was 83 (66-97) points. Results of this study support the observation t

[Post-traumatic infections of the frontal sinus].

Post-traumatic infectious complications of the frontal sinus occur more frequently after multiple fractures than after isolated fractures. The greatest frequency is found with open traumas of the sinus, due to the presence of foreign bodies and to bone and mucosal destruction. Such infectious complications are curatively treated both medically and surgically, to eliminate the bony and mucosal infection and to restore the normal physiology of the frontal sinus. Stress is laid on the preventive treatment, especially on the quality of the initial surgical treatment, either by restoring the sinus

Results of AO plate fixation of forearm shaft fractures in adults.

A series of 111 forearm fractures in 108 individuals involving 177 individual bones, and treated by AO plating is reported. There were 18 different surgeons. Open fractures occurred in 24 per cent of limbs. Reviews at a mean of 3 years after the accident showed 97 per cent of bones to be solidly united, and satisfactory function to be achieved in 80 per cent of limbs. Deep infection occurred 6 times, and non-union in 7 bones. Cross-union developed in 6 patients, all of whom had sustained head injuries. Seven patients sustained operative nerve injuries. It is felt that the implant of choice at

Dislocation of the elbow: a retrospective study of 115 patients.

Of the 115 elbow dislocations treated at two New Orleans hospitals between 1959 and 1969, 78% were posterior. Sixty-one percent of the patients were 21 years of age or younger, and the male-female ratio was 1.7:1.0. The most common associated injuries were fractures of the medial condyle or epicondyle. All uncomplicated dislocations were treated by closed reduction and plaster immobilization. Of the 35 cases complicated by medial condylar or epicondylar fractures, 29 required open reduction and internal fixation. Results were excellent or good in 91% of the cases. The three patients (2.6%) wit

A Rare Case of a Bilateral Olecranon Fracture in a Young Adult With a Two-Year Follow-Up.

Olecranon fractures are common in orthopedic wards and can be traumatic or pathological in origin. There are very few cases of bilateral olecranon fractures without any associated injuries to the long bones in the literature. We present a unique case of a young 21-year-old male who has an isolated bilateral olecranon fracture following a road traffic accident. The patient had a closed fracture of the ulna on both sides without any associated injuries or neurovascular compromise. Since the patient was young and had good muscle strength preoperatively, we planned fixation of both sides. The pati

Management of Patella Fractures Non-amenable to Tension Band Wiring: Series of Nine Cases with Review of Literature.

INTRODUCTION: Isolated coracoid process fracture of scapula is a rare. Usually, it happens along with the rotator cuff tear, dislocation of acromioclavicular joint, or glenohumeral joint. Displaced fracture will cause impairment of upper limb movement. Since there are very low numbers of cases reported so far, the treatment is still controversial. CASE REPORT: We report a case of a 42-year-old male laborer with motor vehicle accident, following which he had pain and restricted movements on his left shoulder. On examination, Glasgow Coma Scale 15/15 and left shoulder had swelling and tendernes

Exposing the incidence of ileus in pelvic and acetabular fractures: a retrospective case analysis.

BACKGROUND: Paralytic ileus is a temporary inhibition of gastrointestinal mobility in the absence of mechanical obstruction. Ileus has previously been observed in up to 40% of patients undergoing bowel surgery, leading to increased morbidity and length of stay. Pelvic and acetabular fractures are often caused by high energy trauma and are associated with a risk of visceral injury.  Prior to this study, there were no reported figures for the incidence of ileus in patients presenting with pelvic and/or acetabular fractures. METHODS: All patients over the age of 16 presenting to a major trauma c

[3D-printed template-assisted reduction of long bone fractures].

INTRODUCTION: In multiply injured patients, the time period between the initial treatment with external fixators (damage control) and the definitive treatment can last from days to weeks. A poor reduction result with the fixator (e.g. malposition in axis, length and rotation) and a long delay from trauma to definitive osteosynthesis are associated with longer operation times, higher intraoperative radiation doses, higher infection rates and an increased likelihood for the necessity to perform an open reduction. MATERIAL AND METHODS: In the described technique computed tomography (CT) is perfo
