Sigmoid Colon Obstruction due to Gallstone Ileus: Case Report.

INTRODUCTION: Gallstone ileus of the colon is a rare cause of large bowel obstruction, usually caused by a gallbladder-to-colon fistula allowing stone entry. The classical presentation is with episodic subacute obstruction. Contrast-enhanced CT is valuable for diagnosis, with surgery being the preferred treatment; however, endoscopic therapy has been described and may also be attempted. CASE PRESENTATION: We describe the case of an 81-year-old female with a medical history significant for atrial fibrillation and congestive heart failure. She presented with a 2-week history of constipation, ab

Mapping the 5Ms: Identifying Gaps in a Geriatric Medicine Curriculum at a U.S. Medical School.

BACKGROUND: A strong foundation in the care of older adults is crucial for caring for the growing aging population. While residency training curricula in geriatric medicine are well-studied, data on undergraduate medical education curricula are limited. This study compares the Minimum Geriatrics Competencies for Graduating Medical Students (which follows the 5Ms framework) to the geriatrics curriculum taught at the University of Missouri-Columbia School of Medicine. METHODS: Using the school's curriculum map, we identified curricular elements through a keyword search utilizing 5 M (Mobility,

Exercise attitudes and practices among adults listed for kidney transplantation: a survey of a diverse patient cohort.

BACKGROUND: Given the increasing age and frailty of kidney transplant candidates, there is an emerging drive to optimise patients before transplantation. Lack of exercise has been linked with poor outcomes at all stages of the transplant pathway. The aim of this study was to evaluate the attitudes and perception to exercise in such patients and assess how these practises vary by demographics. METHODS: A single-centre, prospective, survey-based study was conducted on consecutive adult patients being assessed for activation on the deceased-donor kidney transplant waiting list. RESULTS: A total

Patient physical condition and functional sequelae following hospitalization with COVID-19: A cross-sectional observational study.

After hospitalization caused by COVID-19, a high prevalence of physical deterioration has been observed, hence the importance of having tests to evaluate the functional status of patients and to be able to perform a partition and subsequent referral to the physiotherapy service. This cross-sectional observational study describes the physical status according to the short physical performance battery (SPPB) of patients admitted to the hospital setting for COVID-19 and to identify variables potentially related to this outcome. Thirty-six patients admitted to the hospital setting for COVID-19 in

Updated evidence on selection and implementation of an invasive treatment strategy for older patients with non-ST-segment elevation myocardial infarction.

Non-ST-segment elevation myocardial infarction (NSTEMI) is the most common acute coronary syndrome diagnosis in older patients. In the UK, there are ~20 000 NSTEMI cases annually in patients aged ≥75 years. Despite therapeutic advances in pharmacological and invasive management, studies show that older patients with NSTEMI experience worse in-hospital and long-term outcomes than younger patients, suggesting a clear need for robust evidence in this cohort. The European Society of Cardiology guidelines recommend that invasive management should be considered holistically with no specified age cut-

Risk management of cardiovascular disease in older patients with diabetes.

The number of older patients with diabetes has been increasing due to Japan's rapidly aging population. As aging and diabetes are independent risk factors for cardiovascular disease, older adults face a higher risk of cardiovascular disease, with the incidence of heart failure and stroke increasing significantly with age. Preventing cardiovascular events is critical for this population to maintain their activities of daily living and improve their quality of life. To achieve this challenging purpose, it is essential to prioritize the use of glucose-lowering medications with proven cardiovascul

Increasing Social Connection in Permanent Supportive Housing: A Participatory Study on Therapeutic Recreation for People With Experiences of Homelessness.

BACKGROUND AND OBJECTIVES: Social exclusion and isolation due to homelessness are compounded in older people by physical frailty and stigma. Interventions to support older people with experiences of homelessness often neglect social dimensions of well-being. In this participatory action research study, residents, staff, and researchers collaborated to develop, implement, and evaluate therapeutic recreation programming in a permanent supportive housing facility for older people with experiences of homelessness (n = 68). Bridging the age gap: improving representation of older adults in clinical trials for type 2 diabetes and chronic kidney disease.

Chronic kidney disease (CKD) is common among older adults, with more than a third of individuals older than 65 years having moderate to severe CKD globally. Management of CKD in older adults is often complex because of multiple comorbidities, polypharmacy, and frailty. However, older adults remain under-represented in clinical trials for both type 2 diabetes (T2D), which is the leading risk factor for CKD, and CKD itself. Recent analyses show consistent low representation of older adults in T2D trials. Similarly, participants of dialysis trials tend to be younger, healthier, and have a lower m

Heart Failure in Older Patients: An Update.

Heart failure (HF) is a clinical syndrome with high incidence and prevalence and high morbidity and death rate, even in the short term, representing a serious public health issue, mainly in older people. It is a growing cause for hospital admission in this age group, being frequently associated with several comorbidities, further aggravating the disease's course. Moreover, older HF patients are usually affected by clinical conditions, like frailty, malnutrition, and cachexia, which significantly impact the overall management of HF and need to be properly identified and treated. Diagnosing and

Exploring the Association Between Torquetenovirus Viral Load and Immunosuppressive Drug Exposure in Lung Transplantation.

To improve lung transplant recipient (LungTx) outcome, it would be of great interest to measure the net state of immunosuppression to avoid both infection and rejection. Measurement of Torquetenovirus load (TTV load) has been proposed as a biomarker to monitor solid organ transplantation, but its relationship with immunosuppressive drugs, particularly mycophenolic acid (MPA), is not well understood. We performed a prospective study of 53 LungTx, measuring TTV load before transplantation, at week 3, and at month 3. Tacrolimus and MPA doses and levels were recorded, and an area under the MPA cur
