Compound muscle action potentials in newborn infants with spina bifida.

The aim of this study was to investigate the relationship between compound muscle action potentials (CMAPs) and neurological impairment in newborn infants with spina bifida. Thirty-one newborn infants (17 males, 14 females, mean gestational age 39 wks [SD 2]; mean birthweight 3336 g [SD 496]) with spina bifida were investigated at a median age of 2 days (range 1-18 d). Motor and sensory impairment and muscle stretch reflexes were assessed and neuroimaging was performed. CMAPs were recorded from the tibialis anterior muscle and the gastrocnemius muscle after percutaneous electrical nerve stimul

Acute posterior leukoencephalopathy in a patient with nephrotic syndrome.

Reversible posterior leukoencephalopathy syndrome is one of the most serious complications of immunosuppressive therapy. The clinical features include headache, altered mental functioning, seizures, cortical blindness and other visual disturbances, with hypertension. The neuroimaging studies reveal predominant posterior leukoencephalopathy. Usually, antihypertensive therapy and reduction or withdrawal of immunosuppressive agents have been reported to resolve the neurological deficits and imaging abnormalities within a few weeks. We discuss here a 51-year-old woman with nephrotic syndrome who d

Research priorities in the evolving demographic landscape of Alzheimer disease and associated dementias.

The rapid increase of ethnically diverse late onset dementia-affected persons is bringing a special urgency to Alzheimer disease and associated disorders research. By this decade's end, non-European heritage ethnically diverse persons will account for 25% of the dementia-affected cohort and 33% by mid-century. These populations have received some attention in both dementia-focused basic biomedical and social-behavioral research. However, studies differ greatly with regard to the constructs of culture used, the methodologies used, and representativeness of the populations sampled. Moreover, soc

Cerebellar degeneration associated with human immunodeficiency virus infection.

Cerebellar disorders associated with HIV infection are typically the result of discrete cerebellar lesions resulting from opportunistic infections such as toxoplasmosis and progressive multifocal leukoencephalopathy or primary CNS lymphoma. Clinical symptoms and pathologic abnormalities related to the cerebellum may also be observed with HIV dementia. A primary cerebellar degeneration with HIV has not previously been reported. Ten patients were identified over an 8-year period at five medical centers. All patients had clinical, laboratory, and radiologic evaluations, and three had neuropatholo

Pumping the shunt revisited. A longitudinal study.

Most cerebrospinal fluid (CSF) shunts employed in the treatment of hydrocephalus include in their designs a pump mechanism that has been used in office practice as an indicator of whether the shunt is functioning properly; however, the pumping characteristics of shunts have been shown to correlate poorly with clinical status at a single point in time. A longitudinal study was performed to determine whether interval changes in the pumping characteristics of shunts correlate with changes in clinical status. 115 patients were examined on at least two occasions by a single pediatric neurosurgeon.

Geometric brain signatures of Alzheimer's disease progression and subtypes.

Alzheimer's disease (AD) patients suffer from consequential diagnostic delay due to the lack of accessible biomarkers. They also show different responses to treatments due to disease heterogeneity and progression. Here, we developed a novel framework to identify disease progression and subtypes by using geometric brain signatures derived from multiple neuroimaging modalities, including [ 18 F]-Florbetapir (AV45) Positron Emission Tomography (PET), [ 18 F]-Fludeoxyglucose (FDG) PET, and structural Magnetic Resonance Imaging (MRI). These signatures were derived by decomposing corresponding maps

Sleep apnea and the ageing brain: bridging mechanisms and clinical outcomes.

Obstructive sleep apnea (OSA) is a highly prevalent sleep-related breathing disorder increasingly recognized for its association with cognitive impairment and dementia. Emerging epidemiological evidence suggests that individuals with OSA have a higher risk of developing mild cognitive impairment and accelerated cognitive decline. The pathophysiological mechanisms underlying this relationship are multifactorial, involving intermittent hypoxia, sleep fragmentation, and sympathetic activation. These disturbances contribute to oxidative stress, neuroinflammation, impaired glymphatic clearance of a

Brain structure and function in Homo naledi.

Endocasts are casts of the internal surface of skull bones created by the brain and surrounding tissues. These fossil phantoms provide the most direct evidence of the brains of extinct organisms, and are one of the many puzzle pieces that help paleoneurologists reconstruct brain evolution. The recent discovery of a small-brained, recently-extinct human species, Homo naledi, creates a timely opportunity to review what endocasts can and cannot tell us about ancient brains. We first review published evidence about the brain and behavior of H. naledi, including the suggestion that the species may

Pediatric osmotic demyelination syndrome triggered by post-traumatic pituitary stalk injury: a case report.

BACKGROUND: Osmotic demyelination syndrome (ODS) is an acute neurological disorder resulting from demyelination, typically triggered by rapid osmotic shifts. This syndrome is rare in adults and even more uncommon in children. To date, no reported cases have documented ODS secondary to hyponatremia caused by post-traumatic pituitary stalk injury. We report a pediatric patient with traumatic brain injury who developed ODS subsequent to pituitary stalk disruption and hyponatremia, and discuss the corresponding diagnostic and therapeutic strategies. CASE PRESENTATION: We report the case of a 13-ye

Hyperthyroidism-Induced Dural Venous Sinus Thrombosis: A Case Report.

Cerebral dural venous sinus thrombosis is an uncommon and potentially life-threatening cerebrovascular disorder with a broad and often nonspecific clinical spectrum, frequently leading to diagnostic delay. We describe a young woman with no prior comorbidities who presented with a subacute history of progressively worsening headache associated with visual disturbances and pulsatile tinnitus. Neuroimaging with computed tomography and magnetic resonance venography demonstrated acute thrombosis of the right transverse dural venous sinus without parenchymal infarction or hemorrhage. Network spreading and local biological vulnerability in amyotrophic lateral sclerosis.

Amyotrophic Lateral Sclerosis (ALS) is a progressive neurodegenerative disease that predominantly targets the motor system. Spread of pathology is thought to be driven by both local vulnerability and network architecture. Namely, molecular and cellular features may confer vulnerability to specific neuronal populations, while synaptic contacts may also increase exposure to pathology in connected neuronal populations. However, these principles are typically studied in isolation and it remains unknown how local vulnerability and network spreading interact to shape cortical atrophy. Here, we inves

Outlier Detection and Cross-Modal Representation Learning for Multimodal Alzheimer's Disease Diagnosis.

The early diagnosis of Alzheimer's disease (AD) is crucial because individuals may first experience mild cognitive impairment (MCI), which can then develop into AD, enabling timely intervention, slowing disease progression, and advancing the understanding of AD pathology. However, existing methods face two major challenges: first, they lack effective mechanisms to handle abnormal samples in neuroimaging data, which can distort model learning; second, they do not fully exploit complementary structural information across modalities, leading to insufficient discriminative power. To tackle these p

Unsupervised Dimensionality Reduction Techniques for the Assessment of ASD Biomarkers.

Autism Spectrum Disorder (ASD) encompasses a range of developmental disabilities marked by differences in social functioning, cognition, and behavior. Both genetic and environmental factors are known to contribute to ASD, yet the exact etiological factors remain unclear. Developing integrative models to explore the effects of gene expression on behavioral and cognitive traits attributed to ASD can uncover environmental and genetic interactions. A notable aspect of ASD research is the sex-wise diagnostic disparity: males are diagnosed more frequently than females, which suggests potential sex-s

Group membership and stereotype content jointly affect neural processing during resource allocation decisions.

Both stereotypes and in/outgroup categories affect judgment and decision making, but while the former reflect socially learned generalizations about other people's characteristics, the latter specify relationships with whom it is safe to affiliate. With neuroimaging data we substantiate these distinct functions and explore how their underlying neural correlates interact to influence behavioral biases. Participants in the MRI scanner are asked to share money with recipients who are stereotyped along a warm/cold dimension and who belong either to a (minimally defined) in- or outgroup. Whole-brai

Spontaneous Cortical Intracerebral Hemorrhage Causing Ipsilateral Hand and Oral Sensory Change Without Motor Deficit: A Case Report.

Intracerebral hemorrhage (ICH) is the second most common stroke subtype associated with high morbidity and mortality rates. Although various brain regions are susceptible to ICH, putaminal hemorrhage is the most common, whereas cortical ICH is less common. Here, we report the case of a 69-year-old man who developed a parietal cortical ICH. The patient developed hypoesthesia and paresthesia in the right upper lip and hand; however, the weakness was not severe. Twenty-five days after the ICH onset, the manual muscle test results were normal, but he had difficulty eating and shaving because of de

Candidate Biological Markers for Social Anxiety Disorder: A Systematic Review.

Social anxiety disorder (SAD) is a common psychiatric condition associated with a high risk of psychiatric comorbidity and impaired social/occupational functioning when not promptly treated. The identification of biological markers may facilitate the diagnostic process, leading to an early and proper treatment. Our aim was to systematically review the available literature about potential biomarkers for SAD. A search in the main online repositories (PubMed, ISI Web of Knowledge, PsychInfo, etc.) was performed. Of the 662 records screened, 61 were included. Results concerning cortisol, neuropept
