The Journal of bone and joint surgery. American volume2011eng
BACKGROUND: Surgical lengthening is used to address both overactivity and shortening of the spastic agonist muscle in children with cerebral palsy. It has been presumed that the function of the antagonist muscle will improve when the spastic agonist muscle has been surgically lengthened. The purposes of the current study were to use quantitative gait analysis to determine the prevalence of the ankle dorsiflexor muscles (antagonist) dysfunction during the swing phase of the gait cycle and to analyze how this function is affected following surgical lengthening of the ankle plantar flexor muscles

Normal pressure hydrocephalus, defined clinically by Hakim's triad of gait and balance disturbance, cognitive decline, and urinary incontinence, presents a critical challenge in neurology. While the ventriculoperitoneal shunt remains the standard treatment, growing evidence suggests that the lumboperitoneal shunt (LPS) rivals its effectiveness, achieving similar improvements in gait and balance, cognition, and continence, with comparable complication rates. LPS may also offer advantages, such as local anaesthesia, shorter procedures, and shorter hospital stays, which could lead to cost savings

The striatum is a critical component of the basal ganglia and plays a central role in regulating motor initiation and action selection. How cortical and subcortical inputs converging at the striatum regulate locomotion remains unclear. By examining gait changes in head-fixed mice running on a treadmill, we found that mice were capable of performing forward, but not backward, rhythmic locomotion using their forelimbs when the striatum and motor cortex were inactivated. The striatal activity is critical for adjusting initially disorganized gait to efficient rhythmic locomotion during forward run

Normal pressure hydrocephalus (NPH) is characterized by pathologic ventriculomegaly with normal opening pressures on lumbar puncture. It commonly presents with a triad of gait disturbance, cognitive impairment, and urinary bladder detrusor dysfunction. Its pathogenesis is complex but is thought to arise in the setting of imbalanced cerebrospinal fluid (CSF) secretion and absorption. Given that intracranial pressure often remains normal in the setting of NPH, visual symptoms are quite uncommon. Here we present a case of a 70-year-old female with a subacute history of visual aberration described

Limping refers to an asymmetrical gait that deviates from the typical gait pattern expected for a child of a certain age. In most children, limping is caused by a mild, self-limiting event, such as a contusion, strain, or sprain. However, a child's limping is always a pathological finding that poses a particular diagnostic challenge and necessitates a thorough assessment. The pediatrician must weigh a wide range of acute and chronic potential causes of a non-traumatic limp, including infection, neoplasia, and chronic inflammatory disorders. A thorough history and clinical examination will help

Cerebellar involvement in primary Sjögren's syndrome (pSS) is an uncommon condition, with only a limited number of cases described worldwide. A 43-year-old woman affected by cerebellar atrophy associated with pSS was referred to our center to undergo a cycle of physical rehabilitation therapy. Although motor symptoms started when the patient was 23 years of age, the underlying disease remained undiagnosed for several years. Neurological examination before rehabilitation revealed ataxic gait, dysmetria, nystagmus, and hypermetric saccades; the patients complained about unsteadiness while standi

In order to achieve a high speed of skating robot based on the leg structure - with passive wheels on even ground - the motion characteristics of human roller skating are studied in this paper. Utilizing a three dimensional motion capture system, the linear and turning gait are analyzed to express the motion characteristics of human roller skating. According to the observation and analysis, the normal linear gait can be divided into four phases and normal turning can be divided into three phases. The spin angle between the sagittal plane and the roller skate of the supporting leg is mainly gen

People usually develop different kinds of compensated gait in response to local function deficits, such as muscle weakness, spasticity in specific muscle groups, or joint stiffness, in order to overcome the falling risk factors. Compensated walking has been analysed empirically in the impaired gait analysis area. However, the compensation could be identified spatially and temporally. The stability and perturbation resistance of compensated walking have not been analysed quantitatively. In this research, a biomimetic human walking simulator was employed to model one individual paraplegic subjec

Parkinson's disease is a neurological disorder, and early diagnosis is crucial for the treatment and quality of life of patients. Gait movement disorder is a significant manifestation of PD, and automated gait assessment is key to achieving automated detection of PD patients. With the development of deep learning, in order to improve the accuracy of early Parkinson's disease detection and enhance the robustness of motion recognition models, this study introduces an innovative deep learning approach, namely Multi-area Attention Spatiotemporal Directed Graph Convolutional Network (Ma-ST-DGN). INTRODUCTION: Motor symptoms may present throughout the Alzheimer's disease (AD) course. We evaluated the impact of motor symptoms on the risk of progression to severe cognitive impairment, severe neuropsychiatric symptoms (NPSs), and mortality in patients with autopsy-confirmed AD. We also examined the rates of non-AD pathology and identified the predictors of disease progression among these patients.
METHODS: Data from the National Alzheimer's Coordinating Center (NACC) were used for this analysis. Participants (N = 1167) were required to have autopsy-confirmed AD.
RESULTS: Motor symptoms

Frailty is highly prevalent among patients with aortic stenosis (AS). Nonetheless, the impact of AS severity on frailty remains unclear. This study aimed to clarify the association between AS severity and frailty in this population. This retrospective study included consecutive patients with AS who were hospitalized as candidates for transcatheter aortic valve implantation (TAVI). The prevalence of frailty, N-terminal pro B-type natriuretic peptide (NT-proBNP) level, gait speed, and geriatric nutritional risk index (GNRI) were compared between patients with severe and those with very severe AS.

The air-water of the planet's water bodies, such as ponds, lakes and streams, presents an uncertain ecological niche with predatory threats from above and below. As Microvelia move across the water surface in small ponds, they face potential injury from attacks by birds, fish, and underwater invertebrates. Thus, our study investigates the effects of losing individual or pairs of tarsi on the Microvelia's ability to walk on water. Removal of both hind tarsi causes Microvelia spp. to rock their bodies (yaw) while running across the water surface at ±19°, compared to ±7° in non-ablated specimens.

The number of patients with unilateral above-knee amputation (AKA) due to non-vascular causes has remained stable over the years, at 0.92 per 1000 people per year. Post-AKA individuals are at risk of experiencing a higher incidence of chronic pain. Post rehabilitation, it is estimated that between 16−62% of patients with musculoskeletal disabilities fail to meet the minimum criteria for physical activity in comparison to a healthy population. The current study included 14 participants (11 men and 3 women) with a mean age of 46.1 ± 14.2 years, body height of 1.76 ± 0.09 m, and weight of 79.6 ±

BACKGROUND: Idiopathic normal pressure hydrocephalus (iNPH) is characterized by the classic clinical triad of gait, cognitive, and urinary dysfunction, albeit incomplete in a relevant proportion of patients. The clinical findings and evolution of these symptoms have been variably defined in the literature.
OBJECTIVES: To evaluate how the phenomenology has been defined, assessed, and reported, we performed a critical review of the existing literature discussing the phenomenology of iNPH. The review also identified the instrumental tests most frequently used and the evolution of clinical and ra
