RATIONALE: Congenital neutropenia (CN) encompasses a group of disorders characterized by impaired neutrophil differentiation, resulting in persistently low neutrophil counts in the peripheral blood. It presents with recurrent infections and an elevated risk of leukemia. Multiple genetic mutations have been implicated in the pathogenesis of neutropenia.
PATIENT CONCERNS: This paper reports the case of a 3-year-2-month-old boy admitted with a 4-day history of cough and fever, accompanied by recurrent respiratory infections, neutropenia, and growth retardation. Whole-exome sequencing identified

A 63-year-old woman presented with fever, cough, and recurrent presyncope following a lower respiratory tract infection. Initial telemetry demonstrated intermittent complete atrioventricular block with ventricular premature complexes. Despite correction of electrolyte abnormalities and withdrawal of QT-prolonging medications (prescribed for cough), she developed repetitive episodes of polymorphic ventricular tachycardia triggered by R-on-T phenomena, requiring electrical cardioversion and suppression with overdrive pacing after magnesium and phenytoin proved ineffective. A nasopharyngeal bio f

BACKGROUND: Vestibular schwannomas (VSs) are common benign tumors of the cerebellopontine angle, often treated with surgery or stereotactic radiosurgery to preserve nerve function. Malignant transformation of VS (MTVS) is exceedingly rare and has been primarily associated with neurofibromatosis type 1 (NF1), neurofibromatosis type 2-related schwannomatosis (NF2-SWN), or prior radiation exposure. However, cases of MTVS occurring without these risk factors are rare and remain poorly understood.
OBSERVATIONS: A 58-year-old woman without neurofibromatosis underwent subtotal VS resection in 2019,

Pancreatic ductal adenocarcinoma (PDAC) continues to carry a poor prognosis, and only incremental gains in systemic therapy have been made over the past decade. Because activating KRAS mutations are present in more than 90% of PDAC and serve as the inciting oncogenic driver, RAS has long been a coveted but elusive therapeutic target, historically deemed "undruggable." Allele-specific KRASG12C inhibitors provided the first proof of concept, but G12C mutations account for only about 1% of PDAC, in which G12D, G12V, and G12R predominate. Recent advances in the structural and biochemical understan

Acute lymphoblastic leukemia (ALL) is the most common childhood cancer. Thiopurines such as 6-mercaptopurine (6MP) are essential in ALL maintenance therapy. However, dose-limiting toxicities can significantly disrupt treatment. While genetic variants in TPMT and NUDT15 are known to affect thiopurine response, many patients with normal function genotypes in these genes still experience adverse effects, suggesting that additional genes might be involved. We analyzed 663 pediatric ALL patients enrolled in the AALL03N1 trial to identify novel genetic determinants of 6MP sensitivity, focusing on in

BACKGROUND: Cardiovascular diseases (CVDs) are the leading cause of mortality worldwide, with hereditary genetic factors contributing substantially to disease burden. Current treatments, including lifestyle modifications, pharmacotherapy, and surgical interventions, focus primarily on symptom management but fail to address underlying genetic causes, often resulting in disease progression or recurrence. Gene therapy has emerged as a transformative approach, offering a potential treatment. This review explores its efficacy and safety in animal models, identifying opportunities for future advance

BACKGROUND: Dual antiplatelet therapy with aspirin and a P2Y₁₂ inhibitor is standard for patients with acute coronary syndrome (ACS) undergoing percutaneous coronary intervention. While ticagrelor offers superior efficacy in reducing ischemic events compared to clopidogrel, its use is limited by a higher incidence of dyspnea, an adverse reaction whose underlying predictors remain incompletely understood. Genetic variations in CYP3A4/5, the principal enzymes responsible for ticagrelor metabolism, may influence interindividual susceptibility to this side effect.
METHODS: In a prospective cohort

BACKGROUND: Congenital dysfibrinogenemia is a rare autosomal dominant disorder involving abnormal fibrinogen function, leading to variable risks of bleeding and thrombosis. Its management during pregnancy is particularly challenging due to the potential for complications such as miscarriage, stillbirth, placental abruption, and fetal growth restriction. The absence of reliable predictive laboratory markers further complicate individualized risk assessment and clinical decision-making. CASE PRESENTATION: A 23-year-old primigravida was diagnosed with congenital dysfibrinogenemia after low fibrin

Miyoshi myopathy is a muscular dystrophy disease characterized by muscle weakness and atrophy generally in distal muscle groups, such as in the legs and arms. Miyoshi myopathy is thought to occur due to genetic mutations in the DYSF gene, which codes for the dysferlin protein, which is critical for muscle cell membrane integrity and muscle fiber adhesiveness. The first symptoms begin in early adulthood and include weakness and atrophy in the calves, gait abnormalities, pain and discomfort in affected muscles, and difficulty jumping or walking on tiptoes. Patients generally are diagnosed by a c

BACKGROUND: Episodic ataxia type 2 (EA2) is a rare, autosomal dominant paroxysmal neurological disorder caused by mutations in the CACNA1A gene. Its hallmark features include transient episodes of ataxia and dysarthria, often responsive to acetazolamide. However, misdiagnosis as epilepsy may occur due to overlapping electroencephalographic findings.
CASE PRESENTATION: We report a 64-year-old woman with no prior history nor family history of neurological illness, who developed daily episodes of dysarthria and ataxic gait. Brain MRI did not show definite signs of cerebellar atrophy. Interictal

BACKGROUND: Diffuse large B-cell lymphoma (DLBCL) secondary to acute lymphoblastic leukemia (ALL) is a rare disease with poor prognosis, usually attributed to delayed diagnosis. To date, only four cases of ALL developing DLBCL have been reported, while none of them exhibiting central nervous system (CNS) symptoms.
CASE DESCRIPTION: Here, we report an unusual case of a 15-year-old boy diagnosed with ALL and treated based on the SCCLG-ALL 2016 protocol. While he was receiving maintenance treatment, the patient developed dizziness and vomiting. An Epstein-Barr virus (EBV)-positive DLBCL with CNS

The increasing prevalence of breast cancer presents a significant global health challenge, highlighting the urgent need for improved diagnostic and treatment monitoring methods. The non-invasive nature of cell-free circulating epigenomic biomarkers, such as methylated DNA (metDNA) and microRNAs (miRNAs), offers a reassuring approach to identifying breast cancer patients in the early stages and assessing their response to therapy. This approach holds great promise for diagnosis and treatment evaluation, prioritizing patient comfort and well-being. Cell-free circulating metDNA and miRNAs are rel

Recently, the International Consensus Classification (ICC) and the 5th edition of the World Health Organization classification (WHO2022) introduced diagnostically similar yet distinct approaches, which has resulted in practical confusion. This review compares these classification systems for acute myeloid leukemia (AML), building up on the revised 4th edition of WHO (WHO2016). Both classifications retain recurrent genetic abnormalities as a primary consideration. However, they differ in terms of blast threshold. The ICC mandates a minimum of 10% blasts in the bone marrow or peripheral blood, w

INTRODUCTION: The mechanism of relapsed CD19(-) B-ALL after anti-CD19 immunotherapy (Kymriah [CART-19] and blinatumomab) is under active investigation. Our study aims to assess LILRB1 as a novel B-cell marker for detecting CD19(-) B-lymphoblasts and to analyze the clinicopathologic/genetic features of such disease to provide biological insight into relapse.
METHODS: Six patients (3 males/3 females, median age of 14 years) with relapsed CD19(-) B-ALL were analyzed for cytogenetic/genetic profile and immunophenotype.
RESULTS: CD19(-) B-ALL emerged after an interval of 5.8 months following anti

Pituitary stalk interruption syndrome (PSIS) is a rare disease associated with either isolated growth hormone deficiency (GHD) or combined pituitary hormone deficiency (CPHD). In older children and adults, most patients experience short stature or hypogonadism. Neonatal PSIS is extremely rare and is difficult to recognize due to absence of dwarfism. However, when this condition occurs in newborns, it is often life-threatening. Here, we collected patients with neonatal PSIS to clarify its characteristics to improve its early diagnosis. The patients included in this study were treated at the pedi

Since several years targeted therapy has been part of treatment in NSCLC in subsets of patients with specific genetic alterations. One of these alterations involves HER2, a member of the ERBB family of tyrosine kinase receptors. Despite that HER2 alterations in NSCLC have been studied for years, there is still no consensus about subgroup definitions. In this review HER2 alterations in NSCLC are discussed, including diagnostic challenges and treatment strategies. Three principal mechanisms of HER2 alterations can be identified: HER2 protein overexpression, HER2 gene amplification and HER2 gene

The paper deals with a case of TRAPS in a Ukrainian family. The manifestations of this syndrome appeared at the age of 2,5 years and gradually the attacks of fever became morefrequent and the recurrence was typical of this diagnosis. Classically, besides fever, there was an intense abdominal pain, such as an "acute abdomen", arthralgia in the right hip and headache. Micropoliadenia was also detected. This patient did not have any other symptoms. A genetic study found a mutation in the TNFRSF1A gene (substitution in exon 4 with 3449T> G: p.C117G). This mutation has not been recorded in the inte

Eosinophilia is a severe disease with increased eosinophil count. The transcript of FIP1L1-PDGFRA fusion gene is a genetic biomarker of clonal eosinophilia screened routinely by reverse transcript PCR (RT-PCR) during diagnosis. Another significant genetic biomarker is the PDGFRA gene alone as some of its mutations are targets of imatinib. In this study, we identified a patient who had typical symptoms of Eosinophilia but had no response to the first-line treatment of hormonotherapy. This patient also showed bone rupture and eosinophil bone infiltration, which are extremely rare among all known

Phosphate plays essential roles in many biological processes, and the serum phosphate level is tightly controlled. Chronic hypophosphatemia causes impaired mineralization of the bone matrix and results in rickets and osteomalacia. Fibroblast growth factor 23 (FGF23) is a bone-derived hormone that regulates phosphate metabolism. FGF23 excess induces hypophosphatemia via impaired phosphate reabsorption in the renal proximal tubules and decreased phosphate absorption in the intestines. There are several types of genetic and acquired FGF23-related hypophosphatemic diseases. Among these diseases, X
