GLP-1RAs versus metformin and Parkinson's risk in type 2 diabetes.

BACKGROUND: Parkinson's disease (PD) is an increasingly prevalent neurodegenerative condition, particularly among individuals with type 2 diabetes mellitus (T2DM). While prior studies have suggested that GLP-1 receptor agonists (GLP-1RAs) and metformin may confer neuroprotective effects, most were limited by small sample sizes, short follow-up, uncontrolled designs, or lacked direct comparisons between therapies-making their findings inconclusive for clinical decision-making. METHODS: Using the TriNetX Global Collaborative Network, we identified 92 485 patients with T2DM initiating GLP-1RA th

Semaglutide in patients with hidradenitis suppurativa and obesity.

BACKGROUND: Hidradenitis suppurativa (HS) is a chronic inflammatory dermatosis strongly associated with obesity and metabolic dysfunction. GLP-1 receptor agonists (GLP-1 RAs), such as semaglutide, promote weight loss and exhibit anti-inflammatory properties, but evidence regarding their effects in HS remains limited. OBJECTIVE: To evaluate the effects of 6 months of semaglutide treatment at doses >1mg/week on anthropometric, dermatologic, metabolic, inflammatory, and quality-of-life outcomes in patients with HS and obesity. METHODS: We conducted a prospective, open-label cohort study includi

Challenges in Diagnosing Central Adrenal Insufficiency in Children: Cortisol-Stimulating Tests are Safe and Often Required.

INTRODUCTION: The accuracy and safety of cortisol-stimulating tests (CSTs) for assessing hypothalamic-pituitary-adrenal (HPA) axis integrity, including the diagnosis of central adrenal insufficiency (CAI), in children remain uncertain. Although these tests can simultaneously evaluate cortisol and growth hormone secretion, the present study focuses exclusively on cortisol measurements. METHODS: We evaluated the diagnostic accuracy and safety profile of CST in pediatric patients submitted to insulin tolerance test (ITT) or glucagon-stimulation test (GST) in a tertiary center for 15 years in thi

Evaluation of transduction efficiency in pancreatic beta and alpha cells utilizing various AAV serotypes.

Adeno-associated viruses (AAVs) have emerged as powerful tools for delivering genes to various cell types, including pancreatic endocrine cells. Currently, AAV serotype 8 (AAV8) is the primary AAV vector employed for transducing pancreatic cells for transgene expression. We aimed to determine whether alternative serotypes, specifically AAV2, AAV6, and AAV9, commonly used for gene transfer, can efficiently transduce pancreatic cells in vitro. Murine pancreatic β-cells, α-cells, and fibroblasts were transduced with AAV serotypes 2, 6, and 9 carrying the transgene for enhanced green fluorescent p

Sex-Specific Differences in the Revascularization of Grafted Pancreatic Islets.

Islet transplantation can improve glycemic control in a subset of patients with type 1 diabetes mellitus (T1DM). This therapeutic approach is often limited by scarcity of adequate donor islets and an insufficient revascularization capacity of grafted islets. Recent findings reveal that sex is an important determinant for the outcome of islet transplantation. However, it is still unknown how the biological sex of islet donors and recipients affects the revascularization of the grafts during the initial ischemic post-transplantation phase. In this study, we observed in a mouse dorsal skinfold ch

Off-label GLP-1 weight-loss medicine use among online bodybuilders: Folk pharmacology, risk and harm reduction.

BACKGROUND: GLP-1 medicines (e.g. Semaglutide; Tirzepatide) are diabetes medicines that have recently been approved for weight loss purposes. These drugs are increasingly common, with many users accessing these medicines for off-label lifestyle-oriented purposes. While recent research has explored increased interest in these drugs, to date no studies have explored their use in bodybuilding cultures. With bodybuilders often at the forefront of experimental drug use, and diffusing knowledge of drugs to other populations, this research examines bodybuilders' understandings of use and risk of GLP-

Fusion Outcomes of GLP-1 Agonist Therapy in Multilevel Cervical Spinal Fusion: A Propensity-Matched Analysis.

STUDY DESIGN: Retrospective analysis. OBJECTIVE: To evaluate the effects of GLP-1 agonist therapy upon the incidence of pseudarthrosis in patients undergoing multilevel cervical spinal fusion. SUMMARY OF BACKGROUND DATA: The rising prevalence of obesity and diabetes mellitus has rendered the usage of glucagon-like peptide-1 receptor (GLP-1) agonists increasingly commonplace since their introduction in 2005. However, there is a dearth of evidence to suggest whether outcomes of multilevel cervical spinal fusion differ in patients treated with GLP-1 agonists. This study assesses rates of pseuda

Orthostatic hypotension after kidney transplantation in a patient with diabetic kidney disease: an underrecognized and therapeutic challenge.

Orthostatic hypotension (OH) is characterized by an excessive drop in blood pressure upon standing, leading to impaired quality of life, increased fall risk, and potential cardiovascular complications. It is frequently associated with autonomic dysfunction in patients with neurodegenerative diseases, diabetes mellitus, and aging. Despite its potential impact, OH may be underrecognized in kidney transplant (KT) recipients, particularly in the early post-transplant period, when diuresis-induced hypovolemia may serve as a precipitating factor. We present a case of severe OH in a woman in her 50s

Unraveling the impact of semaglutide in a diabetic rat model of testicular dysfunction: Insights into spermatogenesis pathways and miRNA-148a-5p.

BACKGROUND: Diabetes has been a long-known risk factor for male sexual dysfunction, which may be caused by persistent hyperglycemia, oxidative stress, and spermatogenesis inhibition. This study explored the potential of Semaglutide (Sem) to alleviate testicular dysfunction and spermatogenesis impairment in diabetic rats to understand the molecular mechanism of this protective effect. METHODOLOGY: A controlled experiment was conducted where 28 adult male rats were divided into four groups: control, Semaglutide, diabetic, and diabetes + Sem. Diabetes was induced using a single STZ dose (50 mg/k

Vagal activation alters prandial bile acid composition and glycemia in patients with hypoglycemia after Roux-en-Y gastric bypass surgery.

BACKGROUND: Altered prandial glycemic response after Roux-en-Y gastric bypass (RYGB) is exaggerated in patients with post-RYGB hypoglycemia. Increased contribution of glucagon-like peptide 1 (GLP-1) to prandial insulin secretion plays a key role in developing hypoglycemia after RYGB, but the role of nonhormonal gut factors remains unknown. Here, the effect of vagal activation on prandial bile acid (BA) composition in relation to glucose, insulin and gut hormone responses was examined in a small size group of nondiabetic subjects after RYGB with intact gallbladder compared to nonoperated contro

Effects of GLP-1 receptor agonists on kidney and cardiovascular disease outcomes: a meta-analysis of randomised controlled trials.

BACKGROUND: GLP-1 receptor agonists reduce the risk of major adverse cardiovascular events (MACE) and can also have kidney benefits. However, whether GLP-1 receptor agonists improve clinically important kidney outcomes remains uncertain. We aimed to comprehensively assess the effects of GLP-1 receptor agonists on kidney and cardiovascular disease outcomes by performing a meta-analysis of randomised controlled trials. METHODS: For this meta-analysis, we searched MEDLINE, Embase, and the Cochrane Central Register of Controlled Trials for randomised controlled trials that included at least 500 p

Incretin mimetics and acute pancreatitis: enemy or innocent bystander?

PURPOSE OF REVIEW: The incretin enhancers and mimetics, including dipeptidyl peptidase-4 (DPP-4) inhibitors, GLP-1 receptor agonists (GLP-1RA) and GLP-1/GIP co-agonists, have become mainstays in the treatment of type 2 diabetes (T2D). Recently, the approval of certain GLP-1RA and GLP-1/GIP co-agonists for the treatment of obesity has broadened their popularity and use. In this review, we summarize the evidence for an association of these drugs with acute pancreatitis and other adverse events of special interest to gastroenterologists. RECENT FINDINGS: In addition to pancreatic islets, GLP-1 r

Hepatocyte-derived DPP4 regulates portal GLP-1 bioactivity, modulates glucose production, and when absent influences NAFLD progression.

Elevated circulating dipeptidyl peptidase-4 (DPP4) is a biomarker for liver disease, but its involvement in gluconeogenesis and metabolic associated fatty liver disease progression remains unclear. Here, we identified that DPP4 in hepatocytes but not TEK receptor tyrosine kinase-positive endothelial cells regulates the local bioactivity of incretin hormones and gluconeogenesis. However, the complete absence of DPP4 (Dpp4-/-) in aged mice with metabolic syndrome accelerates liver fibrosis without altering dyslipidemia and steatosis. Analysis of transcripts from the livers of Dpp4-/- mice displa
