Estimates of the annual number and cost of new HIV infections among women attributable to trichomoniasis in the United States.

BACKGROUND: Clinical evidence suggests that trichomoniasis facilitates the sexual transmission and acquisition of HIV. GOAL: The goal of this study was to estimate the annual number and cost of new HIV infections among women in the United States attributable to trichomoniasis. STUDY: We used a mathematical model of HIV transmission to estimate the probability that a woman with trichomoniasis would acquire HIV as a result of her trichomoniasis-mediated increased susceptibility to HIV infection or as a result of increased HIV infectiousness in a trichomoniasis-infected male partner. RESULTS:

Comparative yield of different respiratory samples for diagnosis of Pneumocystis carinii infections in HIV-seropositive and seronegative individuals in India.

Respiratory specimens were prospectively examined for Pneumocystis carinii from 53 patients. The majority of specimens were comprised of expectorated sputum, induced sputum, broncho-alveolar lavage (BAL), and tracheal aspirates. In only four patients Pneumocystis carinii (P. carinii) was detected. All the samples were produced by broncho-alveolar lavage. Candida spp and Aspergillus spp were also identified in a small number of patients. Acid-fast-bacilli were not detected in any of the cases under study. There were no sex-related differences in distribution. The present prospective study was u

Mucosal Immune Responses in People Living with HIV May Confer Protection from SARS-CoV-2 Infections After COVID-19 Vaccination.

Background/Objectives: The induction of anti-SARS-CoV-2 antibodies by COVID-19 vaccination reduces morbidity and mortality, but immune responses may be compromised in people living with HIV (PLWH). The aims of the current study were to determine whether viral suppression (VS) or immune reconstitution (IR) in PLWH directly affected their ability to produce effective levels of anti-SARS-CoV-2 antibodies in mucosal secretions or blood induced by vaccination. Methods: Anti-SARS-CoV-2 spike IgG, IgA and secretory IgA (SIgA) antibodies and their avidities were measured by ELISA in HIV-negative healt

Prevalence and characterization of West Nile virus infections among people living with HIV in Northern Italy: a sero-repository analysis.

BACKGROUND: West Nile virus (WNV) is increasingly reported in Italy, yet its population prevalence remains uncertain. People living with HIV (PLWH) may be at increased risk of WNV severe manifestations, including neuroinvasive disease. This study assessed the seroprevalence and factors associated with WNV seropositivity among PLWH in a large Northern Italian cohort. METHODS: This retrospective single-center study included all PLWH followed at the Brescia HIV Outpatient Clinic with at least one serum sample collected in 2022 and stored in the institutional sero-repository. Samples were screene

Multiple Opportunistic Infections as the Initial Presentation of Human Immunodeficiency Virus (HIV)/Acquired Immunodeficiency Syndrome (AIDS) in an Ecuadorian Immigrant.

Advanced human immunodeficiency virus (HIV)/acquired immunodeficiency syndrome (AIDS) continues to occur in those with social, educational, and cultural barriers to accessing healthcare. A 27-year-old Ecuadorian man presented with vomiting, epigastric pain, fever, weakness, and weight loss. He had advanced AIDS, with opportunistic infections including disseminated histoplasmosis, cerebral toxoplasmosis, and JC virus brain infection. Despite prior repeated contact with local, US-based healthcare, sexual history and HIV screening were never performed. Missed opportunities to identify HIV infecti

Blood demand and utilization among pregnant and postpartum women with and without HIV infections at the university teaching Hospital, Zambia.

BACKGROUND: Complications from pregnancy, primarily post-partum hemorrhage, including infections like HIV, are the leading causes of maternal mortality in sub-Saharan Africa (SSA). Blood products are not always available for obstetric emergencies because of chronic shortages in SSA countries. Understanding the quantity of blood products used by pregnant and post-partum women with and without HIV infections could inform strategies for the appropriate use of blood in resource-limited settings. METHODS: A prospective study was conducted of medical charts of all pregnant or postpartum (42 days fo

Predictors of opportunistic infections among people living with HIV: a prospective cohort study from a tertiary care setting in India.

Opportunistic infections (OIs) remain a leading cause of morbidity and mortality among people living with HIV (PLHIV). Early identification of high-risk individuals is vital to guide the preventive measures and the efficient use of healthcare resources. We conducted a prospective longitudinal cohort study involving 223 PLHIV at a tertiary care center in Western India. Baseline demographic, clinical, and laboratory data were collected, including CD4 count, nutritional status, serum albumin, and ART (antiretroviral therapy) history. Patients were followed for the development of OIs. Logistic reg

Subpopulations with frequent healthcare barriers have increased risk of sexually transmitted infections and dropping out from HIV preexposure prophylaxis care.

OBJECTIVE: Certain subpopulations (i.e., <25 years, transgender, sex worker, uninsured or migrant) were prioritized for inclusion in the Dutch preexposure prophylaxis (PrEP) pilot. We compared incidence of sexually transmitted infections (STI) during and drop-out from HIV PrEP care between prioritized and nonprioritized subpopulations. DESIGN: Retrospective longitudinal study using routinely collected data at the Centre for Sexual Health Amsterdam, 2019-2024. METHODS: We modelled incidence rates (IR) for any STI while on PrEP using Poisson regression, adjusting for testing frequency and cale

Unveiling the Hidden Burden of TORCH Infections in HIV Patients: A Residual Serum Seroepidemiological Study in a Korean Metropolitan City.

BACKGROUND: Although human immunodeficiency virus (HIV) is a legally notifiable disease in Korea, surveillance data are reported in aggregate without regional breakdowns. This limits the ability of local governments to design targeted prevention and care strategies. The coronavirus disease 2019 pandemic underscored the importance of region-specific epidemiological data for timely public health responses. TORCH infections-comprising Toxoplasma gondii, Treponema pallidum, rubella virus, cytomegalovirus (CMV), and herpes simplex virus (HSV)-can cause severe complications in immunocompromised indi

Prevalence of HIV, HBV, and HCV in United States blood donations, 2015-2023: The transfusion-transmissible infections monitoring system (TTIMS).

BACKGROUND: The Transfusion-Transmissible Infections Monitoring System assesses trends in ~60% of the US blood supply. Donors with high-risk behaviors, including injection drug users, men having sex with other men, or those exchanging sex for money/drugs, were deferred for 12 months (12 M) from 2016 to 2020 and 3 months (3M) from 2020 to 2023. Here we evaluate HIV, HBV, and HCV donation prevalence annually from 2015 to 2023 and in two comparison periods of pre-3M (2015-2020) and 3M (2020-2023) to identify differences between the two periods. METHODS: Annual prevalence was assessed for 8 TTIMS

Opportunistic Infections and Malignancies in a Patient With HIV/AIDS and a Critically Low CD4 Count of 1 Cell/μL.

We present a 45-year-old African American male with a medical history of advanced-stage HIV/AIDS (CD4 count: 1 cell/μL) and poor adherence to highly active antiretroviral therapy (HAART), who presented with symptoms of diarrhea, weakness, and respiratory distress. Upon admission, duodenal and colonic biopsies revealed a diffuse histiocytic infiltrate consistent with Mycobacterium avium complex (MAC), and a cecal biopsy was positive for Kaposi sarcoma (KS). Further workup showed consolidation and a right pleural effusion on chest X-ray, suggesting a pneumonia infection. The patient's hypoglycem

Oral lymphangioma-like Kaposi sarcoma: a Brazilian case report in a scenario of a still high number of HIV infections.

We present a challenging case of HIV-related lymphangioma-like Kaposi sarcoma (LLKS) affecting the oral cavity. A 54-year-old Brazilian male patient was referred to our center complaining of bleeding lesions affecting the oral cavity for 2 months. Interestingly, these oral lesions were the first clinical manifestation of HIV infection. Clinically, multiple erythematous nodular and patch lesions were observed. An incisional biopsy was performed, revealing numerous microscopic angled and irregular vascular channels lined with flattened endothelial cells. More cellularized and solid areas consist

Tribute to John C. Martin at the Twentieth Anniversary of the Breakthrough of Tenofovir in the Treatment of HIV Infections.

At Bristol-Myers (BM) (1985-1990), John C. Martin started his HIV career with directing the clinical development of didanosine (ddI) and stavudine (d4T). During this period, he became aware of the acyclic nucleoside phosphonates (ANPs), such as (S)-HPMPA and PMEA, as potential antiviral drugs. Under his impulse, BM got involved in the evaluation of these ANPs, but the merger of BM with Squibb (to become BMS) incited John to leave BM and join Gilead Sciences, and the portfolio of the ANPs followed the transition. At Gilead, John succeeded in obtaining the approval from the US FDA for the use of

Recently acquired HIV infections and associated factors among men who have sex with men diagnosed at Dutch sexual health centres.

INTRODUCTION: Surveillance for recent HIV infections was implemented at Dutch sexual health centres (SHCs) for planning and evaluation of HIV prevention initiatives. The study objective is twofold: (1) to explore trends in recent HIV infections and associated socio-demographic and behavioural risk factors among men who have sex with men (MSM )attending SHCs, to gain insight into subgroups at risk for acquiring HIV and(2) to illustrate how comparison with different reference groups affect associations with these risk factors. METHODS: Residual plasma or serum samples from MSM newly HIV diagnos

Assessment for sexually transmitted infections in men who have sex with men attending a nurse-run HIV preexposure prophylaxis clinic.

BACKGROUND: Male-to-male sexual contact remains the primary mode of transmission for HIV and other sexually transmitted infections (STIs) in the United States. Reported STI rates in men who have sex with men (MSM) taking HIV preexposure prophylaxis (PrEP) are higher than expected, possibly due to decreased condom use while on PrEP or more frequent STI screening with routine PrEP care. LOCAL PROBLEM: Hamilton County, located in southwestern Ohio, consistently has the highest rates of gonorrhea and syphilis and second highest rate of chlamydia in the state. The purpose of this project was to in

Predicting Sexually Transmitted Infections Among HIV+ Adolescents and Young Adults: A Novel Risk Score to Augment Syndromic Management in Eswatini.

BACKGROUND: Despite poor predictive power, syndromic screening is standard of care for diagnosing sexually transmitted infections (STIs) in low-resource, high HIV-burden settings. Predictive models may augment syndromic screening when diagnostic testing is not universally available for screening high-risk patient populations such as adolescents and young adults living with HIV. SETTING: Four hundred fifteen adolescents and young adults living with HIV, age 15-24 years, participated from 3 clinical sites in Eswatini, provided urine, sexual and medical history, and completed physical examinatio
