Auditory neuropathy as the first sign of leptomeningeal metastasis.

INTRODUCTION: Auditory neuropathy is a hearing disorder characterized by impaired temporal coding of acoustic signals in the auditory nerve fibers, resulting in abnormal auditory perceptions relying on temporal cues. We describe the case of a woman with a history of cancer who developed auditory neuropathy as the first manifestation of leptomeningeal metastasis. CASE SUMMARY: A woman in her sixties complained of severe difficulties in understanding speech despite preserved sensitivity to sounds, which had begun two months before audiological assessment. Her past medical history was remarkable

Congenital middle ear anomalies: endoscopic outcomes, facial nerve anomalies, and bilateral consistency in 41 ears.

BACKGROUND: Congenital middle ear anomalies are primarily managed surgically, but data on facial nerve anomalies and bilateral cases are limited. OBJECTIVE: To report endoscopic outcomes for congenital middle ear anomalies, focusing on facial nerve anomalies and bilateral consistency. METHODS: Retrospective review of 33 patients (41 ears) from 2021 to 2025. Anomalies were intraoperatively classified using the Teunissen system. All underwent endoscopic, class-specific ossicular reconstruction. Facial nerve anomalies recorded. Audiometric outcomes (ABG, 8 kHz) were analyzed. Complications reco

Poly(amino acid) nanoprodrugs mitigate cisplatin-induced ototoxicity.

Cisplatin is a first-line chemotherapeutic agent for solid and metastatic tumors, yet its ototoxicity remains a major clinical challenge, causing irreversible, bilateral, dose-dependent sensorineural hearing loss. This toxicity severely impairs patients' quality of life and, in children, disrupts language and socio-cognitive development. Conventional strategies using antioxidants, anti-inflammatory agents, or molecular inhibitors only partially mitigate ototoxicity, while systemic administration compromises cisplatin's antitumor efficacy. Intratympanic delivery reduces systemic interference, b

Long-term hearing outcomes and determinants on patient-perceived benefits after bone cement fixation in stapedotomy.

BACKGROUND: Stapes surgery using Teflon prostheses is widely performed for otosclerosis; however, prosthesis dislocation has been reported in approximately 3.5% of cases within the first two postoperative years. AIMS/OBJECTIVE: To evaluate the seven-year audiological and patient-reported outcomes of stapedotomy with bone cement fixation. METHODS: This retrospective cohort study with prospective follow-up included patients who underwent stapedotomy with bone cement fixation between 2012 and 2022. Air conduction and bone conduction thresholds were assessed at short-term (0-1 years), mid-term (

Effects on hearing after long-term use of iron chelators in beta-thalassemia: Over twenty years of longitudinal follow-up.

OBJECTIVE: The role of iron chelation in causing hearing loss (HL) is still unclear. The present study assessed the prevalence of HL among transfusion-dependent thalassemia (TDT) patients who underwent audiological follow-up over a 20-year period. METHODS: We retrospectively analyzed clinical records and audiological tests from January 1990 (T0) to December 2022 (T22) of a group of TDT patients who received iron chelation therapy with deferoxamine (DFO), deferiprone (DFP) or deferasirox (DFX), in monotherapy or as part of combination therapy. RESULTS: A total of 42 adult TDT patients (18 mal

Usefulness of radiological findings for predicting cochlear implantation outcomes in children with cochlear nerve deficiency: a pilot study.

CONCLUSION: Children with CND received limited benefits from CIs and their results varied. The size of the vestibulocochlear nerve relative to the facial nerve could potentially be used as a predicator for CI outcomes in children with CND. OBJECTIVE: This study aimed to (1) retrospectively review the outcomes of cochlear implants (CIs) in children with cochlear nerve deficiency (CND) and (2) evaluate the clinical usefulness of radiological findings as predictors for post-implantation outcomes. METHODS: Study participants included 10 children with bilateral CND and profound sensorineural hear

Necrotizing otitis externa: 19 cases' report.

Necrotizing otitis externa is an uncommon but severe infectious disease of the external auditory canal. Patients at risk are those immunodepressed or having diabetes. The causal germ is often Pseudomonas aeruginosa. Over a period of 10 years (1997-2006), we treated 19 patients: 94.7% had diabetes (insulin dependent in 6 cases). The causal germ was P. aeruginosa in 59% of cases. The pretherapeutic work-up included a computed tomography and a scintigraphy practiced in order to confirm diagnosis and assess the extension. Medical treatment was based on a parenteral antibiotic therapy using a third

Human otopathology in scleroderma.

OBJECTIVE: Scleroderma is a complex chronic progressive immune-mediated disease that causes fibrosis of the skin and internal organs, and vasculopathy.Ear involvement has been poorly studied in patients with scleroderma. Vasculitic and autoimmune mechanisms are considered as possible etiologies on hearing impairment, however, this etiology still unclear.Herein, we reviewed three cases of scleroderma from a temporal bone repository. METHODS: The national temporal bone database was reviewed for cases with scleroderma. Clinical case review and correlative otopathologic analysis. Middle and inner

Effects of Varying Laser Parameters During Laser Stapedotomy on Intracochlear Pressures.

OBJECTIVE: Sensorineural hearing loss is a known complication of stapes surgery. We previously showed that laser stapedotomy can result in intracochlear pressures that are comparable to high sound pressure levels. Optimizing laser settings to those that correspond with the lowest pressure changes may mitigate risk for postoperative hearing loss. Here we quantify the effects of various laser parameters on intracochlear pressures and test the hypothesis that intracochlear pressure changes are proportional to the laser energy delivered. STUDY DESIGN: Basic and translational science. SETTING: Ca

Hair Cell Generation in Cochlear Culture Models Mediated by Novel γ-Secretase Inhibitors.

Sensorineural hearing loss is prevalent within society affecting the quality of life of 460 million worldwide. In the majority of cases, this is due to insult or degeneration of mechanosensory hair cells in the cochlea. In adult mammals, hair cell loss is irreversible as sensory cells are not replaced spontaneously. Genetic inhibition of Notch signaling had been shown to induce hair cell formation by transdifferentiation of supporting cells in young postnatal rodents and provided an impetus for targeting Notch pathway with small molecule inhibitors for hearing restoration. Here, the oto-regene
