Endovascular Repair of Intraoperative Penetrating Injury to the Subclavian Vein: Case Report, Technical Considerations and Literature Review.

Penetrating injury to the subclavian vein is an uncommon, but potentially life-threatening event. Surgical repair is technically demanding due to the vessel's deep location beneath the clavicle, the narrow operative field, and the proximity of vital structures. Reported experience on endovascular techniques for subclavian venous injuries (SVI) remains limited. We present the case of a 60-year-old woman who sustained iatrogenic injury of the left subclavian vein during lateral cervical lymph node dissection for invasive thyroid carcinoma. Attempts at open hemostasis were unsuccessful, due to th

Hierarchically sized chitosan hemostatic particles with electrostatic-hydrophobic synergy for rapid and effective control of arterial hemorrhage.

Chitosan is widely employed as a hemostatic material for hemorrhage control due to its excellent biocompatibility, and intrinsic cationic property that promotes red blood cell aggregation. However, the insufficient stability under high-pressure arterial flow and the poor mechanical strength of the formed clots can lead to a high risk of secondary bleeding. Here, machine learning-guided hierarchical sized chitosan hemostatic particles (CSHP) with electrostatic-hydrophobic synergy were fabricated through multistage particle size distributions and lauric acid loading. The CSHP combined physical p

Astaxanthin inhibits platelet-mediated thrombosis via suppression of bidirectional αIIbβ3 signaling.

Platelets are central mediators of hemostasis and thrombosis, and dysregulated platelet activation substantially contributes to the pathogenesis of cardiovascular diseases. Astaxanthin (AZ), a xanthophyll carotenoid and a bioactive compound derived from Haematococcus lacustris, possesses potent antioxidant and anti-inflammatory properties. Nevertheless, its regulatory effects on platelet function remain insufficiently elucidated. This study investigated the impact of AZ on platelet function and thrombosis. In vitro experiments demonstrated that AZ significantly inhibited agonist-induced platel

Molecular regulators of thromboinflammation and angiogenesis in pediatric cancer: emerging roles of noncoding RNAs, epigenetics, and extracellular vesicles - narrative review.

Pediatric cancers, although less prevalent, remain a significant cause of mortality among children worldwide. Thromboinflammation arises from a complex dysregulation of inflammation and hemostasis, leading to tissue injury and cell death. Angiogenesis, the formation of new blood vessels, is influenced by inflammation. Both processes are closely linked to the progression of various tumors. Emerging evidence increasingly highlights the pivotal role of certain molecular markers in stimulating thromboinflammatory and angiogenic pathways in several types of pediatric cancers, including gliomas, oth

Recurrent multilevel postoperative spinal epidural hematoma in a patient with essential thrombocythemia: failure of repeated decompression and hematological correction. Illustrative case.

BACKGROUND: Postoperative spinal epidural hematoma (SEH) is an uncommon but serious complication after lumbar endoscopic surgery, with an incidence of 0.1%-0.2%. Although percutaneous endoscopic lumbar discectomy (PELD) minimizes tissue disruption, SEH may still occur, particularly in patients with hematological disorders. Essential thrombocythemia (ET) presents a hemostatic paradox of simultaneous bleeding tendency and hypercoagulability, increasing postoperative hematoma risk. Massive multilevel SEH after PELD is rare. OBSERVATIONS: A patient developed severe left leg pain immediately after

Cardiac tamponade following endovascular therapy due to acute aorto-caval leakage after superior vena cava stenting in tuberculosis mediastinal fibrosis.

Superior vena cava (SVC) syndrome results from impaired venous return to the right atrium. While malignancy is the most common cause, benign etiologies such as mediastinal fibrosis are increasingly recognized. Endovascular therapy is considered the first-line treatment due to its high technical success and rapid symptom relief. We report the case of a 47-year-old woman with tuberculosis-related mediastinal fibrosis presenting with progressive SVC syndrome. She underwent uncomplicated endovascular stenting of the SVC using an uncovered stent. Twelve hours after the procedure, the patient develo

Thromboelastography-Guided Laparoscopic Splenectomy in a Patient With Immune Thrombocytopenia and Recent Drug-Eluting Stent Placement: A Case Report.

BACKGROUND: Perioperative management of patients with immune thrombocytopenia (ITP) is particularly challenging when complicated by recent coronary stenting requiring dual antiplatelet therapy (DAPT), due to competing risks of bleeding and thrombosis. CASE PRESENTATION: We report the case of an 80-year-old male with refractory ITP and recent drug-eluting stent placement who underwent elective laparoscopic splenectomy. Although preoperative optimization with corticosteroids and intravenous immunoglobulin increased platelet count from < 20 × 109/L to 177 × 109/L, functional hemostatic adequacy

Neonatal Thromboembolic Disease: Clinical Spectrum and Emerging Evidence for Diagnostic and Therapeutic Strategies Addressing Unmet Needs.

Neonatal thromboembolic (TE) disease is an increasingly recognized clinical entity, driven by the physiological uniqueness of developmental hemostasis, improved survival of preterm infants, and the widespread use of invasive supportive technologies. Most neonatal thrombotic events are associated with central venous and arterial catheterization which are considered the most significant modifiable risk factors. The clinical spectrum encompasses catheter-related thrombosis, perinatal stroke and site-specific entities such as renal vein and portal vein thrombosis. Each condition presents distinct

Surgical Significance of Middle Hepatic Vein Proximity to the Gallbladder Fossa During Canine Cholecystectomy in Two Dogs.

The middle hepatic vein (MHV) may lie in close proximity to the gallbladder fossa and represent a potential source of severe hemorrhage during cholecystectomy. While this anatomical relationship is well recognized in human surgery, the clinical relevance of MHV proximity during canine cholecystectomy has rarely been described. Two dogs underwent cholecystectomy for gallbladder disease. In Case 1, an 8-year-old Pomeranian with gallbladder mucocele and necrotizing cholecystitis developed sudden massive hemorrhage during dissection of the gallbladder from the hepatic fossa between the quadrate an

Safety Considerations in Blepharoplasty: A Comprehensive Review.

Blepharoplasty is one of the most frequently performed aesthetic facial procedures worldwide. It is widely regarded as safe when performed with appropriate surgical planning and technique. Nevertheless, the complexity of eyelids makes periocular surgery uniquely sensitive to technical errors and potential postoperative untoward sequelae. This review summarizes contemporary safety considerations in cosmetic blepharoplasty. We will focus on reported complication patterns, patient- and procedure-related risk factors, and strategies for prevention, recognition, and management. Due to heterogeneity

ADAR RNA editing for cardiovascular disease: Targeting B4GALT1 to modulate lipid metabolism through reduced galactosyltransferase activity.

Cardiovascular disease remains a leading cause of mortality despite current therapies targeting low-density lipoprotein cholesterol (LDL-C). Beta-1,4-galactosyltransferase 1 (B4GALT1), a central glycosyltransferase enzyme that regulates lipoprotein metabolism and hemostasis, is a promising therapeutic target. To evaluate its therapeutic potential as a protective variant, B4galt1 p.Asn352Ser was introduced into the ribonucleic acid (RNA) of healthy wild-type and APOE∗3-Leiden.CETP transgenic mice, a well-established model for hyperlipidemia with a humanized lipoprotein metabolism, using editing

A coexisting pseudoaneurysm hidden behind a saccular distal middle cerebral artery aneurysm in long-standing postirradiation vasculopathy: illustrative case.

BACKGROUND: Delayed cerebrovascular complications after childhood cranial irradiation may include progressive vasculopathy and aneurysmal disease. However, attributing a ruptured aneurysm directly to prior irradiation can be difficult in patients with complex preexisting cerebrovascular pathology. The authors describe a diagnostic pitfall in a patient with long-standing postirradiation vasculopathy who presented with hemorrhagic distal middle cerebral artery (MCA) aneurysmal lesions. OBSERVATIONS: A 51-year-old man who had undergone whole-brain irradiation for pineal germinoma at 8 years of a

Delayed Distal Coronary Perforation After Successful Covered-Stent Sealing of a Proximal Ellis Type III Rupture: A Dual-Site Mechanism.

Coronary perforation is a rare but potentially life-threatening complication of percutaneous coronary intervention. We report a case of delayed hemorrhagic pericardial effusion caused by an initially occult distal wire-related perforation after successful covered-stent sealing of a proximal Ellis type III rupture. A 67-year-old man underwent PCI for a severely calcified mid-right coronary artery lesion complicated by proximal Ellis type III perforation after high-pressure post-dilatation. The perforation was immediately sealed with a covered-stent, and early echocardiographic follow-up showed

Delayed Recognition of Glycoprotein IIb/IIIa Antibody-Associated Platelet Dysfunction in Chronic Liver Disease: A Platelet-Mediated Bleeding-Thrombosis Dysregulation.

Recurrent bleeding in chronic liver disease is frequently attributed to structural gastrointestinal pathology, often without detailed interrogation of platelet function. We report a patient with prolonged transfusion-dependent gastrointestinal bleeding in whom repeated endoscopic findings failed to account for the severity and persistence of hemorrhage. The clinical course was characterized by a reproducible pattern in which bleeding episodes led to temporary withdrawal of aspirin with partial hematologic stabilization, whereas recurrent cardiac biomarker elevation prompted reintroduction of a

Proteomic Biomarker Discovery in Cerebrospinal Fluid Identifies CD74 and CD14 for Differentiating Multiple Sclerosis from Neuromyelitis Optica.

Multiple sclerosis (MS) and neuromyelitis optica (NMO) are distinct autoimmune demyelinating diseases of the central nervous system with overlapping clinical features, complicating early diagnosis. Accurate differentiation is essential to avoid inappropriate treatment and improve outcomes. We conducted a comprehensive proteomic analysis of cerebrospinal fluid (CSF) from MS and NMO patients using both conventional proteomics and a nanoparticle-based low-abundance protein enrichment strategy (LAPE). Deep CSF proteome coverage yielded 3,816 proteins. Proteomic profiling revealed shared and distin
