Trauma in older people leads to substantial morbidity and mortality. The National Hip Fracture Database (NHFD) has driven improved practice with units compared to identify outliers. In 2013, our unit was an outlier for mortality post hip fracture (30-day mortality 12.2% vs. 8.3% nationally). This triggered external review. In 2019 the unit was highlighted as an exemplar in the UK. We describe the process that moved us from outlier to outstanding. BACKGROUND: The objective of this study was to evaluate the effectiveness of our strategy for managing floating hip injuries.
METHODS: From January 2014 and December 2019, all patients with a floating hip underwent surgical treatment in our hospital were included in the retrospective study, with a minimum follow-up of 1 year. INTRODUCTION: The proximal femur and acetabulum are frequent sites for benign active and aggressive lesions. The risk of pathologic fracture is great when a bone-destroying pathology involves an anatomic location such as the hip joint that undergoes profound mechanical loading. If the destruction involves a large area around the joint, secure fixation cannot be achieved with internal fixation implants. The study investigates use of articulated hip distraction to protect reconstructions performed for the treatment of benign active or aggressive tumors presenting with pathologic fracture.

UNLABELLED: Osteoporosis care in men is suboptimal due to low rates of testing and treatment. Applying biomechanical computed tomography (BCT) analysis to existing CT scans, we found a high proportion of men with osteoporosis have never been diagnosed or treated. BCT may improve identification of patients at high risk of fracture.

Eighteen patients with fractures of the pelvis were examined by computer tomography. The results were compared with those of conventional radiological diagnosis. In each case the bone injuries were accurately diagnosed, in some patients additional information was obtained or the findings corrected. It was also possible to demonstrate injuries to pelvic, abdominal and retroperitoneal organs and injuries to other regions of the body, such as the skull and thorax. Haematomas could be directly demonstrated and additional information, useful for plannng surgery, was obtained.
