Maternal vascular-placental axis in the third trimester in women with gestational diabetes mellitus, hypertensive disorders, and unaffected pregnancies.

BACKGROUND: Hypertensive disorders of pregnancy and gestational diabetes mellitus are characterized by vascular dysfunction and are associated with long term cardiovascular risks. OBJECTIVE: This study aimed to compare different markers of maternal vascular function in women with gestational diabetes mellitus, preeclampsia, or gestational hypertension and in women whose pregnancies were unaffected by these complications and to assess the association between maternal vascular function and markers of placental perfusion and maternal vascular-placental axis in 4 groups of women. STUDY DESIGN: T

[Effect of exclusive breastfeeding on infant hospitalizations].

INTRODUCTION: Exclusive breastfeeding (EBF) provides essential nutrients and strengthens the immune system, significantly reducing the risk of infant hospitalization during the first six months of life. This study aims to evaluate the association between EBF and the risk of hospitalization during the first year of life in a cohort of 370 postpartum women. MATERIALS AND METHODS: A prospective cohort study was conducted with postpartum women recruited from a maternity ward between August 2022 and 2023, with a one-year follow-up period. All postpartum women admitted during the same period who vo

Evaluation, Stabilization, and Transfer of Pregnant and Postpartum Patients Presenting to Emergency Departments Without Inpatient Obstetric Services.

More than 50% of rural critical access hospitals in the United States no longer provide inpatient obstetric services. As more hospitals close their hospital-based obstetric services, emergency physicians must still be ready to care for pregnant patients who present emergently for care. Ideally, this will include immediate medical screening examination, stabilization and transfer to a hospital with obstetric services. In other circumstances, it will result in the need to provide basic obstetric emergency care for conditions such as unanticipated delivery, postpartum hemorrhage, eclamptic seizur

Childhood adversity, benevolent experiences, and perceived stress among pregnant women in Mexico: psychosocial determinants of maternal well-being in a middle-income setting.

BACKGROUND: Adverse childhood experiences (ACEs) are strongly associated with poor physical and mental health across the life course. Maternal ACEs have been linked to prenatal stress, hypertensive disorders, gestational diabetes, shorter gestation, lower infant birth weight, and increased risk of prenatal and postpartum depression. Benevolent childhood experiences (BCEs) and social support may buffer these effects; however, ACE-informed perinatal care remains uncommon in low- and middle-income settings. This study aimed to estimate the prevalence of ACEs and BCEs among pregnant women in Mexic

Socioeconomic risk factors and obstetric outcomes of adolescent pregnancies in Vietnam.

BACKGROUND: Adolescent pregnancy remains a pressing concern in the Pacific and Southeast Asia, with birth rates of 51 and 43 per 1000 girls. Teenage mothers face a heightened risk of complications and mortality, while their newborns are more likely to experience challenges such as preterm birth, low birth weight, and poor Apgar scores. In Vietnam, however, evidence on this issue remains limited. OBJECTIVE: Our research aims to examine the socioeconomic characteristics and outcomes of adolescent pregnancy in Vietnam. STUDY DESIGN: We conducted a cross-sectional study recruiting pregnant women

Prevention and treatment of postpartum hypertension.

BACKGROUND: Postpartum blood pressure (BP) is highest three to six days after birth when most women have been discharged home. A significant rise in BP may be dangerous (e.g., can lead to stroke), but there is little information about how to prevent or treat postpartum hypertension. OBJECTIVES: To assess the relative benefits and risks of interventions to: (1) prevent postpartum hypertension, by assessing whether 'routine' postpartum medical therapy is better than placebo/no treatment; and (2) treat postpartum hypertension, by assessing whether (i) one antihypertensive therapy is better than

The Maternal Lifestyle Study: drug exposure during pregnancy and short-term maternal outcomes.

OBJECTIVE: Reports of maternal effects resulting from drug exposure during pregnancy are inconsistent. The Maternal Lifestyle Study (MLS) is a multicenter, prospective, observational study that was initiated to better define the effects of exposure to illicit drugs during pregnancy on the mother, fetus, and infant. METHODS: Between May 1993 and May 1995, of 19,079 mother-infant dyads that were screened after delivery for cocaine and opiate exposure at four clinical centers (Brown University, University of Miami, University of Tennessee, Memphis, and Wayne State University), 16,988 (89%) met e

Incidence of congenital rubella syndrome at a hospital serving a predominantly Hispanic population, El Paso, Texas.

OBJECTIVE: The current epidemiology of rubella reveals an increase in the number of cases among adult Hispanics and an increase in the number of congenital rubella syndrome (CRS) cases among infants of Hispanic mothers. Recent rubella outbreaks have occurred primarily among adult Hispanics, many of whom are foreign-born natives of countries where rubella vaccination is not routine or has only recently been implemented. The objective of this study was to estimate the incidence of CRS in a hospital serving a predominantly Hispanic population. METHODS: Hospital charts of infants <1 year old disc

Associations between parenting styles, problematic smartphone use, and depression severity in adolescents with depressive disorder.

BACKGROUND: Adolescent depression represents a significant public health issue in China. Parenting styles and problematic smartphone use (PSU) are closely associated with adolescent depression. Consistent with the I-PACE model, depression is considered a distal vulnerability factor for PSU; however, among adolescents already diagnosed with depressive disorder, whether PSU is associated with increased symptom severity remains unclear, and the potential indirect role of parenting styles warrants further investigation. METHODS: A cross-sectional study was conducted among adolescent patients diag

Experiential Classes Plus Digital Logging in Antenatal Care for Pregnant Women in China: Mixed Methods Study.

BACKGROUND: Maternal health during the perinatal period is a global public health priority. While antenatal education is widely implemented, conventional lecture-based models often fail to achieve sustained behavior change. Innovative approaches that integrate experiential learning with digital support may enhance maternal knowledge, self-management, and pregnancy outcomes. OBJECTIVE: The aim of this study is to evaluate the feasibility and preliminary effectiveness of a combined experiential class and online logging intervention for pregnant women in China and to explore the mechanisms under

A secondary analysis of a prospective cohort study: association between aspirin regime and early- and late-onset preeclampsia.

BACKGROUND: Preeclampsia (PE) poses a threat to maternal and perinatal health. Low-dose aspirin prophylaxis is recommended for women at high risk of developing PE. Here, we investigated how aspirin regimens and maternal characteristics in early pregnancy are associated with early- and late-onset PE. METHODS: This was a retrospective secondary analysis of prospective cohorts. A total of 975 participants who registered and delivered at Beijing Obstetrics and Gynecology Hospital between the 1st of January 2023 and the 30th of June 2024 were divided into three groups: non-PE, early-onset PE, and l

Reported Dietary Patterns in Pregnant Women with and Without Gestational Diabetes Mellitus: A Post-Diagnosis Comparative Study in Guadalajara, Mexico.

Background: Gestational diabetes mellitus (GDM) affects between 1% and 14% of pregnancies worldwide. Major risk factors include advanced maternal age, excess adiposity, family history of type 2 diabetes, and unhealthy dietary habits. In Mexico, evidence on the association between dietary patterns and GDM remains scarce, particularly in socioeconomically vulnerable populations with limited access to specialized nutrition services. This study aimed to evaluate the association between dietary patterns and the presence of GDM in pregnant women attending the outpatient obstetrics clinic of a teachi

Feasibility of multi-component intervention for blood pressure control in pregnancy in Nigeria and Ghana: a pilot randomized clinical trial.

BACKGROUND: Hypertensive disorders in pregnancy are a leading cause of maternal and fetal morbidities and mortalities. They are 5 times as prevalent in Sub-Saharan Africa with relatively higher rate of adverse outcomes. Challenges in management are as a result of physiologic changes in pregnancy, restricted choice of antihypertensives, sub-optimal response to medications in pregnancy and the exclusion of pregnant women from most drug trials. Objectives of this study are to determine the feasibility of delivering multi-component intervention for blood pressure control in women with chronic hype

Exploring factors influencing mother-infant interaction trajectories post-NICU discharge in low-birth-weight infants during COVID-19 in China: a longitudinal qualitative study.

BACKGROUND: Annually, over 20 million newborns are affected by low-birth-weight, posing a global public health challenge. Interactions between mothers and low-birth-weight infants during the first year post-discharge from neonatal intensive care unit are crucial for developmental trajectories. However, longitudinal research to explore the experiences of mother-infant interactions within the corrected age of 12 months and to analyze the factors influencing these interactions in low-birth-weight infants is limited. METHODS: A longitudinal qualitative study based on semi-structured interviews was

Effect of Intraoperative Sublingual Nitroglycerin on the Rate of Active Uterine Segment Incision in Preterm Cesarean Birth.

INTRODUCTION AND AIM: Active segment uterine incisions, including classical, T-shaped, and high transverse incisions, are often performed during preterm cesarean deliveries (CD), especially in patients delivering at less than 30 weeks of gestation, when the lower segment can have inadequate space for a safe maternal or fetal delivery. These uterine incisions increase the risk of maternal morbidity due to longer operative time, increased blood loss, and the rate of uterine dehiscence in future pregnancies compared to lower segment uterine incisions. Patients with uterine incisions in the active
