Atrial and ventricular pressures in atrial flutter.

The hemodynamic effects of atrial flutter (AF) are unknown. The purpose of the present study was to investigate the changes in atrial and ventricular pressures after induction of AF. In 23 patients with paroxysmal AF (age 59 +/- 9 years), a hemodynamic study was performed both during sinus rhythm and after induction of the tachyarrhythmia. During AF, 13 patients showed a fixed 2:1 AV conduction and 10 patients showed variable conduction. Mean right and left atrial pressures increased (P < 0.001) and right and left ventricular end-diastolic pressures decreased (P < 0.001) after induction of AF.

Is local myocardial contractility related to endocardial acceleration signals detected by a transvenous pacing lead?

The availability of sensors monitoring cardiac function parameters may offer many interesting new applications in cardiac pacing. A microaccelerometer sensor (BEST, Biomechanical Endocardial Sorin Transducer) located at the tip of a pacing lead (PL) has been developed by Sorin Biomedica. The signal detected by the accelerometer, peak endocardial acceleration (PEA), was shown to reflect cardiac contractility and to be related to the dP/dt signal. Whether the PEA detected by the BEST sensor in different cardiac locations is the expression of local acceleration forces or reflects the whole heart

Perioperative cardiac pacing using an atrioventricular pacing pulmonary artery catheter.

An atrioventricular pacing thermodilution pulmonary artery catheter was evaluated in 40 patients undergoing cardiac surgery. The catheter was inserted in all study patients in a timely fashion without difficulty and functioned well during the perioperative period. Before the start of cardiopulmonary bypass, atrial capture was achieved in 98% of the patients (threshold mean 4.9 mA), ventricular capture in 100% (threshold mean 3.0 mA) and atrioventricular sequential (AVS) pacing in 98%. After cardiopulmonary bypass, atrial capture was achieved in 95% of the patients (threshold mean 5.2 mA), vent

Effects of postural change from supine to head-up tilt on the skin sympathetic nerve activity component synchronised with the cardiac cycle in warmed men.

KEY POINTS: Humans are unique in controlling body temperature in a hot environment by a large amount of skin blood flow; however, the decrease in total peripheral resistance due to systemic cutaneous vasodilatation and the reduction of venous return to the heart due to blood pooling in the cutaneous vein threatens blood pressure maintenance in the upright position, and occasionally causes heat syncope. Against this condition, cutaneous vasodilatation is reportedly suppressed to maintain arterial pressure; however, the nerve activity responsible for this phenomenon has not been identified. In t

Laparoscopic aortofemoral bypass grafting: human cadaveric and initial clinical experiences.

PURPOSE: Postoperative complications are mainly related to the surgical trauma derived from the extensive abdominal incision and dissection after a conventional aortofemoral bypass grafting procedure. In an attempt to reduce postoperative complications, a concept of video-endoscopic vascular surgery on the infrarenal aortoiliac artery has been developed. On the basis of our experience with the practicability of video-endoscopic vascular surgery in the pelvic region in an animal study and in a pilot study of human cadavers, the purpose of this report was to describe three different methods that

[Individualized implant selection for an anatomically normal cochlea : Cochlear implant surgery on the way to personalized medicine].

BACKGROUND: Due to the anatomical variety of the human cochlea and the many factors that may determine the choice of the cochlear array, there is no one-size-fits-all method for determining the right implant. The cochlear size, preoperative residual hearing, anatomical variants, operative technique, and cause of deafness must be taken into consideration when making this decision. OBJECTIVE: This work aims to spread awareness of precision surgery and provide an overview. MATERIALS AND TECHNIQUES: Factors that contribute to individualization are explained, and future advances are outlined. RE

Abdominal CT scan findings of a child with hepatic fascioliasis: A case report on rarely reported emerging disease.

Fasciolosis is a zoonotic infection caused by trematodes fasciola hepatica and fasciola gigantic, and humans are incidental hosts. Although infrequently reported in developed nations, it is common in developing countries. Few cases have been reported in Africa, specifically in Ethiopia. This article reports a case of a 4-year-old Ethiopian child who presented with right upper quadrant abdominal pain. His complete blood count showed eosinophilia, and imaging demonstrated lesions at peripheral subcapsular parenchyma and central along the biliary tree. Serologic tests confirmed liver fluke infect

Brown tumor of mandible: A conundrum towards the diagnosis.

Brown Tumor is a rare, reactive, giant cell associated, late-stage bone lesion occuring as a result of long term untreated primary or secondary hyperparathyroidism (HPT). It clinically manifests as a high turnover bone disease, which typically shows increased parathyroid hormone (PTH) secretion in response to low serum calcium concentration thereby enhancing calcium reabsorption and osteoclastic bone resorption, hence called osteoclastoma or giant cell reparative granuloma or osteitis fibrosa cystica. Brown tumor associated with HPT can be seen in any part of the human skeleton, However clinic

Myxoid lipoma in the perioral mandibular region: two case reports.

Lipomas are one of the most common mesenchymal tumors in the human body, exhibiting a heightened prevalence between the ages of 40 and 60 years. However, primary intraoral lipomas are rare. Myxoid lipoma, which is characterized by abundant mucoid components, is a particularly rare histological subtype of lipoma. This study presents two cases of myxoid lipoma that occurred outside the common age range for occurrence, one in the right submandibular area of a 67-year-old male and the other in the lower lip of a 3-year-old child. Through these case reports, the aim was to introduce myxoid lipoma,

Frequency-specific cortico-subcortical interaction in continuous speaking and listening.

Speech production and perception involve complex neural dynamics in the human brain. Using magnetoencephalography, our study explores the interaction between cortico-cortical and cortico-subcortical connectivities during these processes. Our connectivity findings during speaking revealed a significant connection from the right cerebellum to the left temporal areas in low frequencies, which displayed an opposite trend in high frequencies. Notably, high-frequency connectivity was absent during the listening condition. These findings underscore the vital roles of cortico-cortical and cortico-subc

Nail Squamous Cell Papilloma: A Rare Case Report.

INTRODUCTION: Squamous cell papilloma is a benign tumor whose pathogenesis is generally related to the human papillomavirus. Despite affecting several organs, we did not find cases reported in the nails. CASE PRESENTATION: A 67-year-old female presented with a painful lesion in the nail of the right hallux that started in 2021, with an erythematous appearance evolving to black and expansive growth. Due to the evolution and symptoms, she opted for excision, with a histopathological report of squamous cell papilloma. Complementary treatment with imiquimod was performed, with good healing and pa

mNGS-identified cellulitis due to quinolone-resistant Edwardsiella tarda: a case report.

Edwardsiella tarda is frequently isolated from aquatic animals and environments. While human infections caused by E. tarda are rare, some extraintestinal infections can be severe. This case report describes a patient with cellulitis of the right upper extremity of unknown origin. Metagenomic next-generation sequencing (mNGS) indicated that the patient was infected with E. tarda. Antimicrobial susceptibility testing revealed that the isolate was resistant to quinolones and trimethoprim/sulfamethoxazole. The isolate, positive for four virulence genes (fimA, gadB, mukF, and sodB), was confirmed t

Generation of human induced pluripotent stem cell lines UKJi001-A and UKJi006-A from patients with heterozygous mutation in the PKP2 gene.

One of the main signs we do not know enough about arrhythmogenic right ventricular dysplasia 9/cardiomyopathy (ARVCD9, OMIM #609040, autosomal dominant) is the lack of early markers and therapeutic alternatives. To better study disease pathways in vitro, we generated human induced pluripotent stem cell (hiPSC) lines from the father (UKJi006-A) and son (UKJi001-A), who both shared the same heterozygous mutation in the PKP2 gene (OMIM *602861). While the father had a clinical diagnosis of ARVC, the son lacked the ARVC phenotype. To generate hiPSC lines, non-integrating Sendai virus (SeV) vectors

Adenitis as Initial Mycobacterium marinum Presentation.

Mycobacterium marinum is a ubiquitous and opportunist agent that may cause infections related to water activities in humans. It causes mainly skin and soft tissue infections, and other forms of presentation are uncommon. A 27-year-old man presented to the Emergency Department of a tertiary hospital due to a cervical foreign-body sensation that evolved into right cervical swelling and consumption symptoms. He was a waiter on a cruise in the Douro river. Weeks after the initial presentation, the diagnosis of Mycobacterium marinum infection was made by positive nucleic acid amplification tests (N

Encephalic meningioangiomatosis in a dog with high-grade undefined glioma.

Meningioangiomatosis (MA) is a rare proliferative meningovascular disorder that affects mainly the cerebral cortex, brainstem and spinal cord of humans and animals and can coexist with other proliferative disorders. A 7.5-year-old male Brazilian Campeiro Bulldog died after a convulsive crisis and cardiorespiratory arrest. At necropsy, a firm, white mass involving the piriform and right occipital lobes was seen. Histologically, the mass consisted of two morphologically distinct entities that collided: a congenital malformation characterized by a proliferation of meningothelial cells around bloo

A Rare Case of Left-Sided Gastroschisis in a Human Museum Specimen.

Gastroschisis is a congenital defect in the anterior abdominal wall resulting in herniation of the abdominal viscera without any fetal membrane covering it. It usually occurs to the right of a normally inserted umbilical cord. The anomaly is associated with intrauterine growth retardation, stillbirth, and preterm delivery. We found a preserved specimen of a 17- to 20-week-old male human fetus presenting with gastroschisis in the Departmental Museum of Anatomy of the Institute of Medical Sciences and SUM Hospital, Bhubaneshwar, a medical college in Eastern India. The fetus showed a hiatus on th

Hydatid Cyst In A Rare Site; The Left Ventricle.

Echinococcosis is a disease that has been affecting human beings since decades and is primarily a zoonotic disease. The larvae of genus Echinococcus causes this disease. The most common sites for these cysts are the lungs and liver. Cardiac hydatidosis is a rare phenomenon and the incidence of such cases is about 1.3%. The case presented is of a 15- year-old boy who would have shortness of breath on exertion and acute right leg pain. To diagnose cardiac variant of the cystic disease, computed topography (CT) and echocardiography were done. Doppler studies confirmed acute limb ischemia. The tre
