Treatment results of carotid artery stenting in a developing country.

OBJECTIVE: The purpose of this study was to investigate the details of minor complications of carotid artery stenting in a developing country. METHODS: This was a retrospective, single-center study conducted on the target group consisting of 65 symptomatic patients who underwent carotid artery stenting. We assessed technical success rate, periprocedural complication within 30 days (hypotension, bradycardia, acute kidney injury, vasospasm, a transient ischemic attack, stroke, myocardial infarction, and death), and the differences between groups with and without complications. RESULTS: Minor p

A Rare Cause of Pulmonary Edema in the Postoperative Period.

With the increasing longevity of the population, the annual rates of hip arthroplasties performed have been steadily increasing over the past decade. Given the presence of medical comorbidities in the older patients, the peri-operative care of these individuals requires multi-specialty care, now more than ever. Hip arthroplasty is generally well tolerated, with early mortality after the procedure being <1%. Bone cement implantation syndrome (BCIS) is an entity that is occasionally encountered during or after the surgery. It is characterized by hypoxemia, hypotension, cardiac arrhythmias, and c

How should we manage adverse intradialytic blood pressure changes?

Variations in intradialytic blood pressure (BP) are a common and predictable occurrence in ESRD patients. These are caused by a decrease in blood volume provoked by ultrafiltration, lack of normal compensatory responses to fluid removal, underlying cardiac disease, and electrolyte changes that may adversely affect cardiovascular function. Intradialytic hypotension is the most frequent complication of the hemodialysis (HD) procedure and is fundamentally a consequence of an ultrafiltration rate that surpasses mechanisms activated to avert a decline in BP. Intradialytic hypertension is a less wel

Cardiac Arrest during Total Hip Arthroplasty in a Patient on an Angiotensin Receptor Antagonist.

Angiotensin-converting enzyme inhibitors (ACEI) and angiotensin receptor antagonists (ARA) are effective and well-tolerated first-line drugs in the therapy of hypertension and, therefore, are frequently encountered in the perioperative setting. Hemodynamic compensation for volume depletion seen in the perioperative period is normally mediated by the renin-angiotensin system, which is blocked by ACEI/ARA. These drugs may contribute to severe hypotension during anesthesia induction and may have contributed to the cardiac arrest seen in this patient. Additional factors such as increased intra-abd

Cardiovascular autonomic dysfunction in MSA and Parkinson's disease: similarities and differences.

In Parkinsons disease and multiple system atrophy (MSA), cardiovascular dysfunction may occur for a variety of reasons and may manifest itself through inappropriate changes and/or levels in blood pressure, heart rate and/or regional vascular perfusion in a range of situations. The early occurrence of orthostatic hypotension often leads to consideration of MSA, especially in the presence of other features of autonomic failure. Orthostatic hypotension, however, is increasingly recognised in PD, and especially with increasing age, severity of disease and as a result of drug therapy, sometimes for

Patent ductus arteriosus ligation and post-operative hemodynamic instability: case report and framework for enhanced neonatal care.

Ligation of a patent ductus arteriosus can lead to severe cardiorespiratory compromise in preterm infants. This report reviews the postoperative course of a patient with significant cardiorespiratory instability following surgical ligation of the patent ductus arteriosus and presents a framework for enhanced cardiovascular care in this population. A preterm infant, born at 24 wk gestation underwent ligation of a large haemodynamically significant ductus arteriosus after failure of 2 courses of indomethacin. He developed systemic hypotension, which was aggressively treated with high doses of mu

Cardiac denervation in patients with Parkinson disease.

More than 40 neuroimaging studies have reported evidence for loss of sympathetic noradrenergic nerves in PD. Cardiac sympathetic denervation is virtually universal in patients with PD and neurogenic orthostatic hypotension. About one half of patients with PD who do not have orthostatic hypotension also have evidence for loss of noradrenergic innervation. The loss progresses over years, in a pattern suggesting "dying-back". Because patients with familial PD from mutation of the gene encoding alpha-synuclein or from triplication of the normal gene have low myocardial concentrations of 6-[18F]flu

Acute renal failure after massive honeybee stings.

Two clinical cases of patients who survived after numerous attacks of Africanized bees (600 and 1500 bee stings, respectively) are reported. Clinical manifestation was characterized by diffuse and widespread edema, a burning sensation in the skin, headache, weakness, dizziness, generalized paresthesia, somnolence and hypotension. Acute renal failure developed and was attributed to hypotension, intravascular hemolysis, myoglobinuria due to rhabdomyolysis and probably to direct toxic effect of the massive quantity of injected venom. They were treated with antihistaminic, corticosteroids and flui

[Diabetic cardiovascular neuropathy].

Impairment of the autonomic nervous system is frequently detectable in diabetic patients. Symptoms and signs are less often observed but if present may be very important with regard to quality of life, metabolic control, and prognosis. Currently used methods to detect disturbances of the autonomic innervation of the cardiovascular system are easily performed, noninvasive, reliable and reproducible. They are not very time consuming and require only standard technical equipment allowing the quick identification of patients at risk. Damage of the cardiovascular autonomic nerve function may explai

Cardiovascular autonomic insufficiency in a patient with metastatic malignancy.

A case of cardiovascular autonomic insufficiency is described in a cachectic 70-year-old man with widely metastatic small-cell tumor. The patient experienced disabling syncopal episodes in association with severe postural hypotension. Tests of cardiovascular autonomic insufficiency were abnormal. The introduction of fludrocortisone and the use of elastic stockings produced palliation of the symptomatology, but his condition deteriorated and he died. Autopsy demonstrated unexpectedly extensive tumor invasion of autonomic nervous tissue. Although cardiovascular autonomic insufficiency (CAI) has

Bilateral vocal cord paralysis with Shy-Drager syndrome.

Shy-Drager syndrome consists of progressive autonomic nervous system failure with Parkinson's disease-like symptoms and orthostatic hypotension. It can also result in airway compromise from bilateral vocal cord paralysis. Fewer than 30 cases of severe bilateral vocal cord paresis or paralysis associated with the Shy-Drager syndrome have been reported in the English literature. We present a case of a 72-year-old man who had a 2-year history of orthostatic hypotension, neurogenic bladder, impotence, anhydrosis, and extremity weakness and paresthesias. Hoarseness and dyspnea with stridor develope

Bezold-Jarisch reflex in postoperative pediatric cardiac surgical patients.

The Bezold-Jarisch reflex is an inhibitory reflex that originates from the heart, is mediated by the vagus nerve, and is manifested by hypotension and bradycardia. We present 4 pediatric cardiac surgical patients, aged 1 day to 9 months, who exhibited cardiovascular collapse in their early postoperative course. In each patient, cardiovascular deterioration was marked by an insidious decrease in arterial blood pressure without an associated change in heart rate, central venous pressure, or airway pressure. Bradycardia followed the decrease in blood pressure. The Bezold-Jarisch reflex was suspec

Differential effects of clonidine on pain, arterial blood pressure, and heart rate in the cat: lack of interactions with naloxone.

In cats anesthetized with alpha-chloralose and urethane, intravertebral administration of clonidine (4 and 10 micrograms/kg) dose-dependently suppressed the jaw-opening reflex, arterial blood pressure, and heart rate. For a given dose, there was a differential degree of inhibition in the order of analgesia much greater than hypotension greater than bradycardia. Naloxone injections (0.4 and 1.0 mg/kg, i.vert.) essentially failed to antagonize these effects, suggesting the lack of involvement of the opiate receptors or endogenous opioids in these processes. Furthermore, pain suppression by cloni

The use of imipramine in depressed patients with congestive heart failure.

Previous studies of left ventricular performance (LVP) in depressed patients receiving tricyclic antidepressants have been performed on patients without severe heart disease. This study reports the effect of imipramine hydrochloride on LVP, assessed by radionuclide angiography, in a group of depressed patients with notable preexisting left ventricular dysfunction. Ejection fraction was measured at rest by first-pass radionuclide angiography before and after treatment with imipramine. Ejection fraction was unchanged during treatment, but seven of 15 patients experienced orthostatic hypotension

A case of pure autonomic failure with a long-term-45-year-clinical course since the initial detection of abnormal blood pressure fluctuation.

This report describes the case of an 85-year-old male with a clinical phenotype consistent with pure autonomic failure (PAF) for more than four decades. Despite the presence of neurological manifestations, including bradykinesia, olfactory dysfunction, and visual hallucinations, phenoconversion to another α-synucleinopathy has not been confirmed based on established diagnostic criteria. Orthostatic and postprandial hypotension, low supine plasma norepinephrine levels, and reduced cardiac uptake on 123I-metaiodobenzylguanidine myocardial scintigraphy were observed, consistent with the postgangl

[Low blood pressure in pregnancy].

There has been a controversial discussion for decades about the significance of low blood pressure during pregnancy. This review should answer the question, whether low blood pressure itself or the decrease in blood pressure on standing have a negative effect on pregnant women. In addition, we will focus on physiologic and pathophysiologic pregnancy related mechanisms of changes in blood pressure. Subjective symptoms occur more frequently with low blood pressure, however, they disappear at the end of pregnancy. Physiologically, blood pressure decreases towards midpregnancy and rises to preconc

A large ventricular septal defect complicating resuscitation after blunt trauma.

A young adult pedestrian was admitted to hospital after being hit by a car. On arrival to the Accident and Emergency Department, the patient was tachycardic, hypotensive, hypoxic, and acidotic with a Glasgow Coma Scale of 3. Despite initial interventions, the patient remained persistently hypotensive. An echocardiogram demonstrated a traumatic ventricular septal defect (VSD) with right ventricular strain and increased pulmonary artery pressure. Following a period of stabilization, open cardiothoracic surgery was performed and revealed an aneurysmal septum with a single large defect. This was r

[Mesenteric ischemia in hemodialysis patients: case report and review of the literature].

Mesenteric infarction is increasingly observed in uremic elderly patients with widespread atherosclerosis. A 77-year-old man on renal replacement therapy since June 1997 was admitted because of abdominal pain. The surgical diagnosis was massive intestinal infarction and the patient died a few hours later. A colonoscopy had been performed a few weeks before and a well-limited necrosis of the caecum mucosa had been detected. Hypotensive episodes were frequent during his hemodialysis sessions. In this work we discuss age, symptoms, laboratory investigations, risk factors and the evolution of case

Spinal anaesthesia in obstetrics.

Spinal anaesthesia has been used since the 1800s but, due to a number of complications, the popularity of this technique has waxed and waned. In the 1950s, it was the most widely used method of anaesthesia and analgesia in obstetrics but it fell out of fashion with the arrival of the epidural technique which allowed a continuous method of delivering analgesia with relatively few complications. Hypotension and the high incidence of postdural puncture headaches were two reasons for the decline in the popularity of spinal anaesthesia in the young, otherwise healthy pregnant population. With the d
