Placenta accreta spectrum (PAS) disorder is a potentially life-threatening condition which rarely complicates second trimester miscarriages. This report describes a woman who was pregnant at 17 weeks with a history of previous caesarean section and uterine curettage. She presented with hypovolaemic shock following massive per-vaginal bleeding at home. A 180 g fetus was spontaneously aborted, but the placenta was retained in the uterus. After resuscitation, she underwent surgery, during which a morbidly adherent placenta was identified necessitating an emergency hysterectomy. Histopathological

In this report, to the best of our knowledge, we present the first case of genital-sparing robot-assisted radical cystectomy (RARC) using the hinotori™ Surgical Robot System (Medicaroid Corporation, Kobe, Japan) in a 78-year-old woman with muscle-invasive bladder cancer and a history of total hysterectomy. An anterior vaginal wall-sparing cystectomy and intracorporeal ileal conduit reconstruction were completed without complications. The postoperative course was uneventful, and pathological analysis revealed pT0N0M0. This case demonstrates the feasibility and safety of genital-sparing RARC wit

We report a rare case of a female patient in her early 50s who presented with unilateral transient visual symptoms that resolved shortly. On examination, the anterior segment exam was unremarkable bilaterally; however, her fundus examination showed unilateral anaemic retinopathy, including Roth spots. Her blood tests showed iron deficiency anaemia due to chronic menorrhagia, and her abdominal ultrasound scan showed large uterine fibroids. The gynaecology team was involved; medical therapy failed to control her heavy menstrual bleeding and subsequently correct her anaemia; thus, surgical manage

BACKGROUND: Situs inversus totalis (SIT) is a rare autosomal recessive inheritance at which the abdomino-thoracic organs are mirror-image transposed. Germ cell tumors originate from the primitive germ cell of the ovary and testis.
CASE REPORT PRESENTATION: A rare association between malignant ovarian mixed germ cell tumor and SIT was presented in a 32-years-old Egyptian female, successfully treated with laparoscopic total abdominal hysterectomy, right salpingo-oophorectomy, and retroperitoneal lymphadenectomy (laparoscopic retroperitoneal lymphadenectomy) of both sides. This case is considere

Ganglioneuroma, a rare benign neuroblastic tumor, typically arises in the posterior mediastinum, but it can be found in the anterior mediastinum and thymus. Predominantly affecting the young, these asymptomatic tumors are often discovered incidentally through imaging. In our reported case, a 44-year-old woman post-hysterectomy with persistent jaundice was diagnosed with a neuroganglioma in the right posterior mediastinum via a computed tomography (CT) scan. Thoracotomy and resection revealed a 10-cm neuroganglioma untangled from mediastinal planes. Post-surgery, chylothorax emerged, which was

This case report describes bloodless management of a 32-year-old Jehovah's Witness who had severe obstetric hemorrhage at 23 weeks of gestation, spontaneously delivering twin fetuses. After resuscitation with crystalloids and colloids, the patient was still bleeding and hemoglobin fell to 3.3 g/dL. She had emergency hysterectomy. On postoperative day 1, her hemoglobin was 1.3 g/dL and Glasgow Coma Scale 6 of 15. Electrolyte derangement and fluid overload were detected and treated promptly. She regained consciousness and received intravenous iron and erythropoietin. She recovered fully without

BACKGROUND: This article reports a unique case of cesarean scar pregnancy, demonstrating importance of early management and diagnosis.
CASE PRESENTATION: A 30-year-old pregnant woman with prior history of two cesarean sections found to have cesarean scar pregnancy at approximately 13 weeks' gestation and underwent a gravid hysterectomy.
CONCLUSIONS: While rare, cesarean scar pregnancies should be considered on the differential diagnosis of any pregnant patient with history of cesarean section who presents in early pregnancy with vaginal bleeding and/or cramping. Given the increased rates of

An 83-year-old woman with a long-standing history of both invasive and vulval extramammary Paget's disease (EMPD) was referred to a tertiary gynaecological oncology service for suspicion of contiguous extension to the vagina and cervix. Vaginal biopsies confirmed EMPD; however, a loop excision of the cervix demonstrated invasive adenocarcinoma arising from Paget's disease. The patient subsequently underwent laparoscopic radical hysterectomy, bilateral salpingo-oophorectomy and radical upper vaginectomy, confirming FIGO stage 1B1 cervical adenocarcinoma. She was recommended but declined to have

This case report discusses an unlikely occurrence of massive subcutaneous emphysema in an elective robotic-assisted laparoscopic total hysterectomy in a 45-year-old, ASA class 1 woman. The patient's perioperative course was otherwise uncomplicated, with the subcutaneous emphysema developing at surgical closure. The patient presented with substantial crepitus spanning from her face to her lower extremities and hypercarbia with end-tidal carbon dioxide readings persistent between 60 and 70 mm Hg. This case did not result in clinically significant airway obstruction because of provider vigilance.

A 62-year-old Japanese woman developed numbness of the extremities and megaloblastic anemia. She had undergone total abdominal hysterectomy, whole-pelvis radiation therapy and chemotherapy for gynecological cancer 10 years before. Chronic abdominal pain, diarrhea and intermittent small-bowel obstruction had afflicted her for a long time. We diagnosed her with vitamin B12 deficiency anemia and polyneuropathy due to chronic radiation enteritis causing malabsorption. Vitamin B12 injections improved her numbness and anemia. The early diagnosis and treatment of deficiency of vitamin B12 are importa

Placenta accreta spectrum disorders, especially placenta percreta (PP) and placenta praevia (PLP), are major risk factors for massive obstetric haemorrhage which is a common cause of maternal morbidity and mortality in our environment. This risk becomes exponential and life-threatening when the two conditions co-exist in the same patient. Even in advanced countries with readily available expertise and state of the art resuscitative and supportive facilities, these conditions are associated with grave maternal and perinatal morbidity and mortality. We present a challenging case of PP co-existin

A 43-year-old woman with a medical history of cervical cancer treated with curative hysterectomy 12 years earlier developed progressive dyspnea, chest discomfort, and hoarse voice over a 7-month period. The patient never smoked and had no exposure history. Imaging at an outside hospital showed a mediastinal mass with hilar adenopathy (Fig 1A), which was biopsied via endobronchial ultrasound transbronchial needle aspiration (EBUS-TBNA) and revealed noncaseating granulomas with surrounding rims of lymphocytes (Fig 1B). The patient was given the diagnosis of sarcoidosis and started on prednisone

CASE: The authors report a case with spontaneous renal hemorrhage caused by invasive mole. The diagnosis was gestational trophoblastic disease (GTD), with metastasis to brain, kidneys, and lungs at Stage IV. The patient was given etoposide-methotrexate-actinomycin D plus cyclophosphamide-vincristine (EMACO) treatment regimen for 11 times including three times with consolidation chemotherapies. Laparoscopically-assisted vaginal hysterectomy (LAVH) + laparoscopic-assisted left renal excision + evacuation of the left perirenal hematoma were performed during the eighth chemotherapy.

BACKGROUND: Pregnant women with obstetric fistula are at high risk for obstetric complications and present a unique challenge in management.
CASE: A young, HIV-positive woman with a history of rectovaginal and vesicovaginal fistulae after a prior delivery presented in premature labor. Upon cesarean delivery, fecal matter entered the uterus. The uterus was irrigated, closed, and a sigmoid colostomy was created. Subsequently, she underwent repair of both fistulae with bilateral ureteral reimplantation, hysterectomy, primary rectal repair, and colostomy reversal. She remains continent of urine a

Clostridium perfringens (CP) is an anaerobic, Gram-positive bacillus associated with malignant diseases and near-term pregnancies. The necrotic tissue that results from these disease processes fuels the proliferation of CP, leading to gas gangrene and subsequently sepsis. Herein, we report a case of a 41-year-old female patient with a history of invasive molar pregnancy that was further complicated with a CP infection. Although past research has shown a link between Clostridium infection and choriocarcinoma (Chern-Horng and Hsieh, 1999), no previous cases of CP infection have been associated w

Since 2007, we have identified 2 cases of central uterine necrosis after uterine arterial embolization for postpartum hemorrhage. Contrast-enhanced sonography showed an absence of enhancement of the internal myometrium. Magnetic resonance imaging with gadolinium confirmed the diagnosis. The images obtained for the first case were corroborated by histologic analysis from a hysterectomy done for sepsis. For the second case, contrast-enhanced sonography performed during a follow-up period of conservative treatment revealed a reduction of necrosis. Our study shows that contrast-enhanced sonography

BACKGROUND: Antemortem discovery of a malignant pericardial effusion secondary to cervical cancer is uncommon and management should focus on symptom control.
CASE: A patient was diagnosed with widely metastatic cervical cancer following a simple hysterectomy for presumed benign etiology. Sixteen months later, she was diagnosed with pericardial tumor involvement and a malignant pericardial effusion resulting in severe dyspnea. The patient underwent a pericardial window procedure that temporarily alleviated her symptoms. She again developed symptoms 2 weeks following the procedure and ultimatel

Placental site trophoblastic tumor (PSTT) is the rarest type of gestational trophoblastic tumors. Common presentation is irregular vaginal bleeding. A 26-year-old G1P1 woman presented with 15 month amenorrhea, a large uterine mass and plateau low level of serum human chorionic gonadotropin (hCG) which raised the possibility of PSTT. Since whole myometrium had been substituted with the large tumoral mass, hysterectomy was considered despite the young age of the patient. The patient has been in remission 36 months postoperatively without receiving any adjuvant treatment. It seems that plateau lo
