Eleven haemophiliac boys infected with HIV were screened for irregular red cell antibodies and were compared with nine haemophiliac boys who did not have antibodies to HIV. Seven (64%) of the children who had antibodies to HIV also had cold agglutinins, mostly of anti-I specificity, compared with one (11%) of those who did not have antibodies to HIV. The children with antibodies to HIV and cold agglutinins had a significantly increased mean IgM concentration. Five patients with chronic hepatitis delta virus (HDV) infection suffered spontaneous episodes of liver enzyme elevation on a background of otherwise biochemically stable liver disease. In all five patients these episodes were accompanied by a rise in serum levels of anti-HDV IgM, HDV antigen and HDV RNA. These episodes of increased HDV replication accompanied by biochemical evidence of liver injury are reminiscent of reactivation in chronic hepatitis B. Surges of increased HDV replication may be important in the progression of liver disease in chronic HDV infection.

The sequence of events following infection of mice with the intestinal nematode, Strongyloides stercoralis, has been observed. Most infective larvae passed to the muscles where they did not develop further. In mice given primary infections, larvae were found in muscles for the first 9 d or so, then disappeared spontaneously. The immunofluorescent staining patterns of oral lesions of discoid lupus erythematosus were examined by use of monoclonal antibodies AE1, AE2 and AE3. During a 6 yr period, 105 (69%) of 153 patients in whom a histological diagnosis of minimal change glomerular disease was made had renal biopsy tissue suitable for immunofluorescence examination. Thirty seven (35%) patients showed glomerular mesangial deposits of IgM. All the patients presented with the nephrotic syndrome. We found no significant difference in age and sex prevalence, presentation, response to therapy and glomerular morphology between IgM positive and IgM negative groups. Four primarily healthy patients with granulomatous hepatitis during acute cytomegalovirus infection are described. Granulomatous hepatitis associated with cytomegalovirus infection has been described previously in only 7 patients. As the authors have observed 4 such cases within a period of one year, this disease pattern may occur more frequently than was formerly assumed.

Circulating low molecular weight (LMW) IgM was demonstrated in five of 38 patients with B cell lymphoproliferative disorders. The specificity and sensibility of IgM-IFAT for diagnosis of acute acquired and congenital toxoplasmosis may increase after separation of IgM fractions by simple gel filtration method. In the presence of IFA-IgG titers greater than 250 IU/ml with Remington test less than 1/50, specific IgM antibodies to Toxoplasma gondii should be detected by separation. The separation of IgM seems a necessary step for detecting IgM antibodies by IgM-IFA in the newborn sera.
