Pulmonary mucormycosis without underlying systemic disease.

Mucormycosis (or zygomycosis) is an opportunistic fungal infection which usually is seen in patients who are immunosuppressed or who have diabetes. It is uncommon in healthy persons and also is uncommon in Australia. We report a case of a 45-year-old, otherwise-healthy man with an indolent lung infection that was caused by Absidia corymbifera, who was cured by a combination of surgical and medical therapy.

Filarial chyluria with opportunistic infections.

A patient with long-standing filarial chyluria developed severe lymphocytopenia, extensive mucosal candidiasis and disseminated cryptococcosis when given small doses of prednisolone and azathioprine, for the treatment of a co-existing chronic active hepatitis. This report suggests that patients with chyluria are particularly susceptible to the complications of immunosuppressive therapy. This susceptibility is probably due to the immunological abnormalities associated with chyluria.

[Liver changes in Wegener's granulomatosis (author's transl)].

A 54-year-old woman is described, who had been treated with immunosuppressive therapy for Wegener's granuloma for several years. The clinical course was surprisingly long and benign. An incidental sonographic and scintigraphic finding was a change in the liver which healed at the same time as the lung lesions and which is therefore assumed to have been due to inflammatory involvement of the liver.

Systematic review and network meta-analysis of interventions for articular involvement in patients with systemic lupus erythematosus.

Several immunosuppressive therapies beyond corticosteroids have demonstrated efficacy in controlling symptoms and reducing inflammation in lupus-related arthritis. The current standard of care (SOC) remains the combination of antimalarials and low-dose steroids, with the addition of other immunosuppressants in refractory cases. This study aimed to compare the efficacy of available biological and non-biological immunosuppressive therapies in controlling articular activity in patients with systemic lupus erythematosus (SLE). A systematic review and network meta-analysis were conducted following

Single-cell profiling delineates the tumor microenvironment and immunological networks in patient-derived uterine leiomyosarcoma.

BACKGROUND: Uterine leiomyosarcoma (ULSA) is a highly aggressive gynecologic malignancy characterized by early metastasis, profound immunosuppression, and resistance to conventional therapies, including immune checkpoint blockade (ICB). The intricate tumor microenvironment (TME) and cellular heterogeneity driving its progression and therapy resistance remain poorly defined. METHODS: We performed single-cell RNA sequencing (scRNA-seq) on metastatic lesions (pelvic cavity, rectum, peritoneum, bladder) from a treatment-naïve ULSA patient and compared them to normal uterine myometrium, MMM (n=5).

MicroRNA signature associated with treatment response in myasthenia gravis: A further step towards precision medicine.

Myasthenia gravis (MG) is an autoimmune disorder affecting neuromuscular transmission currently treated with chronic immunosuppression. Inter-subject variation in treatment response and side effects highlight the need for personalized therapies by identification of biomarkers predictive of drug efficacy in individual patients, still lacking in MG. MicroRNAs (miRNAs) play a key role in immune response and drug metabolism modulation. This study, part of an Italian-Israeli collaborative project, aimed to identify specific miRNAs as biomarkers associated with immunosuppressive treatment response i

Acute hepatitis B virus infection with delayed appearance of hepatitis B core antibody in an immunocompromised patient: a case report.

BACKGROUND: Despite the introduction of universal hepatitis B immunization programs worldwide, outbreaks of acute infection still occur in unimmunized individuals. A timely diagnosis of hepatitis B is necessary to ensure adequate clinical care and public health interventions that will reduce transmission. Yet, interpretation of hepatitis B serological markers can be complex. We present a case of hepatitis B with atypical markers, including delayed appearance of hepatitis B core antibody. CASE PRESENTATION: A 62-year-old white woman was identified as a sexual contact of a male individual with

Glucocorticoid resistant asthma: T-lymphocyte steroid metabolism and sensitivity to glucocorticoids and immunosuppressive agents.

We have previously shown that T-lymphocytes from clinically glucocorticoid (GC) resistant asthmatics are more refractory to dexamethasone suppression in vitro than those of GC sensitive asthmatics. We wished to extend these observations to compare three GCs used topically for asthma therapy (budesonide, beclomethasone dipropionate and fluticasone 17 alpha-propionate) and three immunosuppressive drugs (cyclosporin A, FK506 (tacrolimus) and mycophenolate mofetil) with dexamethasone for their antiproliferative effects on T-lymphocytes from GC sensitive and resistant asthmatics, and also to compar

The Sleeping Wolf - Widefield OCT Angiography Revelations of Insidious Systemic Lupus Erythematous-Associated Retinal Vasculitis.

PURPOSE: To describe a case of minimally symptomatic, insidious systemic-lupus-erythematous (SLE) retinal vasculitis revealed with widefield multimodal imaging. METHODS: Report of a case. RESULTS: A 56-year-old woman presented with a vague history of visual blur in her left eye for 3 weeks. While her central acuity was relatively preserved, fundus examination was notable for scattered cotton-wool spots in a Purtscher-like retinopathy presentation. Widefield angiographic imaging revealed profound peripheral non-perfusion with some focal arteriolar vasculitis. Systemic work-up confirmed the di

Disarming the "Neural Hijack": Repurposing neuroactive drugs for cancer therapy.

The emerging field of cancer neuroscience has fundamentally reshaped our understanding of malignancy, revealing tumors as complex ecosystems deeply integrated with and actively shaped by the nervous system. Malignancies recruit, reprogram, and exploit neural components, a phenomenon we conceptualize as "neural hijack" to fuel growth, metastasis, immunosuppression, and therapy resistance. Concurrently, drug repurposing, leveraging the established safety profiles of approved neuroactive agents, has emerged as a powerful strategy to therapeutically dismantle this tumor-neural axis. This review sy

Evolving Therapeutic Strategies in ANCA-Associated Vasculitis: Current Standards and Emerging Targets for GPA and MPA.

Granulomatosis with polyangiitis (GPA) and microscopic polyangiitis (MPA) are severe autoimmune disorders characterized by necrotizing small-vessel inflammation, and are classified among anti-neutrophil cytoplasmic antibody (ANCA)-associated vasculitides (AAV). Standard induction therapy combines glucocorticoids (GCs) with rituximab (RTX) or cyclophosphamide (CYC), with growing emphasis on GC minimization and selective use of avacopan in patients at high risk of GC toxicity. For maintenance therapy, fixed-interval RTX generally outperforms conventional oral agents and biomarker-guided re-dosin

Cerebral toxoplasmosis: Parasitology, diagnosis, treatment, and prevention - a narrative review.

Cerebral toxoplasmosis is a major opportunistic infection caused by Toxoplasma gondii, particularly in individuals with advanced HIV/AIDS and other forms of immunosuppression. This narrative review synthesizes current knowledge on the parasitology, diagnosis, treatment, and prevention of cerebral toxoplasmosis, with emphasis on clinical relevance and public health implications. We outline the parasite's life cycle and mechanisms of central nervous system invasion, distinguishing latent CNS infection in immunocompetent hosts from reactivated disease in immunocompromised patients. Fluorinated Proteolysis Targeting Chimeras-Sorafenib Nanoassembly for Epigenetic Remodeling to Combat Multi-Pathway Drug Resistance in Hepatocellular Carcinoma.

Although sorafenib (SF) is the standard first-line therapy for hepatocellular carcinoma (HCC), extended exposure frequently induces multipathway resistance driven by tumor hypoxia and an immunosuppressive microenvironment. Here we report a fluorinated Proteolysis TArgeting Chimeras (PROTAC)-sorafenib nanoassembly (FCP@SF/FPro) that boosts efficacy by degrading bromodomain-containing protein 4 (BRD4) and rewiring epigenetic programmes. This innovative system integrates fluorinated PROTAC targeting BRD4 (FPro) and sorafenib within a fluorinated poly(ethylene glycol)-conjugated poly(ethylenimine)

Paradoxical development of reactive squamous atypia and aggressive, invasive squamous cell carcinoma following 5-fluorouracil for field cancerization with concurrent immunomodulation.

5-fluorouracil (5-FU) is a widely prescribed therapy for field cancerization. However, the emergence of cutaneous squamous cell carcinoma (cSCC) after 5-FU treatment is poorly described. We report five patients treated for field cancerization at four university hospitals between March 2019 and May 2021 who had eruptive cSCCs of varying severity following 5-FU. We examined the indications for treatment, immunosuppression status, onset of cSCCs, staging and outcomes. The patients (three female, two male; median age 74 years) were treated with 5-FU for field cancerization. Four, including two sol

Iodine-starch chromogenic nanoplatforms for photothermal precision and IDO-mediated colorectal cancer therapy.

Colorectal cancer (CRC) remains a significant global health challenge, as current treatments such as surgery and chemotherapy are limited by poor precision, severe side effects, and the complex tumor microenvironment. Photothermal therapy (PTT) offers a promising alternative; however, conventional photothermal agents largely rely on synthetic materials, raising concerns about stability, toxicity, and metabolic degradation. To overcome these limitations, this study aimed to develop a biocompatible photothermal nanoplatform using iodine (I2)-loaded acetylated starch nanoparticles (ASt NPs). The

Liver Disease-Associated Glomerulopathies.

Hepatitis B virus (HBV) and hepatitis C virus (HCV) infect a significant number of individuals globally and their extra-hepatic manifestations, including glomerular disease, are well established. Additionally, liver disease-associated IgA nephropathy is the leading cause of secondary IgA nephropathy with disease course varying from asymptomatic urinary abnormalities to progressive kidney injury. Herein we provide an updated review on the epidemiology, pathogenesis, clinical manifestations, and treatment of HBV- and HCV-related glomerulonephritis as well as IgA nephropathy in patients with live
