Hemodynamic effects of dexmedetomidine in critically ill neonates and infants with heart disease.

The primary objective of this study was to evaluate the hemodynamic effects of dexmedetomidine (DEX) infusion on critically ill neonates and infants with congenital heart disease (CHD). The secondary objective of the study was to evaluate the safety and efficacy profile of the drug in this patient population. A retrospective observational study was conducted in the cardiovascular intensive care unit (CVICU) of a single tertiary care university children's hospital. The charts of all neonates and infants who received DEX in the authors' pediatric CVICU between August 2009 and June 2010 were retr

Transient evoked otoacoustic emissions: feasibility in the nursery.

OBJECTIVE: To assess the feasibility of obtaining transient-evoked otoacoustic emissions (TEOAEs) directly in the nursery, to examine the nature of failures in this population, and to determine the time requirements for this test relative to the auditory brain stem response (ABR). DESIGN: TEOAEs and ABRs were recorded from 149 ears, at bedside, in the recovery or intensive care nursery. Subjects were selected according to the risk criteria developed by the Joint Committee on Infant Hearing (1991). Parametric and nonparametric statistical procedures were conducted to describe demographics and

Clinical progress note: Hepatitis B.

Hepatitis B virus is a well-known infectious etiology of acute and chronic liver disease worldwide. Notably, 90% of children infected with hepatitis B in infancy develop chronic infection and subsequent progressive liver disease, including cirrhosis and hepatocellular carcinoma. Significant reduction in perinatal transmission in the United States (US) was seen after implementation of universal infant vaccination 30 years ago, as well as dramatic reductions in HBV incidence across Asia, coinciding with widespread hepatitis B vaccination. However, with ongoing changes to federal vaccine guidance

Pediatric Acute Lymphoblastic Leukemia, Version 2.2025, NCCN Clinical Practice Guidelines In Oncology.

The NCCN Clinical Practice Guidelines in Oncology (NCCN Guidelines) for Pediatric Acute Lymphoblastic Leukemia (ALL) were developed as a result of meetings convened by a multidisciplinary panel of pediatric ALL experts, with the goal of providing recommendations on standard treatment approaches based on current evidence. The NCCN Guidelines for pediatric ALL focus on risk assessment and stratification of risk-adapted therapy; treatment strategies for BCR::ABL1 (Philadelphia chromosome [Ph])-negative and BCR::ABL1-positive B-cell lineage, T-cell lineage, and infant ALL; and supportive care cons

Microbiological treatment failure associated with macrolide-resistant Bordetella pertussis.

BACKGROUND: Despite successful vaccination programs, many countries are experiencing a marked rise in pertussis cases since 2024, compounded by the emergence of macrolide-resistant Bordetella pertussis (MRBP). Here, we report the clinical, laboratory, and genomic features of MRBP isolated from a child with severe pertussis. CASE SUMMARY: An 11-week-old term infant with pertussis who required intensive care unit support remained B. pertussis culture-positive despite azithromycin therapy. Pre- and post-treatment isolates demonstrated phenotypic drug resistance and carried known 23S rRNA macroli

Citrobacter Sedlakii as a Rare Cause of Brain Abscess in a Late-Preterm Omani Infant: Case report and literature review.

Brain abscess in neonates is rare but remains a severe, life-threatening complication of meningitis with significant morbidity and mortality. While Citrobacter koseri and Citrobacter freundii are commonly implicated in brain-abscess formation, Citrobacter sedlakii is an uncommon causative pathogen. To date, only two cases of neonatal brain abscess caused by C. sedlakii have been reported in the literature. Early diagnosis, timely surgical intervention and prompt antibiotic therapy are critical to achieving a favourable outcome. A third case of a C. sedlakii brain abscess in a preterm infant, p

Emergent initial presentation of congenital malrotation with midgut volvulus in 68-year-old.

Congenital malrotation is a pathology nearly exclusive to the infant population. In the rare instance when it is diagnosed in an adult, it is typically associated with a longstanding history of gastrointestinal symptoms. Unfortunately, this unique presentation in an unexpected population has the potential to be confounding, leading to delayed or mismanaged care. Here, we describe an intriguing case of congenital malrotation complicated by midgut volvulus in a 68-year-old woman. Even more curious, the patient did not have a medical history plagued by abdominal complaints. Careful, comprehensive

Emphysema as a possible complication of infant respiratory distress syndrome leading to lung transplantation.

Infant respiratory distress syndrome (IRDS) develops among premature infants due to structural immaturity of the lungs and insufficient production of pulmonary surfactant. Nowadays, treatment takes place under conditions of intensive care and includes oxygen therapy, mechanical ventilation, exogenous supplementation of pulmonary surfactant and antenatal corticosteroid therapy. The treatment of IRDS, especially mechanical ventilation, may lead to complications which can contribute to developing a severe dysfunction of the respiratory system. Unavailability of pharmacological treatment of IRDS a

Role of Anesthesiologist in the Management of a Child with Cerebral Palsy.

Cerebral palsy (CP) refers to a spectrum of nonprogressive neurological disorders with disturbances in posture and movement, resulting from perinatal intrauterine insult to developing infant brain. Many conditions associated with CP require surgery. Such cases pose important gastrointestinal, respiratory, and other perioperative considerations. Anesthetic management in these cases is delicate. Intraoperative complications including hypovolemia, hypothermia, muscle spasms, seizures, and delayed recovery might complicate the anesthetic management. A thorough preanesthetic evaluation allows for a

X-Linked Lissencephaly With Absent Corpus Callosum and Abnormal Genitalia: An Evolving Multisystem Syndrome With Severe Congenital Intestinal Diarrhea Disease.

X-linked lissencephaly with abnormal genitalia is a rare and devastating syndrome. The authors present an infant with a multisystem phenotype where the intestinal manifestations were as life limiting as the central nervous system features. Severe chronic diarrhea resulted in failure to thrive, dehydration, electrolyte derangements, long-term hospitalization, and prompted transition to palliative care. Other multisystem manifestations included megacolon, colitis, pancreatic insufficiency hypothalamic dysfunction, hypothyroidism, and hypophosphatasia. A novel aristaless-related homeobox gene mut

A 10-Month-Old With Intermittent Hypotonia and Paralysis.

A 10-month-old boy presented with a 1-day history of flaccid quadriplegia and dysconjugate gaze. His history was remarkable for stereotyped episodes of flaccid quadriplegia or hemiplegia, oculomotor abnormalities, and limb or neck posturing, beginning in the first days of life and becoming more frequent and more prolonged over time. The patient was healthy and developmentally normal between episodes. Results of extensive laboratory evaluations, including EEG and brain imaging studies, were negative. The patient's history, diagnostic evaluation, and final diagnosis are reviewed. This case illus

Committee Opinion Summary No. 639: The Importance of Vital Records and Statistics for the Obstetrician-Gynecologist.

Information from vital records is critical to identify and quantify health-related issues and to measure progress toward quality improvement and public health goals. In particular, maternal and infant mortality serve as important indicators of the nation's health, thereby influencing policy development, funding of programs and research, and measures of health care quality. Accurate and timely documentation of births and deaths is essential to high-quality vital statistics. This Committee Opinion describes the process by which births, maternal deaths, and fetal deaths are registered; the challe

Harlequin Infant Born to a Varicella Infected Mother: A Case Report.

Harlequin ichthyosis is a rare, severe form of congenital ichthyosis characterised by distinct physical appearance of the infant. It has occurrence of 1 in 1 million births and over 100 cases have been reported so far. It is caused by mutation in ABCA12 gene involved in lipid transport leading to profound thickening of stratum corneum. The mainstay of management relies on supportive care & administration of retinoid derivatives. Outcome is usually fatal and most of the babies die during neonatal period. In this article, we report a case of harlequin baby born of non-consanguineous marriage wit

A close call: does the location of incision at cesarean delivery matter in patients with vasa previa? A case report.

We present here a case of vasa previa in a multipara, diagnosed at the time of her late second trimester ultrasonogram. The patient subsequently underwent an elective cesarean section after 37 weeks gestation, giving birth to a healthy child with an uneventful post-partum, neonatal and infant course. At the time of cesarean section, the incision was gradually deepened in layers through the myometrium by utmost care allowing the amniotic sac to protrude through the uterine incision hereby avoiding laceration of the vasa previa and its branches. Fetal exsanguination and a need for blood transfus

Common hemodynamic problems in the neonate.

Hemodynamic problems are common in neonatal intensive care. They occur in the context of incomplete myocardial and vascular development and in cardiovascular responses to interventions which are, as a result, limited and often uncertain and unpredictable. In this review, I outline the hemodynamic features of 4 neonatal conditions which often require intervention: (1) persistent pulmonary hypertension of the newborn, (2) cardiogenic shock (most commonly in the context of hypoxic ischemic injury), (3) sepsis and (4) low blood pressure in the transitional period of the extremely preterm infant. I

Successful extracorporeal membrane oxygenation for respiratory failure in an infant with DiGeorge anomaly, following thymus transplantation.

We report the first successful use of venovenous extracorporeal membrane oxygenation (ECMO) for refractory respiratory failure in an infant with DiGeorge anomaly, following thymus transplantation. A 23-month-old female with complete immune-incompetent DiGeorge anomaly 65 days after allogenic thymus transplantation was treated in our pediatric intensive care unit for acute respiratory failure secondary to bacterial sepsis. She subsequently developed acute hypercarbic respiratory failure unresponsive to conventional medical therapy. She was successfully managed with venovenous ECMO for 4 days, w

Dexmedetomidine as the primary anesthetic agent during cardiac surgery in an infant with a family history of malignant hyperthermia.

Malignant hyperthermia (MH) is an acute hypermetabolic crisis triggered in susceptible patients by the administration of succinylcholine or a volatile anesthetic agent. When providing anesthetic care for MH-susceptible agents, a total intravenous anesthetic (TIVA) technique is frequently chosen. When choosing the components for TIVA, several options exist including the combination of propofol or dexmedetomidine with an opioid. We present our experience with the use of dexmedetomidine as a key component of the anesthetic regimen in a 5-month-old infant with a family history of MH. Previous repo

Botulism: the challenge of diagnosis and treatment.

Botulism is a rare paralytic disease caused by a neurotoxin produced from the spore-forming bacterium Clostridium botulinum and in rare cases Clostridium butyricum and Clostridium baratii. Botulism has 4 naturally occurring syndromes: foodborne, wound, infant botulism, and adult intestinal toxemia. Inhalational botulism could result from aerosolization of botulinum toxin, and iatrogenic botulism can result from injection of toxin. All of these produce the same clinical syndrome of symmetrical cranial nerve palsies followed by descending, symmetric flaccid paralysis of voluntary muscles, which

Mal-positioned umbilical venous catheter causing liver abscess in a preterm infant.

Neonatal liver abscess is uncommon, carries a high mortality and is difficult to diagnose. We report an unusual case of liver abscess in a preterm infant presenting with abdominal distension and suspected gastrointestinal perforation, rather than the more usual features of fever, hepatomegaly, abdominal tenderness, right-sided pleural effusion, and leukocytosis. We discuss current treatments for neonatal liver abscess and argue that in view of the high mortality and difficulty in diagnosis, prevention should be the primary objective. We believe that mal-positioning the umbilical venous cathete

Surfactant protein B deficiency in infants with respiratory failure.

Surfactant Protein B (SP-B) deficiency has been recently identified as an uncommon, autosomal recessive lung disorder in term infants. This inability to produce SP-B leads to progressive, lethal, hypoxemic respiratory failure in the first year of life. A frameshift mutation (121 ins 2) is the predominant but not exclusive cause. The clue to diagnosis is to have a high suspicion of SP-B deficiency in any term infant with severe respiratory distress without any apparent cause. SP-B deficiency can be diagnosed prenatally or postnatally. The only current treatment options available include lung tr
