A case-control study of singleton low birthweight babies at the Port Moresby General Hospital.

A retrospective study of 432 consecutive singleton low birthweight babies and 432 unmatched controls was carried out at the Port Moresby General Hospital from January to December 1988. Of the 432 low birthweight babies 65% were preterm, 27% were light for gestational age, 6% were both preterm and light for gestational age and 2.5% could not be classified. The results of the analysis showed low birthweight to be significantly associated with the past delivery of a low birthweight infant, very young and elderly mothers, lack of antenatal care, poor family planning, hypertensive disease in pregna

[Acute fatty liver in pregnancy].

Acute fatty liver in pregnancy (AFLP) is a rare disease with sudden onset in the third trimester of pregnancy characterized by jaundice, bleeding, liver failure and encephalopathy. The cause of AFLP is unknown. It is of short duration and high risk of mortality. The aim of this paper is to describe a patient who was hospitalised in the 36th week of pregnancy due to jaundice and bleeding. Upon arrival the patient delivered a dead infant, and thereafter jaundice, severe bleeding and encephalopathy deteriorated. The patient's laboratory results revealed AFLP which was confirmed with increased ech

Diagnosis of vasa previa with transvaginal and color flow Doppler ultrasound.

Vasa previa, which is associated with high fetal mortality, is present when fetal vessels cross the internal cervical os as a velamentous insertion of the umbilical cord with or without a succenturiate lobe or bilobate placenta. This is the first case report of vasa previa not associated with a succenturiate lobe or bilobate placenta and in which the diagnosis was made using a combination of transvaginal ultrasonography and color flow Doppler ultrasound. The infant was delivered by elective cesarean, and the ultrasound findings were confirmed. Color flow Doppler and transvaginal ultrasound fac

Multiple pregnancy in the Maltese population.

This study analyses the multiple birth statistics for the Maltese Islands since 1959. Hospital twin births delivering during the period of 1983-1985 are analysed for a number of variables. The incidence of multiple pregnancy for the Maltese Islands appears to have decreased slightly since 1959 with an overall rate of 10.21 per 1000 maternities. The ratio of dizygotic to monozygotic twinning was computed to be 1.64. Patients with multiple pregnancies are shown to be generally elderly and multiparous. The pregnancy outcome is more likely to be complicated by an operative delivery, while the infa

The Impact of Donating Human Milk on the Health of the Donor and Their Infant: Evidence from Two Systematic Reviews.

Using human milk has been associated with decreased morbidity and mortality in preterm/low birth weight infants. Donor human milk is recommended when maternal milk is unavailable. The benefits of donor human milk for the recipient are well documented, but the impact of donation on donors and their infants is not clear. We aimed to evaluate the effects of donation human milk on donors and their infants. Literature searches were conducted (April 2024) to identify studies (observational, quasi-experimental, and randomized control trials) assessing the impact of human milk donation on donor health

Feasibility of implementing the World Health Organization case management guideline for possible serious bacterial infection among young infants in Ntcheu district, Malawi.

BACKGROUND: Neonatal sepsis is a leading cause of mortality, yet the recommended inpatient treatment options are inaccessible to most families in low-income settings. In 2015, the World Health Organization released a guideline for outpatient treatment of young infants (0-59 days of age) with possible serious bacterial infection (PSBI) with simplified antibiotic regimens when referral was not feasible. If implemented widely, this guideline could prevent many deaths. Our implementation research evaluated the feasibility and acceptability of implementing the WHO guideline through the existing hea

Early Inhaled Budesonide for the Prevention of Bronchopulmonary Dysplasia.

BACKGROUND: Systemic glucocorticoids reduce the incidence of bronchopulmonary dysplasia among extremely preterm infants, but they may compromise brain development. The effects of inhaled glucocorticoids on outcomes in these infants are unclear. METHODS: We randomly assigned 863 infants (gestational age, 23 weeks 0 days to 27 weeks 6 days) to early (within 24 hours after birth) inhaled budesonide or placebo until they no longer required oxygen and positive-pressure support or until they reached a postmenstrual age of 32 weeks 0 days. The primary outcome was death or bronchopulmonary dysplasia,

Early additional food and fluids for healthy breastfed full-term infants.

BACKGROUND: Health organisations recommend exclusive breastfeeding for six months. However, the addition of other fluids or foods before six months is common in many countries. Recently, research has suggested that introducing solid food at around four months of age while the baby continues to breastfeed is more protective against developing food allergies compared to exclusive breastfeeding for six months. Other studies have shown that the risks associated with non-exclusive breastfeeding are dependent on the type of additional food or fluid given. Given this background we felt it was importa

Measuring ENAP interventions for small and/or sick newborns in routine health information systems: indicators and considerations from a WHO expert consultation.

BACKGROUND: Current trends indicate 63 low- and middle-income countries (LMICs) are not on track to achieve the 2030 Sustainable Development Goal 3.2 target of a neonatal mortality rate ≤12 per 1000 live births. The Every Newborn Action Plan (ENAP) prioritised four life-saving interventions for small and/or sick newborns (SSN) in health facilities: neonatal resuscitation, kangaroo mother care, antibiotic treatment of possible serious bacterial infections, and antenatal corticosteroids for women at risk of preterm birth at <34 weeks of gestation. Limited indicator reporting on the use of these

Evaluation of the Helping Babies Breathe programme in Migori County, Kenya.

INTRODUCTION: Kenya's 2021 neonatal mortality rate was 18.42 per 1000 live births, far exceeding the Sustainable Development Goal of 12 per 1000 live births. Helping Babies Breathe (HBB) was developed to equip birth attendants with necessary skills for neonatal resuscitation in low-resource settings. This study aims to evaluate the implementation and impact of HBB training in Migori County, Kenya. METHODS: This single-group pretest-posttest study was conducted at 26 healthcare centres in Migori County, Kenya. Preimplementation neonatal health data were collected between January 2022 and June

Estimated neonatal survival of very preterm births across the care pathway: a UK cohort 2016-2020.

OBJECTIVE: Currently used estimates of survival are nearly 10 years old and relate to only those babies admitted for neonatal care. Due to ongoing improvements in neonatal care, here we update estimates of survival for singleton and multiple births at 22+0 to 31+6 weeks gestational age across the perinatal care pathway by gestational age and birth weight. DESIGN: Retrospective analysis of routinely collected data. SETTING: A national cohort from the UK and British Crown Dependencies. PATIENTS: Babies born at 22+0 to 31+6 weeks gestational age from 1 January 2016 to 31 December 2020. INTERV

Determinants of birth asphyxia among preterm newborns in Ethiopia: a systematic review and meta-analysis of observational studies protocol.

BACKGROUND: Birth asphyxia accounted for nearly 50% of neonatal mortality in Sub-Saharan African countries. This scenario has been worst in Ethiopia where every two out of three deaths attributed to birth asphyxia. Moreover, studies conducted in Ethiopia were highly variable and inconclusive to estimate the pooled prevalence and determinants of perinatal birth asphyxia among preterm babies. OBJECTIVE: This study aimed to estimate the pooled prevalence of birth asphyxia and its determinants among preterm newborns in Ethiopia. METHODS: The protocol for this review is registered at PROSPERO wit

Antibiotic treatment for newborns with congenital syphilis.

BACKGROUND: Congenital syphilis continues to be a substantial public health problem in many parts of the world. Since the first use of penicillin for the treatment of syphilis in 1943, which was a notable early success, it has remained the preferred and standard treatment including for congenital syphilis. However, the treatment of congenital syphilis is largely based on clinical experience and there is extremely limited evidence on the optimal dose or duration of administration of penicillin or the use of other antibiotics. OBJECTIVES: To assess the effectiveness and safety of antibiotic tre

Outcomes in oligohydramnios: the role of etiology in predicting pulmonary morbidity/mortality.

Objective Early-onset oligohydramnios is typically secondary to renal-urinary anomalies (RUA) or preterm premature rupture of membranes (PPROM). We compared neonatal pulmonary outcomes between these etiologies. Methods We conducted a retrospective cohort study of women with oligohydramnios identified before 24 completed weeks of gestation attributed to either PPROM or RUA. Patients were excluded if other fetal anomalies were noted. Respiratory morbidity was assessed by the need for oxygen at 36 corrected weeks or at hospital discharge. Results Of 116 eligible patients, 54 chose elective pregna

Air pollution during pregnancy and lung development in the child.

Air pollution exposure has increased extensively in recent years and there is considerable evidence that exposure to particulate matter can lead to adverse respiratory outcomes. The health impacts of exposure to air pollution during the prenatal period is especially concerning as it can impair organogenesis and organ development, which can lead to long-term complications. Exposure to air pollution during pregnancy affects respiratory health in different ways. Lung development might be impaired by air pollution indirectly by causing lower birth weight, premature birth or disturbed development o

The Antenatal Corticosteroids Trial (ACT)'s explanations for neonatal mortality - a secondary analysis.

BACKGROUND: The Antenatal Corticosteroid Trial assessed the feasibility, effectiveness, and safety of a multifaceted intervention to increase the use of antenatal corticosteroids (ACS) in mothers at risk of preterm birth at all levels of care in low and middle-income countries. The intervention effectively increased the use of ACS but was associated with an overall increase in neonatal deaths. We aimed to explore plausible pathways through which this intervention increased neonatal mortality. METHODS: We conducted a series of secondary analyses to assess whether ACS or other components of the
